The WMSDs are known to occur more in upper extremity than lower extremity, and such a ratio is increasing each year. The motions or repeated work requiring excessive strength beyond worker's maximum grip strength were known as a major cause of the WMSDs in upper extremity. To prevent the WMSDs in upper extremity, research on the grip strength characteristics analysis of field workers that can be used as basic data for work design and manual tool design is needed. The purpose of this study is to identify various variables affecting grip strength and is to find out the impacts of grip strength on WMSDs. This research measured the grip strength of 327 field workers at Heavy Industries, and also conducted a questionnaire survey on individual characteristics and job characteristics. As a result of analyzing grip strength, the grip strength was statistically significant (P<0.1) according to the body surface area (BSA) of the research subjects. The differences between percentile groups of grip strength were statistically significant (P<0.1) according to pain levels of hand/wrist/finger and arm/elbow. The comparison results between the average grip strength of Korean adult males and the average grip strength of the survey-targeted heavy industry workers show that the average grip strength of the heavy industry workers was higher by 9.75 kg. This study analyzed relationship between grip strength and the pain levels of hand/wrist/finger and arm/elbow, and compared the findings in this study with those of existing preceding studies. Also, this research comparatively analyzed the grip strength of Korean adult males and survey-targeted heavy industry workers. The findings of this study can be used as useful data for ergonomic work design and manual tool design to prevent WMSDs at industrial worksites, given that almost no data on the grip strength of workers in the industrial sites are found in Korea.
This study was carried out in order to examine the health status of arthritis patients living in K city of southern part of Korea. The data were collected with AIMS2 instrument from 56 subjects who were asked to complete the questionnaire by the trained surveyor. The AIMS2 has been approved useful instrument for assessing the outcome of various treatments and programs in rheumatic diseases. The component variables of the AIMS2 are Mobility, Walking and Bending, Hand and finger function, Arm function, Self-care tasks, Household tasks, Social activity, Support from family and friends, Arthritis pain, Work, Mood, Level of tension, The total number of items of AIMS 2 are 78. The results are as follows: The average age of the subjects was 58 years old and no gender difference. The mean scores of the 12 sub-concepts of AIMS2 are 15.0 of mobility, 12.16 of walking and of bending, 9.32 of hand and finger function, 9.03 of arm function, 13.53 of self-care tasks, 10.19 of household tasks, 15.09 of social activity, 19.58 support from family and friends, 13.82 of arthritis pain, work, 14.85 of level of tension, 15.12 of mood. The Cronbach Cronbach $\alpha$. is 0.72-91 for RA patients and 0.74-96 for OA patients. This results showed that the health status of Arthritis patients at home in K-city was almost not so serious, but the level of tension and mood were quite higher than the average score. Considering the results of this particular subjects, an appropriate rehabilitation program should be developed and implemented.
Frozen food processing workers work under a cold environment which can cause several adverse health effects.This study explored factors affecting workers' health in the frozen food industry in Thailand. Participants comprised 497 workers exposed to a cold working environment and 255 office workers who served as the controls. Data were collected by a survey on the work environment, and the interview of workers for abnormal symptoms. The exposed group had the following characteristics: 52.7% male, overall average age of 27 (SD 6.6) years old, attained elementary education (Grade 4 and Grade 6) (54.1%), married (48.9%), smokers (21.3%), alcohol consumption (31.0%), duration of work was between 1 and 5 years (65.2%), working 6 days a week (82.7%), 1-5 hours of overtime per week (33.8%), office workers (33.9%); work category: sizing (6.9%), peeling (28.3%) dissecting (22.2%), and in the warehouse (8.6%). The temperature in the work environment ranged from $17.2^{\circ}C$ to $19.2^{\circ}C$ in most sections, $-18.0^{\circ}C$ in the warehouse, and $25^{\circ}C$ in the office areas. Warehouse workers had more abnormal symptoms than controls including repeated pain in the musculoskeletal system (OR 11.9; 95% CI 6.12-23.45), disturbance throughout the body (OR 4.60; 95% CI 2.00-10.56), respiratory symptoms (OR 9.73; 95% CI 3.53-26.80), episodic finger symptoms (OR 13.51; 95% CI 5.17-35.33). The study results suggest that workers' health should be monitored especially with regard to back and muscle pain, respiratory symptoms, episodic finger symptoms, and cardiovascular symptoms. Health promotion campaigns such as antismoking and reduction of alcohol consumption should be established because smoking and alcohol consumption are contributing factors to the pathogenesis of Raynaud's phenomenon and peripheral vascular disorders such as hypertension and heart disease.
