Effects of Sensorimotor Training on Postural Stability and Pain in Patients with Chronic Low Back Pain

  • Background: Back pain is associated with a high risk of recurrence. Various physical therapy techniques for back pain have been studied, including reprogramming the central nervous system by integrating sensation and motion with sensory exercise training. Objectives: To aimed verify the effectiveness of sensorimotor training in improving postural stability and pain levels. Design: A randomized controlled trial. Methods: The study population was randomized into a sensory exercise training group and trunk stabilization training group and treated three times a week for 4 weeks. Each group took part in sensorimotor training for 15 minutes or lumbar stabilization exercise for 15 minutes. Results: After the intervention both groups showed Improvements in the variables. There was a significant difference in the dynamic postural stability, limit of stability, and modified visual analog scale scores in the sensorimotor training group compared to the lumbar stabilization exercise group (P<.05). Conclusion: Sensorimotor training appears to be an effective physical therapy exercise program that can be applied in patients with low back pain to improve muscle control ability.

Xylan으로부터 단리한 Glucuronic Acid가 유산소 운동 후 흰쥐 백근의 항산화계에 미치는 영향 (Effect of Glucuronic Acid Derivertives Isolated from Xylan on Antioxidative Defense System in Rat White Gastrocnemius after Aerobic Exercise)

  • The purpose of this study was to investigate the effects of glucuronic acid (isolated from xylan) on antioxidative defense system in rat after aerobic exercise. The glucuronic acid was isolated from xylan. Sprague-Dawley male rats weighing 150$\pm$10 g were randomly assigned to one normal group and three exercise training groups. Exercise training groups were classified to T (glucuronic acid free diet), TU (250 mg glucuronic acid/kg bw) and 2TU (500 mg glucuronic acid/kg bw) according to the level of glucuronic acid supplementation before exercise training. The experimental rats in exercise training groups (T, TU and 2TU) were exercised on glucuronic acid supplementation or rats in normal group (N) were confined in cage for 4 weeks. And rats were sacrificed with an overdose of pentobarbital injection just after running. Body weight, food intakes and food efficiency ratio (FER) were lower in the exercise training group than in the normal group. White gastrocnemius xanthine oxidase (XOD) activity in the T group was 85% greater than that of the normal group, whereas in the TU and 2TU groups it did not differ from the normal group. White gastrocnemius superoxide dismutase (SOD) activity in T group, that was decreased by 22% compared with that of N group, but those of TU and 2TU groups were increased by 38% and 42%, respectively, compared with that of T group. White gastrocnemius glutathione peroxidase (GSHpx) activity in T group, that was decreased by 42% compared with that of N group, but those of TU and 2TU groups were increased by 67% and 68%, respectively, compared with that of T group. Glutathione S-transferase (GST) activity of white gastrocnemius in N group was not significantly different from that in the T and TU groups, but 2TU group were increased by 12%. Contents of thiobarbituric acid reactive substance (TBARS) in T group was increased by 54%, compared with that of normal group but those of TU group and 2TU group were lower 44% and 36% than that of T group. In conclusion, the effects of glucuronic acids in exercise training rats would appear to reduce peroxidation of tissue as an antioxidative defense mechanism.

  • Cervical pain is a rapid increase that is owing to a flexion-extension whiplash injury, unappropriated posture, chronical repetition injury from abdominal position of head and neck, excessive repeating work, chronical deficiency of excercise. Because of that is bring about muscle unbalance, tightness of cervical extensor muscle, weakness of cervical deep flexor muscles, instability of cervical region and reduction of proprioceptive sensor. Recent the role of muscle is more emphasized for preservation of sine stabilization. And cognition of integrated muscular system, importance for the operation and relation is increased to maintain stability of the motor system and pertinent function. Therefore we are going to introduce the sling exercise and stabilization exercise method for advanced efficient of cervical and upper limb and for the muscle strengthening to importance cervical stabilization through neurological program as control the reaction of cervical stabilization. Sling exercise therapy(SET) concept consists of a system of diagnosis and treatment. The system of diagnosis involves testing the muscle's tolerance through progressive loading in open and close kinetic chains. The SET system contains elements such as relaxation, increasing the range of movement, traction, training the stabilizing musculature, sensory-motor exercises, training in open and close kinetic chains, dynamic training of the mobilizing musculature, cardiovascular exercise, group exercise, personal exercise at home Sensory-motor training is an essential element of the SET concept. The emphasis is on closed kinetic chain exercise on an unstable surface, there by achieving optimum stimulation of the sensory-motor apparatus.

