Surgical Management of Aortic Insufficiency in Behcet`s Syndrome - An Experience of 8 Cases - (Behcet 씨 증후군에 의한 대동맥판 폐쇄부전의 수술치험 -3례 보고-)

  • In Behcet syndrome, cardiac involvements are rare and have been reported pericarditis, myocarditis, right heart endocardial fibrosis, right ventricle mural thrombus with pulmonary embolism, active endocarditis, granulomatous endocarditis, conduction disturbance, acute aortic insufficiency, mitral valve prolapse. Our three patients underwent AVR because of aortic insufficiency and ascending aorta enlargement combined with Behcet syndrome. Two patients had mitral regurgitation too. So one underwent MAP and the other underwent MVR concomitantly. One who underwent AVR have been well for 50 months. Another who underwent AVR+MAP and redo AVR due to aortic paravalvular leakage was died of congestive heart failure. The other who underwent AVR+MVR and repeated AVR three times because of aortic paravalvular leakage is in condition of aortic paravalvular leakage. Paravalvular leakage is considered to recur due to progressive dilatation and fragility of aortic root that is the result of pathologic change of Behcet syndrome in it. If Open heart surgery is needed in Behcet`s syndrome during inflammatory reaction is active, postoperative complications such as paravalvular leakage or suture line rupture may be prevented with pre- and postoperative anti-inflammatory management.

Clinical Experience of Carbomedics Prosthetic Heart Valve (Carbomedics 인조 심장판막의 임상적 경험)

  • The CarboMedics Medical valve has become our mechanical valvular prosthesis of choice because of favorable hemodynamic results that associated with marked clinical improvement and low incidence of thromboembolism after it,s uses. The data for this study was collected from August 1988 to July 1993,five years period. There were total of 57 patients[female 40,male 17 in this series with 4 isolated aortic valve,26 isolated mitral valve,11 double valve and a triple valve replacement. The mean follow up time was 32 months. Postoperatively,58% of cases were in New York Heart Association[NYHA functional class I,and mild and moderate symptoms[NYHA class II were present in 36% and there were very few patients remaining in higher functional classes. In postoperative echocardiographic study, showed marked improved cardiac function. The overall early mortality was 3.5% and the late mortality was one case after triple valve replacement due to sudden death. The causes of early death were attributed to early prosthetic valve endocarditis and heart failure.

Congenitally Corrected Transposition of the Great Arteries Surgical Experience, 4 cases (교정형 대혈관전위증의 외과적 치험 4)

  • The clinical, investigative, and surgical experiences were reviewed in four patients with congenitally corrected transposition of the great arteries who presented to the National Medical Center between August 1983 and August 1985. This condition is very rare congenital anomaly defined as the combination of atrioventricular discordance and transposition of the great arteries. Examples of primitive [single] ventricle inverted [that is, left sided in situs solitus] with outflow chamber were excluded in this paper. According to the sequential arrangement of the hearts there were two cases of [S,L,L] and two cases of [I,D,D]. The surgical approach should be focused on minimizing the risk of heart block and increasing the degree of relief of pulmonary outflow tract obstruction [POTO]. We experienced complete heart block in two cases of [S,L,L] and significant residual POTO in one case of [S,L,L] and one case of [I,D,D] postoperatively. There were one hospital mortality caused by complete heart block and residual POTO and two delayed mortalities caused by congestive heart failure and sepsis respectively.

  • In the conventional operation of ice-storage system based on operator's experience and judgement, the failure in forecast of cooling load occurs frequently due to operator's misjudgement and unskilled operation. This study presents the method of constructing self-organizing fuzzy models which forecast tomorrow temperature, humidity and cooling load periodically for economic and efficient operation of ice-storage system. To check the effectiveness and feasibility of the suggested algorithm, the actual example for forecasting temperature, humidity and cooling load of ice- storage system in KEPCO training institute, Sokcho, is examined. The computer simulation results show that the accuracy of temperature, humidity, cooling load forecast of the suggested algorithm is higher than that of the conventional methods.

  • In order to establish preventive maintenance standards through analysis & reliability prediction of about 60,000pcs of 20kindsof relays and contractors used for Seoul subway trains, several life prediction methodologies were applied. Firstly, Occurrence, Severity, Detection were defined and predicted by applying operation characteristic of EMU to the number of actions of relays & contactors which the manufacturers generally offer as the life cycle data. Secondly, failure distribution and average life of parts were analyzed through interpretation of field data based on a lot of experience which had built up in the field for a long time. Finally, using the 217PLUS standard as a reliability prediction program, comparative analysis of use reliability and inherent reliability was done through reliability prediction at the part level and system level.

