An investigation has been performed to develop an analysis tool based on a nonlinear beam theory, which can be used to predict the long-term behavior of an artificial hip joint. The nonlinear behav ior of the femur arise from the coupled dependence of the bone density and the mechanical properties on each other. The beam theory together with its numerical algorithm is developed to take into account the nonlinear bone remodeling process of the femur that is long enough to be assumed as a beam. A piecewise linear curve for the bone remodeling rate is used in the bone remodeling theory and the surface area density of bone is modeled as the third order polynomial function of bone density. At each section of the beam, a constant curvature is assumed and the longitudinal strains are also assumed to vary linearly across the section. The Newton-Rhapson iteration method is used to solve the nonlinear equations for each cross section of the bone and a backward method is used to march along the time. The density and the remodeling signal ar, calculated along with time for the various time steps, and the developed beam theory has been verified by comparing with the results of finite element analysis of a remodeling bone with an artificial hip joint of titanium prosthesis subjected to uni-axial loads and pure bending moment. It is concluded that the developed beam theory can be used to predict the long-term behavior of the femur and thus to design the artificial hip prosthesis.
A recent study reported that a diet rich in isoflavones is beneficial for bone formation in growing rats. It therefore seemed desirable to find out whether the beneficial effect of isoflavones in ovariectomized rats could also be reproduced with same amount of isoflavones which used for growing rats. To study the effect of isoflavones, an equal amount of isoflavones which used for growing rats, on bone mineral density and bone mineral content in ovariectmized rats were performed. Forty female Sprague-Dawley rats (body weight $210{\pm}5g$) were divided into two groups, ovariectomy and sham groups, which were each randomly divided into two subgroups that were fed casein and casein supplemented with isoflavones diets for 9 weeks after operation. All rats were fed on experimental diet and deionized water ad libitum for 9 weeks. Bone mineral density (BMD) and bone mineral content (BMC) were measured using PIXImus (GE Lunar Co, Wisconsin) in spine and femur. Serum alkaline phosphatase activity (ALP) and osteocalcin and urinary DPD crosslinks value were measured as markers of bone formation and resorption. The results of this study indicate that body weight gain and food intake were higher in ovariectomy groups than in sham groups regardless of diets. Serum Ca concentration was lower in ovariectomy groups than in SHAM groups. Serum ALP, osteocalcin, and crosslink value were increased in ovariectomy groups. Spine BMD/weight, femur BMD/weight, and femur BMC/weight of ovariectomy groups were significantly lower than SHAM groups after 9 weeks. However, isoflavones supplemented group in ovariectomy groups, serum ALP and osteocalcin concentrations, spine BMD/weight and spine BMC, femur BMD/weight and femur BMC/weight were significantly increased after 9 weeks. In conclusion, the beneficial effect of isoflavones on bone in ovareiectomized rats was shown on 9 weeks after feeding with an equal amount of isoflavones supplementation which used for growing rats.
To evaluate the applicability of osteoporosis management by statistical analysis of the correlation between bone mineral density (BMD) changes after menopause by dividing the T-score of bone mineral density measured by dual energy X-ray absorptiometry do. Between January 1, 2016 and July 31, 2017, women who visited the medical center of W Medical Center were enrolled in this study. The postmenopausal period was divided into 5 groups, There were 18 patients within 5 years, 44 patients in 6~10 years, 134 patients in 11~15 years, 109 patients in 16~20 years and 21 patients in 21 years or older. And postmenopausal women. Bone mineral density (BMD) of the lumbar spine and femur was measured using a dual energy X-ray absorptiometry. The lowest value among lumbar spine 1, 2, 3 and 4 and the lowest value among the femoral neck, greater trochanter, total femur, and ward Values were measured. The statistical significance was analyzed by using bivariate correlation coefficient method and one - way ANOVA. In 326 patients who underwent BMD, the correlation between bone mineral density and postmenopausal BMD showed a negative correlation (-.159, p<.01) with BMD of femur and BMD of lumbar spine The correlation between the menopausal period and negative (-.208, p<.01) was shown. There was a significant difference (p<.012) between the postmenopausal femur bone density and the mean value of the lumbar spine BMD (p<.000). The relationship between bone mineral density (BMD) and postmenopausal women's postmenopausal status can be estimated by estimating the bone mineral density and using it as a basic data for osteoporosis management.

Purpose: Incidence of chronic osteomyelitis in femur is lower than that of tibia due to abundantsurrounding soft tissue like muscles and subcutaneous fat. However, if the femur is infected, surgical approach would be very difficult because of surrounding soft tissue and bony defects would be getting larger due to the late detection. Chronic osteomyelitis of femur is an intractable disease with frequent recurrence and remained bone instability in spite of multiple classical operations. Methods: From August 1998 to October 2005, we had 7 cases of fibular osteocutaneous free flap to reconstruct the femur. Those were followed-up for 23 months. All 7 cases were male. 4 cases were in midshaft and the others are distal part of femur. Results: The 7 cases that had not been healed in spite of average 9.1 times previous operations were reconstructed successfully without the recurrence of chronic osteomyelitis. Continuous rehabilitation therapy and brace were very helpful for the ambulation. It took 5.6 months for complete union of bone, and 9.8 months for the ambulation. Conclusion: After wide resection, reconstruction of the femur using fibular osteocutaneous free flap guaranteed bone stability and prevented recurrence of osteomyelitis through rich blood supplying fibula and muscle. Double barrel graft of fibula would be needed in case of the sufficient strength and thickness of femur. We report the successful results of reconstruction of femur with fibular osteocutaneous free flap for chronic osteomyelitis of femur.

