General Characteristics for Poisoning-Induced Transient or Sustained Hyperammonemia (급성 약물중독에 합병된 일과성 혹은 지속성 고암모니아혈증의 특성)

  • Purpose: In patients with altered mentality caused by drugs or unknown causes, ammonia is checked to facilitate differential diagnosis or diagnose hepatic coma. This helps early prevention and treatment of brain damage due to hyperammonemia. This study was conducted to evaluate clinical characteristics of intoxicated adult patients with hyperammonemia. Methods: We evaluated 95 patients with hyperammonemia among intoxicated patients above the age of 15 who visited our ED from January 2013 to December 2015. We analyzed the demographic characteristics and type of poisoning substance, reason for ingestion, toxicological characteristics such as elapsed time from ingestion to hospital visit, lab, clinical progression and complications. Data were evaluated using the student's t test or Mann-Whitney U test for continuous variables, and Chi-square test and Fisher's exact test for frequency analysis of categorical variables. Results: When compared to healthy individuals, patients with hyperammonemia showed statistical significance on their SOFA score (p=0.016) and poison severity score (p<0.001). Additionally, patients with hyperammonemia showed significantly different initial serum AST level (p=0.012) and maximum serum AST level during the hospital stay (p=0.026) when compared to healthy individuals. Moreover, individuals with sustained hyperammonemia compared to transient hyperammonemia showed clinically significant SOFA scores (p<0.001), poison severity scores (p=0.007), mortality rates in the ICU (p=0.021), as well as different duration of hospital stay (p=0.037), serum creatinine level (p=0.002), erythrocyte sedimentation rate (p=0.025), and serum myoglobin (p=0.015). Conclusion: Most poisoning-induced hyperammonemia cases were transient and recovered without special treatment. Therefore, hyperammonemia is almost non-specific among poisoning patients.

Medication Errors in Chemotherapy Preparation and Administration: a Survey Conducted among Oncology Nurses in Turkey

  • Background: Medication errors in oncology may cause severe clinical problems due to low therapeutic indices and high toxicity of chemotherapeutic agents. We aimed to investigate unintentional medication errors and underlying factors during chemotherapy preparation and administration based on a systematic survey conducted to reflect oncology nurses experience. Materials and Methods: This study was conducted in 18 adult chemotherapy units with volunteer participation of 206 nurses. A survey developed by primary investigators and medication errors (MAEs) defined preventable errors during prescription of medication, ordering, preparation or administration. The survey consisted of 4 parts: demographic features of nurses; workload of chemotherapy units; errors and their estimated monthly number during chemotherapy preparation and administration; and evaluation of the possible factors responsible from ME. The survey was conducted by face to face interview and data analyses were performed with descriptive statistics. Chi-square or Fisher exact tests were used for a comparative analysis of categorical data. Results: Some 83.4% of the 210 nurses reported one or more than one error during chemotherapy preparation and administration. Prescribing or ordering wrong doses by physicians (65.7%) and noncompliance with administration sequences during chemotherapy administration (50.5%) were the most common errors. The most common estimated average monthly error was not following the administration sequence of the chemotherapeutic agents (4.1 times/month, range 1-20). The most important underlying reasons for medication errors were heavy workload (49.7%) and insufficient number of staff (36.5%). Conclusions: Our findings suggest that the probability of medication error is very high during chemotherapy preparation and administration, the most common involving prescribing and ordering errors. Further studies must address the strategies to minimize medication error in chemotherapy receiving patients, determine sufficient protective measures and establishing multistep control mechanisms.

