Background: The aim of this study was to evaluate radiation exposure to the eye and thyroid in pain physicians during the fluoroscopy-guided cervical epidural block (CEB). Methods: Two pain physicians (a fellow and a professor) who regularly performed C-arm fluoroscopy-guided CEBs were included. Seven dosimeters were used to measure radiation exposure, five of which were placed on the physician (forehead, inside and outside of the thyroid protector, and inside and outside of the lead apron) and two were used as controls. Patient age, sex, height, and weight were noted, as were radiation exposure time, absorbed radiation dose, and distance from the X-ray field center to the physician. Results: One hundred CEB procedures using C-arm fluoroscopy were performed on comparable patients. Only the distance from the X-ray field center to the physician was significantly different between the two physicians (fellow: 37.5 ± 2.1 cm, professor: 41.2 ± 3.6 cm, P = 0.03). The use of lead-based protection effectively decreased the absorbed radiation dose by up to 35%. Conclusions: Although there was no difference in radiation exposure between the professor and the fellow, there was a difference in the distance from the X-ray field during the CEBs. Further, radiation exposure can be minimized if proper protection (thyroid protector, leaded apron, and eyewear) is used, even if the distance between the X-ray beam and the pain physician is small. Damage from frequent, low-dose radiation exposure is not yet fully understood. Therefore, safety measures, including lead-based protection, should always be enforced.
This study aims to find geometric parameters that the radiologist can change from time to time to reduce dose in angiography examinations. Depending on the geometric characteristics, the values calculated by effective dose were compared, while filming in fluoroscopy mode and Digital subtraction angiography, respectively. The study found that the lower the dose was in FPS mode, the lower the dose was reduced to 30-40%. Doses according to the X-ray angle were measured highest in AP View and lower as the angle went in the head direction. The greater the FOV, the higher the dose was 1.2-1.6 times, and the closer the distance between the X-ray tube and the table, the greater the dose was about 10%. In conclusion, various geometric characteristics in angiography examinations are parameters that can be applied by radiographers as frequently as possible, and appropriate geometric properties can be considered and applied in various situations, resulting in appropriate dose reduction.

Background: The physician's hands are close to the X-ray field in C-arm fluoroscopy-guided pain interventions. We prospectively investigated the radiation attenuation of Proguard RR-2 gloves. Methods: In 100 cases, the effective doses (EDs) of two dosimeters without a radiation-reducing glove were collected. EDs from the two dosimeters-one dosimeter wrapped with a glove and the other dosimeter without a glove-were also measured at the side of the table (Group 1, 140 cases) and at a location 20 cm away from the side of the table (Group 2, 120 cases). Mean differences such as age, height, weight, radiation absorbed dose (RAD), exposure time, ED, and ratio of EDs were analyzed. Results: In the EDs of two dosimeters without gloves, there were no significant differences ($39.0{\pm}36.3{\mu}Sv$ vs. $38.8{\pm}36.4{\mu}Sv$) (P = 0.578). The RAD ($192.0{\pm}182.0radcm^2$) in Group 2 was higher than that ($132.3{\pm}103.5radcm^2$) in Group 1 (P = 0.002). The ED ($33.3{\pm}30.9{\mu}Sv$) of the dosimeter without a glove in Group 1 was higher than that ($12.3{\pm}8.8{\mu}Sv$) in Group 2 (P < 0.001). The ED ($24.4{\pm}22.4{\mu}Sv$) of the dosimeter wrapped with a glove in Group 1 was higher than that ($9.2{\pm}6.8{\mu}Sv$) in Group 2 (P < 0.001). No significant differences were noted in the ratio of EDs ($73.5{\pm}6.7%$ vs. $74.2{\pm}9.3%$, P = 0.469) between Group 1 and Group 2. Conclusions: Proguard RR-2 gloves have a radiation attenuation effect of 25.8-26.5%. The radiation attenuation is not significantly different by intensity of scatter radiation or the different RADs of C-arm fluoroscopy.

