Lymphaticovenular anastomosis for Morbihan disease: a case report

  • Morbihan disease (MD) is a very rare condition characterized by rosaceous or erythematous lymphedema on the upper two-thirds of the face. A definitive management strategy for MD is lacking, and treatment is challenging. Herein, we present a case of persistent bilateral eyelid edema treated by lymphaticovenular anastomosis (LVA) and lymph node-vein bypass surgery. The patient experienced persistent bilateral eyelid edema. Indocyanine green lymphography was performed, and the diagnosis of bilateral facial lymphedema was confirmed. On the right side, a preauricular lymphatic vessel was anastomosed to a vein. On the left side, lymphostomy on the preauricular lymph node was done, with anastomosis to the transected proximal end of the concomitant vein of the transverse facial artery. Furthermore, a preauricular lymphatic vessel was anastomosed to a vein. Eyelid edema decreased and progressively improved on both sides. The outcome of this case suggests that LVA and lymph node-vein bypass surgery are appropriate for treating persistent eyelid edema related to MD.

A Case Study of Eyelid Edema Post Craniotomy (개두술 후 발생한 안검부종에 대한 치험 1례)

  • Objectives: This study considered the effectiveness of Korean medicine for the treatment of eyelid edema post craniotomy. Methods: The patient was diagnosed with Taeumin-one of the four constitution types in Korean medicine-and treated with herbal medicines: Cheongsimyeunja-tang, Chungpyesagan-tang, and Hwangryunhaedok-tang pharmacoacupuncture treatment. The severity of eyelid edema was assessed using Kara and Gokalan's scale, daily imaging of the patient's eyelids, and the ratio of the length of the left eye to that of the right. Results: Following treatment, the Kara and Gokalan score decreased from 3 to 0. The changes in the ratios were 29% and 50%, respectively. The visual field defect due to edema in the left eye was also repaired. Conclusion: This study suggests that Korean medical treatment could be an effective option for treating eyelid edema following craniotomy.

Effects of Cryosurgery of the Third Eyelid Gland on Tear Production and Histological Changes in Dogs (개에서 제3안검선 냉동수술이 눈물생산량 및 조직학적 변화에 미치는 영향)

  • The effects of cryosurgery on tear production and histological changes of the third eyelid were studied in dogs. Clinically normal 12 mixed breeds weighing 2∼6 kg were divided into three groups and treated as follows; 45 seconds double freeze-thaw treated group, 60 seconds double freeze-thaw treated group and 90 seconds double freeze-thaw treated group. The significant decrease of the tear production after cryosurgery was shown in all groups throughout the observed periods(p<0.05). However, there was no difference among groups. The main complications after cryosurgery were chemosis and conjunctival injection. Other complications such as eyelid edema, eyelid depigmentation and keratitis were more preominent in group III compared to those of groups I and II. On histopathological examination, chronic inflammatory changes and regeneration of the third eyelid glands were noted in group I and predominated loss of the third eyelid glands and necrosis of hyaline cartilage were observed in group III However, such changes were less appeared in group II. The results of this study suggested that double freeze-thaw cryosurgery for 60 seconds on the third eyelid glands would be the most effective method for treating tear staining syndrome.

  • A five-day old female Thoroughbred foal appealed mild edema and pain of a lower eyelid on left eye. She was diagnosed with a typical entropion which clinical signs were inward rolling of the eyelid edge and excess lacrimation. Modified Hotz-Celsus technique that resect and suture only skin was operated. Result was very successful without recurrence of entropion and other complications. We conclude this method is of value for permanent correction of severe entropion in young foals.

  • Objectives : The purpose of this study is to investigate the research trend of Korean traditional medicine for edema under Korean healthcare facilities. Methods : We collected data from search engines for research including domestic online database using the keywords edema, Hydrocele, emphysema and Pneumocranium Results : Total of 41 articles were about Lower body edema, General edema, edema of the eyelid, Lymphedema, Hydrocele, Bone marrow edema, hand edema, Brain edema, emphysema, Pneumocranium The treatment methods used were acupuncture and pharmacopuncture treatment, moxibustion treatment, herbal medicine treatment, cupping treatment, massage treatment, radio frequency treatment, and Chuna treatment. Twenty-nine studies used only oriental medicine treatment, and 12 studies combined oriental medicine and western medicine. the most commonly used measurement was Measurement of the perimeter of the area. Conclusions : This study shows the research trend of edema. Through the collected data, the treatment methods and for specific information of treatments were organized. This study can be used in clinical environments and will contribute for further study in edema.

