  • Ramsay Hunt syndrome is a viral associated disease with severe otalgia, vertigo, fever, herpetic eruptions on either side of the external auditory meatus and cavum concha, ipsilateral facial nerve palsy and cochleovestibular dysfunction. This syndrome may be the most common cause of unilateral facial paralysis and involvement of both the vestibular and cochlear branch of 8th cranial nerve. And loss of taste sensation may be developed in same involement site. This syndrome affects adults in most cases, and a samll number of children with herpes zoster oticus have been reported. And concomittantly CNS invlovement of this snydrome is very rare. We experienced a 7 years old aged patient of Ramsay Hunt syndrome who had evidence of aseptic meningitis, and this patient showed well reponses with Acyclovir and symptomatic therapy. So, we report this case with brief review of related literatures.

한국재래산양에서 발생한 리스테리아병 (A Case of Listeriosis Occurred in the Korean Native Goat)

  • A survey was performed on the outbreak of listeriosis in a Korean native goat in Animal Breeding Station of Gyeongsang National University, Chinju, Korea. Clinical signs in the affected goat were fever, dullness, inappetence, nasal discharge, slobbering, dysphagia, circling, incoordination, recumbency, unilateral facial paralysis, torticollis, dyspnea, spasmodic paddling movement of the limbs and death at 6 days after onset. No significant gross lesions were seen. The histopathological lesions were consisted of perivascular edema and cuffs by round cells and microabscess with infiltration of neutrophils, lymphocytes and macrophages in the cerebrum. Gram-positive organism was observed in the microabscess lesions. Listeria monocytogenes was isolated exclusively from the brain tissue by cultural examination and the lesions induced by experimental infection with the isolate were characterized by severe conjunctivitis in rabbit and hepatic necrotic foci in mouse. This seems to be the first report of listeriosis in Korean native goat.

전할(全蝎)에 대한 문헌적(文獻的) 고찰(考察) (The study on Buthus martensii Karsch)

  • Objective: Through the literatures on the effects of Buthus martensii Karsch, we are finding out the clinical possibility and revealing the more effctive to intractable diseases. Method: We investigated the literatures of Oriental Medicine and experimental reports about Buthus martensii Karsch. Results: 1. The taste of Buthus martensii Karsch is salty, hot and toxic, and the effect of this is tetanus, headache, facial palsy and convulsion. 2. The venom of Buthus martensii Karsch is anaesthetic and toxic protein, composed of buthotoxin, lecithin, trimethylamine, betaine, taurine, cholesterol, stearic acid and palmitic acid and similar to the snake venom. 3. The pharmacological effects ofButhus martensii Karsch are anti-convulsion, depressor, anesthesia, anti-thrombosis and anti-cancer. 4. Symptoms of Buthotoxin poisoning are local pain, vomiting, fever, hypertension and palpitaion, and critical condition to Dyspnea, coma and death.

Osteoma of the Frontal Sinus with Secondary Subdural Empyema Formation

  • Osteomas of the paranasal sinuses rarely lead to intracranial manifestations. The authors report an unusual case of a frontal sinus osteoma leading to subdural emyema formation. A 19-year-old man presented with headache and fever one month after minor facial trauma. Neuroradiological studies revealed subdural empyema in left frontal lobe with moderate cerebral edema and a osteoma in the left frontal sinus with sinusitis of maxillary sinus. The patient was surgically treated in one stage operation of decompressive craniectomy, removal of subdural empyema with frontal sinus osteoma, and endoscopic sinus surgery via cranial and nasal route. The patient recovered very well after surgery and postoperative antibiotic therapy. The etiology of intracranial infection and the treatment strategy are to be discussed.

성상신경절 차단후에 발생한 경부혈종 (Hematoma in Neck after Stellate Ganglion Block)

  • A 43 year old male patient injured in a traffic accident was admitted to the department of general surgery for the treatment of spleen rupture and multiple rib fractures. After subphrenic abscess occurred after splenectomy was treated. After 50 days of admission, left facial palsy occurred with sensory neural hearing loss, and tinnitus by longitudinal fracture of left temporal bone. The patients was consulted to pain clinic for further evaluation and treatment. The patient was treated with stellate ganglion block with 1% lidocaine 6ml one time daily. On 19th day, stellate ganglion block was given as usual, and the patient complained of pain in the neck and headache the next day. Two days later, mild fever elevation and hematoma in the neck were found. Hematoma was drained with hemovac. Ruptured muscular branch of vertebral artery was ligated surgically but the ligation was released in the next day and the hematoma was removed and the artery religated. Five units of packed RBC were transfused during the period and the patient was discharged without any sequelae.

태음인(太陰人) 병증(病證) 진단 알고리즘 개발 연구 (The Study on the Development of Diagnosis Algorithm of Taeeumin Symptomology)

  • Objectives : This study is aimed to develop the algorithm to diagnose Taeeumin's symptomology, by the method of literature research on Sasang Constitutional Medicine. Methods : Applying the sequential differentiations of Taeeumin's symptomology, or exterior-interior disease differentiation, favorable-unfavorable pattern differentiation, and mild-severe-dangerous-urgent pattern differentiation, "Donguisusebowon" and related literatures have been reviewed. Results and Conclusions : 1) 1st step: Taeeumin's exterior pattern and interior pattern are differentiated by the indexes of whole-body cold or heat pattern, sweating, and facial complexion. 2) 2nd step: The favorable pattern of the Taeeumin's exterior disease can be detected by indexes of the existence of fever, generalized pain while the unfavorable one by indexes of the abnormal condition of digestion and feces, and fearful throbbing. The favorable pattern of the Taeeumin's interior disease can be diagnosed based on indexes of eye pain, dry nose, dry throat, and heat symptoms that occur in various parts of the body, while the unfavorable one by indexes of thirsty, urination, feces and specific symptoms which can be induced by dryness. And in the both unfavorable patterns the dark complexion on the faces is revealed. 3) 3rd step: The mild-severe patterns of the favorably exterior disease are differentiated in terms of the condition of fever, while the mild-severe patterns of the favorably interior disease are in differentiated based on whether abnormal symptoms are revealed in the gastrointestinal tract. Both of the unfavorably dangerous-urgent patterns in exterior and interior diseases are differentiated by the symptoms such as tinnitus, dim vision, weakness of legs and back pain, and lack of strength in legs and thighs.

