Effect of Horizontal Load on the Performance of Track Roller (수평방향 하중이 트랙롤러의 특성에 미치는 영향)

  • Track rollers are the key parts that support the weight applied to the caterpillar track of construction machinery. The operating conditions of construction machinery are harsh; hence, the track rollers exposed to these conditions can experience failures such as oil leakage and breakdown of the machine, because of impacts during driving. The failure of track-rollers has a major effect on the operation of the construction machinery. Therefore, each manufacturer tries to evaluate the performance of their products by conducting a self-test. However, only the vertical loads applied to the track rollers are considered in most of the self-tests. This method has the problem of ignoring the horizontal loads that are applied to the track rollers while rotary driving, and hence it does not create the practical operating condition of the construction machinery. In this study, we have determined the experimental conditions that effectively consider the magnitudes of the vertical and horizontal loads. The results of simulations and experiments conducted to assess the influence of horizontal loads on the failure of track rollers are presented in this paper.

  • Purpose: Infantile hepatic hemangioendothelioma (IHHE) is the most common type of hepatic vascular tumor in infancy. We conducted this study to review our clinical experience of patients with IHHE and to suggest management strategies. Methods: We retrospectively analyzed the medical records of 23 IHHE patients (10 males, 13 females) treated at the Asan Medical Center between 1996 and 2009. Results: Median age at diagnosis was 38 days (range, 1 to 381 days). Seven patients (30%) were diagnosed with IHHE based on sonographically detected fetal liver masses, 5 (22%) were diagnosed incidentally in the absence of symptoms, 5 (22%) had congestive heart failure, 3 (13%) had skin hemangiomas, 2 (9%) had abnormal liver function tests, and 1 (4%) had hepatomegaly. All diagnoses were based on imaging results, and were confirmed in three patients by histopathology analysis. Six patients were observed without receiving any treatment, whereas 12 received corticosteroids and/or interferonalpha. One patient with congestive heart failure and a resectable unilobar tumor underwent surgical resection. Three patients with congestive heart failure and unresectable tumors were managed by hepatic artery embolization with/without medical treatment. At a median follow-up of 29 months (range, 1 to 156 months), 21 (91%) patients showed complete tumor disappearance or >50% decrease in tumor size. One patient died due to tumor-related causes. Conclusion: IHHE generally has a benign clinical course with low morbidity and mortality rates. Clinical course and treatment outcome did not differ significantly between medically treated and non-treated groups. Surgically unresectable patients with significant symptoms may be treated medically or with hepatic artery embolization.

Clinical Experience of Exposed Pacemakers (노출된 영구 심박조율기의 치험례)

  • In the cases of severe arrythmia and conduction failure, a permanent implanted pacemaker is considered an essential treatment modality with less complication rate, broad indications and low morbidity. However, some pacemakers needs to be removed or replaced due to infection, in need of a lead upgrade, elective replacement, conduction failure or insulation failure. The most common indication for pacemaker extraction is infection. Conservative treatments such as administration of intravenous antibiotics and limitation of debridement are not effective and the removal of the entire pacing system is considered to be the best approach to pacemaker pocket infection. Although a locking stylet, a laser sheath and other newer methods of transvenous lead extraction have been proven to be effective, all leads cannot be removed. Moreover, major complications such as, cardiac tamponade and respiratory arrest during leads extraction procedure should not be ignored. We experienced two cases of exposed pacemakers in the subclavicular region. After removing the pacemaker body, exposed proximal lead was pulled out and cut off. The end of remnant external insulation tube was tied to prevent infection propagation between external insulation tube and inner metalic coil. Wounds were covered by local flap coverage. No other problems were detected during the one-year follow-up. Since there are few reports on lead-preserving method of treating limited infection of exposed pacemakers, we would like to present our new method for treating exposed pacemakers.

  • Over the past twenty years, the maintenance system has been developed and its importance has been increased. For the effective maintenance of the urban transit, we have developed the maintenance system based on the concept of RCM(Reliability Centered Maintenance). RCM analysis is a systematic approach to developing a cost-effective maintenance strategy based on the various components's reliability of the system in question. It is performed according to process that includes the following steps; definition of function and functional failures of the systems, construction of RB D(Reliability Block Diagram), performance of FMEA(Failure Modes & Effects Analysis) and calculation of the reliability index. The final process of RCM is to determine appropriate failure maintenance strategies. This paper aims to define the procedure of maintenace based on the concept of RCM for urban transit. The key for a successful maintenance system is an automated scheduling to the maximum extent possible and timely executions. The developed system issues maintenance plan and repair request based on analyzed data and maintenance experience.

