Systematic Review of the Cause of Drug-Induced Liver Injuries in Korean Literature (약인성 간손상의 원인물질에 관한 국내연구의 체계적 고찰)

  • Background: The safety of herbal products is an important issue in Korea. It is more complicated because of the concomitant use of conventional western drugs and herbal medicine, including prescribed medicine, folk herbs and food supplements. Although both western and traditional Korean medical doctors have studied on the safety of HMP, their results did not show relevant or consistent conclusions because of the poor quality of studies, including bias. Objectives: The aim of this study was to review the studies in Korea related to drug induced liver injury (DILI) systematically. We tried to estimate the proportion of causative materials of DILI and evaluate the clinical difference of DILIs from different materials. Methods: Systematic searches were performed on MedRIC and MEDLIS in Korea. The extraction of data as well as selective screening of the studies was carried out independently by two of the authors. There were no restrictions on the types of publication, including grey literature. Results: Twenty-one articles were included. Of them only 7 adapted prospective design and only 6 were published in peer-reviewed journals. The proportion of conventional drugs associated with hepatotoxic injuries in all DILI ranged from 15.8% to 83.3% and that of herbal medicine was from 28.9% to 44.7%. However, the criteria for herbal medicine were not clearly defined and concomitant medications were not fully investigated in most studies. There were limited objective data in the clinical differences of liver injuries from different materials and their results were conflicting. Conclusions: The causality assessments regarding DILI of herbal medicine were not performed properly and causative materials were misclassified in most of the studies published in Korea. These make the safety issue still ambiguous because of the limitations and lack of objectivity of the studies. More rigorous studies are required for clearly addressing these conflicting issues with cooperative investigation between traditional Korean and western medicine.

Anti-inflammatory Effects of Aurantio-obtusin isolated from Cassia tora L. in RAW264.7 Cells (결명자로부터 분리된 Aurantio-obtusin의 항염증 활성)

  • Cassia tora L. have been used as a folk medicine in Korea. This study investigated anti-inflammatory effect of aurantio-obtusin isolated from C. tora. We isolated aurantio-obtusin from 50% ethanol extracts of C. tora L. We investigated the anti-inflammatory effects of aurantio-obtusin on the lipopolysaccharide (LPS)-stimulated inflammatory response in murine macrophage cell line (Raw 264.7). To investigate the cytotoxicity of aurantio-obtusin on RAW 264.7 cells, MTS assay was performed. RAW 264.7 cells were treated with aurantio-obtusin at different concentrations (12.5, 25, 50, $100{\mu}M$) for 30 h. The result showed that aurantio-obtusin had no cytotoxic effect in a concentration range of $12.5-100{\mu}M$. To determine the effect of aurantio-obtusin on LPS-induced NO production, the NO concentration measurement was performed. RAW 264.7 cells were treated with aurantio-obtusin at 12.5, 25, 50 and $100{\mu}M$ for 24 h, and the results showed that the NO production of aurantio-obtusin-treated cells compared to LPS alone treated group was significantly decreased in a dose-dependent manner. Pretreatment of aurantio-obtusin inhibited LPS-induced NO production in a dose-dependent manner. To find out inhibitory mechanisms of aurantio-obtusin on inflammatory mediators, we examined the $PGE_2$ pathways. As a result, $PGE_2$ were decreased in a dose-dependent manner by aurantio-obtusin. The release of interleukin-$1{\beta}$ (IL-$1{\beta}$) and IL-6 were also reduced. Moreover, aurantio-obtusin suppressed LPL-induced $I{\kappa}B-{\alpha}$ degradation. These results suggest that the down regulation of NO, $PGE_2$, IL-$1{\beta}$ and IL-6 expression by aurantio-obtusin are achieved by the downregulation of NF-${\kappa}B$ activity.

