On-X$^{circledR}$ 기계판막을 이용한 대동맥판 치환술의 조기 임상 경험 (Early Clinical Experience in Aortic Valve Replacement Using On-X$^{circledR}$Prosthetic Heart Valve)

  • On-X 기계판막은 1997년 처음으로 사용된 이후 전 세계적으로 사용빈도가 증가하고 있다. 본 연구는 이 새로운 기계판막에 대한 유용성과 안전성, 그리고 술 후 혈역학적 변화에 대해 알아보고자 시행되었다. 대상 및 방법: 1999년 4월부터 2002년 8월까지 전남대학교병원 흉부외과에서 On-X 판막을 사용하여 대동맥판막 치환술을 시행했던 52명의 환자를 대상으로 분석하였다. 환자 중 52%가 남자였으며 평균연령은 50$\pm$13세였다. 연구는 판막수술 후 결과보고에 대한 AATS/STS 지침에 따라 분석하였다. 술 전 뉴욕 심장병학회의 기능분류상 class III 이상의 환자가 32명(61.5%)이였으며, 2명의 환자는 이전에 대동맥판막 수술을 받았던 환자였다. 71.1%의 환자에서 동반된 질환에 의해 대동맥판막 치환술과 함께 동반 수술을 시행하였고, 이식된 판막의 크기는 19 mm 13명, 21 mm 26명, 23 mm 10명과 25 mm 3명이었다 평균 외래 추적관찰 기간은 16.6$\pm$10.5 (1∼39)개월이었다. 수술 전과 수술 직후 그리고 3, 6, 12개월에 심초음파를 반복 시행하여 술 후 혈역학적 변화양상을 분석하였다. 결과: 평균 체외순환시간과 대동맥 차단시간은 각각 191$\pm$94.7분과 142$\pm$51.7분이었다. 술 후 조기 및 만기 사망은 없었다. 술 후 12개월간 event-free rate는 혈전색전증 95.6$\pm$6%; 출혈성 합병증 90.2$\pm$4%: 판막 주위누출 92.3$\pm$4%이었으며 전체적인 event-free rate는 76.6$\pm$3%이었다. 수술 후 판막혈전이나 심내막염, 구조적 판막부전은 발생하지 않았다. 술 후 12개월의 평균심박출량은 62.7$\pm$9.8%로 수술 전(55.8$\pm$15.9%)에 비해 의미 있는 증가를 보였다(p=0.006). 좌심실 체적지수와 최대 판막압력차는 수술 전에 247.3$\pm$122.3 g/$\textrm{cm}^2$, 62.5$\pm$38.0 mmHg을 보였으나 술 후 12개월에 각각 155.5$\pm$58.2 g/$\textrm{cm}^2$(p=0.002), 18.0$\pm$10.8 mmHg (p<0.0001)로 감소하는 소견을 보였다. 결론: On-X 기계판막을 이용한 대동맥판막 치환술은 임상적 및 혈역학적으로 만족스러운 조기 결과를 보여주었다. 그러나 대상 환자들에 대한 장기간의 외래 추적관찰과 함께 기존 판막과의 비교가 이루어짐으로써 이 새로운 판막에 대한 유용성과 안정성이 확립될 수 있을 것으로 생각한다.

삶의 여정을 통한 만화 히어로 성장유형 연구 (A Study on Growth Type of Comic strips Heroes through Journey of Life)

  • 긴 이야기구조의 기승전결인 네 국면의 플롯은 패턴화 되어 도식화된 경향이 있다. 인물의 행동에는 인류의 시작과 궤를 같이 하게 되는데 장편연재만화 플롯은 기본적으로 4국면이지만, 단순 배열이 아닌 인류의 추진력이나 행동과 느낌의 패턴을 정리하여 개발한 복잡 배열이다. 이 연구는 만화전문대학생 설문과 답변으로 결과지를 얻은 후, 통합적으로 추출된 장편연재만화작품의 설문 자료를 통하여 장편연재만화 아리스토텔레스 이론인 플롯의 국면, 네 단계와 캐롤 피어슨의 인물원형이론을 통한 4개의 인물의 유형화, 조셉 캠벨의 영웅의 여정이론을 네 단계로 유형화한다. 복잡한 장편연재만화의 이야기 구조가 가진 요소 중 하나인, 인물이 겪는 삶의 과정을 제시하여 간략성과 체계적 기술을 통해 유형화하는 이 연구는 결국 만화작가가 장편연재만화의 복잡하고 긴 이야기의 구조에서 은유하는 것은 무엇인가를 밝히고자함이다. 아리스토텔레스의 플롯이론을 근거로 하는 도입, 발전, 전개 결말구조의 의미가 인류의 삶과 운명의 메타포이며, 융 이론에 영향 받은 융 연구가 캐롤 피어슨의 인물 원형론의 성장단계가 인류의 삶과 운명의 메타포임을, 역시 융 이론에 영향 받은 인류학자 조셉 캠벨 이론들은 희노애락의 복합감정을 영웅원형에 투사하여 영웅의 여정이란 은유를 통해 인류의 삶과 운명의 메타포임을 밝히고 있다. 최종적으로, 제기된 이론들은 크리스토퍼 보글러의 저서에 작가의 '시나리오작성법'으로 접근하여 소개되고 있다. 한편으로 이 메타포는 이 연구목표이자 목적이기도 하다. 연구에서 추출된 장편연재만화는 긴 이야기가 복잡하고 얽힌 듯이 보여도 인물이 집을 떠나 모험과 난관을 겪고 세계와의 또 다른 만남을 통해 거부와 갈등, 그리고 경쟁과 전쟁, 시련을 겪으며 보상을 얻어 귀환하는 여정의 과정 중 정신적, 심리적, 상징적 성장으로 결국, 영웅으로 획득되며 만화라는 이미지를 통하여 우리의 내면 성찰에 대한 의미 부여를 플롯에 의해 서사적으로 표현하고 있다. 복잡해보이지만 장편연재만화이야기의 기본구조가 플롯 4단계이며 장편연재만화작품에서 범상치 않은 인물이 여행과 모험을 떠나는 과정에서 성장하는 삶의 방식을 크게 나누어 볼 때, 네 단계의 영유아동기, 청년기, 중년기, 말년기의 상징성을 지닌 심리적 성장과정이며, 인물이 유형화된 특성도 크게 네 단계의 부류로 나누어 성장과정을 설명할 수 있게 한다. 성장할 인물의 표상화인 영웅의 여정, 역시 크게 네 단계의 여정으로 압축되어 설명된다. 즉 이론을 통하여 만화의 4국면의 플롯의 복잡 배열은 삶의 순서를 통한 성장구조의 진행과의 흐름이 일치한다는 것을 발견하였다. 동시에 인물이 영웅으로 거듭나는 성찰의 메타포이며 장편연재만화작가들의 작품을 통하여 인생국면 4단계인 인물의 성장과 삶을 통찰하는 은유를 발견하였다. 만화작가가 의도하는 장편연재만화작품의 긴 이야기란, 플롯의 논리적구성법에 의해 인물이 여정을 떠나 역경과 어려움을 겪으며 삶이 변화되고 그의 성장과정은 유형화된 이미지로 제시하여 성찰에 도달함을 영웅적으로 표현하는 것이며, 독자는 삶과 인생에서 만화이야기의 이미지의 시공간을 공유하는 동시에 감동을 통하여 정서상 인물과 같은 체험을 하게 되었음을 인식하게 한다.

한국인(韓國人) 중고교생(中高校生)들의 흡연실태(吸煙實態)에 관(關)한 연구 (A Study on the Smoking Status of the Korean Middle and High School Students)

  • I investigated actual conditions of smoking of teenagers who were randomly chosen middle and high school students. 1. Juvenile smoking 1) Parents' opinions of juvenile smoking Most parents do not want their children to smoke after growth : 88.6% of fathers (middle school students: 88.9%, high school students: 88.4%) and 95.1% of mothers (middle school students: 93.4%, high school students :95.5%). 2) Teenagers' opinions of smoking after growth The rate of students who will smoke after growth is 10.8% (middle school students: 12.0%, high school students: 9.9%): students in agricultural areas show the higher rate than those in cities. 