Approach in the Surgical Management of Cardiac Myxoma - Clinical Experience and Long-term Result - (심장 점액종의 외과적 고찰임상 경험 및 장기 성적)

  • Between 1977 and 1987, 8 patients underwent excision of cardiac myxomas at the Hanyang University Hospital. All had a left atrial myxoma. There were 4 female and 4 male patients ranging from 15 to 62 years of age. Preoperative findings consist of symptoms and signs of congestive heart failure except one. Diagnosis was confirmed by echocardiography[8 cases] and angiography[2 cases], preoperatively. A biatrial operative approach was utilized in all but 2, who were small sized. Complete excision of the tumor with a cuff of normal tissue[1 was atrial wall and 7 were atrial septum] was performed. all heart chambers were carefully explored for presence of multicentric myxomas or tumor debris. There were no operative deaths or intraoperative embolization. Follow-up has been 1/3 to 10 years. There has been 1 late death, due to recurrence and 1 patient had reoperation for mitral regurgitation due to dilatation of the annulus by a huge tumor mass. Surgical excision of the myxoma can be performed with low morbidity, and it provides excellent and sustained symptomatic relief. The recurrence rate is low, but long-term follow-up and serial echocardiography are advisable.

Clinical Experience of Pericardial Effusion (심낭삼출 환자의 임상적 고찰)

  • We have experienced the 37 patients of pericardial effusion during last 8 years[1986-1993 . They were 17 male and 20 female patients, and their age range was 6months to 80 years.The causes of this pericardial effusion were 10 cases of tuberculosis, 7 cases of malignancy, 2 cases of acute pyogenic infection, 2 cases of postpericardiotomy pericardial effusion, 2 cases of trauma, 1 case of congenital heart disease, 1 case of SLE, and other 12 cases of unknown origin. All patients, except one, were managed by pericardiotomy with subxiphoid tube drainage. One case was managed by pericardiectomy. After operation, their symptoms and sign were dramaticaly improved. However, 3 patients were died postoperatively. Two of them were died of respiratory failure. One of them was died 28 months later with unrelated cause. They have already far advanced lung cancer. We conclude that the pericardiotomy with subxiphoid tube drainage was effective treatment in the patients with pericardial effusion.

An Exploratory Study of Consumer Return Experiences (소비자 반품경험에 관한 탐색적 연구)

  • This study attempted to understand post-purchase consumer return experiences which had not received much attention in consumer behavior research. As a first step to understand return experiences from consumers' viewpoint, the study took an exploratory approach and identified themes and patterns from subjective consumer experiences. Whenever possible, attribution theory was applied to interpret the identified patterns. A total of 99 cases were collected from an open-ended questionnaire and analyzed based on a qualitative method. The reasons of product return was classified into five groups: changing mind; product defects; unsuitable products; products not expected; and for a better deal. External attribution was observed for return decisions caused by consumers' changed mind as well as by product defects. Dissatisfaction with defected products seemed to be intensified or reduced by service failure or success in the post-purchase service encounter. Consumer expectation and situations seemed to affect this process. Overall, the study supported attribution theory in the post-purchase return experience, and insights for research questions were proposed.

  • Although the prevalence of rotator cuff tears is dependent on the size, 11% to 94% of patients experience retear or healing failure after rotator cuff repair. Treatment of patients with failed rotator cuff repair ranges widely, from conservative treatment to arthroplasty. This review article attempts to summarize the most recent and relevant surgical options for failed rotator cuff repair patients, and the outcomes of each treatment, except arthroplasty.

The Durability of Ships Considering Fatigue Cracking

  • The larger trends related to cracking in ocean going vessels (primarily tankers and bulk carriers) are reviewed on the basis of available data. The typical interrelated causes of such cracking are: high local stresses, extensive use of higher strength steels, inadequate treatment of dynamic loads, adverse operational factors (harsh weather, improper vessel handling), and controllable structural degradation (corrosion, wear, stevedore damage). Three consequences of cracking are then discussed: structural failure, pollution, and increased maintenance. The first two, while rare, are potentially of high consequence including loss of life. The types of solutions that can be employed to improve the durability of ships in the face of fatigue cracking are then presented. For existing vessels, these solutions range from repairs based on structural analysis or service experience, control of corrosion, and enhanced surveys. For new vessels, the use of advanced design procedures that specifically address dynamic loads and fatigue cracking is necessary. As the preferred solution to the problem of cracking in ships, this paper advocates prevention by explicit design by first principles.