Effect of Water Extract of Cynanchi Wilfordii Radix in RANKL-induced Osteoclast Differentiation (백하수오(白何首烏) 물 추출물의 파골세포 분화에 미치는 영향)

  • Osteoporotic fracture became a serious social problem, which related with mortality and morbidity in old age population. Osteoclast which is responsible for bone resorption is originated from hematopoietic cell line and plays a key role osteoporotic bone loss. Cynanchum wilfordii (Asclepiadaceae) roots have been used in Korean folk medicine for the treatment of diabetes mellitus and aging progression. Also, recent studies have shown that the extract and fractions of Cynanchi Wilfordii Radix have various pharmacological actions including scavenging free radicals, enhancing immunity, reducing high serum cholesterol, and anti-tumor activity. However, the effect of extract of Cynanchi Wilfordii Radix in osteoclast differentiation had not been reported. Thus, we evaluated the effect of Cynanchi Wilfordii Radix on receptor activator of nuclear factor-${\kappa}B$ ligand (RANKL)-induced osteoclast differentiation. Through our study, we found that Cynanchi Wilfordii Radix significantly inhibited osteoclast differentiation induced by RANKL. Cynanchi Wilfordii Radix suppressed the activation of p38 pathway and $NF{\kappa}B$ in bone marrow macrophages (BMMs) treated with RANKL. Also, Cynanchi Wilfordii Radix significantly inhibited the mRNA expression of c-Fos, tartrate-resistant acid phosphatase (TRAP), osteoclast-associated receptor (OSCAR), nuclear factor of activated T cells (NFAT)c1 and cathepsin K in BMMs treated with RANKL. Particularly, Cynanchi Wilfordii Radix inhibited the protein expression of c-fos and NFATc1. Taken together, our results demonstrated that Cynanchi Wilfordii Radix may be useful treatment option of bone-related disease such as osteoporosis leads to fracture of bone and rheumatoid arthritis.

Anti-Inflammatory Effect of Sedum takesimense Nakai Water Extract in RAW 264.7 Cells (섬기린초 물 추출물의 마우스 대식세포에서 항염증 효능)

  • Background: Sedum takesimense Nakai has been used as folk medicine in Korea. The present study aimed to determine the biological activity of S. takesimense by investigating the anti-inflammatory effects of S. takesimense water extract (SKLC) on the lipopolysaccharide-induced inflammatory response in RAW 264.7 cells. Methods and Results: Cytotoxicity of SKLC on RAW 264.7 cells was determinded by performing MTS assay was found to have no cytotoxic effect on RAW 264.7 cells at a concentration range of $62-500{\mu}g/m{\ell}$. Further, pretreatment of SKLC inhibited lipopolysaccharide-induced nitric oxide (NO) production in a dose-dependent manner. To determined the inhibitory mechanisms of SKLC on inflammatory mediators, we assessed the inducible nitric oxide synthase (iNOS) and cyclooxygnease-2 (COX-2) pathways. The activities of these pathways were decreased in a dose-dependent manner by SKLC. The production of tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$), interleukin $(IL)-1{\beta}$, and IL-6 were also reduced. Conclusions: These results suggest that the down regulation of iNOS, COX-2, TNF-${\alpha}$, IL-$1{\beta}$, and IL-6 expression by SKLC are mediated by the down regulation of nuclear factor-${\kappa}B$ (NF-${\kappa}B$) activity, a transcription factor necessary for pro-inflammatory mediators. This might be the mechanism underlying the anti-inflammatory effects of SKLC.

Screening of Antioxidative, Anti-atherosclerotic Effect of Alisma Rhizome Extracts (택사 열수 및 에탄올 추출물의 항산화활성과 human LDL 산화억제 및 ACE 저해효과에 미치는 영향)

  • Objectives : The study was conducted to evaluate antioxidative, anti-atherosclerotic and anti-hypertensive effects of natural remedies. Alisma Rhizome (AR) has been used for a long time in Asia in folk remedies for treatment of hypertension and stroke and has been used in Korean traditional medicine for the treatment of glycosuria, gonorrhea, hypercholesterolemia, hypertension, and jaundice and its diuretic effect. These pharmacological effects of AR might come from antioxidant properties of phytochemicals in these materials. Methods : In this study, the antioxidant activity of extract from AR was studied with in vitro methods by measuring the antioxidant activity by TEAC, measuring the scavenging effects on reactive oxygen species (ROS) [superoxide anion, hydroxyl radical] and on reactive nitrogen species (RNS) [nitric oxide and peroxynitrite] as well as measuring the inhibitory effect on $Cu^{2+}$ induced human LDL oxidation and on ACE. Results : The AR extracts were found to have a potent scavenging activity, as well as an inhibitory effect on LDL oxidation and on ACE against all of the reactive species tested, with the water extract showing particularly strong antioxidant activities. Conculsions : The AR extracts have antioxidative, anti-atherosclerotic and anti-hypertensive effects in an in vitro system, which can be used for developing pharmaceutical drugs against oxidative stress and atherosclerosis.