3) Parents' opinions of their children's smoking now 1.5% of fathers want their children to smoke now (middle school students: 1.3%, high school students: 1.6%) and 1.1% of mothers do (middle school students: 0.6%, high school students: 1.5%). This shows that most parents do not want their children to smoke now. 4) Students' opinions of their friends' smoking now Students who want their friends smoke now cover 7.8% (middle school students: 7.1%, high school students: 8.4%). This rates are higher than those of parents shown in (3). And more high school students and more girl students gave the positive reponse than middle school boy and girl students, respectively. 5) Students' views of smoking "Look like an adult" covers the rate of 4.0% (boy: 7.8%, girl:3.6%) 6.7% of middle school students have this view, while 3.7% of high school students have. 16.1% of students had an experience of smoking during the last one year (boy: 29.9%, girl: 8.6%): this shows that the rate of the boy students is more than 3 times greater than that of the girl students and high students who experienced smoking last year covers 20.2%, while middle school students shows 10.9%. 6) Actual conditions of students' smoking The present rate of students' smoking is 22.4% (boy:38.3%, girl:13.8%): the rate of boy students is greater than that of girl students. Students who smoke more than pack of cigarettes a day cover 8.2% (boy: 17.5%, girl: 3.2%): 5.2% of middle school students (boy:11.4%, girl: 2.1%) smoke more than one pack while 10.7% of high school students do (boy:21.5%, girl: 4.2%). This shows that the rate of boy students' smoking is greater than that of girl students' smoking. 7) The rate of smoking of students' parents 75.4% of fathers (city: 74.5%, agricultural area:75.9%) smoke: and more than a half (62.4%) smoke more than a pack cigarettes a day. On the other hand, the rate of smoking mothers is 5.2%(city: 4.3%, agricultural area: 7.3%): the rate is higher in agricultural areas. 8) Opinions of smoking population in the future 61.4% of students answered that smoking population will increase, while 27.0% have the opinion that smoking population will decrease. 2. Opinions of the effects of smoking on health 1) Have you heard that smokers are likely to suffer from tuberclosis? 78.3% of students said yes (boy: 80.8%, girl: 76.4%): it is shown that the rate of boys is greater than that of girls. 2) Have you heard that smokers are likely to get out of endurance? 76.6% of students (boy: 69.3%, girl: 49.7%) answered yes: it is shown that the rate of boys is greater than that of girls. 3) Have you heard that heart-beats get fast when one smokes? 32.5% of students (boy: 35.5%, girl: 30.9%) answered yes: 32.2% in cities(boy: 33.0%, girl: 31.8%) and 33.5% in agricultural areas(boy: 41.8%, girl: 28.8%): and 28.7% middle students and 35.5% of high school students answered yes. 4) Have you heard that smokers are likely to have heart-diseases? 35.1% of students (boy: 34.0%, girl: 34.1%) answered yes: 35.3% in cities (boy: 37.2%, girl: 34.2%) and 36.7% in agricultural areas (boy: 39.0%, girl: 33.9%): 34.8% of middle school students and 35.4% of high school students. 5) Have you heard that smokers are likely to have a lung cancer? 91.4% of students (boy: 93.2%, girl: 89.9%) answered yes: 90.35% in cities and 94.2% in agricultural areas. 6) Have you heard that the life of smokers gets shorter? 94.3% of students (boy:94.6%, girl: 92.2%) answered yes. 7) Have you heard that pregnant smokers will deliver a baby with low birth weight? 29.6% of students (boy: 29.8%, girl: 29.4%) answered yes: the rates of boys and girls almost the same. 8) Have you heard that one feels calm when one smokes? 80.1% of students (boy: 81.8%, girl: 79.2%) answered yes: boys and girls showed almost the same rate. 3. Preventive measures Smoking people continued to increase all over the world because smoking not only mitigated emotional uneasiness such as loneliness, nervousness and so on, but also could be very helpful from the social perspective. This was so because they did not consider harmful effects of smoking on health, and victims. However, because any -one can have physical disorders caused by smoking, people should always keep in mind the following preventive measures. 