Applicability of AE for the Prediction of Rock Slope Failure (암반비탈면 붕괴시 예측가능한 AE의 적용성에 관한 연구)

  • In general, many instrumentations of slope rely on theory or experience because on-site accessibility and long term instrumentation are difficult to conduct the instrumentation of slopes. Also the prediction of disaster is very difficult. Therefore experimental research was conducted about an effective method to predict collapse of slope and on-site applicability in this study. The collapse of slope was able to be predicted by applying AE sensor which we call WEAD to the failure criteria. The parameters of AE generated during the collapse of slope were secured through bending shear test. Test construction was applied to the slope with a history and a possibility of collapse. As a result, it is shown that AE parameters do not exceed the failure criterion and is found to be stable slopes. As the real symptoms of collapse did not appear, AE was found to have excellent applicability.

Short-Term Outcome of Infliximab Therapy in Pediatric Crohn's Disease: A Single-Center Experience

  • Purpose: Studies on the efficacy of infliximab (IFX) in a large population of pediatric patients with Crohn's disease (CD) are limited, and prognostic factors are not well-known. The aim of this study was to evaluate outcomes of IFX in pediatric patients with CD and to identify factors associated with poor prognosis. Methods: We retrospectively analyzed medical data of 594 pediatric patients with CD between 1987 and 2013 in a tertiary center. Of these, 156 children treated with IFX were enrolled and were followed up for at least a year with intact data. Outcomes of induction and maintenance, classified as failure or clinical response, were evaluated on the tenth and 54th week of IFX therapy. Results: We treated 156 pediatric patients with CD with IFX, and the median duration of IFX therapy was 47 months. For IFX induction therapy, 134 (85.9%) patients experienced clinical response on the 10th week. Among the 134 patients who showed response to induction, 111 (82.8%) patients maintained the clinical response on the 54th week. In multivariate analysis, low hematocrit (p=0.046) at the time of IFX initiation was associated with the failure of IFX induction. For IFX maintenance therapy, longer duration from the initial diagnosis to IFX therapy (p=0.017) was associated with maintenance failure on the 54th week. Conclusion: We have shown the acceptable outcomes of IFX in a large cohort of pediatric CD patients in Korea. Hematocrit and early introduction of IFX may be prognostic factors for the outcomes of IFX.

Effects of Regret in Consumer Failure Situation: Moderation of Self-Compassion (소비자 실패 상황에서 후회의 효과: 자기자비의 조절 효과)

  • In this study, we examined whether the intention to choose varies according to the experience of the regret of consumers when experiencing a failure in the status quo or non-status quo, and how self-compassion plays a moderating role in this relationship. A experiment was conducted to achieve the purpose of the study. First, this study also confirmed that regret was a mediator between dissatisfaction with choice outcomes and behavioral intentions. Second, like previous studies, this study confirmed the effect of status quo in general. However, it can be seen that the effect depend on self-compassion. Or, the regrets of those with high self-compassion and their future behavioral intentions did not change significantly whether it was wrong due to maintaining on or changing of the status quo. On the other hand, those with low self-compassion showed the bigger differences, so they were typically showing the status quo effect.

Experimental Study of Hybrid Super Coating (HSC) and Cast Reinforcement for Masonry Wall (하이브리드 슈퍼코팅(HSC)과 유리섬유를 통한 조적조 내진보강 연구)

  • Many Korean domestic masonry structures constructed since 1970 have been found to be vulnerable to earthquakes because they lack efficient lateral force resistance. Many studies have shown that the brick and mortar suddenly experience brittle fracture and out-of-plane collapse when they reach the inelastic range. This study evaluated the seismic retrofitting of non-reinforced masonry with Hybrid Super Coating (HSC) and Cast, manufactured using glass fiber. Four types of specimen original specimen (BR-OR), one layered HSC (BR-HS-O), two-layered HSC (BR-HS-B), one layered HSC, and Cast (BR-CT-HS-O) were constructed and analyzed using compression, flexural tensile, diagonal compression, and triplet tests. The specimen responses were presented and discussed in load-displacement curves, maximum strength, and crack propagation. The compressive strength of the retrofit specimens slightly increased, while the flexural tensile strength of the retrofit specimens increased significantly. In addition, the HSC and Cast also produced a considerable increase in the ductile response of specimens before failure. Diagonal compression test results showed that HSC delayed brittle cracks between the mortar and bricks and resulted in larger displacement before failure than the original brick. The triplet test results confirmed that the bonding strength of the retrofit specimens also increased. The application of HSC and Cast was found to restrain the occurrence of brittle failure effectively and delayed the collapse of masonry wall structures.