A 1:1 exercise-to-rest period ratio needed by animals to restore energy sources and replenish anti-oxidative status after exercise

  • BACKGROUND/OBJECTIVES: Successful recovery of an animal from exercise is essential, especially prior to the next exercise session. This study was conducted to find an effective exercise-to-rest period ratio for the restoration of energy sources and replenishment of anti-oxidative status in tissue after exercise. MATERIALS/METHODS: Thirty-two rats were assigned to either non-training or training exercise groups for 5 weeks. After that period, the two groups were subdivided into four smaller groups: non-exercise (NE), exercise 0.5 hour and rest 1 hour (ER0.5:1), exercise 1 hour and rest 1 hour (ER1:1), exercise 2 hours and rest 1 hour (ER2:1). RESULTS: In the training group animals and compared to the NE group, the levels of plasma glucose after the rest period were significantly high in all ER groups but highest in the ER2:1 group. Similarly, the liver glycogen level was highest in the ER2:1 group. The plasma FFA level reached the highest level in the ER2:1 group but was similarly high in the ER0.5:1 group. Liver TG level was unchanged in the ER2:1 and ER1:1 groups but was significantly high in the ER0.5:1 group. Muscle TG levels were decreased in all three ER groups. Plasma protein levels were significantly high in the ER2:1 and ER0.5:1 groups. In both training animal and non-training animals, the liver protein levels did not change significantly between the NE and ER groups, irrespective of the exercise-to-rest ratio. In the training animal group, muscle protein level was significantly low in the ER2:1 and ER0.5:1 groups. The activity levels of superoxide dismutase and catalase, as well as the malondialdehyde concentration, were not significantly different between NE and ER groups, irrespective of the exercise-to-rest period ratio. CONCLUSIONS: These results indicate that animals provided with a 0.5:1 to 1:1 exercise-to-rest period ratio can restore their muscle energy sources and recover their anti-oxidative defense system.

  • Dysfunction of the anterior and dorsal muscles of the trunk have been studied in relation to low back pain of many years. Many muscles of the trunk are capable of contributing to the stabilization and protection of the lumbar spine, recent evidence has suggested that transversus abdominis may be critically involved and has been the focus of rehabilitation. The delay in onset of contraction of trunk muscles associated with movement of the upper or lower limb in patients with low back pain indicates a significant deficit in the automatic motor command for control of disturbance to the spine. The function of transversus abdominis has been largely ignored in the evaluation of spinal stabilization and protection. The most essential stabilizing muscles for the lumbar column are the transversus abdominis and the multifidus. Sling exercise therapy(SET) concept consists of a system of diagnosis and treatment. 일회성 탈진운동과 장기간 지구성 트레이닝이 심장근의 Mn-SOD, HSP70 및 PPAR-γ 단백질 발현에 미치는 영향 (The Effect of Acute Exhaustive Exercise and Long-term Endurance Exercise Training on the Protein Expression of Mn-SOD, HSP70 and PPAR-γ in the Heart of rats)

  • The purpose of this study was to investigate whether two different exercises, acute exhaustive exercise and long-term endurance exercise training could affect to the expression of Mn-SOD, HSP70, and PPAR-γ protein in myocardium. The Wistar-Kyoto rats(n=24, 4 weeks) were used and randomly divided into 3 groups; endurance exercise training group (EET, n=8), acute exhaustive exercise group (AEE, n=8) or control group (CON, n=8). EET performed treadmill exercise for 12 weeks (5 days/week, 30~60 min/day). AEE exercised treadmill running (speed increased gradually to 14-26 m/min, 60 min ±10min) until exhausted when EET finished the program. Then, all the rats were sacrificed 48 hours rest at least after the last session of their own exercise program. Hearts were isolated and then the expression of Mn-SOD, HSP70, and PPAR-γ were analyzed by western blotting. One-way repeated ANOVA was used and p value under 0.05 was considered as statistical significance. The results were followed as; the expression of Mn-SOD of AEE was decreased compared with CON. However, the expression of Mn-SOD of EET was increased compared with CON. There was significant difference between AEE and EET in the expression of Mn-SOD. The expressions of HSP70 and PPAR-γ in the both AEE and EET were significantly increased compared with CON. In conclusion, acute exhaustive exercise might induce oxidative stress wheres endurance exercise training could ameliorate the oxidative conditions by increase of Mn-SOD, HSP70, and PPAR-γ. Therefore, we suggested that endurance exercise training could enhance the complementary antioxidant system and improve to prevent apoptosis. Further, a long-term moderat aerobic exercise program might play a important role in mitochondrial biogenesis in the heart.

