고유수용성신경근촉진법의 팔 폄 패턴이 반대편 다리의 근활성도에 미치는 영향 (The Effect of Arm Extension Patterns of PNF on Muscle Activity of Opposite Lower Extremity)

  • Purpose: The purpose of this study is to analyze the effect of the arm extension pattern of proprioceptive exercise program on the muscle activity of the opposite lower extremity. Methods: In this study, electromyogram MP150(Biopac system, USA) was applied to 20 healthy male subjects. Arm extension-adduction-internal rotation pattern was applied within initial, mid and end range in sling position and supine position for measurement. And the effect on the activity of rectus femoris and tibialis anterior muscle of the opposite lower extremity was compared and analyzed. Results: The results of this study were summarized as follows: First, there was a statistically significant difference of the activity of the tibialis anterior muscle within the Initial range in sling position and supine position(P<0.05). Second, there was a statistically significant difference of the activity of the tibialis anterior muscle within the end range in sling position and supine position(P<0.05). Conclusion: Rectus femoris and tibialis anterior muscles shows the higher activity in the supine position than in the sling position. Therefore, the supine position is more appropriate than the sling position to make irradiation on lower extremity muscle with the extension pattern.

한쪽 다리에 적용한 수축-이완 기법이 반대쪽 무릎관절 폄 범위에 미치는 효과 -예비연구- (The Immediate Effects of Unilateral Contract-Relax Stretching on Contralateral Knee Extension Range -A Preliminary Study-)

  • Purpose: This article was conducted to determine the immediate effects of unilateral contract-relax (CR) stretching on contralateral knee extension range and to compare both sides of the knee extension range between experimental and control groups. Methods: This study recruited 16 adult males and females with straight leg raising abilities below $90^{\circ}$. The subjects were randomly divided into an experimental group and a control group comprising 8 subjects each. The experimental group performed direct CR stretching on the right hamstring muscles with straight hip extension adduction, and the control group performed indirect CR stretching on the right hamstring muscles with straight hip flexion abduction. Each group performed CR stretching 4 times with 4 repetitions comprising 10 sec of contraction and a 10 sec break between repetitions. Before and after the CR stretching exercises, the subjects' passive knee extensions were measured at the hip in a $90^{\circ}$ flexed position. The subjects' peak force on the right leg and peak pressure on the left leg during each CR stretching exercise were also measured. Results: After doing CR stretching 4 times, each group showed a significantly increased passive knee extension range on both sides, and there was no difference in the passive knee extension ranges between the groups. The peak force on the right leg was significantly higher in the experimental group than the control group. There was no difference in peak pressure between the groups. Conclusion: After applying unilateral CR stretching, the study subjects experienced a significantly increased passive knee extension range on the contralateral side. For patients who find it difficult to apply stretching techniques to knee joints directly, the use of the proprioceptive neuromuscular facilitation technique of CR stretching may be useful in improving the range of the knee joint on the contralateral side without direct treatment.

캐딜락 기구를 이용한 필라테스 전후방 팔 뻗기 동작이 체간 근 활성도에 미치는 효과 (Effect of Forward and Backward Arm Extension Movement of Pilates Exercise Using Cadillac Instrument on Trunk Muscle Activity)

  • Purpose: This study analyzed the muscle activity changes induced by motions of reaching forward and chest expansion that were examined from the bilateral muscles with rectus abdominis, external oblique, multifidus, and longissimus thoracic using Pilates cadillac instrument. Methods: Nine young adult women, who have no musculoskeletal disorder and any of chronic diseases, were participated. Surface electromyography system was used for recording of all signals produced by muscles, and then normalized as percentage of maximum voluntary isometric contraction (%MVIC). The paired t-test and repeated measures of analysis of variance was performed. Results: Reaching-forward motion showed a higher muscle activity from non-dominant external oblique muscle than that of the chest-expansion motion. During both reaching-forward motion and chest-expansion motion, MVIC values collected from dominant side of external oblique muscle were shown a significantly lower than the values obtained from non-dominant side (p<0.05). Conversely, %MVIC values in external oblique muscle collected from dominant side showed a significantly higher than the values obtained from non-dominant side of the same oblique muscle (p<0.05). Reaching-forward motion was caused a higher %MVIC on non-dominant external oblique muscle than that of the chest-expansion motion (p<0.05). Regardless of dominant or non-dominant sides, external oblique muscle was shown the highest activation rate of all the other muscles during reaching forward action, and longissimus thoracic muscle was shown the highest activation rate of all the other muscles during chest expansion action. Conclusion: Reaching-forward motion is suitable for activating an external oblique muscle, and chest-expansion motion is an effective enough in activating of longissimus thoracic muscle.

