The Effects of Foot Reflexology on Depression and Anxiety in Coal Workers' Pneumoconiosis (발반사 요법이 탄광부 진폐증 환자의 우울과 불안에 미치는 영향)

  • Purpose: This study was to evaluate the effects of foot reflexology on depression and anxiety in coal workers' pneumoconiosis. Method: The research design used was a non-equivalent control group pretest-posttest design. The foot reflexology was performed for 60 minutes twice a week through five weeks in the experimental group, not in the control group. The subjects of this study consisted of both the experimental group of twenty-nine and the control group of thirty coal workers' pneumoconiosis. The data were collected from December 10, 2002 to February 15, 2003. To evaluate the effects of foot reflexology the scores of depression and anxiety were measured before and after the experiment in both groups. Data of this experiment were analyzed by Chi-square test, t-test, unpaired t-test, ANCOVA and Repeated Measures ANOVA with the SAS Program. Results: The score of depression decreased in the experimental group, not in the control group. There was a significant difference of depression between the two groups. The score of anxiety decreased in the experimental group, not in the control group. There was a significant difference of anxiety between the two groups. Conclusion: As a summary of the study results, foot reflexology was regarded that it attributed to the emotional promotion of coal workers' pneumoconiosis. Therefore the foot reflexology is considered that might be used as an effective skill of nursing intervention program for coal workers' pneumoconiosis.

The Effects of Foot Reflexology on Fatigue and Insomnia in Patients suffering from Coal Workers' Pneumoconiosis (발반사 마사지가 탄광부 진폐증 환자의 피로와 불면증에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effects of foot reflexology on fatigue and insomnia in patients suffering from coal workers' pneumoconiosis. Method: This study was a quasi-experimental study of pre-test and post-test design in a non-equivalent control group. The subjects of this study consisted of both the experimental group of twenty-nine and the control group of thirty coal workers' pneumoconiosis patients. Data was collected from December 10, 2002 to February 15, 2003. Foot reflexology was performed for 60 minutes twice a week through five weeks in the experimental group, but none in the control group. To evaluate the effects of foot reflexology, the scores of fatigue and insomnia were measured before and after the experiment in both groups. Fatigue was evaluated by Fatigue Symptoms Inventory. Insomnia was measured with the visual analogue scale (VAS). Data of this experiment was analyzed by Chi-square test, t-test, unpaired t-test and Repeated Measures ANOVA with the SAS Program. Results: The scores of fatigue and insomnia decreased in the experimental group but not in the control group. There was a significant difference of fatigue and insomnia between the two groups. Conclusion: It is suggested that foot reflexology might have beneficial effects on reducing fatigue and insomnia in patients suffering from coal workers' pneumoconiosis, and can be recommended as a nursing intervention program for patients with coal workers' pneumoconiosis.

A Study on the Early Diagnosis of Pneumoconiosis (진폐증의 조기진단에 관한 연구)

  • In order to assess the method which is more sensitive one to detect the early change of lung tissue by the inhaled dust, we have performed the various medical examinations such as chest radiography, pulmonary function test, high resolution chest CT, brnchoalveolar lavage and lung biopsy used bronchoscope and ultrathin bronchoscopy examination to 48 persons. The control group were 8 persons who did not exposed to dust, 40 cases of the experimental group have professionally exposed to the mineral dust. The results were as follows 1. The total number of cells in bronchoalveolar lavage was significantly increased in all of the pneumoconiosis group classified by chest and high resolution chest CT. 2. The composition rate of macrophage to the total number of cells in bronchoalveolar lavage fluid was significantly decreased in all of the pneumoconiosis group compared with the control group. 3. The composition rate of neutophils and lymphocytes to the total number of cells in bronchoalveolar lavage fluid was significantly increased in all of the pneumoconiosis group compared with the control group. 4. The forced expiratory volume in one second ($FEV_{1-0}$), maximal mid-expiratory flow (MMF), and maximal voluntary ventilation (MVV) were significantly increased only in the group of the progressed pneumoconiosis relatively. 5. We observed submucosal edema, anthracotic pigmentation and granuloma formation in transbronchial lung biopsy of the suspected pneumoconiosis (category 0/1) case which is thought to the early change of coal workers' pneumoconiosis.

Factor Analysis on the Variables Related to Workers Occupationally Exposed to Mixtures of Organic Solvents (인자분석을 통한 유기용제 취급자의 자각증상 호소에 관여하는 변수에 관한 연구)

  • By using Factor analysis on the variables relating to the subjective symptoms of total 56 workers, who have exposed to mixture of organic solvents at the musical instrument manufacturing plant, as experinmental group, and 70 workers, who worked in noisy working environment as control group, the study was performed. It was confirmed that the variables found in hematological figures, age specification, occupational history and physical examination have a influential factor on the subjective symptoms of both groups. It was explicated that the ratio of these to total Eigen Values showed 31.1 percent in experimental group and the control group revealed 37.7 percent.

