INTERRATER RELIABILITY OF CARIES DIAGNOSIS BY LASER FLUORESCENCE (레이저형광 우식진단법의 검사자간 신뢰도에 관한 연구)

  • The purpose of study was to measure the interrater reliability of the caries diagnostic instrument, Diagnodent(KaVo, Germany), using a new laser fluorescence technique. Occlusal surfaces and smooth surfaces of one hundred extracted human premolars and molars were examined by four dentists. Pearson correlation coeffcients were 0703 to 0.870 for occlusal surfaces and 0.764 to 0.932 for smooth surfaces (P<0.01, all). Reliability coefficients which were calculated by intraclass correlation coefficients were 0.9450 for occlusal surfaces and 0.9605 for smooth surfaces. Therefore, the interrater reliability of caries diagnosis by Diagnodent is very high, and the rater training would be necessary more for occlusal surfaces than for smooth surfaces.

  • The 5-aminolevulinic acid (5-ALA)-induced tumor fluorescence is a useful intraoperative marker for the diagnosis and the detection of various malignancies, but its use in meningioma is only reported infrequently. In meningioma, a complete resection of the tumor mass is crucial for the prevention of recurrence and postoperative morbidities. Deep sylvian meningioma is a rare type of meningioma where complete tumor removal is complicated by its deep anatomical location and close involvement with the middle cerebral artery. From our experience, 5-ALA-mediated fluorescence facilitated a safe excision whilst preserving critical neurovascular structures. To our best knowledge, this is first report from use of 5-ALA in a deep sylvian meningioma.

  • To evaluate the quantitative nature of laser fluorescence(LF) and dye-enhanced laser fluorescence(DELF) on early enamel caries, artificial caries were induced on the bovine enamels and argon laser was irradiated on the surfaces of lesions. Fluorescence radiance from enamel specimen was recorded with CCD camera which was connected to PC image analyzing system. The difference of fluorescence of radiance(DFR) between the carious and the sound enamel in each sample of the LF and the DELF groups were measured by image analyzing program. The DFR of lesion surfaces measured with laser fluorescence and lesion depths were evaluated and the results are as follows: The caries lesions were discriminated from sound enamels by the darkness with LF and by the brightness with DELF. The DELF was more sensitive than the LF at the earlier stage of demineralization.

  • Recently, there have been improvements in diagnostic methods for the assessment of early caries lesions. The reason is that dental professionals are seeking methods to reliably detect incipient dental caries and to remineralize them. This review examines the literature on principles, theoretical background, and history of the Quantitative Light-Induced Fluorescence (QLF) system (Inspektor Research Systems BV, The Netherlands). Furthermore, this paper discusses the potential application of QLF system to clinical practice for educational purpose, enabling dental hygiene students to perform oral health assessment using the QLF system. In addition, the clinical application of QLF system can motivate patients by providing additional visual information about caries and bacterial activity. The evidences on validity and reliability of the QLF system for detection of longitudinal changes in de/remineralization and caries were examined. The QLF system is capable of monitoring and quantifying mineral changes in early caries lesions. Therefore, it can be used to assess the impacts of caries preventive measures on the remineralization and reversal of the caries process. And the QLF system is a very promising equipment to assess educational effectiveness for dental hygiene students in their learning process. In conclusion, the QLF system is the most effective technology for more sensitive staging of caries and treatment without surgical intervention.

  • Cancer serum biomarkers have advanced our ability to more accurately predict tumor classification, prognostic/metastatic potential, and response potential to novel chemotherapies. Serum amyloid A (SAA) and Vascular endothelial growth factor (VEGF) have potential utility as a serum biomarker for lung cancer. Quantum dots, nanometer-sized crystals, have a high quantum yield, sensitivity, and pronounced photostability. The properties of quantum dots can be efficiently applied to the detection of serum biomarkers in immunoassays as fluorescent probe. We used quantum dots as fluorescent probes in immunoassays and attempted to detect serum amyloid A and vascular endothelial growth factor as serum biomarkers of lung cancer. This fluorescence immunoassay based on the properties of quantum dots is applicable to the detection of serum biomarkers for lung cancer. The fluorescence immunoassay with quantum dots should allow the efficient and specific detection of serum amyloid A (SAA) for the possible diagnosis of lung cancer.

DEVELOPMENT OF FILTERING SYSTEM OF LASER FLUORESCENCE FOR IMPROVEMENT OF THE DIAGNOSTIC SENSITIVITY FOR DENTAL CARIES (레이저 형광법의 치아우식증 진단감도를 증진시키기 위한 필터링 시스템 개발)

  • The objective of this study is to improve the optical sensitivity of laser fluorescence for detection of incipient enamel caries. An incipient carious lesion was formed in various stages by placing an enamel specimen of a bovine tooth in STPP demineralization solution. After measuring the optical density of the lesion surface by laser fluorescence induced by argon laser and various alter of yellow(500-520nm), amber(520-540nm), orange(540-560nm), and red(560-580nm), the specimen was cut vertically to measure the depth of the lesion using a polarizing microscope. SAS statistical program was used to analyze the relationship between the optical density of the lesion suface and the depth of the lesion. The results were as follows: 1. The optical density of early carious lesion, measured by laser fluorescence with amber and orange filter, and lesion depth observed by polarizing microscope, were increased as demineralization time increased. 2. The correlation coefficient between optical density of the lesion surface and the histological depth of the lesion was the highest in orange filter(r=0.49), followed by amber(r=0.32), yellow(r=0.13) and red(0.01). 3. Regression analysis showed that the most linear relationship between the optical density and the lesion depth was existed in orange filter group. In regard above results, laser fluorescence could be considered to be reliable for optical diagnosis of dental caries.

  • It was reported that the etiologies of recurrent spontaneous abortion are immunologic factors, endocrinologic problems, anatomical abnormalities, genetic abnormalities, infection, and unexplained factors. Among those etiologic factors, genetic abnormalities occur in about 5% of the couples who experience recurrent spontaneous abortions, and most common parental chromosomal abnormality contributing to recurrent abortion is balanced translocation. The advent of in vitro fertilization (IVF), the development of skills associated with the handling of human embryo, and an explosion of knowledge in molecular biology have opened the possibility of early diagnosis of genetic disease in preimplantation embryos. Therefore preimplantation genetic diagnosis (PGD) is indicated for couples, infertile or not, at risk of transmitting a genetic disease. A case of successful pregnancy and term delivery by PGD using fluorescence in situ hybridization (FISH) technique in patient with recurrent spontaneous abortion due to balanced translocation is presented with brief review of literatures.

Prenatal diagnosis of an unbalanced translocation between chromosome Y and chromosome 15 in a female fetus

  • We report the prenatal diagnosis of an unbalanced translocation between chromosome Y and chromosome 15 in a female fetus. Cytogenetic analysis of parental chromosomes revealed that the mother had a normal 46,XX karyotype, whereas the father exhibited a 46,XY,der(15)t(Y;15) karyotype. We performed cytogenetic analysis of the father's family as a result of the father and confirmed the same karyotype in his mother and brother. Fluorescence in situ hybridization and quantitative fluorescent-polymerase chain reaction analysis identified the breakpoint and demonstrated the absence of the SRY gene in female members. Thus, the proband inherited this translocation from the father and grandmother. This makes the prediction of the fetal phenotype possible through assessing the grandmother. Therefore, we suggest that conventional cytogenetic and molecular cytogenetic methods, in combination with family history, provide informative results for prenatal diagnosis and prenatal genetic counseling.
