These days, in Korean Traditional Medicine, acupuncture is attracting our attention. As a result, many kinds of acupuncture method are developed. Especially, Saam's Acupuncture, Hwa Acupuncture are usually used in clinic, today. All of them use Five Transporting Point(五輸穴) and are based on theory of intergeneration/interinhibition among the Five Element(五行相生相克理論). They belong to Five Element Acupuncture(五行鍼法). This paper introduce the Medicine-Changes Three Point Acupuncture which is another form of Five Element Acupuncture and study its significance.

Objectives : This research was performed to investigate the effect of five element constitutional acupuncture(Sa-am acupuncture) therapy at acupoints on the Yin meridians on obese rats(Sprague Dawley strain, male) induced by high fat diet. Methods : The control and acupuncture groups were provided with high fat diet, and the acupuncture groups were treated with five element constitutional acupuncture(Sa-am acupuncture) every other day for five weeks. We observed the body weight gain, food intake, riglyceride, total cholesterol, HDL-cholesterol, AST, and ALT. Result : The acupuncture group treated with five element acupuncture on the meridians of spleen, heart, pericardium showed significant decrease of body weight, the groups treated on the meridians of lung, heart, pericardium showed signigicant decrease of food intake. Concentration of HDL-cholesterol was significantly increased in the groups treated on the meridians of spleen, heart, pericardium, liver. Conclusion : Reviewing these experimental results, it appears that the five element constitutional acupuncture(Sa-am acupuncture) therapy at acupoints on the meridians of spleen, heart and pericardium have an efficacy on obesity induced by high fat diet.

The purpose of this study was to report the effect of a combination of Sa-am five-element acupuncture and eight-principle pharmacopuncture (EPP) for the treatment of an essential tremor (ET). This study reviewed the medical records treated at OO Korean medical hospital for ET by using diverse types of acupuncture without herbal medicine, other types of physical therapy, and western medication related ET or Parkinson's disease and was performed after the approval of the institutional review board (IRB). The three cases that were finally selected were then extracted and reviewed. The three cases that were finally selected involved three women in their 70s to 80s. The evaluation of the progress was made by using the numeric rating scale. A combined treatment, the method of liver excess (肝乘格), from among Sa-am five-element acupuncture, and Hwangyeonhaedoktang EPP at CV23 and CV17, was applied to all cases. In all three cases, the ET was improved, and recurred ETs improved with the same treatment. The results suggest that the combined treatment of Sa-am five-element acupuncture and Hwangyeon-haedoktang EPP may be effective for treating an ET, even though this conclusion is based on only three cases.

Objective: To report the case of a patient suffering from Broca’s aphasia after cerebral infarction, which was improved by Korean medical treatments, especially five-element acupuncture.Methods: To relievesymptoms, Korean medicine treatments such as acupuncture, moxibustion, and herb medication were performed. To evaluate the effect of the treatments, articulation accuracy, vowel accuracy, and the sequential motion rate of articulator were checked.Result: After treatments for 28 days, the symptoms were improved.Conclusion: This report suggests that five-element acupuncture can be effectively used for patients with Broca’s aphasia after cerebral infarction.

Objectives: To investigate validity and effectiveness of Saam five element acupuncture treatment in psychological problems. Methods: We selected a Korean medicine doctor known to apply Saam five element acupuncture treatment to the psychiatry field. An investigator visited the doctor's clinic to observe the cases and interview patients. Duration of each interview was 30 minutes to one hour. A semi-structured questionnaire was used for in-depth interviews. All interviews were recorded and converted into texts. Converted texts were then summarized and analyzed qualitatively. Results and Discussion: Treatment process of Saam five element acupuncture designed for psychiatric use is divided into four stages: qi transformation (氣化), acupuncture treatment, deep breathing, and acupressure. In the first stage, patients are asked to express their feelings and emotions in a physical sense; to imagine their feelings have colors, weight, and physical form, and to describe those characteristics. Next, the doctor evaluates each patient's qi according to the six qi (六氣), and then determines protocol of Saam five element acupuncture treatment. This treatment is focused on suffering of the patient and specific symptoms or emotions. The treatment duration is brief and changes in symptoms are frequently checked during treatment sessions. Without discussing details of stressful events, the doctor enables patients to express physical aspects of their feelings. He also provides them with psychoeducational resources. As a result, patients experienced immediate changes in body sensation, emotions and thinking, decreased response to triggers, and increased self-control after treatment. Conclusions: The Saam five element acupuncture treatment has the potential to be used as a treatment for psychiatric disorders. Treatment can be effective in managing psychiatric disorders when applied by primary Korean medicine clinics.