1) Doctors or teachers should set an example of giving up smoking. Informing patients or students of harmful effects of smoking to persuade their family and relatives not to smoke. 2) Through mass media like newspapers, periodicals or broadcasting, to make people know harmful effects of smoking and not smoke. 3) To prohibit selling teenagers cigarette by law. 4) To prohibit smoking in public places like work places, offices, lecture rooms, recreation rooms, buses, trains and so on. 5) To decrease the rate of life insurance for non-smokers as in foreign countries and to give a warming of the harmful effects on cigarette packets or ads.

해외 취업 및 인턴쉽에 대한 인식과 만족도에 관한 연구 -조리 및 외식관련 전공자를 대상으로- (Evaluation of the Perception and Satisfaction of Working and Internship Abroad -By Undergraduates Studying in Culinary and Foodservice Departments-)

  • 조리 및 외식 관련 전공자 가운데 해외 취업과 인턴쉽에 경험이 있는 사람을 대상으로 해외 취업 및 인턴쉽에 대한 인식과 만족도를 조사하였으며, 그 결과는 다음과 같다. 1. 조사 대상자는 남자가 50.9%, 여자가 49.1%이었으며, 20대가 주를 이루었다. 응답자의 42.1%는 해외 취업을, 57.9%는 인턴쉽을 경험하였으며, 체류 기간은 1년 미만이 42.1%로 가장 많았다. 2. 해외 취업 또는 인턴쉽을 하게 된 동기는 남녀 모두 다양한 해외 문화 체험과 견문 확대의 기회를 꼽았으며, 종사 업무는 남녀 모두 양식 분야가 54.4%로 가장 많았다. 3. 해외 취업 또는 인턴쉽을 희망하는 후배들에게 권하고 싶은 취업 분야는 남자의 경우 51.7%가 양식을, 37.9%는 일식을 권장한 반면 여자의 경우는 양식에 대해서는 46.4%가, 일식에 대해서는 7.1%가 권장하여 유의적인 차이를 보였다(p<0.001). 또한, 후배들에게 권하고 싶은 준비 항목으로 남녀 모두(72.4%, 60.7%) 언어 능력을 가장 많이 선택하였으며, 다음은 새로운 환경에 대한 적응력(남녀 각각 10.3%, 32.1%)으로 남녀간 유의한 차이가 나타났다(p<0.01). 4. 해외 취업 또는 인턴쉽 수행에 대한 인식을 5점 Likert법으로 조사한 결과, 남자는 '나는 현재의 업무를 계속하고 싶다(4.21)'가 가장 높았으며, 다음은 ‘나의 업무는 매우 가치가 있다(4.14)’로 나타났다. 여자는 '주방의 동료 및 선배와의 관계가 원활하다(4.11)'가 가장 높은 점수이었고, 다음은 '나는 업무 외의 해외 생활에 만족한다(4.00)'와 '나는 현재의 업무를 계속하고 싶다(4.00)'로 나타났으며, 전체 평균은 남자가 3.84와 여자가 3.78로 비교적 높은 인식 점수를 보였다. 5. 해외 취업 및 인턴 업무 수행에 대한 중요도 및 만족도를 5점 Likert법으로 조사한 결과, 중요도의 경우, 남자는 '부서간의 협력(4.52)', '언어(4.38)', '주방 환경(4.34)'을, 여자는 '언어(4.36)', '대인관계(4.21)' '급여(4.18)'를 매우 중요하게 생각하였다. 한편, 만족도의 경우는 남자는 '업체의 규모(4.28)', '주방 환경(4.21)', '업무시간(4.10)'을, 여자는 '주방 환경'과 '인센티브(4.14)', '대인관계(4.11)'를 만족스럽게 생각하였다. 중요도에 대한 전체적인 반응은 남자가 4.18, 여자가 4.06인 것에 비해 만족도에 대한 전체적인 생각은 남자가 3.75, 여자가 3.78로 나타나 중요하다고 생각하는 것에 비해 만족도는 상대적으로 낮음을 알 수 있었다. 본 연구는 조리 및 외식 관련 전공자들 중 해외 취업과 해외 인턴 경험자를 추적하기가 쉽지 않았던 관계로 표본수가 제한적이어서 그 결과를 일반화하는데 다소 무리가 있음을 간과할 수 없으나, 본 자료를 근거로 이들 전공자에 맞는 맞춤형 인턴 제도의 개발과 이를 통한 취업의 활로 개척이 시급함을 엿볼 수 있었다. 부분적으로 나마 이를 해소할 수 있는 방안으로는 조리 및 외식 관련 분야 인턴의 선발, 교육, 업체 확보, 관리 등 인턴 업무에 관한 모든 업무를 총괄하여 처리할 수 있는 센터나 기구의 설립이 그 예가 될 수 있을 것이다. 지금까지 주로 민간업체의 주도하에 인턴 파견 업무 알선을 이용함으로써 대행료 등의 부담스러운 비용 문제 발생과 신뢰성 여부의 부담을 주기는 하였지만, 조리 및 외식업계의 해외 진출은 거의 이들 사설업체의 기여도가 높은 편이다. 그러나 앞으로는 노동부와 한국산업인력공단이 함께 운영하는 월드 잡(world job) 사이트를 활용하면 보다 쉽고 보다 유리한 조건으로 해외 취업의 길이 열릴 것이므로 이에대한 적극적인 홍보가 필요하다. 또한, 자료를 통해 언급했던 2015년까지 정부 주도하의 글로벌 청년 리더 양성 프로그램을 이용하여도 좋은 기회가 될 것이다. 다만, 현장 경력이나 기본적인 의사 소통을 위한 외국어의 준비도 함께 병행해야함은 물론이다. 그리고 본 설문 조사에서 낮은 점수로 언급된 것과는 다르게 한식 자격증을 기본적으로 갖추는 것이 필요하다고 권장하고자 한다. 그 이유는 한국산업인력공단의 해외 취업고를 살펴보면 실제로 외국 주재 대사관이나 해외 진출 건설업계의 급식 시설에 파견할 파견 조리사중에는 한식 조리사를 찾는 비중이 높기 때문이다. 2005년 해외 취업 가이드에서 언급되었듯이 한식 분야에 대한 현지의 수요가 점차 늘어나는 추세인 만큼 이에 대한 새로운 각오로 임하는 것이 무엇보다도 필요하며, 드라마를 통한 한류 바람을 타고 일본, 중국을 비롯한 동남아 권에도 한식 열풍이 일기 시작하고 있으므로 한식 조리사에 대한 수요가 많아질 전망이다. 따라서 이런 현상에 대한 실제적인 수요 조사를 좀 더 구체적으로 실시한 후, 현지화 전략이 가능한, 한국 음식에 관한 제대로 된 조리교육을 받은 조리사 양성이 필요할 것으로 판단된다.

UCC(user-created-contents) 웹 사이트에서 사용자의 인성이 감정적, 인지적 평가와 UCC 활용에 미치는 영향 (The Effect of Users' Personality on Emotional and Cognitive Evaluation in UCC Web Site Usage)