Diagnostic reference level (DRL) is employed to optimize the radiation doses of patients. The objective of this study is to review the DRLs for interventional procedures in Korea and abroad. Literature review was performed to investigate radiation dose index and measurement methodology commonly used in DRL determination. Dose area product (DAP) and fluoroscopy time within each major procedure category were systematically abstracted and analyzed. A wide variation was found in the radiation dose. The DAP values and fluoroscopy times ranged 0.01-3,081 Gy·㎠ and 2-16,878 seconds for all the interventional procedures, 8.5-1,679 Gy·㎠ and 32-5,775 seconds for the transcatheter arterial chemoembolization (TACE), and 0.1-686 Gy·㎠ and 16-6,636 seconds for the transfemoral cerebral angiography (TFCA), respectively. The DRL values of the DAP and fluoroscopy time were 238 Gy·㎠ and 1,224 seconds for the TACE and 189 Gy·㎠ and 686 seconds for the TFCA, respectively. Generally, the DRLs of Korea were lower than those of other developed countries, except for the percutaneous transluminal angioplasty with stent in arteries of the lower extremity (LE PTA and stent), aneurysm coil embolization, and Hickman insertion procedures. The wide variation in the radiation doses of the different procedures suggests that more attention must be paid to reduce unnecessary radiation exposure from medical imaging. Furthermore, periodic nationwide survey of medical radiation exposures is necessary to optimize the patient dose for radiation protection, which will ultimately contribute to patient dose reduction and radiological safety.

The use of ultrasonography has recently been increasing in musculoskeletal diagnosis or intervention treatment. Ultrasound guided procedure offers a reliable alternative to fluoroscopy or computed tomography for lumbar medial branch block, facet joint block and peripheral nerves of lower extremity. Further, there is no exposure to radiation and additional equipment necessary for the protection against radiation is required. And ultrasound guided procedure needs smaller space than fluoroscopy guided procedure with real time images in the outpatient department. This article reviews ultrasound guided procedure at lumbar vertebra and peripheral nerves of lower extremity.
The purpose of our study was to determine the entrance surface dose and absorbed dose in ovary when using the metal speculum and plastic speculum in hysterosalpingography respectively. The examinations was performed in anthropomorphic phantom into which calibrated photoluminescence glass dosimeter were placed on symphysis pubis level surface and ovary area. We checked average fluoroscopy time and spot expose times during the hysterosalpingography. It was average fluoroscopy time 58 sec, spot expose 5 times. We divided the subjects into two different groups to used metal and plastic speculum. We measured 10 times of absorbed dose in the same condition of the anthropomorphic phantom. We compared two groups adsorbed dose on ovary with speculum material-related. The entrance surface dose on of plastic Speculum using group was average 17.23 mGy, absorbed dose on ovary was average 3.51 mGy. The entrance surface dose on ovary of metal Speculum using group was average 19.95 mGy, absorbed dose on ovary was average 4.14 mGy. Plastic speculum using group shows a decrease absorbed dose(17.9%) as compared with metal speculum using group. The method of plastic speculum using in hysterosalpingography. might provide us with lower radiation dose, especially in patients with childbearing stage.

Objective : Fixation of the C1-2 segment is challenging because of the complex anatomy in the region and the need for a high degree of accuracy to avoid complications. Preoperative 3D-computed tomography (CT) scans can help reduce the risk of complications in the vertebral artery, spinal cord, and nerve roots. However, the patient may be susceptible to injury if the patient's anatomy does not match the preoperative CT scans. The intraoperative 3D image-based navigation systems have reduced complications in instrument-assisted techniques due to greater accuracy. This study aimed to compare the radiologic outcomes of C1-2 fusion surgery between intraoperative CT image-guided operation and fluoroscopy-guided operation. Methods : We retrospectively reviewed the radiologic images of 34 patients who underwent C1-2 fusion spine surgery from January 2009 to November 2018 at our hospital. We assessed 17 cases each of degenerative cervical disease and trauma in a study population of 18 males and 16 females. The mean age was 54.8 years. A total of 139 screws were used and the surgical procedures included 68 screws in the C1 lateral mass, 58 screws in C2 pedicle, nine screws in C2 lamina and C2 pars screws, four lateral mass screws in sub-axial level. Of the 34 patients, 19 patients underwent screw insertion using intraoperative mobile CT. Other patients underwent atlantoaxial fusion with a standard fluoroscopy-guided device. Results : A total of 139 screws were correctly positioned. We analyzed the positions of 135 screws except for the four screws that performed the lateral mass screws in C3 vertebra. Minor screw penetration was observed in seven cases (5.2%), and major pedicle screw penetration was observed in three cases (2.2%). In one case, the malposition of a C2 pedicle screw was confirmed, which was subsequently corrected. There were no complications regarding vertebral artery injury or onset of new neurologic deficits. The screw malposition rate was lower (5.3%) in patients who underwent intraoperative CT-based navigation than that for fluoroscopy-guided cases (10.2%). And we confirmed that the operation time can be significantly reduced by surgery using intraoperative O-arm device. Conclusion : Spinal navigation using intraoperative cone-beam CT scans is reliable for posterior fixation in unstable C1-2 pathologies and can be reduced the operative time.