Status of Hypertensive Patients' Drug-taking in Health Center and Its related Factors (보건소 방문 고혈압환자의 약물복용 실태와 관련요인)

  • The purpose of this study is to show that, before registration of chronic degenerative diseases and enforcing management system in health centers, the management of hypertension could achieved comprehensively and continuously by knowing the realities of drug-taking and its related factors those who are being cared for in health centers. For one year, a questionnaire about drug-taking realities of hypertensive patient was distributed at 8 health centers in Taegu from March 24. 1997 to April 24. 1997. The questionnaires were then collected and analysed. The results are as follows: Of 691 patients, 77.4% of patient were taking medicine regularly every day. The reasons why patients weren't taking medicine every day was as follows: the blood pressure became normal(34%, the highest of the group), patients forgot(28.2%), it was difficult to buy the medicine(15.4%), there was no effect even though the patient took medicine continuously. The experience of medical treatment outside of health centers was 28.9%. The types and percentages of alternative medicine are herbs (50.5%), health food(24.5%), folk remedy(13.7%). Among them, 44.6% of patients didn't know if the medicine was effective. Medical treatment places excluding health centers was pharmacies(63.5%-the highest rate). In simple analysis, the older one was the more regularly one took the medicine, but there was no statistical relations. Of health activities, patients eating low-salt diet showed that they took the medicine regularly. Nonsmoking patients and those who experienced drug side-effect and those who doubted doctor's prescriptions and instructions showed that they took alternative, excluding modem medicine. Anti-hypertensive drug medication status according to recognition, attitude, experiences of hypertension showed that patients who thought that they should take anti-hypertensive drug during the whole life time took the medicine regularly(82.3%). The reasons for patients turning to alternatives varied. In case of having subjective symptom(34.1%), those who thought they knew the hypertension well(36.6%), they decided to use alternatives. In multiple logistic regression analysis the key statistics were as follows: The older patients, patients who ate low-salt diet, patients who thought that they should take anti-hypertensive drug during the whole life time, all showed that they took medicines regularly. And also patients who experienced drug side-effect, doubted prescriptions and instructions, and patients who had subjective symptoms, patients who thought that he knew the hypertension very well, all showed higher rate of using alternative medicine. In the future, at the health centers, to register and manage hypertension patients effectively, we must educate patients about regular drug-taking, and alternatives without doctor's prescriptions.

  • The roots and leaves of Angelica keiskei (AK) have been used for the treatment of various diseases including coronary heartdisease, hypertension, and cancer in the Korean folk medicine. However, the mechanism by which methylenechloride fraction (MF) from AK exerts anti-tumorigenic activity in human prostate cancer cells has not been fully understood. In the present study, we report the MF exerted the highest cytotoxicity against prostate cancer DU145 cells compared with other fractions. Especially, MF caused the accumulation of sub-G1 DNA contents of cell cycle and increased annexin V-positive apoptotic bodies and DNA fragmentation. MF down-regulated several proliferative (Cyclin D1) and anti-apoptotic (Bcl-xl, Bcl-2, IAP-1/2, and survivin)gene products in these cells. Hence, MF induced apoptosis through the caspase-3 activation in DU145 cells. We further confirmed that caspase-3 plays an importance role in MF-induced apoptosis in DU145 cells by using caspase-3 inhibitor. Additionally, we observed that MF potentiated Dox-induced apoptosis in DU145 cells. Taken together, our data demonstrate the evidence that MF induces apoptosis depend on caspase-3 activation of and overcomes resistance to chemotherapy in human prostate cancer cells.