  • The research conducted here focuses on the effect of factors that affect the behavior of UCC (User Created Content) website users, other than user's rational recognition of how useful a UCC website can be. Most discussions in the existing literature on information systems have focused on users' evaluation how a UCC website can help to attain the users' own goals. However, there are other factors and this research pays attention to an individual's 'personality,' which is stable and biological in nature. Specifically, I have noted here that 'extroversion' and 'neuroticism,' the two common personality factors presented in Eysenck's most representative 'EPQ Model' and 'Big Five Model,' are the two personality factors that affect a site's 'usefulness,' by this I mean how useful does the user consider the website and its content. How useful a site is considered by the user is the other factor that has been regarded as the antecedent factor that influences the adoption of information systems in the existing MIS (Management Information System) research. Secondly, as using or creating a UCC website does not guarantee the user's or the creator's extrinsic motivation, unlike when using the information system within an organization, there is a greater likelihood that the increase in user's activities in relation to a UCC website is motivated by emotional factors rather than rational factors. Thus, I have decided to include the relationship between an individual's personality and what they find pleasurable in the research model. Thirdly, when based on the S-O-R Paradigm of Mehrabian and Russell, the two cognitive factors and emotional factors are finally affected by stimulus, and thus these factors ultimately have an effect on an individual's respondent behavior. Therefore, this research has presented an assumption that the recognition of how useful the site and content is and what emotional pleasure it provides will finally affect the behavior of the UCC website users. Finally, the relationship between the recognition of how useful a site is and how pleasurable it is to useand UCC usage may differ depending on certain situational conditions. In other words, the relationship between the three factors may vary according to how much users are involved in the creation of the website content. Creation thus emerges as the keyword of UCC. I analyzed the above relationships through the moderating variable of the user's involvement in the creation of the site. The research result shows the following: When it comes to the relationship between an individual's personality and what they find pleasurable it is extroverted users who have a greater likelihood to feel pleasure when using a UCC website, as was expected in this research. This in turn leads to a more active usage of the UCC web site because a person who is an extrovert likes to spend time on activities with other people, is sensitive to new experiences and stimuli and thus actively responds to these. An extroverted person accepts new UCC activities as part of his/her social life, rather than getting away from this new UCC environment. This is represented by the term 'Foxonomy' where the users meet a variety of users from all over the world and contact new types of content created by these users. However, neuroticism creates the opposite situation to that created by extroversion. The representative symptoms of neuroticism are instability, stress, and tension. These dispositions are more closely related to stress caused by a new environment rather than this creatingcuriosity or pleasure. Thus, neurotic persons have an uneasy feeling and will eventually avoid the situation where their own or others' daily lives are frequently exposed to the open web environment, this eventually makes them have a negative attitude towards the web environment. When it comes to an individual's personality and how useful site is, the two personality factors of extroversion and neuroticism both have a positive relationship with the recognition of how useful the site and its content is. The positive, curious, and social dispositions of extroverted persons tend to make