[Purpose] Sex hormones deficiency leads to dramatically bone loss in particular postmenopausal women. Royal jelly has anti-osteoporosis effect due to maintain bone volume in that condition. We hypothesized that royal jelly protein (RJP, a latent residue after extracting royal jelly) also prevents bone deficient in ovariectomized (OVX) female rats, the animal model of postmenopausal women. [Methods] Female Sprague-Dawley rats (n = 30, 6 weeks age old) were sham operated (Sham; sham operated group, n = 7), OVX control group (OC, n = 7), OVX with low RJP intake group (ORL, n = 8), and OVX with high RJP intake group (ORH, n = 8) during 8 weeks experimental periods. In the end point of this experiment, the bone samples (lumbar spine, tibia, and femur) were surgically removed under anesthesia. These bone samples were evaluated bone mineral density (BMD) and bone strength. [Results] BMD of lumbar spine in RJP intake groups (ORL, ORH) were higher than that in OC group (p < 0.05 and p < 0.01) in RJP intake volume dependent manner. BMD of tibial proximal metaphysis and diaphysis in RJP intake groups were also higher than these in OC group (p < 0.01 and p < 0.01 / p < 0.05 and p < 0.001). In addition, breaking force of femur in RJP intake groups were significantly increase compared with that in OC group (p < 0.001 respectively). [Conclusion] These findings indicate that RJP contribute to prevent sex hormone related bone abnormality.
This study was performed to evaluate the effect of sodium cholride supplementation on bone metabolism in female rats consuming a low calcium diet. Twenty five female rats were divided into three dietary groups (control Na : 0.1038%, 1% Na : 1.036%, 2% Na : 2.072%). All experimental diets contained 0.27% Ca and were fed to rats with deionized water for 7 weeks. Bone mineral density(BMD) and bone mineral content(BMC) of total body, spine and femur were measured using energy x-ray absorptiometry(DEXA) by small animal software. Then Ca efficiency was calculated from BMD and BMC. Serum Ca, P, Na and urine Ca, P, Na were determined. Urinary pyridinoline, serum ALP were measured to monitor bone resorption. Following 7 weeks, sodium cholride supplemented groups had higher urinary Ca excreteion, urinary pyridinoline, crosslinks value and serum ALP. There was no significant difference in case of serum Ca among all groups. Sodium chloride supplemnted groups had lower Ca effciency of total, spine and femur BMD and BMC than that of control group. In conclusion high salt intake not only increases urinary Ca excretion as urinary Na excretion does but also increase bone resorption and decrease Ca efficiency of each bone. It is been suggested that high salt intake may be harmful for bone maintenance. Therfore, the decrease of salt intake to the level of recommendation would be desirable.

Purpose: As well as patient survival, the restoration of postoperative function such as ambulation is important in limb salvage operations for treatment of malignant bone tumors involving the proximal femur. The authors analyzed clinical outcomes of limb salvage operations using tumor prostheses for metastatic or primary malignant bone tumors in the proximal femur. Materials and Methods: From February 2005 to January 2014, 20 cases (19 patients) with malignant bone tumor involving the proximal femur with pain or complicated pathologic fracture were treated with segmental resection and limb salvage operations with tumor prostheses. Mean age was 63.1 years (range 35-86). Fourteen patients were male and six ones were female. The mean follow-up period was 20 months (1-94 months). There were 15 cases of metastatic bone tumor, 4 cases of osteosarcoma, and 1 case of multiple myeloma. The primary tumors of the metastatic bone tumors included 4 lung cancers, 3 hepatocellular carcinomas, and 3 renal cell carcinomas. Other primary tumors were breast cancer, thyroid cancer, colon cancer, prostate cancer, and malignant spindle cell tumor, each in 1 case. Modular tumor prostheses were used in all cases; (Kotz's$^{(R)}$ Modular Tumor prosthesis (Howmedica, Rutherford, New Jersey) in 3 cases, MUTARS$^{(R)}$ proximal femur system (Implantcast, Munster, Germany) in 17 cases). Perioperative pain was assessed with Visual Analogue Scales (VAS). Postoperative functional outcome was assessed with Musculoskeletal Tumor Society (MSTS) grading system. Results: Out of 20 cases (19 patients), 11 cases (10 patients) survived at the last follow-up. Average postoperative survival of the 9 deceased patients was 10.1 months (1-38 months). VAS score improved from pre-operative average of 8.40 (5-10) to 1.35 (0-3) after operation. Average postoperative MSTS function score was 19.65 (65.50%, 7-28). The associated complications were 2 local recurrences, 3 hematomas, 3 infections, 2 scrotal swellings, and 1 dislocation. There was no case of periprosthetic fracture or loosening. Conclusion: Limb salvage operation with tumor prosthesis is an appropriate treatment for early pain reduction and functional restoration in malignant bone tumors in the proximal femur with pain an/or complicated pathologic fractures.
