Catastrophic Bronchial Spasm Due to a Severe Anaphylactic Reaction to Protamine

  • Fatal anaphylactic reactions to protamine sulfate during cardiac surgery are very rare. We report a case of catastrophic bronchial spasm due to an anaphylactic reaction to protamine. The patient was managed successfully using a bronchodilator, steroid treatment, and extracorporeal membrane oxygenation.

  • Fracture of tracheostomy tube with subsequent migration into the tracheobronchial tree is rare, but tracheobronchial foreign body in child carries the potentially fatal risk of respiratory obstruction, We report a case of a 5-year-old girl who had aspirated a fractured tracheostomy tube which was removed under rigid bronchoscope.

  • Acetabular and Pelvic ring fractures are major high-energy trauma injuries and are often combined with other injuries. In particular, cause of long duration of immobilization and combined injuries, venous thromboembolism is a common complication in trauma patients with pelvic or acetabular fractures. We report a case of a fatal pulmonary thromboembolism during the acetabulum fracture operation in a 62-year-old male patient.

  • Pulmonary edema and respiratory failure is uncommon but fatal complications associated with major operations of the lung, especially after pneumonectomy. The extracorporeal membrane oxygenation[ECMO] is quite often used in infants with severe respiratory failure and congenital heart disease which is well documented in the literature. In adults, the results of ECMO is comparatively poor to those found in neonates. We have experienced a case of ECMO applied on a 48 year old male who had respiratory failure after pneumonectomy, and the patient was successfully weaned from bypass. Unfortunately however, the patient expired on postoperative 15 day due to multiorgan failure.

  • Acute phlegmonous esophagitis (APE) is a rare and fatal disease. Phlegmonous infection involves the submucosal layer and muscularis propria but not the mucosal layer. Because surgery is not the first treatment option for this disease, an accurate diagnosis is crucial. Herein, we report three cases of APE with various clinical features. All patients were successfully treated with antibiotics and appropriate medical procedures.

  • Aortic dissection is a challenging disease and the causes of that are well-known. Blunt chest trauma is one of the causes of aortic dissection. In such cases, nearly all cases involves the isthmic portion of descending aorta, but ascending aorta is involved in about 10. We experienced a patient who had ascending aortic dissection due to automobile accident and who showed spontaneous rupture of the aorta during operation. In this case, after installation of aortic line via left femoral artery, ascending aorta ruptured and a large amount of blood gushed out, which was suckered by cardiotomy sucker. A little delay of cardiopulmonary bypass may cause the fatal outcome in such a case because the bleeding from aorta is too much to be controlled. Fortunately, we controlled the bleeding with cardiopulmonary bypass and got the good outcome of this patient by interpositioning the vascular graft. One should suspect the possibility of aortic dissection in blunt chest trauma, and prepare all the facilities against bleeding due to rupture.

  • Mediastinum is the space located between the right & left lung lobes in the center of the thorax, and contains many vital organs such as heart, aorta, trachea, esophagus, vagus nerve, lymphatics. So the infection of mediastinal space causes a serious and potentially fatal process. Like other infections, surgical drainage, aggressive antibiotic therapy and supportive care are recommended for optimal outcome. Airway management in the presence of the neck, glottic edema, elevation of the tongue is a formidible problem especially in the case from odontogenic origin. We have recently encountered such a case resulting in a number of local and systemic complications.

  • Extranodal NK/T-cell lymphoma (NTCL), nasal type is rare and highly fatal malignant neoplasm. Early diagnosis and establishing treatment plan are very difficult. Furthermore, NTCL in the mandible is an extremely rare condition. The clinical significance of presented case is the very rare location of NTCL. To the best of author's knowledge, this is the first reported case of NTCL of the mandible in the literature.