'Nei Jing(內徑)' first defined the interrelationship of the true and tile false between evil factor affecting health(雅氣) and vital essence energy(精氣). According to 「'Nei Jing(內徑)', the above interrelationship is explained as 'If state of evil domination is considered as sthenia-syndrome(雅氣盛則實), if the consumption of healthy energy Is considered as asenia-syndrome(精氣尊則虛): 'Nei Jing(內徑)', proposed major features of the medicall treatment by 'regluate the vatal energy of asthenia and sthenia, treat the sthenia-syndrome by purgation, and treat the asenia-syndrome by therapy of invigoration(調其氣之虛實, 實則瀉之, 虛則補之): The above interrelationship was interpreted as 'treat the asthenia-syndrome of child organ by invigorating the mother organ(虛者補其母)'in the 69th of 'The Classic on Difficulty',(難經 六十九難). Go-Mu(高武) of Myung-dynasty describe therapy for invigoration and purgation of itself-meridian(自經 補瀉法), which locating acupuncture points according to the Therorr of Five Element in the five shu points of itself-meridian(自經 五유穴), based on the generation in the ${\ulcorner}$A Synthetical Book of Acupuncture and Moxibustion(針灸聚英)${\lrcorner}$, Sae-hyun Jang(張世賢) further extended location acupuncture points of the five shu points to the other-meridian in the ${\ulcorner}$Gyeo Jung Do Ju Nan Gyung(校正圖註難經)${\lrcorner}$ Sa-Ahm's 5 Element Acupuncture Method(舍嚴五行鍼法) was originated in 1644, the middle of the Yi-dynasty. It linked the reinforcing and reducing in acupuncture therapy which incorporated tlle asthenia-syndrome and sthenia-syndrome of the hollow organs, based on principle of the Yin Yang 5 Element Theory(陰陽五行學說), not only to the generation in the 5 element(相生關係) but also to the restriction in the 5 element(相剋關係). Furthermore it was devised for the medical treatment by comning therapy for invigoration and purgation of itself-meridian(自經 補瀉法) with that of the other-meridian. Even though many original forms(正形) of the therapy for invigoration and purgation of the Yin Yang 5 Element Theory comply with the principle of the generation and the restriction based on the principle of the Yin Yang 5 Element Theory are abailable, variational forms(變形) are also recognized by examining the nature of the Sa-Ahm's 5 Element Acupuncture Method(舍嚴五行鍼法), For this reason, it is very difficult to understand the Sa-Ahm's 5 Element Acupuncture Method(舍嚴五行鍼法) thoroughly. therefore, those variational forms are obstacles for the beginners to study the Sa-Ahm's 5 Element Acupuncture Method. In order to understand the principle of the practical clinical application of the Sa-Ahm's 5 Element Acupuncture Method, this study investigated which principle was based on the variations of the locating acupuncture points' method for the acupuncture prescription.

Objective : The aim of this research was to investigate the effect on five element constitutional acupuncture(Sa-am acupuncture) at 4 acupoints by needle manipulation to sedate Six Fu-Organ on hyperlipidemia induced by high fat diet in rats. Method : We recorded data of weight gain, food intake, food efficiency, weight of liver, heart, spleen, lung, kidney, levels of total cholesterol, triglyceride, HDL-cholesterol, AST, ALT in serum of rats fed high fat diet for 5 weeks. Spraque Dawley rats were divided into eight groups(normal; normal diet and non-acupuncture group, control; non-acupuncture group, Acu-LI; acupuncture at $SI_5{\cdot}LI_5{\cdot}BL_{66}{\cdot}LI_2$ group, Acu-ST; acupuncture at $GB_{41}{\cdot}ST_{43}{\cdot}LI_1{\cdot}ST_{45}$, Acu-SI; acupuncture at $BL_{66}{\cdot}SI_2{\cdot}ST_{36}{\cdot}SI_8$, Acu-BL; acupuncture therapy at $ST_{36}{\cdot}BL_{40}{\cdot}GB_{41}{\cdot}BL_{65}$, Acu-TE; acupuncture at $BL_{66}{\cdot}TE_2{\cdot}ST_{36}{\cdot}TE_{10}$, Acu-GB; acupuncture at $LI_1{\cdot}GB_{44}{\cdot}SI_5{\cdot}GB_{38}$. After needles inserted, it was rotated clockwise and anticlockwise 12 times on rats fed high fat diet. Results : The body weight was decreased in Acu-SI, Acu-TE, Acu-GB group, /the food intake was decreased in Acu-TE, / the food efficiency was decreased in Acu-SI, /serum triglyceride was decreased in Acu-LI, Acu-ST, Acu-BL, Acu-GB, /serum total cholesterol was decreased in Acu-LI, Acu-BL, serum ALP was decreased in Acu-LI, Acu-ST, Acu-BL, Acu-TE, Acu-GB. While the relative weight of heart was increased in Acu-LI, Acu-SI, the relative weight of kidney was increased in Acu-SI, Acu-GB. Conclusion : These results suggest that five element constitutional acupuncture(Sa-am acupuncture) may be having an therapeutic influence on hyperlipidemia induced by high fat diet. Further researches of acupuncture manipulation are needed in the future based on our study.

