PURPOSE: Improving elderly people's balance ability through pilates which may be easily applied in ordinary life is considered an important intervention method for elderly people. Therefore, this study examines the effects of pilates training on elderly people's static and dynamic balance abilities and provides clinical data to improve this ability. METHODS: The pilates program was applied to 19 elderly subjects three times per week for 8 weeks. Their training level took into consideration their age and was set so that the rating scales of perceived exertion became 13 to 14. Prior to the initiation of the experiment, the subjects had an adaptation period in order to practice the order and motions of pilates. After the adaptation period, the subjects received training. The program was conducted three times per week, for eight weeks. Their dynamic balance ability was observed through the timed up and go (TUG) test and tandem walk test (TWT), and their static balance ability was evaluated by the center of pressure (COP) area, medial-lateral displacement, and anterior-posterior displacement. RESULTS: The results of the TUG test and TWT and evaluations of the COP area and medial-lateral displacement were significantly different after the pilates exercise program compared to those before the program began. CONCLUSION: The results show that an 8-week pilates exercise program is an effective method to increase elderly people's static and dynamic balance abilities. In addition, the application result of the pilates exercise program will provide useful information for future research on elderly people's balance ability.
Forty two patients with chronic constrictive pericarditis, who were admitted to the Yonsei University College of medicine over a period of 18 years from January, 1970 to August, 1988, were analyzed retrospectively. Mean age of the patient was 33.5 year ranging from 6.8 to 60 years old. Male to female ratio was 1.3 to 1. Twenty-one cases [50%] were tuberculous origin [based on either associated pulmonary tuberculosis and/or caseous necrosis in thickened pericardial specimen] and 17 cases [40.5%] were idiopathic [non specific chronic inflammatory change was considered to be idiopathic]. Dyspnea on exertion was evident in 30 cases [71.4%] and abdominal distention in 21 cases [50%]. On physical examination, hepatomegaly [83.3%], neck vein distention [54.8%], distant heart sound [47.6%] and ascites were found. Thirty-nine patients showed low voltage of QRS and/or T wave flattening or inversion on EKG. Thirty-one cases had undergone cardiac catheterization which showed data compatible with chronic constrictive pericarditis. Midsternostomy group [n=15] had shown the most remarkable CVP decline [12.20 mmHg] as compared with bilateral submammary incision group [n=25, 8.96 mmHg] and left thoracotomy group [n=2, 7.75 mmHg] but difference was not significant statistically There was four early death among 42 patients [9.5%] including 3 cases of left ventricular failure and one cardiac tamponade. Main postoperative complications were wound infection [6 cases] and arrhythmia [3 cases]. Follow-up of 24 patients [mean; 55.3 months, ranging from 2 months to 155 months] revealed good functional status.
This is a case report of successful surgical correction of coarctation of the aorta associated with the patent ductus arteriosus and the persistent left superior vena cava. The patient was a 15 year old girl and congenital heart anomaly was suspected at the sixth month after birth. Afterward there has been no embarrassing symptoms until the day of operation except slight dyspnea on exertion, The diagnosis of coarctation of the aorta and the patent ductus arteriesus detected by physical signs and X-ray studies including aortography. In the first place, coarctation of the aorta was corrected with following procedure: partial resections of the aortic wall just above and below the coarctating line of the aorta and removal of diaphragmatic structure of coarctation performed by. cross clamping aorta above and below coarctation, and then the defect of the aortic wall was closed by lateral aortorrhapic suture with atraumatic needle 3-0 silk continuously [Fig. 6]. In the second place, the patent ductus arteriosus was closed with double ligation method. The persistent left superior vena cava left as it has been, because there has been no evidence of hemodynamic abnormal shunt. After operation, excellent result was obtained; blood pressure in the upper and lower extremities was normalized and subjective complains of hypertension in the upper extremity was disappeared.
Patients with chronic obstructive pulmonary disease (COPD) frequently complain of dyspnea on exertion and reduced exercise capacity, which has been attributed to an increase in the work of breathing and in impaired of gas exchange. Although COPD primarily affects the pulmonary system, patients with COPD exhibit significant systemic manifestations of disease progression. These manifestations include weight loss, nutritional abnormalities, skeletal muscle dysfunction (SMD), cardiovascular problems, and psychosocial complications. It has been documented that SMD significantly contributes to a reduced exercise capacity in patients with COPD. Ventilatory and limb muscle in these patients show structural and functional alteration, which are influenced by several factors, including physical inactivity, hypoxia, smoking, aging, corticosteroid, malnutrition, systemic inflammation, oxidative stress, apoptosis, and ubiquitin-proteasome pathway activation. This article summarizes briefly the evidence and the clinical consequences of SMD in patients with COPD. In addition, it reviews contributing factors and therapeutic strategies.

Purpose: The purpose of this study was to identify the time for young adults to reach resting blood pressure after walking as measured by an automatic and a mercury sphygmomanometer. Methods: Participants were 33 nursing students in their 20s. Blood pressure after walking was measured every minute on both arms simultaneously for a total of 6 times after walking for 12 minutes on a treadmill at ratings of perceived exertion (RPE) of 11. Data were analyzed using paired t-test, Bland-Altman plots and repeated measures ANOVA. Results: Systolic blood pressure after walking was the same as measurements corresponding to resting blood pressure after 3 minutes of resting if measured with the automatic sphygmomanometer and 4 minutes of resting if measured with the mercury sphygmomanometer. Conclusions: In order to measure the resting blood pressure for healthy young adults who performed low-density walking for 12 minutes on flat land, the measurement needs to be made after a resting time of at least 3 minutes in the case of an automatic sphygmomanometer and 4 minutes in the case of a mercury sphygmomanometer.