Objective : This study was carried out to investigate the effects of the hormones and cytokines associated with bone metabolism in ovariectomized rats. Method : Twenty-four Female-Sprague-Dawley rats were divided into sham operated(normal) group, ovariectomized(control)group, ovariectomized and treated with extract Caraganae Sinicae Radix(treated) group. Each group was investigated the changes of body weight at 3,5,7weeks after treatment, and femur weight, femur/body weight, thickness of compact bone of body of femur, area of cancellous bone of distal epiphysis of femur, serum estrogen, serum calcitonin, serum parathormone, serum Tumor Necrosis $Factor-{\alpha}$, serum $lnterleukin-1{\beta}$ at 7weeks after treatment. The results were summerized as follows; 1. The treated group showed significant change in body weight compared with the control group at 5,7weeks after treatment. 2. The treated group revealed significant increases in femur/body weight compared with the control group. 3. The treated group showed a little thicker compact bone of body of femur than the control group. 4. The area of cancellous bone of distal epiphysis of femur in treated group was increased significantly compared with control group. 5. The level of serum estrogen showed no change compared with control group. 6. The level of serum calcitonin showed no change compared with control group. 7. The level of serum parathormone was decreased in treated group significantly compared with control group. 8. The level of serum $TNF-{\alpha}$ was decreased in treated group significantly compared with control group. 9. The level of serum $interleukin-1{\beta}$ showed no change compared with control group. The results indicate that Caraganae Sinicae Radix inhibits bones resorption in ovariectomized rats by it's lowering effects on serum parathormone and $TNF-{\alpha}$.
This study explored the effect of calcium levels and/or ovariectomy on bone metabolism using female Sprague-Dawley weanling rats as a model . Rats received a low (0.1%) calcium diet for 8 weeks. The rats were then divided into three subgroups that were fed 0.1% ,0.5% and 1.5% calcium diets for 8 weeks after operation. The results of this experiment indicate that body weight gin was higher in ovariectomy groups than in sham groups regardless of calcium level and food intake. Serum Ca and P concentrations were of normal level regardless of calcium level and ovariectomy. Estrogen concentration was low in the ovariectomized group. Serum alkaline phophatase activity and urinary hydroxyproline have been used as markers of bone formation and resorption. These values were increased in ovariectomized groups. The weight, length and breaking force of femur were not significantly different between the groups. Ash, Ca, P and total lipid contents in femur and lumbar were decreased in the groups fed low calcium . Mg content was decreased in the ovariectomy and total protein content was not affected by calcium level and ovariectomy. The effect groups of ovarectomy on calcium contents of bone was more prominent in lumbar than in femur. In conclusion, though low calcium intakes during growth period may retard the attainment of peak bone mass, calcium supplementation after this period increased bone growth and mineral contents, but not significant effect in three calcium levels. Furthermore, calcium intake was shown to have a greater influence on the mineral contents of femur than of lumbar, and removal of endogenous estrogen production by ovariectomy was shown to be more deleterious on the ash and calcium contents of the lumbar than of femur.
The purpose of the present study was to investigate the bone-conserving effects of Rubus coreanus-Cheonggukjang (RC-CGJ) supplemented with more intensified phytochemicals compared to general Cheonggukjang (CGJ) in growing rats. Eighteen rats were divided into 3 treatment groups (Control, CGJ, and RC-CGJ) and were given experimental diets for 9 weeks. All of the rats in this study were fed a AIN-93G-based diet. Both CGJ groups were fed with 33.1% CGJ and RC-CGJ powder, respectively. The results of this study indicate that weight gain, mean food intake, and food efficiency ratio were not significantly different by the experimental diets among all groups. Spine bone mineral density (BMD) and femur BMD were not significantly different by the experimental diets. Spine bone mineral content (BMC) was significantly higher in the RC-CGJ and CGJ groups than in the control group, regardless of CGJ type. The femur BMC of the CGJ supplemented group was significantly higher compared with the control group and the RC-CGJ group. Compared with the control group, spine BMD and femur BMD per weight were markedly increased in the RC-CGJ and CGJ group regardless of CGJ type. Also, spine BMC per weight was significantly higher in the RC-CGJ group than in the CGJ group. However, femur BMC per weight was significantly higher in the CGJ group than in the RC-CGJ group. It can be concluded that RC-CGJ and CGJ supplemented diets have more beneficial effects on spine and femur peak bone mass in growing rats.