Purpose : The research was done to investigate the effectiveness of 2D bony morphometry and microstructure of micro-computed tomography (micro-CT) on the osteoporotic bony change. We performed the bone morphometric analysis of proximal femur in ovariectomized rabbits with BMD and micro-CT examination. Materials and Methods : Twenty-one female (Newzealand, about 16 weeks old, 2.9-3.4kg) rabbits were used. Three rabbits were sacrificed on the day when experiment began (Baseline). The remaining 18 rabbits were divided into two groups. One group was ovariectomized bilaterally (OVX) and the other animals were subjected to sham operation (Sham). Bone specimens were obtained from the right and left femur of sacrificed rabbits. At intervals of 1, 2, 3, 5, 6 months respectively, BMD tests were performed on the proximal femur by using PIXlmus 2 (GE Lunar Co. USA), 2-dimensional bone morphometric analysis by custome computer program and 2D/3D bone structure analysis by micro-CT (Skyscan 1072, Antwerpen, Belgium). Statistical analysis was carried out for the correlation between bone morphometry, micro-CT and BMD Result : BV/TV, Tb.Th, Tb.N of micro-CT parameters showed higher values in sham group than OVX group. N.Nd/Ar.RI, N.NdNd, N.NdTm, N. TmTm, PmB/Ar.RI, 3-D BoxSlope of 2D morphometric parameters showed higher values in Sham group than OVX group. The micro-CT parameters of Tb.Sp, Tb.N were statistically significant correlated with BMD respectively. Several 2D morphometric parameters were statistically significant correlated with BMD respectively. Conclusion : Several parameters of 2D bony morphometry and micro-CT showed effective aspects on the osteoporotic bony change.
This study explored the effects of dietary calcium levels and/or ovariectomy on bone formation, bone composition and calcium metabolism using female Sprague-Dawley weanling rats(mean body weight$\pm$SEM : 232.3$\pm$6.7g) as a model. Rats received high(1.5%) calcium diets for eight weeks during the growth period and were randomly assigned to ovariectomy and sham groups. The two groups were than each randomly divided into three sub-groups and fed 0.1%, 0.5% and 1.5% calcium diets for eight weeks after operation. The results indicate that body weight gain was higher in ovariectomy groups than sham groups regardless of dietary calcium levels and food intakes. Serum Ca concentration was decreased in low Ca groups after operation and serum P concentration increased in ovariectomy groups. Serum Ca concentration was decreased in low Ca groups after operation and serum P concentration increased in ovariectomy groups. Serum alkaline phosphatase activity was increased in ovariectomy groups and was not influenced by dietary calcium levels after operation. Urinary hydroxyproline decreased in high Ca intake groups regardless of whether rats had received an ovariectomy or not. The weight, length and breaking force of the femur were not significantly different in all groups. Ash, calcium, phosphate and magnesium contents in the femur and lumbar were not significantly different regardless of ovariectomy operation and dietary calcium levels. But high/normal calcium intake after ovariectomy and sham operation increased the weight and calcium content in bone. Therefore, high calcium intake influenced the formation of peak bone mass during the growth period and calcium levels and calcium levels and calcium levels continued to influence bone growth and composition after ovariectomy.

As the demand for bone mineral density testing increases in Korea, which is close to an aging society, it is necessary to evaluate the repeatability of equipment such as femur phantom other than l-spine for more accurate diagnosis. However, in clinical practice, it is often not possible to proceed such evaluation due to insufficient quality control conditions. Therefore, this study is to evaluate the usefulness of the femur phantom after fabricating the same using 3D printing technology. The femur phantom was output using GlowFill filament and FDM 3D printing type. Each phantom was repeatedly scaned 20 times to compare whether the existing l-spine phantom and the fabricated femur phantom were suitable as a phantom for quality control. Each time the seven researchers took three times, the location of the femur phantom was readjusted, and then scanned to confirm the error between the researchers. As a result of conducting repeatability evaluation using femur phantom, the coefficient of variation rate was 2%, which was within the minimum precision tolerance of 2.5%. The reproducibility between the researcher was also found to be suitable as the average coefficient of variation was 0.031 and the coefficient of variation rate was 3.1%, which was within the minimum precision error range of 5%. In conclusion, it is considered that the prospective attitude and usefulness of the femur phantom fabricated by 3D printing in clinical practice will be sufficient.

