현재 정보통신의 비약적인 발전은 의료서비스 전달체계에서도 큰 변화와 진전을 야기하고 있으며 세계적으로 점차 확대되어가고 있는 추세이다. 이는 의료정보화라는 이름으로 환자에게는 질병의 진단, 치료에 있어서 보다 신속, 세밀하고 정확히 판단할 수 있게 하여 수준 높은 보건의료서비스를 제공하는 한편 의료기관 및 관련기관은 업무의 효율성을높여가고 있다. 그 중 원격의료는 의료기관의 방문 없이 대기시간의 단축, 일률적인 고도의 의료수준을 기대할 수 있는 등의 장점이 있는 제도이다. 특히 장소와 시간에 구애받지 않고 방대한 의학에 관한 정보접근성이 높아질 수 있다는 점이다. 그러나 현대 과학 기술의 발달의 속도에 비해 운용상의 제반 규정과 마인드가 부족한 것이 현실이다. 현행 우리 의료법에서는 원격의료를 규정하고는 있으나 그 내부적 관계에 따르는 세부적인 법률관계의 부재 및 현장에서 이루어지는 의료행위가 아닌 정보통신망을 이용한 비대면접촉에 의한 특수한 형태라는 측면에서 제도적, 시설적, 환경적 제약이 있음을 부인할 수 없다. 따라서 본 논문에서는 현행 원격의료의 발전 잠재 가능성이 무한함을 전제로 법적 문제점 및 개선점을 고찰해보고 이를 통한 원격의료를 활성화할 수 있는 활로를 모색하고자 한다.
Purpose: This study aimed to explore the social perception of visiting nursing before and after coronavirus disease (COVID-19). Methods: This survey-based study used online big data for comparative analysis by classifying the keywords related to visiting nursing searched on domestic portal sites before and after COVID-19. Results: According to the results of analyzing the Intertopic Distance Map based on Latent Dirichlet Allocation in this study, four topics were extracted, two each before and after COVID-19. The first topic before the COVID-19 period was termed "the expansion of visiting nursing subjects and services visiting nursing," while the second was termed "visiting nursing," which is related to customized welfare. The first topic after the COVID-19 period was termed "the suspension and resumption of visiting nursing services," while the second was "the development of a non-face-to-face home visit healthcare system". Conclusion: The results of this study can be used as useful reference data to contribute to future medical service delivery system reform policies starting at the end of COVID-19 and the revitalization of community care for visiting nursing.
"Personalized medicine," the goal of which is to provide better clinical care by applying patient's own genomic information to their health care is a global challenge for the $21^{st}$ century "genomic era." This is especially true in Korea, where provisions for clinical genetic services are inadequate for the existing demand, let alone future demands. Genomics-based knowledge and tools make it possible to approach each patient as a unique biological individual, which has led to a paradigm-shift in medical practice, giving it more of a predictive focus as compared with current treatment oriented approach. With recent advancements in genomics, many genetic tests, such as susceptibility genetic tests, have been developed for both rare single gene diseases and more common multifactorial diseases. Indeed, genetic tests for presymtomatic individuals and genetic tests for drug response have become widely available, and personalized medicine will face the challenge of assisting patients who use such tests to make appropriate and wise use of genetic risk assessment. A major challenge of genomic medicine lies in understanding and communicating disease risk in order to facilitate and support patients and their families in making informed decisions. Establishment of a health care system with provisions for genetic counseling as an integral part of health care service, in addition to genomic literacy of health care providers, is vital to meet this growing challenge. Realization of the promise of personalized medicine in the era of genomics for improvement of health care is dependent on further development of next generation sequencing technology and affordable sequencing test costs. Also necessary will be policy development concerning the ethical, legal and social issues of genomic medicine and an educated and ready medical community with clinical practice guidelines for genetic counseling and genetic testing.