them consider the future usefulness and possibilities of a new type of information system, or website, based on their positive attitude, which has a significant influence on the recognition of how useful these UCC sites are. Neuroticism also favorably affects how useful a UCC website can be through a different mechanism from that of extroversion. As the neurotic persons tend to feel uneasy and have much doubt about a new type of information system, they actively explore its usefulness in order to relieve their uncomfortable feelings. In other words, neurotic persons seek out how useful a site can be in order to secure their own stable feelings. Meanwhile, extroverted persons explore how useful a site can be because of their positive attitude and curiosity. As a lot of MIS research has revealed that the recognition of how useful a site can be and how pleasurable it can be to use have been proven to have a significant effect on UCC activity. However, the relationship between these factors reveals different aspects based on the user's involvement in creation. This factor of creationgauges the interest of users in the creation of UCC contents. Involvement is a variable that shows the level of an individual's mental effort in creating UCC contents. When a user is highly involved in the creation process and makes an enormous effort to create UCC content (classed a part of a high-involvement group), their own pleasure and recognition of how useful the site is have a significantly higher effect on the future usage of the UCC contents, more significantly than the users who sit back and just retrieve the UCC content created by others. The cognitive and emotional response of those in the low-involvement group is unlikely to last long,even if they recognize the contents of a UCC website is pleasurable and useful to them. However, the high-involvement group tends to participate in the creation and the usage of UCC more favorably, connecting the experience with their own goals. In this respect, this research presents an answer to the question; why so many people are participating in the usage of UCC, the representative form of the Web 2.0 that has drastically involved more and more people in the creation of UCC, even if they cannot gain any monetary or social compensation. Neither information system nor a website can succeed unless it secures a certain level of user base. Moreover, it cannot be further developed when the reasons, or problems, for people's participation are not suitably explored, even if it has a certain user base. Thus, what is significant in this research is that it has studied users' respondent behavior based on an individual's innate personality, emotion, and cognitive interaction, unlike the existing research that has focused on 'compensation' to explain users' participation with the UCC website. There are also limitations in this research. Firstly, I divided an individual's personality into extroversion and neuroticism; however, there are many other personal factors such as neuro-psychiatricism, which also needs to be analyzed for its influence on UCC activities.   • 1. The 'Kao Zheng Pai(考證派) comes from the 'Zhe Zhong Pai' and is a school that is influenced by the confucianism of the Qing dynasty. In Japan Inoue Kinga(井上金娥), Yoshida Koton(吉田篁墩) became central members, and the rise of the methodology of historical research(考證學) influenced the members of the 'Zhe Zhong Pai', and the trend of historical research changed from confucianism to medicine, making a school of medicine based on the study of texts and proving that the classics were right. 2. Based on the function of 'Nei Qu Li '(內驅力) the 'Kao Zheng Pai', in the spirit of 'use confucianism as the base', researched letters, meanings and historical origins. Because they were influenced by the methodology of historical research(考證學) of the Qing era, they valued the evidential research of classic texts, and there was even one branch that did only historical research, the 'Rue Xue Kao Zheng Pai'(儒學考證派). Also, the 'Yi Xue Kao Zheng Pai'(醫學考證派) appeared by the influence of Yoshida Kouton and Kariya Ekisai(狩谷掖齋). 