Schwannomas are benign nerve sheath tumors that are typically located in soft tissue. Occasionally, schwannomas involve osseous structures. These intraosseous schwannomas are generally benign neoplasms that account for less than 0.2% of primary bone tumors. Schwannomas are very rarely observed in long bones. We present a case of a schwannoma affecting the proximal femur with a coincident subchondral fracture of the femoral head. A 38-year-old-male presented with left hip pain without deteriorating locomotor function. Plain film radiographs displayed a lobulating contoured lesion within the intertrochanteric portion of the femur. The magnetic resonance imaging (MRI) scans showed a tumor occupying the intertrochanteric region. Diffuse bone marrow edema, especially in the subchondral and head portions of the femur that was possibly due to the subchondral insufficiency fracture was also noted. The lesion was surgically excised and bone grafting was performed. Histologically, there was diffuse infiltrative growth of the elongated, wavy, and tapered cells with collagen fibers, which are findings that are characteristic of intraosseous schwannoma. Although very rare, intraosseous schwannoma should be included in the differential diagnosis of radiographically benign-appearing, non-aggressive lesions arising in the femur. The concomitant subchondral fracture of the femoral head confounded the correct diagnosis of intraosseous schwannoma in this case.
Purpose: To report the clinical results of the vascularized fibular graft in the treatment of intractable infected nonunion of femur. Materials and Methods: We reviewed 3 patients who were performed vascularized fibular graft in treated for intractable infected nonunion of femur. They had received an average of 5.6 times($4{\sim}8\;times$) surgical treatment at different hospitals. 1 case was of a infected nonunion in a fracture treated with internal fixation, the fracture having occurred after resection of a malignant tumor and transplantation of pasteurized autologous bone. 2 cases occurred after internal fixation in closed fractures. Surgical treatment was performed an average of 4 times($3{\sim}5\;times$) at our hospital and in all of the cases debridement of necrotic tissue and sequestrectomy. And vascularized fibular graft was performed. In all cases unilateral external fixation devices were used, of these, 1 case was changed into internal fixation. The final conclusion was made by assessment of functional outcomes and complications according to the standards of Paley. Results: As a result, in all of the cases bone union was achieved, and in the last follow up the functional results were excellent in 2 cases and good in 1 case. There were not presented leg length discrepancy of more than 2 cm, and further loss of knee joint motion. After previous treatment, average 23.3 months($16{\sim}30\;months$) was taken to eliminate infection and achieve complete bone union via vascularized fibular graft in our hospital. Conclusion: In treatment of intractable infected nonunion of femur, fairly good results can be expected after firm fixation, through debridement and vascularized fibular graft.
This study examined the effects of excess intake of calcium (Ca) and iron (Fe) supplement on bone loss, nephrocalcinosis and renal function in osteoporotic model rats. Seven-week-old female rats were first fed a Ca-deficient diet for four weeks after ovariectomy operation, and then one of nine experimental diets for additional eight weeks, containing three levels of Ca, normal (0.5%) or high (1.5%) or excess (2.5%) and three levels of Fe, normal (35ppm) or high (210ppm) or excess (350ppm). The osteoporotic model rats showed a remarkable increase in body weight, serum alkaline phosphatase (ALP) and decrease in breaking force, Ca, P, Mg contents of femur. Serum Ca concentration was not significantly affected by dietary Ca and Fe levles. Liver Ca content increased in rats fed a high-and excess-Ca diet. Kidney Ca content and microscopic Ca deposition remarkably increased in osteoporotic model rats compared to control group, and showed a tendency to decrease in rats fed a excess-Ca diet. Breaking force of femur increased with increasing dietary Ca levels, but Ca, P contents of femur and serum ALP were not significantly affected by dietary Ca and Fe levels. Serum total protein decreased in rats fed a excess-Ca diet, BUN increased in rats fed a excess-Ca diet, while serum uric acid and creatinine were not significantly affected by dietary Ca levels. Urinary creatinine, GFR increased in rats fed a high-and excess-Ca, diet, and GFR was highest in rats fed a excess-Ca/excess-Fe diet. These results suggest that excess intake of Ca may increase breaking force of femur, but not increase mineral contents of femur, and decrease kidney function. Furthermore, excess intake of Fe and Ca concurrently may aggravate kidney function leading to potential health problems in ovariectomized osteoporotic model rats.
Topology optimization method has a great advantage and capability over a conventional shape optimization technique because it optimizes a topology as well as a shape and size of structure. The purpose of the present study, using topology optimization method with an objective function of minimum compliance as a mechanism of bone remodeling, is to examine which shape factors of femur is strongly related with the curvature of femoral shaft. As is expected, the optimized curvature increased definitely with neck angle among the shape factors and showed a similar trend with the measured curvature to neck angle. Therefore, the topology optimization method can be successfully applied in the analysis of bone remodeling phenomenon in the subsequent studies.
