Transient facial paralysis after myringotomy and ventilation tube insertion under sedation with sevoflurane inhalation and four-quadrant blocks with lidocaine: a case report

  • Myringotomy and ventilation tube insertion are widely performed in pediatric patients with chronic otitis media. This procedure is performed under general anesthesia or sedation with local anesthesia infiltration in pediatric patients. In this case report, we report a case of transient facial paralysis in a pediatric patient who underwent myringotomy and ventilation tube insertion using sevoflurane inhalation and four-quadrant blocks with lidocaine.

Effect of Acupuncture and High Frequency Therapy Applied to the Region Branching to the External Carotid Artery on Reduction of Facial Edema in Patients with Sequelae of Peripheral Facial Palsy: A Case Report (말초성 안면마비 후유증 환자에서 침 치료와 바깥목동맥으로의 분지 영역에 시행한 고주파 병행 치료의 안면부종 감소 효과: 증례보고)

  • This study was performed to evaluate the effect of high frequency therapy applied to the region branching to the external carotid artery for peripheral facial paralysis sequelae. A patient suffering with facial edema due to facial paralysis sequelae had been treated with acupuncture, high frequency therapy on the branch area to the external carotid artery for 7 weeks. The evaluation of clinical outcome was done by degree of swelling by measuring the distance of the face and skin temperature of face through digital infrared thermographic imaging. After treatment, the patient's degree of swelling and the temperature difference between the affected side and normal side was decreased. In addition, the temperature was changed in the entire facial area as well as the treatment point of high frequency therapy. This result shows that acupuncture combined with high frequency therapy at the region branching to the external carotid artery could be an effective way to improve facial blood flow, although further clinical studies will be needed.

Effect of Scratching Method Complex Therapy on the Patients with Peripheral Facial Paralysis (말초성 안면신경마비에 대한 괄법(括法) 복합치료 효과)

  • Objectives : This study was done for reporting the effect of scratching method on patients with peripheral facial palsy. Methods : We investigated 34 cases of patients with peripheral facial paralysis, and devided patients into two groups : We treated one group by complex oriental medical treatment with scratching method therapy, and did the other group by complex oriental medical treatment without scratching method therapy. gross grading system of House-Brackmann(HB score) and Yanagihara's unweighed grading system(Y score) at baseline and final were used for evaluating the effect of the treatment. Results : 1. In scratching method group and non-scratching method group, compared with baseline, at final, HB score was significantly decreased and Y score was significantly increased. 2. At final, scratching method group showed significant decrease on HB score and significant increase on Y score compared with non-scratching method group. Conclusions : scratching method showed significant improvement in peripheral facial palsy.

  • Objectives : The aim of the present study was to evaluate Yandorak for the wide inflection and establishment of a diagnostic system. Methods : We analyzed the measurement value of Yandorak on fifty one patients with idiopathic facial paralysis and eighty four healthy persons from April 1, 2005 to March 1, 2006 by using Yangdorak(YDRAK-94N, Sord Medicom Co., Korea). Statistical analysis was performed using the SPSS 11.0 for Windows, Mann-Whitney test for the comparisons followed side(Right and Left), sex, palsy localization in Idiopathic facial paralysis and normal group and one-way ANOVA(Kruskal Wallis) for the comparisons followed age. p < .05 was considered significant statistically. Results The difference of the electric current value of all meridian except Bladder(BL) of normal group on both side extremity was not significant statistically, Electric current value of all meridian except Gall-Bladder(GB) was more higher in idiopathic facial paralysis group than in normal group. The measurement value of Yandorak followed sex and palsy localization were not significant statistically. Conclusion : It is suggested that the measurement value of six Hand Yang & Eum channel, Liver Meridian of Foot Gworeum(厥陰) and Stomach Meridian of Foot Yangmyeong(陽明) is more higher in idiopathic facial paralysis group than in normal group.

A Research on Quality of Life of Facial Palsy Patients (구안와사 환자의 삶의 질에 대한 연구)

  • Objective : The face is very important for human communication. So facial palsy patients experience not only physical disturbances but also serious emotional stress. Therefore the focus of treatment must be to not only aid the recovery of objective symptoms but also the improvement of subjective quality of life. However there has not been enough Oriental Medical study in this field until now. Thus this study was begun to suggest a useful index for the treatment of facial palsy. Methods : Grade of paralysis, sequelas, and quality of life were used for evaluation. To evaluate Grade of paralysis, House-Brackman Grade was used as the Gross scale and Kim's Grade was used as the Regional scale. Sequelas were evaluated by muscle contraction, synkinesis, NA, and grade of philtrum tilt. Quality of life was evaluated by SF-36v2, Facial Disability Index, and Vas. Results & Conclusion : 1. The change of grade of paralysis between the early and the present time reveals remarkable improvement statistically. 2. There was a remarkable improvement in all parts of quality of life except several areas of SF-36(RP, RE, MH, VT) 3. In only PF(Physical Function) of FDI, quality of life increased remarkably according to the improvement of grade of paralysis. 4. There is the positive relationship among Kim's grade, HB grade and Synkinesis 2. 5. Synkinesis among sequelas has the greatest effect on quality of life. And muscle contraction, change of NA, and grade of philtrum tilt also affect quality to some extent. 6. There was no remarkable relationship between the period of illness and change of quality of life. 7. Change of NA is shown after 3 months of illness and synkinesis becomes worse after 6 months of illness.

