  • This study examined and identified the relationship between the level of hope and self-care agency in hemodialysis patients. The purpose of the study was to reveal that hope is an essential factor to enhance self-care agency and to prepare the basic materials to help nurse the patients. The subjects for this study consisted of 108 persons who undergoing hemodialysis from 2 hospital, 2 hemodialysis center in Pusan, Korea. The degree of hope was measured by the questionnaire that has been developed by Miller. The self-care agency was measured by the questionnaire that has been developed by Kearney B.Y & Fleisher B.J. The collected data were analyzed by descriptive statistics and t-test. one-way ANOVA and Pearson's Correlation Coefficient with SPSS WIN program. Results were obtained as follows: 1) The mean scores of hope were $131.77{\pm}25.77$(range 67-195). The rate of hope classified said that the lowest hope was 2.8% and low hope was 27.8%. 2) The mean scores of self-care agency were $143.78{\pm}23.63$(range 74-187). The degree of self-care agency classified said that the rate of self-care agency deficiency was 0.9% and low self-care agency was 12.0%. 3) The hope was significantly related to self-care agency. That is, the higher the hope score was, the higher the level of self-care agency score(r=0.668,p=0.001) was. In this result, there were hemodialysis patients who have very low hope score and self-care agency score. In this cases self-care agency can be increased by encouraging the patients to be more hopeful. Conclusively nurses should access the degree of hope and self-care agency of hemodialysis patients individually and apply the hope therapy in order that the patient have higher hope than before.

  • Objective : Obesity has been considered to be associated with numerous physical, mental and psychological diseases. Depression, which is a major psychological factor affecting occurrence and treatment of obesity, can be a cause of obesity as well as can be triggered by obesity. This study aimed to find out the need of positive consideration to the existence of depressive mood in the management of obesity. Methods : A total of 101 subjects were enrolled among those who were 60 years old and over at one local clinic. Obese group (n=49) was defined as BMI${\geqq}$25 and normal weight group (n=52) as 20$5.2{\pm}2.5$) was significantly higher than that of the normal weight group ($3.8{\pm}1.7$)(p<0.05). In male subjects, there was no significant difference in the mean GDSSF-K score between both groups, but in female subjects, the mean GDSSF-K score of the obese group, $6.4{\pm}2.6$ was significantly higher than that of the normal weight group ($4.0{\pm}1.8$)(p<0.05). Conclusion : The elderly obese group showed higher depressive scale score than elderly normal weight group, especially in female elderly. Since the elderly obese female group had a depressive predisposition, comprehensive management including mental and psychological approach is required in obesity control programs.

  • The purpose of this study was to investigate the factors affecting falls in 107 elderly living in the city aged 65 or older by evaluating general characteristics, chronic disease status, medical variables related to falls, balance-related confidence, physical ability and depression. Also, the correlations between the significant differences in variables were identified, and the prediction power was determined by deriving the variables with high influence to induce the fall. In the faller group, urinary incontinence, foot pain, lower extremity weakness, number of chronic disease and medication use were significantly higher than those of the nonfaller group. Also, statistically significant differences were evaluated in ABC (Activities-specific Balance Confidence) score, BBS (Berg Balance Scale) score, SGDS (Short Geriatric Depression Scale), FRT (Functional Reach Test) value. The main correlated factor for fall was ABC score, the lower the ABC score, fall risk is increased which is a significant negative impact. When the evaluation is performed by combining those scales, the hit ratio to classify whether faller or nonfaller is increased to 70.01% which is quite higher value.

  • Purpose: This study was conducted to investigate quality of life according to social support of elderly in the rural area. Method: The subjects of this study were 199 people aged over 60 who had been living in three rural areas. Date was collected through questionnaires from July 10th, to August 10th, 2003. Result: The most socially supportive people they answered were friends(80.9%), followed by children(74.9%), neighbors(71.9%), siblings(55.8%), spouse(53.3%), in descending order. Mean social support score for spouse was 13.36, for children 13.27, for friends 11.40, for neighbors 10.21, for siblings 10.20. A comparison of the average grade points per items according to the offerers of social support revealed spouse support(13.36 out of 18), children support(13.27), friends support(11.40), neighbor support(10.21), siblings support(10.20). The average of the quality of life score was 132.26 out of 220. A comparison of the average grade points per items within sub-areas of quality of life revealed the highest score of neighbor relationships(4.29 out of 5.00) and the lowest score of economic conditions (2.61) Quality of life scores correlated positively with social support scores(r=.734, p<.001). Variables significantly influencing quality of life were spouse support(36.1%), neighbor support (5.1%), age(2.2%), religion(1.7%). These variables explained 45.1% of the variance in quality of life. Conclusion: Social support for elderly people in rural areas identified this as a greatly effective factor for their quality of life. Therefore, it is necessary to develop health promotion programs connected with social support in order to enhance the quality of elderly people in rural areas.

  • Purpose: This study aimed to identify 'objective' differential factors for normal frequent loose stool (NFLS) and diarrheal illness with dehydration and nutritional deficiency (DIDN) among infants with chronic frequent loose stool (CFLS). Methods: Data were analyzed from infants under 2 years of age with CFLS who had been transferred from general pediatricians. These 46 patients were divided into 2 groups (NFLS versus DIDN). Nocturnal stool was defined as evacuation between 10 pm and 6 am. Maximal stool amount/day (measured using the mother's hand) was specified as the highest score during the period of CFLS obtained by adding up each evacuation's score (range, 0-2 points). Results: There were 36 cases of NFLS and 10 of DIDN. A failure to gain weight ($P$=0.0001), fever ($P$=0.0079), colic/abdominal pain ($P$=0.0014), gross blood in stool (except allergic proctocolitis) ($P$=0.0113), nocturnal stool ($P$=0.0001), and the score of stool amount ($P$=0.0001) were found to significantly differentiate the groups. A failure to gain weight was observed in 39% of even NFLS. The frequency, mucus content, and microbiological findings of stools, as well as diaper dermatitis were not found to significantly differentiate the groups. Conclusion: NFLS was more common than DIDN in infants with CFLS. The most 'objective' differential factors were nocturnal stool and the score of stool amount (${\geq}7$ points/day).