Analysis of Three Dimensional Positioning Accuracy of Vectorization Using UAV-Photogrammetry (무인항공사진측량을 이용한 벡터화의 3차원 위치정확도 분석)

  • There are two feature collection methods in digital mapping using the UAV (Unmanned Aerial Vehicle) Photogrammetry: vectorization and stereo plotting. In vectorization, planar information is extracted from orthomosaics and elevation value obtained from a DSM (Digital Surface Model) or a DEM (Digital Elevation Model). However, the exact determination of the positional accuracy of 3D features such as ground facilities and buildings is very ambiguous, because the accuracy of vectorizing results has been mainly analyzed using only check points placed on the ground. Thus, this study aims to review the possibility of 3D spatial information acquisition and digital map production of vectorization by analyzing the corner point coordinates of different layers as well as check points. To this end, images were taken by a Phantom 4 (DJI) with 3.6 cm of GSD (Ground Sample Distance) at altitude of 90 m. The outcomes indicate that the horizontal RMSE (Root Mean Square Error) of vectorization method is 0.045 cm, which was calculated from residuals at check point compared with those of the field survey results. It is therefore possible to produce a digital topographic (plane) map of 1:1,000 scale using ortho images. On the other hand, the three-dimensional accuracy of vectorization was 0.068~0.162 m in horizontal and 0.090~1.840 m in vertical RMSE. It is thus difficult to obtain 3D spatial information and 1:1,000 digital map production by using vectorization due to a large error in elevation.

Diagnostic Value of Susceptibility-Weighted MRI in Differentiating Cerebellopontine Angle Schwannoma from Meningioma

  • Background: Differentiation of cerebellopontine angle (CPA) schwannoma from meningioma is often a difficult process to identify. Purpose: To identify imaging features for distinguishing CPA schwannoma from meningioma and to investigate the usefulness of susceptibility-weighted imaging (SWI) in differentiating them. Materials and Methods: Between March 2010 and January 2015, this study pathologically confirmed 11 meningiomas and 20 schwannomas involving CPA with preoperative SWI were retrospectively reviewed. Generally, the following MRI features were evaluated: 1) maximal diameter on axial image, 2) angle between tumor border and adjacent petrous bone, 3) presence of intratumoral dark signal intensity on SWI, 4) tumor consistency, 5) blood-fluid level, 6) involvement of internal auditory canal (IAC), 7) dural tail, and 8) involvement of adjacent intracranial space. On CT, 1) presence of dilatation of IAC, 2) intratumoral calcification, and 3) adjacent hyperostosis were evaluated. All features were compared using Chi-squared tests and Fisher's exact tests. The univariate and multivariate logistic regression analysis were performed to identify imaging features that differentiate both tumors. Results: The results noted that schwannomas more frequently demonstrated dark spots on SWI (P = 0.025), cystic consistency (P = 0.034), and globular angle (P = 0.008); schwannomas showed more dilatation of internal auditory meatus and lack of calcification (P = 0.008 and P = 0.02, respectively). However, it was shown that dural tail was more common in meningiomas (P < 0.007). In general, dark spots on SWI and dural tail remained significant in multivariate analysis (P = 0.037 and P = 0.012, respectively). In this case, the combination of two features showed a sensitivity and specificity of 80% and 100% respectively, with an area under the receiver operating characteristic curve of 0.9. Conclusion: In conclusion, dark spots on SWI were found to be helpful in differentiating CPA schwannoma from meningioma. It is noted that combining dural tail with dark spots on SWI yielded strong diagnostic value in differentiating both tumors.

A Fast Search Algorithm for Raman Spectrum using Singular Value Decomposition (특이값 분해를 이용한 라만 스펙트럼 고속 탐색 알고리즘)

  • In this paper, we propose new search algorithms using SVD(Singular Value Decomposition) for fast search of Raman spectrum. In the proposed algorithms, small number of the eigen vectors obtained by SVD are chosen in accordance with their respective significance to achieve computation reduction. By introducing pilot test, we exclude large number of data from search and then, we apply partial distance search(PDS) for further computation reduction. We prepared 14,032 kinds of chemical Raman spectrum as the library for comparisons. Experiments were carried out with 7 methods, that is Full Search, PDS, 1DMPS modified MPS for applying to 1-dimensional space data with PDS(1DMPS+PDS), 1DMPS with PDS by using descending sorted variance of data(1DMPS Sort with Variance+PDS), 250-dimensional components of the SVD with PDS(250SVD+PDS) and proposed algorithms, PSP and PSSP. For exact comparison of computations, we compared the number of multiplications and additions required for each method. According to the experiments, PSSP algorithm shows 64.8% computation reduction when compared with 250SVD+PDS while PSP shows 157% computation reduction.