소아의 하악에 발생한 Garre 골수염의 근관치료에 관한 증례보고 (GARRE'S OSTEOMYELITIS OF THE MANDIBLE RESOLVED BY ENDODONTIC TREATMENT IN CHILDREN: A CASE REPORT)

  • Garre's osteomyelitis is a unique form of osteomyelitis characterized rediographically by localized thickening of the periosteum and deposition of laminated subperiosteal bone. The most common inciting factor is a mandibular infection in permanent first molar with necrotic pulp. This disease occurs primarily in children and to date in all instances it has occured only in mandible. It usually results in hard swelling over the jaws, producing facial asymmetry with little or no pain. The overlying skin is normal but can occasionally be inflammed mostly when pain is present. Palpation reveals a usually smooth, bone-hard lesion which feel like an inherent part of the mandible. Unlike other forms of osteomyelitis, there is no marked increase in fever, white bloods cell count, sedimentation rate or alkaline phosphatase value. The treatment of Garre's osteomyelitis usually consist of elimination of the sourses of infection, i.e., either extration of an offending infected teeth or root canal therapy. This treatment almost always results in resolution of the Garre's osteomyelitis. Resistant cases have involved secondary surgery, i.e., decortication and sequestrectomy. This report presents three cases of Garre's osteomyelitis resolved by endodontic treatment. Cliniqtl examination revealed swelling on the face with no tenderness. Periapical radiograph showed deep caries lesion extending into pulp chamber and periapical radiolucency. Occlusal radiograph showed an enlargement of bone and stretching the periosteum. A clinical diagnosis of the Garre's osteomyelitis was made. Endodontic treatment was accomplished with conventional method and restored facial symmetry. Long-term check-ups are necessary to evaluate the results of endodontic treatment.

아동의 응급의료서비스 이용실태 (Utilization Status of Emergency Medical Service for Children)

  • Purpose: This study was done to describe utilization status of emergency medical service for children at one university affiliated hospital located in Seoul. Data were obtained from the medical records of patients under 13 years of age who visited the ER from January 1 to December 31, 2006. Method: Medical records missing the time of discharge were excluded in the analysis of waiting time, which resulted in 19,766 cases. Data were analyzed using SPSS WIN 14.0 version. Result: There were slightly more boys (58.4%), average age of the children was 3.97 years of age. More children at the aged 1 to 3 years (51.3%) visited the ER. Fever was the most frequent complaint: 5,180 cases (24.38%). The other complaints were head or facial laceration (10.55%), vomiting (9.63%), abdominal pain (8.06%), cough (7.67%), and painful limb swelling (6.34%). Average waiting time before the first medical examination was 17 minutes, and average ER stay time was 3 hours and 23 minutes. Conclusion: The results suggest the need to assign a nurse specialist for pediatric ER to provide more efficient care for the children. Also, extra staff assignment during the evening shift or extending office hours of local pediatricians should be considered.

The first human case of Trichinella spiralis infection in Korea

  • Three cases of human infection by Trichinella spiralis were first confirmed by detecting encysted larvae in the biopsied muscle in December 1997, in Korea. The patients were one 35-and two 39-year-old males residing in Kochang-gun, Kyongsangnam-do. They had a common past history of eating raw liver, spleen, blood and muscle of a badger, Meles meles melanogenvs, and complained of high fever, facial and periorbital edema, and myalgia. Hematologic and biochemical examinations revealed leukocytosis and eosinophilia, and highly elevated levels of GOT, GPT, LDH and CPK. In the gastrocnemius muscle of a patient, roundly coiled nematode larvae were detected. The larvae measured 0.775-1.050 (av 0.908) mm in length, and 0.026-0.042 (av. 0.035) mm in maximum width. The specific IgG antibody levels in three patients' sera were significantly higher when compared with those of normal controls. The patients were treated with flubendazole and albendazole for 15-30 days, and discharged at 13-34 days post-admission. From the above findings, it was confirmed that T. spiralis is present in Korea, and the badger plays a role of as the natural host.

Complete occlusion of the right middle cerebral artery associated with Mycoplasma pneumoniae pneumonia

  • We report a case of a 5-year-old girl who developed left hemiparesis and left facial palsy, 6 days after the initiation of fever and respiratory symptoms due to pneumonia. Chest radiography, conducted upon admission, showed pneumonic infiltration and pleural effusion in the left lung field. Brain magnetic resonance imaging showed acute ischemic infarction in the right middle cerebral artery territory. Brain magnetic resonance angiography and transfemoral cerebral angiography revealed complete occlusion of the right middle cerebral artery. Mycoplasma pneumoniae infection was identified by a 4-fold increase in IgG antibodies to M. pneumoniae between acute and convalescent sera by enzyme-linked immunosorbent assay. Fibrinogen and D-dimer levels were elevated, while laboratory exams in order to identify other predisposing factors of pediatric stroke were all negative. This is the first reported pediatric case in English literature of a M. pneumoniae-associated cerebral infarction involving complete occlusion of the right middle cerebral artery.