3. In the 'Kao Zheng Pai(考證派)'s theories and views the 'Yi Xue Kao Zheng Pai' did not look at medical scriptures like the "Huang Di Nei Jing"("黃帝內經") and did not do research on 'medical' related areas like acupuncture, the meridian and medicinal herbs. Since they were doctors that used medicine, they naturally were based on 'formulas'(方劑) and since their thoughts were based on the historical ideologies, they valued the "Shang Han Ja Bing Lun" which was revered as the 'ancestor of all formulas'(衆方之祖). 4. The lives of the important doctors of the 'Kao Zheng Pai' Meguro Dotaku(目黑道琢) Yamada Seichin(山田正珍), Yamada Kyoko(山田業廣), Mori Ritsi(森立之) Kitamura Naohara(喜多村直寬) are as follows. 1) Meguro Dotaku(目黑道琢 1739${\sim}$1798) was born of lowly descent but, using his intelligence and knowledge, became a professor as a Shi Jing Yi(市井醫) and as a professor for 34 years at Ji Shou Guan mastered the "Huang Di Nei Jing" after giving over 300 lectures. Since his pupil, Isawara Ken taught the Lan Men Wu Zhe(蘭門五哲) and Shibue Chusai, Mori Ritsi(森立之), Okanishi Gentei(岡西玄亭), Kiyokawa Gendoh(淸川玄道) and Yamada Kyoko(山田業廣), Meguro Dotaku is considered the founder of the 'Yi Xue Kao Zheng Pai'. 2) The family of Yamada Seichin(山田正珍 1749${\sim}$1787) had been medical officials in the Makufu(幕府) and the many books that his ancestors had left were the base of his art. Seichin learned from Shan Ben Bei Shan(山本北山), a 'Zhe Zhong Pai' scholar, and put his efforts into learning, teaching and researching the "Shang Han Lun"("傷寒論"). Living in a time between 'Gu Fang Pai'(古方派) member Nakanishi Goretada(中西惟忠) and 'Kao Zheng Pai' member Taki Motohiro(多紀元簡), he wrote 11 books, 2 of which express his thoughts and research clearly, the "Shang Han Lun Ji Cheng"("傷寒論集成") and "Shang Han Kao"("傷寒考"). His comparison of the 'six meridians'(3 yin, 3 yang) between the "Shang Han Lun" and the "Su Wen Re Lun"("素問 熱論) and his acknowledgement of the need and rationality of the concept of Yin-Yang and Deficient-Replete distinguishes him from the other 'Gu Fang Pai'. Also, his dissertation of the need for the concept doesn't use the theories of latter schools but uses the theory of the "Shang Han Lun" itself. He even researched the historical parts, such as terms like 'Shen Nong Chang Bai Cao'(神農嘗百草) and 'Cheng Qi Tang'(承氣湯) 3) The ancestor of Yamada Kyoko(山田業廣) was a court physician, and learned confucianism from Kao Zheng Pai 's Ashikawa Genan(朝川善庵) and medicine from Isawa Ranken and Taki Motokata(多紀元堅), and the secret to smallpox from Ikeda Keisui(池田京水). He also diagnosed and prescribed for the prince Ming Gong(明宮). He usually analyzed the 'symptoms' of diseases but the composition, measurement, processing and application of medicine were all in the spectrum of 'analystic research' and 'researching analysis'. 4) The ancestors of Mori Rits(森立之 1807${\sim}$ 1885) were warriors but he became a doctor by the will of his mother, and he learned from Shibue Chosai(澁江抽齋) and Isawaran Ken and later became a pupil of Shou Gu Yi Zhai, a historical research scholar. He then became a lecturer of medical herbs at the Yi Xue Guan, and later participated in the proofreading of "Yi Xin Fang"("醫心方") and with Chosai compiled the "Jing Ji Fang Gu Zhi"("神農本草經"). He visited the Chinese scholar Yang Shou Jing(楊守敬) in 1881 and exchanged books and ideas. Of his works, there are the collections(輯複本) of "Shen Nong Ben Cao Jing"(神農本草經) and "You Xiang Yi Hwa"("遊相醫話") and the records, notes, poems, and diaries such as "Zhi Yuan Man Lu"("枳園漫錄") and "Zhi Yuan Sui Bi"("枳園隨筆") that were not published. His thoughts were that in restoring the "Shen Nong Ben Cao Jing", "the herb to the doctor is like the "Shuo Wen Jie Zi"("說文解字") to the scholar", and he tried to restore the ancient herbal text using knowledge of medicine and investigation(考據). Also with Chosai he compiled the "Jing Ji Fang Gu Zhi"("經籍訪古志") using knowledge of ancient text. Ritzi left works on pure investigation, paid much attention to social problems, and through 12 years of poverty treated all people and animals in all branches of medicine, so he is called a 'half confucianist half doctor'(半儒半醫). 