Purpose: The Purpose of this research was to develop the integrative exercise program for the elderly. Method: The first step was to survey the elder's health status and general health problems. The second step was to design the exercise type, intensity, time and frequency. Result: 1. Elders who implement the integrative exercise program had weaker health status, lower physical performance and more depression than any other places. 2. The integrative exercise program developed the preparation stage composed of stretching and Yudongkong-Ki exercise, the central stage was composed of range of motion exercise, muscle strength exercise and aerobic exercise with favorite and popular Korean music. The finishing stage was composed of joint rounding and Yudongkong-Ki exercise. The exercise intensity was RPE(rating perceived exertion) 10-11, maximal pulse rate 40-60%, duration started from 20min to 50min and frequency is 2-3times/wk. Conclusion: The integrative exercise program developed by the researcher is tailored exercise that is more applicable to individual cases in community settings. Eventually, the program is safely applicable to the elders, and is expected to improve the elders' physical function and quality of life.
Fatigue is a significant problem in health care. The relationship between chronic fatigue and psychiatric disorders has generated significant interest in recent years. Increased rates of psychiatric disorder among individuals with chronic fatigue have been reported. Fatigue may be defined as physical and/or mental weariness resulting from exertion. The cause of fatigue is complex, influenced by events occurring in both the periphery and the central nervous system. Because fatigue may result from a number of causes, it should be treated comprehensively. The treatment is largely symptomatic and rehabilitative.
From Jan. 1981 to Dec. 1991, we had treated 25 patients with congenital coronary art-eriovenous fistulas [CAVF] in Seoul National University Hospital. A retrospective review was made to delineate the course and the management of CAVF and to clarify the role of surgical treatment. Fifteen patients were male and 10 were female with The mean age of 17.4 years[from 3 months to 58 years]. The most frequent symptom was dyspnea on exertion[56%]. Other symptoms were angina and palpitation. Sixty-eight percent of the patients were symptomatic. Fifty-three percent of patients less than 20 years old were symptomatic and 100% of patients over 20 years old were symptomatic. Three patients had multiple CAVFs. The fistula drained into the right ventricle in 13, pulmonary artery in 9, left ventricle in 4, right atrium in 2, and left atrium in 1. Thirteen patients had other associated cardiac lesions. The mean pulmonary-to-systemic blood flow[Qp /Qs] in the isolate CAVF group was 2.19. All patients were operated on to correct the fistulas and other associated cardiac lesions. All patients were followed from 1 month to 11 years without late death. Postoperative complication rate was 24% -significant arrhythmia [3], recurred CAVF[1], psychosis[1], pneumonia [1]. Symptomatic improvement was evident postoperatively. Below 20 years old, 94% of patients were asymptomatic, but above 20 years old, symptoms persisted in 25%. In summary, early elective repair of CAVF is indicated in all patients because of higher complication rate and frequent persistent symptoms in older patients.
This report documents the clinical k pathologic features of 33 patients treated for thymoma for 11 years & 6 months between September 1977 and February 1989. At the Thoracic & Cardiovascular Surgery, Yonsei University, College of Medicine of the group, 31 patients treated with surgery were examined for the result of operation & prognosis. Mean age was 50 years. Thirteen were female and twenty were male. Dyspnea on exertion and chest discomfort were common in the patients without myasthenia gravis. Fourteen patients[42.6%] had myasthenia gravis and one patient had autoimmune thyroid disease. Four patients[12.1%] presented without symptoms attributable to their thymoma. Histologic review disclosed 12[36.4%] epithelial thymoma, 10[30.3%] mixed lymphoepithelial, 9[27.3%] lymphocytic, 1[3.0%] spindle cell and 1[3.0%] unknown cell thymoma. They were classified according to Masaoka`s clinical staging criteria; by these criteria, 8 patients were stage I, 5 patients were stage II, 15 patients were stage III and 3 patients were stage IV. Total excision of mass was possible in 20 patients. Partial excision of mass in 4 patients and biopsy in 7 patients were carried out during the operation. There was only one operation mortality. Follow-up was possible in 26 patients and follow-up ranged from 4 months to 10.5 years[mean 28.9 months]. One-year survival rates were 77.8% and eight patients expired during follow-up period. Eleven[78.6%] patients with myasthenia gravis were improved after the operation. This observation suggests that the most significant factor determining the survival is whether or not total surgical excision had been performed.
Between March, 1989, and August, 1993, 10 patients underwent aortocoronary bypass surgery concomittant with cardiac valve replacement. They were 6 men and 4 women, the age ranging from 47 to 64. 7 patients underwent single valve replacement and 2 patients underwent double valve replacement, Another one patient underwent only CABG one year after valve replacement and he had no evidence of prosthetic valve failure. Total number of graft vessels were.15,14 were saphenous venous grafts and 1 was internal mammary artery graft. Dyspnea on exertion was frequent symptom and was found in all patients. 8 patients presented stable angina, only 1 patient presented postinfarct angina and another 1 patient presented no angina symptom. The graft was placed prior to valve replacement and periods of myocardial ischemia were kept at a minimum by maintaining coronary perfusion throughout operation. Postoperative course was uneventful and there was no hospital mortality, as was supported by many reports, it is our opinion that simultaneous valve replacement and aortotomy bypass graft does not increase the risk of cardiac valve replacement substantially.