지속적인 암환자 및 암 관련 의료비용 급증은 환자 개인의 고통 및 경제적 부담으로 작용할 뿐만아니라 사회적인 부담이 되고 있다. 이에 재가암 환자에 대한 통증관리 등의 서비스 제공은 의료 비용을 절감하는데 효과가 있을 것으로 여겨지고 있다. 따라서 재가암환자에 대한 적절한 의료 및 복지 서비스 등을 제공하기 위한 자료로 활용하고자 재가암환자관리사업 수혜자의 서비스 요구도에 대하여 각 보건소의 재가암환자관리사업 담당자가 재가암환자관리사업 수혜자 방문시 면접조사방법으로 조사하였고, 불충분응답자 15명을 제외한 661명을 최종 분석대상으로 하였다. 재가암환자의 서비스 요구도에 대한 요인분석 결과 5가지 영역으로 분류할 수 있었고, 요인별 신뢰도는 0.593~0.890이었다. 요구도의 순위는 사회적 지지, 정보 및 교육제공, 심리적 문제해결, 신체증상해결, 가사도움 순으로 조사되었다. 대상자를 암 치료 및 진행 정도에 따라 말기암 환자, 치료중인 암환자, 암 완치자로 분류하였을 때, 말기암환자의 요구도가 가장 높게 나타났으며, 사회적지지, 심리적 문제 해결, 정보 및 교육 순서로 요구도가 높았다. 나머지 그룹에서는 사회적지지, 정보 및 교육, 심리적 문제 해결 순으로 말기 암환자와 차이를 보였다. 뿐만 아니라, 연령별, 거주 지역별, 종교 유무에 따른 서비스 요구도에 차이가 관찰되었고, 이를 바탕으로 현재 제공되고 있는 재가암환자관리의 효율적인 추진을 위해 대상자 분류, 연령별 및 거주 지역별 특성에 따른 서비스가 제공될 수 있도록 해야 할 것이다.
Objectives : While there are many studies about treatments of facial palsy, no study has been performed on general population of Korea, especially concerning about comparison between western medicine and oriental medicine. This study aimed to investigate magnitude of health visits and treatment patterns for Korean patients with facial palsy through the computerized database of Health Insurance Review and Assessment Service(HIRAS). Methods : According to the HIRAS database over 5 years' period from 2004 to 2008, the medical records of patients with facial palsy as a main diagnosis were extracted. Inclusion criteria of facial palsy are Bell's palsy(G510), Geniculate ganglionitis(G511), Melkersson's syndrome(G512), Other disorders of facial nerve(G518), Disorder of facial nerve, unspecified(G519) in western medicine. And Paralytic face(G016), Deviated eye and mouth(J01), The other facial palsy(J013) were included in oriental medicine. We compared the claim number of western medical care with that of oriental medicine treatment by year and month. Results : The total claim number of facial palsy was increasing on both western medicine and oriental medicine from 2004 to 2008. In western medicine, the claim number of Bell's palsy(G510) is the most. In oriental medicine the inpatients claim number of Deviated eye and mouth(J01) is the most, while outpatients claim number of the other facial palsy(J013) is the most. Conclusions : Medical database of HIRAS provided comprehensive and vast information on epidemiologic characteristics and treatment, which can be more reliable data to expect medical demand for facial palsy in condition that accurate diagnosis and standardized treatment is delivered in clinical settings.
스마트폰을 기반으로 한 앱은 병원의 의료 서비스, 은행 및 카드사의 금융서비스, 기업 및 가정의 유비쿼터스 기술 등 다양한 영역에 활용되고 있다. 이러한 서비스 환경에서 외부인에 의한 스마트폰의 불법적인 노출은 공 사적 정보의 유출을 포함한 자산의 손실이 발생한다. 이를 위한 보호 기법으로 비밀키 및 패턴 인식 기술, 정적인 단일 영상인증 기법이 적용되고 있으나 정적인 킷값의 유출 또는사진과같은영상을활용하여접근이가능하다는문제를 가지고 있다. 본 논문에서는 이러한 위험요소 및 문제로부터 스마트폰을 보호하기 위한 기술로 사용자 얼굴 인증 기술을 제안한다. 제안 기술은 사용자의 얼굴 동영상의 키 프레임을 실시간으로 추출하여 사용자를 인증하고 스마트폰의 접근을 제어한다. 인증 정보는 다수의 키 프레임으로 구성되며, 영상의 화소 및 휘도의 DC 값을 활용한 유사도 판별 알고리즘으로 사용자의 접근을 통제한다.