운동 전 적용된 지속초음파와 맥동초음파가 운동 유발성 근육 손상의 최대등척성근력과 관절가동범위에 미치는 효과 (The effects of pulsed ultrasound and continued ultrasound intervention before an exercise on maximal voluntary isometric contraction and range of motion of exercise-induced muscle damage)

  • Background: The purpose of this study was to investigate the effects of pulsed-ultrasound intervention and continued-ultrasound on the MVIC (maximal voluntary isometric contraction) and active ROM (range of motion) recovery of before EIMD (exercise-induced muscle damage). Design: Randomized controlled trial. Methods: Thirty subjects who are student in their 20s at a university participated in this study, these subjects were assigned into three groups, a control group (n=10), experiment group I (n=10) and experiment group II (n=10). The subjects in experimental group were intervened by pulsed-ultrasound and continued-ultrasound, while ones on control group weren't by any intervention after induced EIMD. Results: First, In comparison of the MVIC, in the among group comparison, the MVIC of continued-ultrasound group was significantly larger than those of other groups (p<.005). Second, In the among group comparison, the active extension angle of continued-ultrasound group was significantly smaller than those of other groups (p<.005). Third, In the among group comparison, the active flexion angle of continued-ultrasound group was significantly lager than those of other groups (p<.05). Conclusion: The above results revealed that the continued-ultrasound intervention before an exercise had a significantly improve of muscle function after EIMD. Therefore we can consider the continued ultrasound as a considerable intervention method to prevent or reduce an exercise injury.

Effects of whey protein supplementation prior to, and following, resistance exercise on body composition and training responses: A randomized double-blind placebo-controlled study

  • [Purpose] The composition of protein supplements, the consumption timing immediately before and after resistance exercise training (RET), and the quantity of protein supplementation may be important factors for the im-provement of muscle mass and function. Although these factors should be considered comprehensively for effective improvement of muscular function in protein supplementation, relatively few studies have focused on this area. Therefore, this study was designed to investigate whether a protein blend supplement before and after resistance exercise for 12 weeks would be effective in increasing muscular function. [Methods] In total, 18 participants were randomly assigned to a placebo (PLA) or protein blend supplement (PRO) group. All subjects followed the same training routine 3 times per week for 12 weeks, taking placebo or protein supplements immediately before and after each exercise session. The protein supplement consisted of 40 g of blend protein, including hydrolyzed whey protein. The RET consisted of lower body (barbell squat, dead lift, seated leg extension, and lying leg curl) and upper body (bench press, barbell rowing, preacher bench biceps curl, and dumbbell shoulder press) exercises. A repetition was defined as three sets of 10-12 times with 80% of one repetition maximum (1RM). [Results] Although the PRO group had a lower protein intake in terms of total food intake than the PLA group, the mean changes in muscle circumference, strength, and exercise volume increased, especially at week 12, compared to the PLA group. [Conclusion] These results suggest that the composition and timing of protein intake are more important than the total amount.

4주간의 혈류를 제한한 걷기운동이 비만여성의 혈중 염증지표와 등속성근기능, 대퇴둘레에 미치는 영향 (Effect of 4 Weeks' Walking Exercise with Blood Flow Restriction on Inflammatory index, Isokinetic Muscle Function, and Thigh Circumference in Obese Women)