Clinical Analysis of Facial Nerve Paralysis Patients Improved by Sasang Constitutional Medical Treat : A Retrospective Cross-Sectional Study (한방병원을 내원한 말초성 안면신경마비 환자의 사상의학적 임상 치료 : 후향적 단면연구)

  • Objectives This study was conducted in order to analyze the distribution of Sasang constitution, herbmed, exterior-interior disease and concomitant symptoms of 36 facial nerve paralysis patients. Methods A clinical study was done on 36 patients, treated as facial nerve paralysis. Sasang constitution specialist diagnosed their constitution and gave prescription. Degree of improvement was evaluated by assessment scale or change of patient's complaint. Results 1) Distribution of sasang constituion : Taeeumin 21 patients, Soyangin 8 patients, Soeumin 7 patients. 2) Distribution of exterior-interior disease : Taeeumin and Soeumin were diagnosed as interior disease more than exterior disease, Soyangin was reverse. 3) Hyeongbangdojeok-san and Hyeongbangjihwang-tang (Soyangin), Galgeunhaegi-tang(Taeeumin) and Gwakhyangjeonggi-san(Soeumin) were used the most in each sasang constitution. 4) Seventeen patients had concomitant symptoms : digestive disorder, sleep disorder, tinnitus, hypertension, diplopia, itching, gout, neck-shoulder pain, constipation and dizziness. Conclusions Patients had different Sasang constitution and in the same Sasang constitution, they had different symptomatology.

A Clinical Study About Factors to Have an Influence on the Prognosis of Bell's Palsy (구안와사 호전요인에 관한 연구)

  • Objective : In Modern society, patients with facial nerve paralysis are increasing because of many factors - irregular life, cold, overwork and stress etc. We have troubles in presuming the prognosis, though how to diagnosis and examine facial nerve paralysis are many. Methods : A clinical study was done on 89 patients who were diagnosed and treated as facial nerve paralysis(Bell's palsy) from January 2001 to May 2003 at the Dep. of Acupuncture and Moxibustion, college of Oriental Medicine, Woo-suk University. We classified 89 patients as the Sasang(四象) constitution, contributing factor, season, age and existence of diabetes items and analyzed, as we would research differences of changing point-Period from on set of Bell's palsy to the day which the change begins to be seen at the face- and improvement -Period which Bell's palsy is improved from on set to H-B grade II. Results : 1. Among the Sasang constitution, Soeumin($10.67{\pm}3.77days$) were faster than other constitutions, but Soyangin($16.25{\pm}6.75days$) were slower than other constitutions in changing point. Taeumin($4.12{\pm}1.49weeks$) were shorter than other constitutions, but Soyangin($4.88{\pm}2.11weeks$) were longer than other constitutions in improvement. 2. Among contributing factors, overwork and stress group were slower than other contributing factors in changing point($13.95{\pm}6.52days$), and longer than others in improvement($4.67{\pm}1.87weeks$). 3. Changing point and improvement of season, age, and existence of diabetes had the difference of the average according to an each item, but they were not statistically significant. 4. In 89 patients with Bell's palsy, average changing-point was $13.11{\pm}5.99$(days) and average improvement was $4.47{\pm}1.82$(weeks). Their correlation was 0.687 and statistically significance(P<0.01), therefore we could decide that their relation is highly correlation.

The Influence of Functional Dyspepsia on Peripheral Facial Paralysis Patients' Improvement (기능성 소화불량이 말초성 구안와사 환자의 호전도에 미치는 영향)

  • Objectives : This study was perfomed to know the effect of functional dyspepsia on the prognosis of peripheral facial paralysis. Methods : We divided 42 patients with Peripheral Facial Paralysis who had admitted at Semyung University Chung-ju Oriental Hospital into two groups, The A group which had Functional Dyspepsia, and The B group which didn't have Functional Dyspepsia. Then we applied acupuncture, physical therapy, herb medicine to the patients. The effects of treatment was evaluated by Yanagihara's unweighed grading system. Results : A group's improvement index which is measured by Yanagihara's unwei ghed grading system was lower than B group's improvement index. Conclusions : Through oriental medicine treatment, the patients recovered significantly. The patients who has functional dyspepsia recovered slowly than the patients who has not functional dyspepsia.

Comparison of Incipient Grade and Improvement Rate between Each Opposite Gender and Lesion on Peripheral Facial Nerve Paralysis (구안와사(口眼喎斜) 환자에서 성별 및 발병부위가 병세 및 호전에 미치는 영향)

  • Objectives : This study was to compare the incipient grade and improvement rate between each opposite gender and part on peripheral facial nerve paralysis. Methods : We investigated 64 cases of patient with peripheral facial nerve paralysis and divided into four groups with each gender and affected part and compared their incipient grade and rate of improvement. All groups were evaluated by Yanagihara's unweighted grading system before treatment and in everyday and after treatment. Results & Conclusion : 1. There's a difference of incipient grade between each opposite affected lesion in male. But there was not significant statistics. 2. There's no difference of incipient grade between each opposite affected lesion in female. 3. There's a difference of incipient grade between each opposite gender, affected their right side. But there was not significant statistics. Left side affecting palsy is more severe than right in male, and reversed results in female. But there were not significant statistics. There's no differences between each affected lesion in female. 4. There's no differences of improvement rate between each affected lesion in female. 5. The group, affected right side had better rate of improvement than another in male. But it's not significant statistics. 6. Male group had better rate of improvement than female in both gender, affected its left lesion. But it's not significant statistics.