Four-constitutional theory proposed by Dr. Lee, Je-ma has been evolve for last 100years by his followers. One of the major progresses was the emergence of constitutional acupuncture proposed by Dr. Kwan Doh-won. Constitutional acupuncture included the new pulse diagnostic method to determine the body type, pulse diagrams and corresponding acupuncture method called five element acupuncture in which five-element points in 12 meridian channels are used. Dr. Sa-ahm proposed five-element acupuncture three and a half hundred years age. Conjunction of Dr Lee's theory to Dr. Kwan proposed eight constitutions with eight pulse diagrams. He also adopted eight acupuncture prescriptions out of twenty-four Sa-ahm's acupuncture prescriptions. The author, Dr. Yom Tae-hwa, proposed more inclusive theory of 24 constitutions with 24 diagnostic pulse diagrams in Las Vegas International Acupuncture Convention in 1986. This article reviews history of constitutional medicine briefly. Each step of theoretical evolution from four-constitutional theory to twenty-four-constitutional theory is explained. By understanding the theoretical connection between twelve meridian channels based on five-element theory and four-constitutional theory in the article, one will easily accept the emergence of twelve of twenty four constitutional theory. Next, the article is focused on four subcategories under Sao-Yin constitution, which are one-sixth of 24 constitution. in this section classification, pulse diagnostic method, pulse diagram acupuncture prescriptions and herbal prescriptions are explained in detail. Then, clinical trial reports of acupuncture treatment and/or herbal formulas for such constitutions are attached for the rest part of the article. the purpose of this paper is to present the new hypothesis with the result of preliminary clinical trial hoping that additional clinical reports from other practitioners in this field triggers well-designed clinical study in future.

Objectives : This survey was accomplished to find out how Korean medical doctors take acupuncture prescriptions for knee osteoarthritis in real clinical practice. Methods : The survey questions were developed by the consensus from 4 professors and 10 residents who major in acupuncture & moxibustion for developing clinical trial protocol on individualized acupuncture treatment for knee osteoarthritis. The questionnaires were distributed via e-mail to 3,306 members of Korea Oriental Medical Association from March 15th to March 23rd in 2005.84 members completed answers, and the computerized data were analyzed by ISP statistical program. Results : 1. 68 out of 84 Korean medical doctors used pattern diagnosis. 2. 61 out of 84 Korean medical doctors used both local and remote points, 20 doctors remote points only, and 3 doctors local acupuncture points only. 3. In case of doctors who use remote acupuncture points only, the acupuncture prescription principle was Saam or five element acupuncture (66%), along the meridian pathway (14%), Eight constitutional acupuncture (11%), Taegeuk acupuncture (2%), and miscellaneous (18%). Conclusion : In our e-mail survey, Korean medical doctors who experienced more than 10 year practice answered that they use five element acupuncture or Saam acupuncture according to meridian pathway theory as the most common principle of their acupuncture treatment prescription.

Objectives : Saam acupuncture initiated by Saamdoin is traditional and originative method, which is characterized by applying the five phases theory and mother-child reinforcement-reduction principle to the selection of acupoints and needling manipulation. Our study was aimed to summarize and assess the use of acupoint SP3(Taebaek) in Saam acupuncture treatment and to further understand Saam acupuncture in an aspect of the combination of acupoints. Methods : We searched the data based on "(Do Hae Kyo Kam) Sa-Ahm's acupuncture method" for SP3 used and acupoint combination including SP3. We performed frequency analysis, network analysis, and cluster analysis for quantitative aspect. To understand clinical implication of SP3 with another acupoint, qualitative and descriptive methods were also carried out. Results and Conclusions : In our study, SP3 was frequently used for tonification of lung, spleen, heart, and kidney meridian and sedation of kidney, heart, and lung meridian. For this, many acupoints such as LU8, LU9, KI3, HT8, KI7, LU10 and LR1 were used with SP3. The combination of SP3 and other acupoints were used to treat stroke, common cold, and pain conditions including headache, low back pain, respiratory disease as well as gastroenteric troubles including stomachache, indigestion, vomiting, and constipation. To further understand Saam acupuncture, an understanding of the five transport points based on five elements characters, pathological changes (deficiency and excess) of viscera and bowels, and concept of source point should be preceded.