The Gastroprotective and Antioxidative Effects of Lonicera japonica water extract on HCl/ethanol-induced Gastric Mucosa Damage in Rats (인동(忍冬) 열수 추출물의 항산화 효과 및 HCl-Ethanol로 유도된 위염 동물 모델에서의 위 점막 손상 보호 효과)

  • Objective : Gastritis is a major complication of gastrointestinal disease. Lonicera japonica is used in folk medicine to treat different diseases such as exopathogenic wind-heat, epidemic febrile diseases, sores, carbuncles and some infectious diseases. Therefore, this study examined the effects of Lonicera japonica water extract (LJE) on HCl/ethano-linduced acute gastric ulceration and anti-oxidants properties. Methods : LC-ESI-IT-TOF MS was employed for rapid identification of major compound from LJE. The antioxidant activities were evaluated through total polyphenol and flavonoid contents and radical scavenging assays and superoxide dismutase (SOD)-like activity. SD rats were randomly divided into five different groups including the normal group, ulcer group, positive group (20 kg/mg of omeprazole, ip), and experimental groups (100 kg/mg and 500 kg/mg of LJE, ip). Results : 4,5-Dicaffeoyl quinic acid, loganic acid, secologanic acid, sweroside, loganin, vogeloside were identified based on the detection of the molecular ion with those of literature data. The LJE was possessed free radical scavenging activities such as DPPH (IC50=189.7 ㎍/㎖), ABTS (IC50=164.5 ㎍/㎖), and SOD-like activity (IC50=405.02 ㎍/㎖). Macroscopic and histological analyses showed LJE treated group were significantly reduced to an extent that it allowed leukocytes penetration of the gastric walls compared with the ulcer group. In addition, an ulcer inhibition rate and prostaglandin E2 levels were increased in rats treated with LJE. Conclusion : The present study has demonstrated the antioxidantive and gastroprotective effect of LJE, these findings suggested that LJE has the potential for use in treatment of gastric disorders.

  • Euonymus alatus(Thunb.) Sieb has been used for cancer theraphy such as Liver cancer, esophagus cancer, stomach cancer, and uterine cancer in folk. Eupolyphaga simemsis(ES) extracted from Euonymus alatus(Thunb.) Sieb increases HDL-C/TC ratio and LCAT activity. TGF-${\beta}$ and Insulin has been Knoen as factors to induce the supression of proliferation of Hep3B, Human hepatoblastoma cell. Plasma HDL3-C involved in carbohydrate methabolism in the liver was maintained in low level by Euonymus alatus(Thunb.) Sieb. The low level of plasma HDL3-C suppresses the transition from normal liver to Fat liver. Finally, this resuly in suppression of cancer in liver. In this study, Euonymus alatus(Thunb.) Sieb showed higher cell toxicity in hepatoblastoma cell(Hep3B) compared to the uterine cancer cell(HeLa). This means Euonymus alatus(Thunb.) Sieb seems not to affect in the all kinds of cancer. In future, we will study whether Euonymus alatus(Thunb.) Sleb have an effect on the liver cancer induced by virus and hepatocellular carcinoma. Also, the mechanims of suppression of cell proliferation in HCC need to investigated.

Anti-ulcer and antioxidant activity of leaves of Madhuca indica in rats

  • The leaves of Madhuca (M.) indica J.f.Gmel. (Sapotaceae) have been used traditionally in folk medicine due to its astringent properties and are effective in treatment of eczema and snake bites. Methanolic extract of M. indica is rich in tannins and has been proven experimentally to possess antibacterial activity. The present study was undertaken to evaluate the anti-ulcer and antioxidant activity of M. indica in rats. The methanolic extract of leaves of M. indica was tested at various doses (75, 150 and 300 mg/kg, p.o.) for its effect on gastric secretion and gastric ulcers in pylorus-ligation and on ethanol- induced gastric mucosal injury in rats. The significant reduction in ulcer index in both the models along with an increase in the pH of the gastric fluid and mucin content of stomach, and the acid secretory parameters such as total acidity and volume of gastric fluid were also significantly reduced along with reduction in the pepsin activity in pylorusligated rats proved the anti-ulcer activity of M. indica. The increase in the levels of superoxide dismutase, catalase and reduced glutathione and decrease in lipid peroxidation in both the models proved the antioxidant activity of M. indica. Thus it can be concluded that M. indica possesses anti-ulcer activity, which can be attributed to its antioxidant mechanism of action.