Objectives: This study was designed to evaluate whether employment status is associated with the experience of unmet care needs. Methods: This study utilized the Korea Health Panel data from 2012 to 2016. A total of 4,083 workers were selected as baseline subjects in 2012 and were followed for four years. This study used the GLIMMIX procedure under the marginal model while adjusting for covariates. Results: A total of 12.4% of 4,083 people said they had failed at least once to have a treatment or checkup despite the needs. Those more likely to experience unmet care needs were women and people of lower income level, with worse health conditions and chronic disease. Precarious workers, the self-employed, and the unemployed were more likely to experience unmet care needs caused by economic burden than permanent workers (Odds Ratio: 2.14, 2.07, 2.74, respectively). Conclusions: This disparity means that precarious workers and the unemployed are more likely to face barriers in obtaining needed health services. Given their insecure employment status, meeting their needs for health care is an important consideration.
목적 : 본 연구의 목적은 음성-텍스트 변환 어플리케이션을 이용한 원격 모니터링이 건강한 성인의 작업균형에 미치는 효과를 알아보기 위함이다. 연구방법 : 총 7명의 건강한 성인을 대상으로 연구를 진행하였다. 대상자는 실험에 참여 전, 원하는 작업 활동을 선택하였고 이를 스마트폰에 설치한 음성-텍스트 변환 어플리케이션에 등록하였다. 음성-텍스트 변환 어플리케이션은 미리 등록된 작업 활동을 시간에 맞춰 수행 여부를 확인할 수 있도록 알람을 제공하였고 대상자는 음성으로 수행 여부를 등록하였다. 원격 모니터링은 일주일 동안 진행하였고 일주일 뒤, 작업 활동의 변화를 살펴보았다. 결과 : 대상자 모두 일주일 동안 어플리케이션 사용에 높은 순응도와 만족도를 보였다. 또한 일주일 뒤, 건강과 관련된 작업 활동의 균형이 유의하게 향상되었다(p<.05). 결론 : 본 연구 결과는 스마트폰을 이용한 원격 모니터링이 코로나19로 인한 봉쇄 기간 동안 작업 균형을 회복하는 방법으로 사용 가능함을 시사한다.
다수 치아를 발치하면 환자는 사회적, 심리적, 심미적인 문제를 마주하게 된다. 이때, 치아를 발치하기 전 임시 즉시 의치를 제작하는 것을 통해 이러한 문제를 최소화할 수 있다. 특히, 디지털 기술을 활용해 임시 즉시 의치를 제작한다면 기공과정이 단순해 지고 환자의 적은 내원 횟수로 임시 즉시 의치를 완성할 수 있다. 한편, 소수의 잔존치가 남은 환자에서 고정성 임플란트 보철수복이 어렵고, 가철성 국소의치의 지지 및 유지가 불량할 것으로 예상되는 경우에 임플란트 지지 가철성 국소의치(Implant-supported removable partial denture)가 대체 치료 옵션으로 주목되며, 임플란트 보철물을 이용해 추가적인 지지 및 유지가 가능하다. 본 증례에서는 다수치아 발거 후 소수 잔존치 상황에서 디지털 기술을 활용해 임시 즉시 의치를 제작하였다. 이후 임플란트를 식립하고 서베이드 크라운을 제작해 가철성 국소의치의 지지, 안정, 유지를 제공하여 임상적으로 만족스러운 결과를 얻었기에 이를 보고하고자 한다.

In 2020, the coronavirus disease 2019 (COVID-19) pandemic has caused unprecedented disruptions to global health systems. The Korea has taken full-fledged actions against this novel infectious disease, swiftly implementing a testing-tracing-treatment strategy. New obligations have therefore been given to the Health Insurance Review and Assessment Service (HIRA) to devote the utmost effort towards tackling this global health crisis. Thanks to the universal national health insurance and state-of-the-art information communications technology (ICT) of the Korea, HIRA has conducted far-reaching countermeasures to detect and treat cases early, prevent the spread of COVID-19, respond quickly to surging demand for the healthcare services, and translate evidence into policy. Three main factors have enabled HIRA to undertake pandemic control preemptively and systematically: nationwide data aggregated from all healthcare providers and patients, pre-existing ICT network systems, and real-time data exchanges. HIRA has maximized the use of data and pre-existing network systems to conduct rapid and responsive measures in a centralized way, both of which have been the most critical tactics and strategies used by the Korean healthcare system. In the face of new obligations, our promise is to strive for a more responsive and resilient health system during this prolonged crisis.