5) Kitamurana Ohira(喜多村直寬 1804${\sim}$1876) learned scriptures and ancient texts from confucian scholar Asaka Gonsai, and learned medicine from his father Huai Yaun(槐園). He became a teacher in the Yi Xue Guan in his middle ages, and to repay his country, he printed 266 volumes of "Yi Fang Lei Ju("醫方類聚") and 1000 volumes of "Tai Ping Yu Lan"("太平禦覽") and devoted it to his country to be spread. His works are about 40 volumes including "Jin Qui Yao Lue Shu Yi" and "Lao Yi Zhi Yan" but most of them are researches on the "Shang Han Za Bing Lun". In his "Shang Han Lun Shu Yi"("傷寒論疏義") he shows the concept of the six meridians through the Yin-Yang, Superficial or internal, cold or hot, deficient or replete state of diseases, but did not match the names with the six meridians of the meridian theory, and this has something in common with the research based on the confucianism of Song(宋儒). In clinical treatment he was positive toward old and new methods and also the experience of civilians, but was negative toward western medicine. 6) The ancestor of the Taki family Tanbano Yasuyori(丹波康賴 912-955) became a Yi Bo Shi(醫博士) by his medical skills and compiled the "Yi Xin Fang"("醫心方"). His first son Tanbano Shigeaki(丹波重明) inherited the Shi Yao Yuan(施藥院) and the third son Tanbano Masatada(丹波雅忠) inherited the Dian You Tou(典藥頭). Masatada's descendents succeeded him for 25 generations until the family name was changed to Jin Bao(金保) and five generations later it was changed again to Duo Ji(多紀). The research scholar Taki Motohiro was in the third generation after the last name was changed to Taki, and his family kept an important part in the line of medical officers in Japan. Taki Motohiro(多紀元簡 1755-1810) was a teacher in the Yi Xue Guan where his father was residing, and became the physician for the general Jia Qi(家齊). He had a short temper and was not good at getting on in the world, and went against the will of the king and was banished from Ao Yi Shi(奧醫師). His most famous works, the "Shang Han Lun Ji Yi" and "Jin Qui Yao Lue Ji Yi" are the work of 20 years of collecting the theories of many schools and discussing, and is one of the most famous books on the "Shang Han Lun" in Japan. "Yi Sheng" is a collection of essays on research. Also there are the "Su Wen Shi"("素問識"), "Ling Shu Shi"("靈樞識"), and the "Guan lu Fang Yao Bu"("觀聚方要補"). Taki Motohiro(多紀元簡)'s position was succeeded by his third son Yuan Yin(元胤 1789-1827), and his works include works of research such as "Nan Jing Shu Jeng"("難經疏證"), "Ti Ya"("體雅"), "Yao Ya"("藥雅"), "Ji Ya"("疾雅"), "Ming Yi Gong An"("名醫公案"), and "Yi Ji Kao"("醫籍考"). The "Yi Ji Kao" is 80 volumes in length and lists about 3000 books on medicine in China before the Qing Dao Guang(道光), and under each title are the origin, number of volumes, state of existence, and, if possible, the preface, Ba Yu(跋語) and biography of the author. The younger sibling of Yuan Yin(元胤 1789-1827), Yuan Jian(元堅 1795-1857) expounded ancient writings at the Yi Xue Guan only after he reached middle age, was chosen for the Ao Yi Shi(奧醫師) and later became a Fa Yan(法眼), Fa Yin(法印) and Yu Chi(樂匙). He left about 15 texts, including "Su Wen Shao Shi"("素間紹識"), "Yi Xin Fang"("醫心方"), published in school, "Za Bing Guang Yao"("雜病廣要"), "Shang Han Guang Yao"(傷寒廣要), and "Zhen Fu Yao Jue"("該腹要訣"). On the Taki family's founding and working of the Yi Xue Guan Yasuka Doumei(失數道明) said they were "the people who took the initiative in Edo era kampo medicine" and evaluated their deeds in the fields of 'research of ancient text', 'the founding of Ji Shou Guan and medical education', 'publication business', 'writing of medical text'. 