Failure Analysis of Carburized Gears by Microstructural Observation (침탄처리된 기어의 미세 조직학적 손상 원인분석)

  • The gear was made of SNC815 case-carburized, quench hardened and tempered steel. The gears were failed far earlier than the expected service life used in the industrial site. Causes of the failed gear were analyzed by microstructure observation. By the SEM and micro structure examinations, the damaged surfaces had been weared and failed by fatigue. Through microscope observation on the damaged surface, it was found that the cause of failure was determined by external overloading and the initial stage of the damage was closely related to complex contact fatigue failure. The overload and contact fatigue contributed to the early failure cause.

  • Many cases of information systems (IS) failure have still continued to be reported ever since computer-based information systems were introduced to process business transactions in the early 1950s. Because an enormous amount of budgets is currently invested on information technology in many organizations, failures and problems of information systems may serve as key culprits to serious business problems which will face the organizations. Thus, there have been a number of studies on IS failures which aimed to identify causes and reasons for such failures and reveal their inherent nature, Some studies developed conceptual frameworks to classify categories of diverse IS failure phenomena. However, little research performed an empirical study to investigate the underlying structure of IS failures perceived by IS professionals by measuring their perception. In this regard, the current study collected systems developers perceptual data towards IS failure phenomena to identify what constitute IS failure. The data was analyzed using a multidimensional scaling program and ten categories of problems were identified to constitute the IS failure structure, It was found that most categories were related to problems with users, hardware, and systems quality.

  • Recently, the load increasement and new regenerative systems of electrified railway system make it a difficult to distinguish between the load current and fault current. The failure of traction system perhaps causes over-current to flow. The high current can collapse other railway systems. If failures of the traction system takes place, the failures are detected and protected lest it should provoke high current flow. The over current from such a traction system failure permit to charge high tension voltage and produces high temperature arc, voltage instability, current cutting, and break down railway systems. The traction system failures detect and the system has to immediately cut off from over-current flow. To isolate the failure, the system can distinguish failure current from current flows. It forces us to adapt such as a new intelligent protection system. The protective system in traction system play a role of detecting and isolating failure points. In this paper, we proposed intelligent algorithm for discriminating normal and abnormal situation instead of the system being operated abnormally.

  • The paper provides the characteristic analyses of porcelain suspension insulators. The comparisons of characteristics were made as a function of failed and unfailed insulators which had ben used in the fields. This paper also describes the failure modes occurred in the fields. The aging of cement corrosion of metal breaking of shed puncture in the head and separation of glaze are different failure modes for porcelain suspension insulators. Among these failure modes the puncture breaking and glaze problems were major concerns in this research. The cracks porosities of porcelain and micro-crystals on the graze were the causes of these failures. These results were confirmed by conducting various failure analyses.

  • In the calendar and the advertising catalog, the surface Is usually coated by coating polypropylene film. The delamination failure of coating film depends on surface roughness and quality of the substrate paper. In this paper, the mechanisms of delamination failure between the coating film and the paper is investigated by using the root cause analysis as one of techniques of reliability evaluation. The papers used in failure analysis are three kind products made by two domestic and one foreign companies. It found that the main causes of delamination failure between the coating film and the paper were the creation of microvoids caused by shape of filler and their growth caused by contraction of paper.

Clinical Evaluation of Mitral Valve Replacement (승모판 치환술의 임상적 고찰)

  • From August, 1986 to December, 1989, mitral valve replacement was performed in 93 patients. Of the valve implanted, 42 were Duromedics, 35 St. Jude Medical, 15 Carpenter-Edwards and 1 Ionescu-Shiley. The hospital mortality rate was 3.2%[3 patients] and the late mortality rate was 4.3% [4 patients]. The causes of hospital death were LV rupture in 1, renal failure in 1 and hypoxic brain damage in l. The causes of late death were congestive heart failure in 1 and sudden death in 3. Follow-up was done on 78 surviving patients; mean follow-up period was 29.22$\pm$9.09 months. The actual survival rate was 91.8% at 4 years. We concluded, therefore, that good clinical results could be achieved with mitral valve replacement in short-term follow-up, and long-term follow-up is also necessary.

  • Few recent reports have indicated that Mycobacterium massiliense causes various infections including respiratory infection. However, there is scarce information on the clinical significance, natural history of the infection, and therapeutic strategy. This report describes a case of an immunocompetent old man infected by M. massiliense that causes acute respiratory failure. In light of the general courses of non-tuberculous mycobacterium infections, rapid progression and fatality are very rare and odd. In addition, we discuss the biological and pathological properties of M. massiliense with the review of cases reported previously including our fatal one.

  • At present, company has to produce a product that consumer like with a competitive price, a good quality, and a fitting time to supply. Process control and qualify control are very important to supply with a product uniformly and inexpensively In this paper, the characteristic of product quality is monitored by control chart during the machining process and construction of quality control cycle is considered to divide into two types in this case that different assignable causes lead to shifts having different magnitudes. Then we are intended to find a process shift magnitude which has economical quality cost policy and are considered to quality cost functions to find a process shift magnitude. Those costs are categorized into the well-known categories of prevention, appraisal, and internal failure and external failure. This paper ends with numerical examples that demonstrate the usefulness of the model.

  • We consider a system whose inherent life follows an Erlang distribution, which is subject to two heterogeneous random shocks. Minor shocks arrive according to a renewal process and each causes the system to fail independently with a certain probability. A major shock whose interarrival times follow an Erlang distribution causes the system to fail with probability one. The Laplace transform of the distribution of the time to system failure is derived in a functional form of the Laplace transform of the interarrival time distribution of minor shocks. An algorithm is given for the computation of the moments of the time to system failure.

  • Electronic control system of the engine is composed of various sensors and actuators, This paper is concerned with fault analysis for the stable operation of it. We suggest the technology that can systematically and reliably analyze fault causes of sensors and actuators by using the fault generating program. In results, we can acquire the systematic road map of occurring faults as well as the valuable information related to the operations of sensors and actuators. These results should be very useful to get the classification of fault causes, develop an electronic control system of engine, and review control strategies of it.