The Analysis of Research about Foot Reflexology in Nursing (발반사마사지에 관한 간호학 논문 분석)

  • Purpose: This study was performed to analyze the trend of foot reflexology research in nursing in Korea. Method: Of studies published in nursing literature between January 1990 and August 2007. The 74 articles were analyzed according to the published year and journal, type of research and study design, subject, and the outcome variables of interventions. Result: Prevailing research designs were experimental research (nonequivalent control group pretest-posttest design). Prevailing characteristics of subjects were patients with hypertension, diabetes mellitus, hemodialysis, cancer and others disease (osteoarthritis, cerebral vascular accident, pneumoconiosis). The most frequently performed intervention was Foot-Reflexo-Massage (FRM). The most frequently used outcome variables were fatigue and sleep. The effect of foot reflexology was inconclusive. Conclusion: The finding suggests that a robust research design in foot reflexology research is needed to accumulate a strong scientific evidence and to adopt nursing intervention from the foot reflexology modalities. Meta analysis of foot reflexology research should be done to analyze and integrate the results of various studies.

Histopathological Changes in Rat Lung Instilled with Natural Coal and Free Silica Dust (자연산석탄분진 및 유리규산분진 주입에 따른 흰쥐 폐에서의 병리조직학적 변화)

  • In order to investigate the changes of hydroxyproline amount and pathologic finding in rat lung which were instilled the natural coal and free silica dust intratracheally, the subjects were divided into two groups as follows. The control group was only administered intratracheally 0.5 ml of normal saline, and the experimental groups were instilled at once the turbid solution containing 10 mg, 30 mg and 50 mg of natural coal and free silica dust each, subjects were sacrified at the 3rd and the 20th week each after the experiment. Hydroxyproline amount in the right upper lung was measured by Woessner method and HPLC (modified Dunphy) method, and the pathlogic finding of lung tissue were observed for hematoxylin-eosin staining, Bielschowski method and Masson's trichrome method. The results were as follows. 1) The wet lung weights of all experimental groups excluding in the groups instilled 10 mg and 30 mg of natural coal dust at 3rd week, were significantly increased (p<0.05) compared with control group. The weight in each free silica group was markedly increased (p<0.05) at 20th week compared with the same dose of natural coal dust group, while the weight in the same dose group of free silica dust was increased significantly at 20th week compared with at 3rd week. 2) The amount of hydroxyproline were significantly increased (p<0.05) in the natural coal and free silica groups at 20th week compared with the control groups, and in each experimental group instilled the same kind and dose of dust, its amount was markedly increased (p<0.05) at 20th week compared with at 3rd week. And also the hydroxyproline in 30 mg and 50 mg free silica groups increased markedly (p<0.05) at 20th week compared with the natural coal dust of the same dose. 3) The polymorphonuclear leukocytes, fibroblasts and macrophages in interstitium and alveolar space showed the increasing tendency in the free silica group more markedly than in the natural coal dust group. The exudate in alveolar space at 3rd week was disappeared at 20th week, and pneumoconiotic nodules observed microscopically in all experimental groups at 20th week, while the nodules apeared already at 3rd week in the 30 mg and 50 mg free silica dust groups. The significant increase of Hydroxyproline content in lung tissue and pneumoconiotic nodule formation in experimental groups found in this study indicate that the observation period, dust amount and kind of dust is important factors associated with pneumoconiosis. And these findings were generally more severe in free silica dust groups than in natural coal groups.

Diagnostic Performance of Diffusion - Weighted Imaging for Multiple Hilar and Mediastinal Lymph Nodes with FDG Accumulation

  • Background: It is sometimes difficult to assess patients who have multiple hilar and mediastinal lymph nodes (MHMLN) with FDG accumulation in PET-CT. Since it is uncertain whether diffusion-weighted magnetic resonance imaging (DWI) is useful in the assessment of such patients, its diagnostic performance was assessed. Materials and Methods: Twenty-three patients who had three or more stations of hilar and mediastinal lymph nodes with SUVmax of 3 or more in PET-CT were included in this study. Results: For diagnosis of disease, there were 20 malignancies (lung cancers 17, malignant lymphomas 2 and metastatic lung tumor 1), and 3 benign cases (sarcoidosis 2 and benign disease 1). For diagnosis of lymph nodes, there were 7 malignancies (metastasis of lung cancer 7 and malignant lymphoma 1) and 16 benign lymphadenopathies (pneumoconiosis/silicosis 7, sarcoidosis 4, benign disease 4, and atypical lymphocyte infiltration 1). The ADC value ($1.57{\pm}0.29{\times}10^{-3}mm^2/sec$) of malignant MHMLN was significantly lower than that ($1.99{\pm}0.24{\times}10^{-3}mm^2/sec$) of benign MHMLN (P=0.0437). However, the SUVmax was not significantly higher ($10.0{\pm}7.34$ as compared to $6.38{\pm}4.31$) (P=0.15). The sensitivity (86%) by PET-CT was not significantly higher than that (71%) by DWI for malignant MHMLN (P=1.0). The specificity (100%) by DWI was significantly higher than that (31%) for benign MHMLN (P=0.0098). Furthermore, the accuracy (91%) with DWI was significantly higher than that (48%) with PET-CT for MHMLN (P=0.0129). Conclusions: Evaluation by DWI for patients with MHMLN with FDG accumulation is useful for distinguishing benign from malignant conditions.