Clinical comparison of surgical approach for orbital fracture. (안와하 골절부의 외과적 접근 방법에 관한 임상적 비교연구)

  • This study was undertaken to compare those 4 most commonly used incisions for exposure of fractures of the infraorbital rim and the orbital floor. The patient's age ranged from 19 to 67 years, the majority being in the 2nd decade. There were 17males and 1 female. Three orbital rim, 5 lower eyelid, 7 subciliary and 3 combined conjunctival-lateral canthotomy incisions were reviewed. Following points were the summury of author's experience. 1. The average scar following any of four incisions is barely noticeable. 2. Adequate fracture exposure was achieved with lower eyelid, orbital rim, subciliary and combined conjunctival-lateral canthotomy incisions. 3. One transient ectropion following subciliary incision occurred in 29-years old man with prolonged postoperative eyelid edema.

  • We have experienced a case of infantile nephrotic syndrome confirmed by renal biopsy in a 13-month-old female patient who showed growth and develop mental retardation and persistent proteinuria. She revealed mild eyelid edema, joint laxity, delayed speech development and adrenal cortical calcification on the radiologic study. Renal biopsy showed microcystic tubular change, micro-glomeruli and marked mesangial proliferation.

  • The eyes, the saying goes, are the windows of the soul. It's the first thing you notice about a person. Therefore, many people want to possess beautiful eyelids. Surgical formation of a palpebral fold and sulcus divides the lid into two well-defined segments (palpebral and pretarsal), producing the double eyelid desired by many Oriental women as well as an increasing number of man recently. Upper lid blepharoplasty is the Oriental eye is one of the variations of standard upper lid blepharoplasty. In Oriental double eyelid surgery, there have been two approaches to form a superior palpebral fold: the buried suture(nonincision) method and the full external incision method. Conventionally, the nonincision technique has been shown to produce little postoperative edema. However, the probability of the fold disappearing is high, and this technique cannot be performed in patients with fatty eyelids. Conversely, the incision technique has contrary characteristics. Recently, partial incision(or semi-open) technique which is combination of mentioned methods is used, this technique is removal of pretarsal tissue, muscle, and/or orbital fat around 2 or 3 incision site to facilitate tarsus-dermal adhesion. Our method is on the basis of this technique, furthermore, compared with conventional semi-open method, Y(Yang's) needle assisted double eyelid operation is more easy, convenient, saving-time method and provide satisfactory results.

  • As carotid-cavernous fistula is the major complication that can be occurred uncommonly after maxillofacial injuries, it is abnormal arteriovenous communication between cavernous sinus and internal carotid artery. Such an arteriovenous communication is most often the result of injuries, but need not be associated with bony fracture. It usually begins soon after an injury, but it may be delayed for as long as several months. It begins undramatically with eye pain, headache and slow protrusion of eye ball. A bruit may be heard above the eye with stethoscope. Close examination will reveal dilatation of superficial veins of the eyelid and forehead and periorbital edema. There will be complete or partial ophthalmoplesia of the affected eye. Compression of the common carotid artery on the ipsilateral side will reduce or obliterate the bruit. The lesion in the cavernous sinus is them demonstrable by angiography.

Dobutamine-Induced Perioperative Anaphylaxis in a Dog

  • A 9-years old spayed female Maltese was referred for the treatment of mass on the right 1st mammary gland and acute weight bearing lameness of right hindlimb. It was diagnosed as malignant mammary tumor and cranial cruciate ligament rupture of right stifle joint. Right upper regional mastectomy followed by cranial closing wedge osteotomy (CCWO) of the right tibia were planned for the present problems. Preanesthetic work-up did not show any remarkable abnormalities. Forty-five minutes after induction of anesthesia dobutamine was administered at a rate of 5 ㎍/kg/min by constant rate infusion due to gradual decrease of blood pressure below MAP 60 mmHg during surgical procedure. Despite of the increase of dobutamine infusion rate up to 20 ㎍/kg/min, blood pressure didn't recover. At the end of regional mastectomy generalized skin redness and eyelid edema were identified. Anesthesia was stopped and CCWO procedure was cancelled. To recover from the anaphylactic reactions dexamethasone and diphenhydramine were administered. After about one hour, the patient completely recovered from hypotension and anaphylactic reactions. After 4 weeks, intradermal skin test (IDST) was performed for all the drugs used during anesthesia. Only dobutamine showed positive reaction in IDST. Therefore, dobutamine was considered as the causative agent of anaphylaxis in this patient during the anesthesia. In case of perioperative anaphylactic reaction, postoperative investigation should be performed to identify causative agent and to provide safe recommendations for future anesthetic procedure.