Active training machine with muscle activity sensor for elderly people

  • For elderly people, an advanced training machine that uses actuator and can adjust load according to muscle activity is proposed. The proposed machine allows users to have a safe and effective training through exercise close to ordinal motion appears in daily life such as stretching or stooping motion. A muscle activity sensor real-timely monitors the activation level of user's muscle during the exercise and the training load is adjusted based on the measured data. The training load is exerted and continuously controlled by electric/pneumatic actuator.

  • 본 논문에서는 개인의 신체적인 특성을 입력하여 이를 전문 트레이너가 미리 입력한 데이터베이스와 비교한 후 개인 특성에 맞는 헬스 트레이닝 방법을 3D 애니메이션으로 사용자에게 제시해주는 개인 맞춤형 헬스 트레이닝 시스템을 설계하고 구현하였다. 스포츠 센터나 헬스클럽에서는 개인별 특성에 따른 맞춤식 운동을 제공하지 못하고 트레이너의 개인적인 경험에 따른 대략적인 운동방법과 운동 기구 사용법만 지도하고 있다. 제안하는 시스템은 개인별 특성을 미리 준비된 표 형식에 입력하고 운동 목적과 운동 장소, 정상 운동 가능 유무, 자신의 RM을 입력하면 보다 정확하게 데이터베이스를 검색하여 개인에게 적합한 헬스 트레이닝 동작과 운동 효과 및 운동 방법을 화면에 제시한다.

사이버위기 경보 기반 사이버 방어 훈련장 설계 및 구축 연구 (Design and Implementation of Cyber Range for Cyber Defense Exercise Based on Cyber Crisis Alert)

  • 사이버 방어 훈련은 최신 사이버공격을 고려한 훈련을 수행해야 할 뿐만 아니라 사이버공격 대응 과정이 실전과 유사해야 그 효과가 크다고 할 수 있다. 또한 최근에는 사이버공격에 관계없이 정상업무를 수행하거나 그에 준하는 업무를 지원할 수 있는 사이버 회복력에 대한 훈련 역시 중요하다. 이에 본 논문에서는 국내에서 사이버공격 발생 시 발령되는 사이버위기 경보를 기반으로 실시간 사이버훈련을 수행함으로 사이버 회복력의 요소들을 강화시킬 수 있는 사이버 방어 훈련장을 제안한다. 사이버위기 경보 수준에 따라 관심·주의·경계·심각 경보 발령 시, 사이버 방어 훈련장은 해당 경보에 따라, 보안 요소를 점검하는 예방보안, 실시간으로 사이버공격 방어를 수행하는 실시간대응, 피해 시스템의 사고조사를 수행하는 사후대응 훈련을 수행할 수 있는 환경을 지원한다. 본 논문에서 제안하는 사이버 방어 훈련장은 사이버위기 환경에서 국내 실정에 맞는 사이버 방어 훈련을 수행함으로써 훈련생들이 사이버위협에 대한 실질적인 대응 역량을 제고 할 수 있다.

Changes of One-Leg Standing Balance of Ipsilateral and Contralateral Lower-Limb Following Unilateral Isokinetic Exercise of Ankle Joint in Young Adults

  • Purpose: The purpose of this study was to investigate the effects of a four week unilateral isokinetic exercise program applied to ankle on the one-leg stance balance performance of ipsilateral and contralateral lower-limbs. Methods: Subjects were randomly assigned to either a right ankle training program (n=12) or a control group (n=12). The training group received unilateral ankle isokinetic exercise of the dominant side for 4 weeks, whereas control group did not. Ipsilateral and contralateral one-leg balance were measured before and after intervention using the Biodex Balance System. Results: Improvements of stability scores, such as APSI, MLSI, and OSI, from pre-test to post-test were significantly different greater for the training group when the control. Conclusion: The results of this study suggest unilateral ankle strengthening exercise transfers benefit to the untrained limb by a cross-education effect, and that this type of exercise should be considered to improve one-leg standing balance of trained and untrained lower-limbs.