Osteoporosis, a well known metabolic bone disease, occurs due to imbalance of bone formings and resorptions. In an attempt investigate if postmenopausal osteoporosis would have influence on the mandible, histomorphometric analysis was performed in the mandible and the femur of the albino rats. Osteoporosis was induced by bilateral oophorectomy. To determine the effect of calcitonin, calcitonin(Asahi Chemical Co., Japan) was injected in the oophorectomized rats. The results were as follows : 1. The width of cortical bone of the femur was decreased in the oophorectomized group compared to the normal control and calcitonin injected groups and the porosity of cortical bone of the femur was increased in the oophorectomized group compared to the normal control and calcitonin injected groups. 2. The osteoid tissue and resorption lacuna of the femur was increased in both oophorectormized and calcitonin injected groups than the normal control group. 3. The width of cortical bone of the mandible was decreased in the oophorectomized group compared to the normal control and calcitonin injected groups. 4. The porosity of cortical bone of the mandible was decreased in both oophorectomized and calcitonin injected groups than the normal control group. 5. The relative volume of trabecular bone of the mandible was decreased in the oophorectomized group compared to the normal control and calcitonin injected groups. 6. The osteoid tissue of the mandible was increased in the calcitonin injected group than the normal control and oophorectomized groups. 7. The resorption lacuna of the mandible was increased in both oophorectomized and calcitonin injected groups, particularly in the oophorectomized group.
Purpose : This study was carried out to investigated the effects of Saenghyeolbosu-Tang on the morphometric changes of femur, and on the hormones and cytokines associated with bone metabolism in ovariectomized rats. Methods : Twenty-four female Sprague-Dawley rats were divided into sham operated group(normal), ovariectomized group(control), and treated with extract of Saenghyeolbosu-Tang group(treated). Each group was evaluated the changes of body weight at 0, 3, 6, 8, weeks after ovariectomy. Morphometric analysis(femur weight, femur/body weight, femur ash weight, femur ash/body weight, cross sectional area of compact bone and cancellous bone of femur) and histopathological examination were performed at 8 weeks after ovariectomy. Estrogen and cytokines($TNF-{\alpha},\;IL-1{\beta}$, IL-6) assay were performed at 8 weeks after ovariectomy. Results : 1. The femur weight(g) of treated group(1.59${\pm}$0.08) was significantly increased(p<0.05)compared with the control group(1.50${\pm}$0.07) at 8 weeks. 2. The femur/body weight(g) of treated group(5.27${\pm}$0.36) was significantly increased(p<0.05) compared with the control group(4.78${\pm}$0.37) at 8 weeks. 3. In the cross sectional area of cancellous bone of femur(${mm^2}$) the treated group(1.60${\pm}$0.20)was significantly increased(p<0.01) compared with the control group(1.19${\pm}$0.19) at 8 weeks. 4. The serum estrogen level(pg/ml) of treated(83.67${\pm}$27.44) and control group(800.49${\pm}$22.27)showed no significantly different changes at 8 weeks. 5. The serum $TNF-{\alpha}$ level(pg/ml) of treated(33.71${\pm}$6.43) and control group(41.35${\pm}$8.47) showed no significantly different changes at 8 weeks. 6. The serum $IL-1{\beta}$ level(pg/ml) of treated group(53.78${\pm}$10.83) was significantly decreased(p<0.001) compared with the control group(79.80${\pm}$11.40) at 8 weeks. 7. The serum IL-6 level(pg/ml) of treated group(55.52${\pm}$9.02) was significantly decreased(p<0.01) compared with the control group(69.69${\pm}$7.95) at 8 weeks. Conclusion : This study shows that Saenghyeolbosu-Tang inhibits bone resolution in ovariectomized rats. These results may be related to the inhibitory effect of Saenghyeolbosu-Tang on the secretion of $IL-1{\beta}$ and IL-6 in the Pathogenesis of osteoporosis.
Purpose: This study was measured to the bone mineral density(BMD) and biochemical bone markers in young women in order to identify the relationship between bone mineral density and biochemical bone markers. Methods: Forty two healthy young women were enrolled. BMD were checked Dual Energy X-ray Absorptiometry and biochemical bone markers were checked ELSA-OSTEO(CIS bio international, France)analyzed kit, Pyrilinks-D(Metra Biosystems Inc., U.S.A)analyzed kit. Data were analyzed with frequencies, percentages, means, and Pearson correlation coefficients. Results: 1) Young women forearm(radius & ulnar) BMD was $0.55g/cm^2$, lumbar($1{\sim}4$) BMD was $0.92g/cm^2$, neck of femur BMD was $0.75g/cm^2$, trochanter of femur BMD was $0.61g/cm^2$, ward's triangle of femur BMD was $0.68g/cm^2$. In biochemical bone marker, Osteocalcin was 21.94ng/ml, Deoxypyridinoline was 11.94nmol/nmolCr. 2) There was no significant correlation between BMD and biochemical bone markers. Conclusion: Results not indicated association between bone mineral density and biochemical markers. As seen in the small sample, future research on BMD and biochemical markers need to studies to the large sample.