The purpose of this study was carried out to rind out the prevalence of occupational musculoskeletal disorders among workers in manufacturing industries, so that the result could provide basic data necessary to prevent musculoskeletal disorders. Information on general characteristics, occupational characteristics, and musculoskeletal symptoms were obtained by a self-administered questionnaire between October and November in 2002, from 345 workers in Kim-hae and Ulsan, Kyung-nam province. The data were analyzed for chi-square test by using SPSS NVIN 10.0 program. The results are as follows: According to the self-reports, among musculoskeletal symptoms complain on shoulders are topping for 50.4%, low back is followed for 40.6%, leg/foot 35.7%, neck 34.5%, wrist/hand/finger 30.1%, and arm 24.3%. According to occupational characteristics prevalence by anatomical site, about neck pains 119 subject, occupational satisfaction is 62,6% in moderate group, work shift is 71.8% in no work shift group, each significant high. Symptom complain rate of 104 patients who complained on wrist, hand and finger pain is significantly related that 36.5% below 5 years and 36.5% above 15 years have been worked group. And each of them are significantly related 77.9% in labor workers group, 70% in no work shift group, 54.8% in frequent transfer group. Among 140 subjects who have back pain, that is significationtly reported on 37.1% below 5 years and 37.9% above 15 years and 60.0% moderate satisfied occupation group have been worked group. Therefore, some efforts should be proceeded such as improvement of working condition, flexibility for changing work, more pleasant and better working environment, and etc.

Objectives : The purpose of this study was to prove the clinical safety of Sweet Bee Venom(BV) and observe the physical reaction(percentage of localized itching & pain) after Sweet BV treatment. Methods : This study was carried out on 130 patients who had been injected with Sweet BV in Koran Medicine Hospital of Woosuk University from March 20, 2012 to June 30, 2013. Patients were treated with Sweet BV daily and we checked the physical reaction. After that, we analyzed those according to treatment times and body parts of injection. Results : 37 patients(28.46 %) complained localized itching and 41 patients(31.54 %) complained localized pain after Sweet BV injection. In 37 patients who complained localized itching, 8 patients were experienced itching in the first treatment. And 27 patients were experienced itching in the 1st~5th treatment. Wrist, ankle and toe were the highest percentage of localized itching. Finger was the highest percentage of localized pain. Knee showed a relatively higher percentage of itching, pain, itching & pain than other body parts. Conclusions : This study suggested that Sweet BV treatment was relatively safe treatment and doctors should explain the physical reaction before treating patients. Further studies are needed to propose a guideline for safety and treatment.
This study was designed to explore use of unconventional treatments and identify related variables of patients with rheumatic disease. One hundred fifty rheumatic patients were recruited from two university based rheumatic centers according to selection criteria. Collected data were analyzed using SAS program through with a structured questionnaire. T-test, ANOVA, and Scheffe's test were adopted. The results were as follows: 1. 77.7% of the subjects were over forties and mean age was 48.5 years. 74.0% were women and 73.2% were living with their spouse. 82.0% of them was good or moderate or good economic status, 38.3% was university graduates. 2. 58.0% of the subjects had rheumatoid arthritis, and mean duration of suffering rheumatic disease was 6.5 years. 89.3% of them had joint pain and their mean pain score was 4.07. Most painful joints were hip(58.0%), finger(42.0%), wrist(42.0%), and the number of painful joint was 5.4. 70% of the subjects using unconventional treatments, the most common treatments using them was herb medicine(42.7%) and acupuncture(36.7%). 3. Level of pain was different according to subject's age, educational background, diagnosis, number of painful joints, and using unconventional treatments or not. It is recommended that use of a larger sample to understand more about unconventional treatments and pain of patients with rheumatic disease. More research is needed to consider developing individual nursing interventions for their well-being and quality of life.