Structural Capacity of RC Beam Retrofitted by CFS with Bond Loss (탄소섬유로 휨보강된 RC 보의 부착 손실에 대한 거동 특성)

  • Recently, various strengthening methods using carbon fiber sheets (CFS) have been developed for the rehabilitation of structures and applied to the concrete member. However, still research need arises in order to verify the structural capacity of RC member which experienced bond loss between concrete and CFS after strengthening. This is because previous research has focused on the development of design process and evaluation of structural capacity only for retrofit. The appearance of this loss may be initiated at just after retrofit construction. And it will be more serious when the layer number of CFS increases. In order to minimize above mistake in retrofit design using CFS, more exact evaluation process to predict the bond loss of CFS is required. The objective of this research is to study the variation of flexural structural capacity of beam which has experienced bond loss after strengthening using CFS. Experimental and analytical study are performed and evaluation of the previous formula is conducted. Test result showed that the significant strength deterioration was not found until the bond loss of 20%. Overall structural behavior of the beams can be predicted by nonlinear sectional analysis.

  • Objectives: The purpose of this research is to obtain basic data about the effects of Constitution-Acupuncture (CA) based on Eight-Constitution Medicine (ECM) by the expert CA Oriental Medical Doctors. Methods: The resources were collected from the internet survey method. 35 samples were collected. The SPSS 13.0 for windows and Answer Tree 3.0 was used for statistical analysis. Results: The results from this research are as follow. 1. The motivation that apply ECM to clinic are 'Because ECM is effective for diagnosis, treatment and prevention against disease' 48.6% and 'Because CA therapy is effective' 25.7%. 2. The advantage that CA therapy compare to the other acupuncture therapy are 'Management and prevention against disease are available by ECM theory to a person who exact constitution is confirmed' 82.9%, 'CA therapy is superior in treatment of hard symptoms to treat by the other acupuncture therapy' 80%, 'Treatment effect is good' 77.1%, 'Needle manipulation of CA is simple comparatively' 57.1%, 'It is definite whether treatment effect is good or not' 54.3% and CA therapy is superior all in diseases of twelve category. 3. When the effects of CA is good, patients expressed much 'Eyesight lightened', 'Took deep sleep at night', 'Fatigue decreased', 'Body is light', 'Feelings became refreshing' as well as 'Symptoms were improved' and also physicians confirmed as those reactions whether treatment is effective. Conclusions: The research suggest that CA is effective therapy in clinic, and so further study is needed for CA therapy.

The design of a single layer antireflection coating on the facet of buried channel waveguide devices using the angular spectrum method and field profiles obtained by the variational method (Variational 방법으로 구한 필드 분포와 Angular Spectrum 방법을 사용한 Buried채널 도파로 소자 단면의 단층 무반사 코팅 설계)

  • We have calculated the optimum refractive index and normalized thickness of a single layer antireflection coating on the facet of buried channel waveguides as a function of waveguide width for several waveguide depths using the angular spectrum method and field profiles obtained by the effective index method (EIM) and the variational method (VM), respectively, and discussed the results. In the area of large waveguide width, the optimum parameters of a single layer antireflection coating obtained by both methods are almost the same. However, as waveguide width decreases, the parameters obtained by the VM approach those of a single layer antireflection coating between cladding layer and air, while those obtained by the EIM do not approach those, and the difference between the two parameters is large. The tolerance maps of the quasi-TE and quasi-TM modes obtained by the VM for square waveguides are located in almost the same area regardless of refractive index contrast, while those obtained by the free space radiation mode (FSRM) method for refractive index contrast of 10% are located in the different area. Thus, we think that the tolerance maps obtained by the VM are more exact than those obtained by the FSRM method.