5. The doctors of the 'Kao Zheng Pai ' based their operations on the Edo Yi Xue Guan, and made groups with people with similar ideas to them, making a relationship 'net'. For example the three families of Duo Ji(多紀), Tang Chuan(湯川) and Xi Duo Cun(喜多村) married and adopted with and from each other and made prefaces and epitaphs for each other. Thus, the Taki family, the state science of the Makufu, the tendency of thinking, one's own interests and glory, one's own knowledge, the need of the society all played a role in the development of kampo medicine in the 18th and 19th century.

  • 《해악전도첩(海嶽全圖帖)》은 금강산과 해금강, 관동팔경을 그린 60점의 실경산수화와 51편의 기문(記文)으로 이루어진 화첩으로 규모와 화풍에 있어 보기 드문 작품이다. 그림의 특징은 화면을 가득 채운 구성과 남종화풍을 따르면서도 거칠고 파격적인 화법, 산석(山石)의 기하학적이고 입체적인 표현 등이다. 1973년의 특별전, '한국미술이천년(韓國美術二千年)'을 처음으로 화첩의 일부만 공개되었던 작품은 2019년 국립중앙박물관 특별전 '우리 강산을 그리다: 화가의 시선, 조선시대 실경산수화'에서 그 전모가 공개되었다. 《해악전도첩》이 김응환(金應煥)(1742~1789)의 작품으로 알려진 것은 화첩의 마지막 장에 쓰여진 관지(款識)와 현재 행방이 묘연한 <칠보대>에 찍힌 '복헌'이라는 도장 때문이었다. 그러나 김응환을 지시하는 관지와 도장은 모두 후대에 더해졌을 가능성이 크다. 본 연구는 화첩을 둘러싼 여러 요소를 고찰하여 제작자를 재검토하는 것을 목적으로 한다. 《해악전도첩》 제작 배경을 이해하기 위해 먼저 18세기 금강산 기행사경도의 전통과 봉명사경을 살펴보았다. 정선(鄭敾)(1676~1759)의 《신묘년풍악도첩(辛卯年楓嶽圖)帖》(1711)을 비롯하여 심사정(沈師正)(1707~1769), 김윤겸(金允謙)(1711~1775), 최북(崔北)(1712~1786 이후), 강세황(姜世晃)(1713~1791) 등은 조선 후기 최고의 여행지였던 금강산을 유람하고 기행사경도를 제작하였다. 화가들은 이전의 전통을 계승하여 내금강의 명승명소를 주로 그렸고, 자신들이 경험한 장소를 새롭게 시각화하였다. 이러한 기행사경도는 여행을 기념하며 동행자나 후원자를 위해 여러 장면을 담을 수 있는 화첩 형식으로 제작되었다. 개별적인 금강산 기행사 경도의 제작이 증가하는 가운데 1788년에 정조(正祖)(재위 1776~1800)가 도화서 화원인 김응환과 김홍도(金弘道)(1745~1806 이후)에게 영동9군과 금강산의 명승(名勝)을 그려오도록 명한 일은 공적 업무였다. 정조는 이들의 관계 및 지방관으로서의 경력, 서로 다른 화풍 등을 고려해 봉명사경의 화원으로 선발하였다. 김응환과 김홍도는 영조(英祖)(재위 1724~1776)조부터 도화서 화원으로 활동하며 선후배이자 동료로서 친밀한 관계를 유지했다. 나아가 이들은 영남 지역의 찰방(察訪)으로서 지방관의 업무를 수행한 경험이 있었다. 두 화원의 화풍은 서로 달라 김홍도는 부드럽고도 섬세하게 필선을 운용한 반면, 김응환은 굳세면서도 울창한 풍치를 잘 표현했다. 두 명의 화원은 각자의 개성으로 봉명사경 기간 동안 100여 폭의 초본을 그렸고, 이를 선별하여 60~70여 폭의 화첩 혹은 두루마리를 완성하였다. 이들의 그림은 18세기 전중반에 내금강과 관동팔경 위주로 제작된 금강산 기행사경도의 전통을 더욱 풍부하게 했고 영동과 외금강의 명승명소를 새롭게 발견하며 소재를 확장시켰다. 현재 《해악전도첩》은 원(元), 형(亨), 이(利), 정(貞) 4책으로 이루어졌는데, 원(元), 형(亨)책은 내금강의 그림 29점이며 이(利)책은 외금강의 장면 17점, 정(貞)책은 해금강과 관동팔경 14점으로 구성되었다. 비단 위에 그려진 각 그림은 기하학적으로 산석을 표현했으며, 연백으로 금강산의 암봉을 흰색 혹은 회청색으로 표현했다. 《해악전도첩》의 구도와 화법은 정선, 강세황, 심사정, 정충엽(鄭忠燁)(1725~1800 이후), 김응환, 김홍도의 화법과 비교할 수 있어 18세기 후반의 시대 양식을 갖는다. 특히 화첩의 일부 그림은 김홍도의 《해동명산도첩(海東名山圖帖)》(1788)과 구성 및 회화적 모티프가 매우 유사하여 두 화첩간의 연관성을 보여준다. 반면에 <영랑호>, <해산정>, <월송정> 등은 김홍도의 그림과는 구별된다. 이를 통해 화가가 김홍도와 영향을 주고받으면서도 자신만의 개성적인 화첩을 제작했음을 추측해 볼 수 있다. 《해악전도첩》에는 다른 화첩에는 등장하지 않는 <자운담>, <백운대>, <안문점망비로봉>, <백정봉> 등이 포함되어 있으며 각 장면마다 경물의 특징을 구체적이고 참신하게 묘사하였다. 특히, 화가는 산석을 기하학적으로 표현하고 선과 면을 도드라지게 하여 입체감을 강조하였다. 그는 남종화풍을 바탕으로 자신만의 화법을 확립했고 이를 자유자재로 운용하면서 화면에 동적인 리듬감을 부여했다. 이처럼 60점의 그림은 거칠고 파격적으로 보이지만 나름의 질서 안에서 일관성을 견지하고 있다. 본고는 화법과 봉명사경의 정황을 종합해 《해악전도첩》의 제작자를 김응환으로 추론하였다. 나아가 김하종(金夏鍾)(1793~1878 이후)의 《풍악권(楓嶽卷)》(1865년 이후)과의 친연성은 《해악전도첩》의 화가를 김응환으로 추정한 또 하나의 이유였다. 《해악전도첩》은 김홍도의 《해산첩》과는 달리 후대에 미친 영향력이 미비한데 김하종의 《풍악권》만이 《해악전도첩》의 소재와 화법을 따르고 있다. 김하종은 《풍악권》에서 50년 전, 춘천부사 이광문(李光文)(1778~1838)을 위해 제작한 《해산도첩》(1816)과는 전혀 다른 화법을 구사했다. 그는 김응환의 《해악전도첩》과 유사한 구성과 회화적 요소, 화보식 인물표현을 따르면서 사의적인 분위기를 강조하였다. 개성김씨의 일원이자 김응환의 종손인 김하종은 가문에 전해지는 《해악전도첩》류의 그림을 감상했고 이를 새롭게 번안했다고 추측된다. 화첩에 포함된 51편의 기문은 그림 다음 장에서 그려진 장소를 설명하고 있어 각 그림을 이해하고 감상하는 데 도움을 준다. 기문은 그림과 밀접한 관계를 가지며 앞의 장소로부터의 이동 정보, 이름의 유래, 지형적 특징, 관련 정보 등이 서술되었다. 이와 같은 백과사전식 혹은 지리지와 같은 기문은 19세기 전반에 나타나기 시작했으며, 금강산 화첩류에 더해졌다. 《해악전도첩》의 백화암 기문에는 1845년의 암자에 대한 중건 내용이 기록되어, 기문의 연대를 추정하는 중요한 근거가 되었다. 김하종에게 《풍악권》을 주문한 이유원(李裕元)(1814~1888)도 각 그림에 글을 붙였는데 이 글들은 김응환의 화첩에 포함된 기문 51편과 내용 및 서술방식이 흡사하다. 이유원의 기문은 《해악전도첩》의 기문이나 그 초고(원본)와 관련성이 높지만 두 화첩의 기문 필사의 선후관계를 판정하는 데에는 좀더 면밀한 검토가 필요하다. 《해악전도첩》은 김홍도의 봉명사경 초본 및 김홍도의 영향으로 제작된 19세기의 금강산 화첩과는 구별된다. 이 화첩은 화원 김응환의 회화세계를 이해할 수 있는 지평을 넓히고 18세기 후반 실경산수화의 또 다른 층위를 보여주고 있어 중요한 의미를 갖는다.