Objective: This study aimed to investigate the short-term effects of flexion-distraction spinal manipulation on intervertebral height, pain, spine mobility in patients with lumbar degenerative disc disease. Design: Randomized controlled trial with a pretest-posttest control group design Methods: A total of 96 participants with degenerative disc disease participated in the study and were randomly divided into two groups. Both groups received intervention for 3-5 minutes a day. The experimental group (n=48) underwent flexion-distraction spinal manipulation for 3-5 minutes, and the control group (n=48) was maintained in the same position as the experimental group for 5 minutes without any intervention. The intervertebral height was measured by computed tomography, pain was assessed using visual analog scale, and the spine in flexion mobility was measured using the finger-to-floor distance test and passive straight leg raise test. Pre-test and post-test measurements were obtained. Results: The experimental group showed significant improvement in intervertebral height, degree of pain, and spinal mobility (p<0.05). The intervertebral height increased from 6.32±1.90 to 6.93±1.85 mm (p<0.05), lower back pain decreased from 69.17±13.35 mm to 48.48±12.20 mm (p<0.05), lumbar spine mobility changed from 17.37±4.49 to 12.69±4.34 cm (p<0.05), and passive straight leg raise test range increased from 46.94±13.05° to 56.01±12.20° (p<0.05). Conclusions: This study suggests that flexion-distraction spinal manipulation could be an effective treatment for decreasing pain and improving function in patients with degenerative disc disease.

Reflex sympathetic dystrophy is an important clinical entity that is characterized by excessive or prolonged pain, vasomotor and other autonomic disturbances, delayed recovery of function, and trophic changes. This syndrome is among the most frequently encountered problems in clinical medicine, and proper diagnosis and therapy are critical. Accidental or surgical trauma or one of a variety of disease states may become a precipitating factor. Proper recognition and treatment result in rapid elimination of symptoms and complete recovery. A 56-years old male accidented total amputation of the proxomal part of the left index finger in May, 1996. Emergently, complete replantation procedure was successfully performed in the department of reconstructive surgery, medical center, Korea University. Afterward, he began to suffer from uncontrolled, prolonged pain in left index finger, proximally spreading pain to the left upper extremity and limited joint movement of the left shoulder. Although many treatments were used for this syndrom, not all of them were effective. Furthermore, the treatments which proved effective had detrimental side effects. However, thoracoscopic left thoracic sympathectomy was performed in our department. This therapy successfully relieved the pain and improved shoulder joint movement.
Purpose: Pachydermoperiostosis is a rare hereditary disease characterized by finger clubbing, periosteal reaction, and pachydermia. The underlying pathogenic mechanism of this disease remains unclear. This disease is known to be associated with a variety of diseases such as cranial suture defect, bone marrow failure, hypertrophic gastropathy, Crohn's disease, and female escuchen. Methods: A 50-year-old male had digital clubbing of both hands, coarse hypertrophic skin changes of face, progressive thickening and furrowing on the scalp(cutis verticis gyrata), persistent pain in the limbs and joints. Other cutaneous features include moderate blepharoptosis, pole-like lower legs and feet. Results: We performed surgical excision for hypertrophic skin change of scalp because of frequent eczematous skin change, severe itching sensation and cosmetic problem. Diagnosis is confirmed by bony proliferative periosteal reaction, pathologic findings, and characteristic clinical findings. Conclusion: Pachydermoperiostosis is manifested by finger clubbing, and hypertrophic skin changes causing coarse facial features with thickening and periosteal bone formation. We experienced a case of pachydermoperiostosis. Brief review of related literature is given.
Raynaud attempted to clarify the situation concerning symmetrical and spontaneous gangrene in a thesis published in 1952. The disease was defined as Raynaud`s phenomenon without associated and contributing conditions or disease, and described as two types, such as, `locale syncope and asphyxia` and `gangrene`. Predilection of Raynaud`s disease for female under 40 years of age wi-thout any vascular occlusive disease beginning in the early decades of life and typical color changes in the skin of the extremities incited by coldness are outstanding features in this disease. One typical case of Raynaud`s disease is presented with relating references. Patient was 24 year old female single patient, who noted pain, numbness, and cyanosis of the finger tips of both hands for 6 years previously in the winter season, and recently such symptoms were aggravated including her both feet for two years even in the summer after exposure to cold water. Physical and laboratory examination revealed nothing specific except slightly glistening tight face and hypertrophy of both finger tips with clammy coldness. FamiliaI and past history revealed nothing specific abnormal contributory factors. Biopsy of skin on the dorsum of right foot one year before this admission revealed no evidence of scleroderma. Treatment was aimed to relieve vasospasmodic reaction to coldness and was very successful with bilateral lumbar and thoracic sympathectomy. Patient is free of symptoms relating to the Raynaud’s phenomenon after sympathectomy for 6 months including winter season.