Comparative Study on Surgical and Conservative Management of Bisphosphonate-related Osteonecrosis of the Jaw (BRONJ) in Disease Stage 2 (비스포스포네이트 관련 악골괴사(BRONJ)의 병기 2기에서의 외과적, 보존적 치료에 대한 비교 연구)

  • Purpose: This study evaluated the prognosis of conservative and surgical treatment according to the staging of bisphosphonate-related osteonecrosis of jaw (BRONJ) by American Association of Oral and Maxillofacial Surgeons and American Society for Bone and Mineral Research. Methods: We evaluated 53 patients of BRONJ who visited Department of Dentistry, Ajou University School of Medicine from May 2007 to February 2013. Twenty eight patients in stage 2, were divided into surgical and conservative groups with cessation of bisphosphonate therapy. Fifteen patients belonged to the conservative treatment group, in which mouth rinsing and antibiotics medication were done. Thirteen patients were treated with debridement or sequestrectomy, in the surgical treatment group. Each study list was analyzed by SPSS ver. 14.0 (SPSS Inc., USA) software and the favorable rate was verified by the Fisher exact test. P-values less than 0.05% were deemed significant. Results: Clinical outcome was evaluated on the basis of both clinical and radiographic findings. Of all the 28 patients of stage 2, 15 patients underwent conservative treatment and 13 patients received surgical treatment. In the surgical group, 9 of 13 (69.2%) showed good prognosis, 4 of 13 (30.7%) showed recurrence. In the conservative group, 13 of 15 (86.6%) showed no change duting the follow-up period. Two of 15 patients even showed a bad prognosis, such as pain and pus discharge, which are criteria for stage 3. P-value was 0.067 (>0.05). Conclusion: The results of the present study suggests that surgical intervention is good choice against the conservative treatment, after proper drug holidays period, while further investigation is needed for a definite solution to BRONJ.

The Prevalence and Risk Factors of Drug Resistant Pulmonary Tuberculosis Investigated at One University Hospital in Seoul (서울지역 한 대학병원에서 조사된 폐결핵 약제 내성률 및 위험인자)

  • Background : The prevalence of tuberculosis is slowly decreasing in Korea. However, the drug-resistance of pulmonary tuberculosis is a major risk factor of treatment failure. Moreover, the National Surveillance System has recently been discontinued. Therefore, a continuous survey is necessary for the exact detection of the rate of drug resistance. We studied the recent 4-year drug resistance rate of tuberculosis at a single University hospital in Seoul. Materials and Methods : The study included 239 pulmonary tuberculosis patients performed with a tuberculosis culture and a drug-sensitivity test at Hanyang University Medical Center from March 1999 to March 2003. Results : Of the 239 patients included in the study during the 4-year period, 52 patients showed resistance to one or more anti-tuberculosis drug (21.8%). The rate of multi-drug resistance was 12.6%. The resistance rates to isoniazid, rifampin, ethambutol, streptomycin and pyrazinamide were 18.4%, 13.8%, 11.7%, 6.7% and 8.4%, respectively. Ninety patients had a history of previous anti-tuberculosis treatment, and the rates of the overall drug resistance and multi-drug resistance of these patients were 36.7% and 25.6%, respectively. The patients with drug-resistance showed a higher rate of a previous tuberculosis treatment history (63.5%) than the drug-sensitive group patients (30.5%). Conclusion : The rate of drug resistant tuberculosis is 21.8%, and multi-drug resistant tuberculosis is 12.6%. The rate of drug resistance is higher in those previously treated for tuberculosis.