  • 혈류를 제한한 운동은 가압벨트를 사용해 사지의 윗부분을 적당한 정도로 압박한 상태에서 트레이닝을 실시하며 인체의 다양한 생리적 기능에 영향을 미치는 것으로 알려져 있다. 이 연구의 목적은 만성질환의 발병 위험성이 높은 비만여성을 대상으로, 낮은 강도의 혈류를 제한한 걷기운동이 신체조성 및 성장호르몬, 근손상지표에 어떠한 영향을 미치는 지를 구명하고자 한다. 연구대상자는 BMI $25kg/m^2$와 체지방률 30% 이상의 직장 비만여성 11명으로 하였으며 혈류제한은 공압방식의 탄성벨트를 양쪽의 대퇴부위에 착용하고 압력을 조절하였다. 걷기프로그램은 트레드밀을 이용하여 실시하였으며(4주, 주 3일, 1일 2회), 2분 걷기와 1분 휴식으로 구성하여 총 5세트 반복하였고(4km/h, 5%) 대응표본 t-test를 실시하였다. 4주간의 혈류를 제한한 걷기운동 후, 체중과 신체질량지수, 체지방량이 유의하게 감소하였으며(p<.05), 우측의 대퇴둘레 또한 유의하게 감소되었다(p<.05). 혈중 염증지표의 농도변화에서 IL-6는 4주간의 혈류를 제한한 걷기운동 후, 유의하게 증가되었으며(p<.05), 통계적 유의수준에는 도달하지 못하였지만 TNF-${\alpha}$는 증가하는 경향을, CRP는 감소하는 경향을 보였다. 등속성 근기능의 변화에서 $60^{\circ}/sec$의 좌 우측 신전력(좌, p<.001; 우, p<.05)과 좌측의 굴곡력(p<.001)이 유의하게 증가되었으며 $180^{\circ}/sec$의 좌측 신전력에서 유의하게 증가되었다(p<.001). 이상의 결과로부터 4주간의 혈류를 제한한 걷기운동이 비만 중년여성의 체중 및 체지방량 감소와 근 기능 향상, 혈중 염증지표 농도의 변화에 긍정적인 효과가 있는 것으로 사료되며, 이는 짧은 운동시간에도 불구하고 장시간의 운동 효과를 얻을 수 있음을 검증한 것과 동시에 각종 만성성인질환 및 대사성 질환의 원인인 비만을 예방하고 치료하는데 효과가 있는 것으로 생각된다.

요추강화 운동기기의 훈련을 통한 유연성 및 근력 특성 분석 (Characteristic Analysis of Flexibility and Muscle Strength according to Exercise using Lumbar Strengthen Exercise Instrument)

  • 본 연구에서는 요추강화 운동기기를 이용하여 훈련 전후에 대한 유연성 및 근력 특성분석을 고찰하고, 요추 재활운동으로서 가능성을 검증하고자 한다. 요추관련 상해가 없으며 신체 건강한 성인 남녀 20명이 실험에 참여하였으며 각각 10명씩 대조군과 요추강화 운동을 제공받는 훈련군으로 나누었다. 실험에 사용된 요추강화운동기기는 하이스파인(Hi-Spine, 메디컬사이언스, 한국)이며 운동은 하루 40분씩 주 3일, 총 4주간 실시하였다. 또한 본 연구에서 피험자들은 누운 자세, 앉은 자세, 직립 자세, 전신스트레칭자세 등 총 4가지의 운동자세를 각 10분씩 제공받았다. 운동 전 후 요추의 유연성과 근력변화를 알아보기 위해 매주 1회 체간 굴곡 및 신전, 기초체력평가와 요추관절토크를 측정하였다. 실험 결과 훈련군에서 유연성과 근력이 모두 증진되었다. 요추강화운동기기의 플랫폼이 3차원 회전을 함에 따라 피험자들의 근육들은 자극받으며, 회전 방향과 각도에 따라 근긴장성과 근수축 작용을 유도시켜 유연성 및 요추 근력을 증가시키는 것으로 판단된다. 본 연구는 향후 요추관련 상해자 또는 고령자들의 요추 재활운동 프로그램에 적용될 수 있을 것이다.

일지역 노인의 관절 건강 상태 (A Study on Health Status of Joints in Elderly People)

  • Purpose: The purpose of this study was to describe the subjective health status of joints, flexibility of joints, muscle strength and state of Joint exercise in elderly people living in the community. Method: The participants were 74 elderly people in a health program of the Gerontological Health Research Center at G University. The data were collected from March to July, 2003. The instrument for the study included items on general characteristics (5 items), subjective health status of joints (10 items), flexibility of Joints and strength of muscle (8 items), and state of joint exercise (7 items). Results: The score for subjective health status of joints was 3.70 (range 1-5). Knee and waist joints were in the worst state. Neck extension and flexion were $17.38{\pm}1.80$ and $.60{\pm}1.08(cm)$ respectively. Trunk extension and flexion were $68.24{\pm}4.47$ and $58.00{\pm}4.91(cm)$ respectively. Flexibility of the Rt./Lt. shoulder joints was $14.63{\pm}12.51/18.82{\pm}13.80(cm)$. Muscle strength of the Rt./Lt. leg was $30.47{\pm}19.68/29.67{\pm}21.22$ (sec.). Shoulder joints were more flexible for men and people in their sixties compared to women and people in their seventies. Hand grip was stronger for men compared to women. The score for state of joint exercise was 2.83(range 1-4). The state of handgrip correlated with the state of joint exercise [r=.423 (Rt.)/r=.273(Lt.)]. Conclusion: To prevent falls and disorders in neuromuscular function, it is important to develop systemic joint exercise programs for elderly people.

