Malignant Transformation of Fibrous Dysplasiainto Osteosarcoma - A Case Report - (골육종으로 악성 전환된 섬유성 골 이형성증 - 1예 보고 -)

  • Fibrous dysplasia is a developmental benign disorder in which normal bone is replaced by fibrous tissue. Malignant transformation of fibrous dysplasia is a rare complication and more commonly occur in the polyostotic form than monostotic form. We report a case in which osteosarcoma developed in area of fibrous dysplasia of proximal femur after treating with curettage, wedge osteotomy, internal fixation, and bone graft. A review of the literature is presented.

  • Fibrous dysplasia is a benign pathologic condition of bone in which fibrous tissue gradually expands and replaces normal bone into fibro-osseous lesion. It is a primary developmental abnormality of bone-forming mesenchyme in origin. This study shows clinical history, radiological and histopathological feature of fibrous dysplasia with the intention of establishing correct diagnosis, treatment plan and evaluation of prognosis. This paper reviews and summarizes the materials from 57 fibrous dysplasias submitted to the Department of Oral and Maxillofacial Surgery in College of Dentistry, Seoul National University. Conclusions obtained were as following : 1. Fibrous dysplasia developed mainly in teenagers and shows female predeliction. 2. Fibrous dysplasia developed much on the maxilla 3. Monostotic fibrous dysplasia was most popular form. 4. Main symptom of fibrous dysplasia was painless swelling. 5. Radiological feature of fibrous dysplasia was ground-glass appearance, 6. Histopathological feature of fibrous dysplasia was irregular immature bony trabeculae(woven bone). 7. Treatment of fibrous dysplasia was mainly conservative contouring surgery.

  • Fibrous dysplasia is a condition characterized by the replacement of the medullary component of predominantly long bone with fibrous tissues. The monostotic form of the fibrous dysplasia occurs more frequent than the polyostotic form. Only the twenty three previous cases of vertebral involvement in monostotic fibrous dysplasia have been reported. Authors experienced a case of monostotic fibrous dysplasia of the third lumbar vertebra, so we report this case. Report details the diagnosis and treatment of a 34 years old man with back pain from monostotic fibrous dysplasia of the third lumbar vertebra. We discuss the experience in the consideration of previous report to recommend the optimal management of this disease.

Malignant Localized Fibrous Mesothelioma - Report of one case - (악성 국한성 섬유성 중피종 - 1예 보고 -)

  • Mesothelioma is a primary tumor of the pleura. There are two forms of mesotheliomas; diffuse form and localized form. The majority of localized mesotheliomas are benign, but the tumor with pedicle recur at 20% after removal of it. Malignant localized fibrous mesothelioma is differential diagnosed pathologically and clinically with diffuse mesothelioma. Symptoms of localized mesothelioma are generally related to the size of the tumor. We presented one case of malignant localized fibrous mesothelioma. The patient was 32 years old female and chief complaints were dyspnea and dry cough. Sputum cytology and needle lung biopsy were not able to diagnose definitely the tumor. Explothoracotomy was carried out for definite tissue biopsy. At the time of operation, huge adult head size mass was in the right thoracic cavity. The surface of the mass was nodular, multilobular and had hard consistency. Pedicle was formed above first rib and its diameter was 4.5cm. There was no adhesion except to RUL. Frozen biopsy suggested mesothelioma. Extirpation of the mass, right upper lobectomy. and partial pleurectomy were carried out.

  • An Al film deposited on the Kovar alloy substrate was anodically-bonded to the borosilicate glass, and the bond interfaces was closely investigated by transmission electron microscopy. Al oxide was found to form a layer ~l0 nm thick at the bond interface, and fibrous structure of the same oxide was found to grow epitaxially in the glass from the oxide layer. The fibrous structure grew with the bonding time. The mechanism of the formation of this fibrous structure is proposed on the basis of the migration of Al ions under the electric field. Penetration of Al into glass beyond the interfacial Al oxide was not detected. The comparison of the amount of excess oxygen ions generated in the alkali depletion layer with that incorporated in the Al oxide suggests that the growth of the alkali-ion depletion layer is controlled by the consumption of excess oxygen to form the interfacial Al oxide.

  • Fibrous dysplasia is a benign disorder of bone consisting of intramedullary proliferation of fibrous tissue and irregularly distributed, poorly developed bone. The disease manifests itself in the monostotic form in which only one bone is involved and the polyostotic form in which multiple bones at different sites are affected. We reported a extensive case of polyostotic fibrous dysplasia with involvement of craniofacial bones, mandible, ribs and extremities. A 18-year-old man showed remarkable right facial swelling who had been treated on right femur 3 years ago with a bone graft for pathologic fracture and he recognized facial swelling 5 years ago. Extraoral radiograms and computed tomogram showed diffuse sclerosis with a ground glass appearance of the most cranial bones, facial bones. The right mandibular lesion showed very expansile lesion with mottled appearance. Bone scans showed mutifocal increased uptakes in craniofacial bones, right mandible, bilaterally in ribs, humerus, femur, tibia and characteristic varus deformity of right femur (shepherd's crook defomity). This case showed exceptionally bilateral, extensive nature of bone lesion and didn't show any features of skin pigmentation and endocrine disturbances.