Can Transradial Mechanical Thrombectomy Be an Alternative in Case of Impossible Transfemoral Approach for Mechanical Thrombectomy? A Single Center's Experience

  • Objective : Until recently, the transfemoral approach (TFA) was used as the primary method of arterial approach in acute ischemic stroke (AIS). However, TFA resulted in longer reperfusion times and worse outcomes in the mechanical thrombectomy (MT) of patients with complex aortic arches and significant carotid tortuosity. We found that the transradial approach (TRA) is a more favorable alternative approach for MT in such cases. Methods : We performed a retrospective review of our institutional database to identify 202 patients who underwent MT for AIS between February 2015 and December 2019. Patient characteristics, cause of TFA failure, procedure time, intra-procedural complications, and outcomes were recorded. Results : Eleven (5.4%) of 202 patients, who underwent MT for AIS, crossed over to TRA for recanalization, and eight (72%) of 11 achieved successful recanalization (≥modified Treatment in Cerebral Infarction 2b). The mean age (mean±standard deviation [median]) was 82.3±6.6 (76) years, and five of the 11 patients were male. The last seen normal to puncture time was 467.9±264.72 (264) minutes; baseline National Institutes of Health Stroke Scale score was 28.9±14.5 (16). Six (55%) of the 11 patients had right vertebrobasilar occlusions, and the remaining five (45%) had anterior circulation occlusive disease. The time from groin puncture to final recanalization time (overall procedural time) was 78.0±20.1 (62) minutes. The mean crossover time from TFA to TRA was 45.2±10.5 (41) minutes. The mean time from radial puncture to final recanalization was 33.8±10.5 (28) minutes. Distal thrombus migration events in previously unaffected territories occurred in 3/8 patients (37%). At 90 days, three patients (28%) had a favorable clinical outcome. Conclusion : Although rare, failure of TFA has been known to occur during MT for AIS. Our results demonstrate that TRA may be an alternative option for AIS intervention for select patients with subsequent timely revascularization. However, the incidence of distal thrombus migration was high, and the first puncture to reperfusion time was prolonged because of the time taken for the crossover to TRA after failure of TFA. This study provides some evidence that the TRA may be a viable alternative option to the TFA for MT of AIS.

Preliminary Experience of Neuroform Atlas Stenting as a Rescue Treatment after Failure of Mechanical Thrombectomy Caused by Residual Intracranial Atherosclerotic Stenosis

  • Objective : The low-profile Neuroform Atlas stent can be deployed directly without an exchange maneuver by navigating into the Gateway balloon. This retrospective study assessed the safety and efficacy of Neuroform Atlas stenting as a rescue treatment after failure of mechanical thrombetomy (MT) for large artery occlusion. Methods : Between June 2018 and December 2019, a total of 31 patients underwent Neuroform Atlas stenting with prior Gateway balloon angioplasty after failure of conventional MT caused by residual intracranial atherosclerotic stenosis (ICAS). Primary outcomes were successful recanalization and patency of the vessel 24 hours after intervention. Secondary outcomes were vessel patency after 14 days and 3-month modified Rankin Scale. Peri-procedural complications, intracerebral hemorrhage (ICH), and 3-month mortality were reviewed. Results : With a 100% of successful recanalization, median value of stenosis was reduced from 79.0% to 23.5%. Twenty-eight patients (90.3%) showed tolerable vessel patency after 14 days. New infarctions occurred in three patients (9.7%) over a period of 14 days; two patient (6.5%) underwent stent occlusion at 24 hours, and the other patient (3.2%) with delayed stent occlusion had a non-symptomatic dot infarct. There were no peri-procedural complications. Two patients (6.5%) developed an ICH immediately after the procedure with one of them is symptomatic. Conclusion : Neuroform Atlas stenting seems to be an effective and safe rescue treatment modality for failed MT with residual ICAS, by its high successful recanalization rate with tolerable patency, and low peri-procedural complication rate. Further multicenter and randomized controlled trials are needed to confirm our findings.