탄력밴드를 이용한 PNF 운동이 유방암 절제술에 속발한 상지 림프부종 환자의 부종, 관절가동범위 및 통증에 미치는 영향 -열린 홀딩과 닫힌 홀딩의 차이- (The Effects of Proprioceptive Neuromuscular Facilitation (PNF) Using Elastic Bands on Edema, Range of Motion, and Pain in Post-Mastectomy Patients with Upper Limb Lymphedema: Differences between Open-Hand and Closed-Hand Grips)

  • Purpose: This study was conducted to examine the effects of a PNF intervention using elastic bands on edema, range of motion (ROM), and pain in post-mastectomy patients with upper limb lymphedema, according to their hand grip type. Methods: The subjects were 14 female patients who were at Stage II lymphedema after undergoing mastectomy for Stage II breast cancer. They were randomly divided into an experimental group (n =7) and a control group (n = 7). Both groups participated in a treatment program four times (one time under the therapist's instructions and three times as self-exercise) a week for 4 weeks. Both groups repeated an upper limb PNF pattern exercise for 30 min using elastic bands after receiving a manual lymph drainage treatment for 1 hour. Here, subjects in the experimental group performed the PNF exercise with an open-hand grip by putting their hands into the loops of elastic bands and keeping their fingers spread out. Subjects in the control group performed the PNF exercise with a closed-hand grip by holding the loops of elastic bands with their fingers. In both groups, the subjects' edema, ROM, and pain levels were measured before the intervention and 2, 3 and 4 weeks after the intervention. Results: As a result of the experiment, both groups demonstrated edema reductions, ROM increases, and pain reductions in four areas of the upper limbs. Notably, the experimental group exhibited larger ROM increases in flexion, extension, and abduction as well as greater edema and pain reductions than the control group. In particular, the most significant effects were found in the elbow of the four upper limb areas for edema reductions and in extension for ROM increases. Conclusion: The present study indicates that exercise therapy-based approaches using elastic bands in post-mastectomy patients with upper limb lymphedema can have different effects depending on the type of hand grip (open or closed), which is the body's most distal part. Therefore, these approaches should be based on the conditions of the distal parts of the patient's body for their effective applications in clinical practice.

운동 후 극저온 냉각치료 적용이 운동유발성 근육 손상의 최대등척성근력과 관절가동범위에 미치는 효과 (The effect of whole-body cryotherapy intervention after an exercise on MVIC and ROM of EIMD)

  • Background: The purpose of this study was to investigate the effects of WBC (whole-body cryotherapy)on the MVIC (maximal voluntary isometric contraction)and active ROM (range of motion) recovery of after EIMD (exercise-induced muscle injury). Design: Randomized Controlled Trial. Methods: Thirty subjects who are student in their 20s at a university participated in this study, these subjects were assigned into three groups, a control group (n=10), experiment group I(n=10), and experiment group II (n=10). The subjects in the experimental group II were intervened by WBC (-130℃, 3 minutes) before induced EIMD, the experimental group were intervened by WBC (-130℃, 3 minutes) after induced EIMD, and the control group weren't by any intervened after induced EIMD. Results: First, In the comparison of the MVIC, there were significant variations with the lapse the time in three groups (p<.001) and there was a significant interaction of time and group (p<.001). In the among group comparison, the MVIC of experimental group II was significantly larger than those of other groups (p<.001). Second, In the comparison of the active extension angle, there were significant variations with the lapse the time in three groups (p<.001) and there was a significant interaction of time and group (p<.001). In the among group comparison, the active extension angle of experimental group II was significantly smaller than those of other groups (p<.001). Third, In the comparison of the active flexion angle, there were significant variations with the lapse the time in three groups (p<.01) and there was a significant interaction of time and group (p<.001). In the among group comparison, the active flexion angle of experimental group II was significantly larger than those of other groups (p<.05). Conclusion: The above results revealed that the WBC intervention after an exercise had a positive effect of muscle function after EIMD. Therefore we can consider the WBC as a considerable intervention method to prevent or reduce an exercise injury.