  • Fibrous dysplasia is a benign pathologic condition of bone which medullary bone is replaced and disturbed by poorly organized, structually unsound fibro-osseous tissue. When facial bones are involved, considerable esthetic deformity may result. The term monostotic fibrous dysplasia has been applied when one bone is involved : when more than one bone is affected, the term polyostotic used. The polyostotic form may be accomplished by pigmented skin lesion (Jaffe type), or by pigmented skin lesions with endocrine disturbance (Albright syndrome). No general agreement exists on the cause of fibrous dysplasia. A few authors have suggested that fibrous dysplasia as a result of trauma. It occurs predominantly in infant, adolescent females and runs a variable clinical coures. When several bones are involved, it tends to be unilateral. Involvements of alveolar bone may produce displacement of teeth with malocclusion, or loss of teeth, or both. Now, we will present a case of fibrous dysplasia on the left facial region treated by conservative contouring surgery.

Computer modeling of elastoplastic stress state of fibrous composites with hole

  • The paper represents computer modeling of the deformed state of physically nonlinear transversally isotropic bodies with hole. In order to describe the anisotropy of the mechanical properties of transversally-isotropic materials a structurally phenomenological model has been used. This model allows representing the initial material in the form of the coupled isotropic materials: the basic material (binder) considered from the positions of continuum mechanics and the fiber material oriented along the anisotropy direction of the original material. It is assumed that the fibers perceive only the axial tensile-compression forces and are deformed together with the base material. To solve the problems of the theory of plasticity, simplified theories of small elastoplastic deformation have been used for a transversely-isotropic body, developed by B.E. Pobedrya. A simplified theory allows applying the theory of small elastoplastic deformations to solve specific applied problems, since in this case the fibrous medium is replaced by an equivalent transversely isotropic medium with effective mechanical parameters. The essence of simplification is that with simple stretching of composite in direction of the transversal isotropy axis and in direction perpendicular to it, plastic deformations do not arise. As a result, the intensity of stresses and deformations both along the principal axis of the transversal isotropy and along the perpendicular plane of isotropy is determined separately. The representation of the fibrous composite in the form of a homogeneous anisotropic material with effective mechanical parameters allows for a sufficiently accurate calculation of stresses and strains. The calculation is carried out under different loading conditions, keeping in mind that both sizes characterizing the fibrous material fiber thickness and the gap between the fibers-are several orders smaller than the radius of the hole. Based on the simplified theory and the finite element method, a computer model of nonlinear deformation of fibrous composites is constructed. For carrying out computational experiments, a specialized software package was developed. The effect of hole configuration on the distribution of deformation and stress fields in the vicinity of concentrators was investigated.

Fabrication of gelatin-amorphous CaP nano fibrous mat forusing as fast bone healing material

  • Using the favorable resorption behavior of amorphous Calcium phosphate (CaP) we fabricated a gelatin basednano fibrous mat by electrospinning for using as a fast healing patch for minorbone defects. Bone is predominantly formed by an inorganic phase of nano-crystalline HAp materials and nano fibrous protein material of collagen. The osteoblast cells, which are the bone formation cells and are key to the new bone formation, receive these materials to form new bone. Taking these considerations we make a new nano fibrous mat of amorphous CaP and gelatin, which is derived from collagen itself. A polymer carrier of poly caprolactone(PCL) was used in the system to stabilize the materials in biological condition. The electrospinning conditions were optimized for smooth mat without any droplet formation. The fabricated mat was characterized for its morphologyby SEM. Mechanical properties like tensile strength was evaluated. To investigate the bio-compatibility we performed the MTT assay and investigated its resorption behavior and apatite formation behavior by SBF immersion.

  • A bed configuration wherein sheets of modified fibrous polyethylene are potted within a Millipore Filter Cartridge matrix has been developed. Polyethylene fibers form sturdy beds but the native hydrophobicity and inertness of polyethylene have precluded their use in protein chromatography The polyethylene fibers used in this system were modified by plasma oxidation and further derivatization. The resulting fibers are hydrophilic, bind protein reversibly and serve as an anion-exchange stationary phase. Separation of Bovine Serum Albumin on this bed, as well as results of basic studies on capacity and reversibility of binding within a fibrous bed and experimental data handling system are shown.
