Temporomandibular joint dislocation: experiences from Zaria, Nigeria

  • Objectives: Dislocation of the temporomandibular joint may occur for various reasons. Although different invasive methods have been advocated for its treatment, this study highlights the value of non-invasive treatment options even in chronic cases in a resource-poor environment. Materials and Methods: A seven-year retrospective analysis of all patients managed for temporomandibular joint dislocation in our department was undertaken. Patient demographics, risk factors associated with temporomandibular joint dislocation and treatment modalities were retrieved from patient records. Results: In all, 26 patients were managed over a seven-year period. Males accounted for 62% of the patients, and yawning was the most frequent etiological factor. Conservative treatment methods were used successfully in 86.4% of the patients managed. Two (66.7%) of the three patients who needed surgical treatment developed complications, while only one (5.3%) patient who was managed conservatively developed complications. Conclusion: Temporomandibular joint dislocation appears to be associated with male sex, middle age, yawning, and low socio-economic status, although these observed relationships were not statistically significant. Non-invasive methods remain an effective treatment option in this environment in view of the low socio-economic status of the patients affected.

  • Oral lichen planus is a premalignant chronic inflammatory mucosal disorder with unknown etiology. It is a multifactorial disease and in addition to genetic background, infections, stress, drug reactions are suggested as risk factors. Helicobacter pylori which is involved in development of many gastrointestinal lesions may also be implicated in oral lichen planus induction. This is of clear importance for cancer prevention and the present study was performed to determine any association between H. pylori infection and oral lichen planus in southwestern Iran. Anti H. pylori IgG levels were determined in 41 patients and 82 sex-age matched controls. The results showed no association between H. pylori infection and oral lichen planus (51% in patients vs. 66% in control). or any of its clinical presentations.

Detection of Human Papillomavirus in Laryngeal Squamous Cell Carcinomas (후두편평세포암종에서 인유두종 바이러스의 검출)

  • Human papillomavirus(HPV) is epitheliotrophic virus invading the anogenital tract and the upper aerodigestive tract HRV produces a diversity of benign and maljgnant tumors. In this study, the author determined the frequency of association of human papillomavirus(HPV) and laryngeal carcinomas and investigated the significance of HRV infection of different subtypes in the tumorigenesis of laryngeal carcinoma. Laryngeal squamous cell cancinomas from 34 patients who did not have preexisting papillomas by clinical history were retrieved from formalin-fixed, paraffin-embedded blocks and analyzed for HPV. Nineteen cases were tumors of the true vocal folds, 11 were supraglottic and 4 were transglottic. HPV detection was dane using polymerase chain reaction amplification with HPV L$_1$consensus primer. HPV type was determined by the same method using HPV-6, 11 and 16,-18 type-specific E6 primers. The results were as follows : 1) HPV DNA was detected in 7 cases among the 34 patients(20.6%). According to the type of HPV DNA HPV-11 was detected in 3 cases, HPV-16 was detected in 2 cases and HPV-6 and HPV-18 were detected in 1 case, respectively. 2) These 7 HPV-positive patients were advanced cancinoma cases. From these results, we concluded that HPV was thought to be the etiological factor of laryngeal squamous cell carcinomas.

Knowledge Base Associated with Autism Construction Using CRFs Learning

  • Knowledge base means a library stored in computer system providing useful information or appropriate solutions to specific area. Knowledge base associated with autism is the complex multidimensional information set related to the disease autism for its pathogenic factor and therapy. This paper focuses on the knowledge of biological molecular information extracted from massive biomedical texts with the aid of widespread used machine learning methods. Six classes of biological molecular information (such as protein, DNA, RNA, cell line, cell component, and cell type) are concerned and the probability statistics method, conditional random fields (CRFs), is utilized to discover these knowledges in this work. The knowledge base can help biologists to etiological analysis and pharmacists to drug development, which can at least answer four questions in question-answering (QA) system, i.e., which proteins are most related to the disease autism, which DNAs play important role to the development of autism, which cell types have the correlation to autism and which cell components participate the process to autism.   • The purpose of this study was to generalize the conception of chronic pelvic pain(CPP)through the literature of oriental medicine and occidental medicine. The results are obtained as follows. First, chronic pelvic pain(pain of more than 6 months duration) may include pain of gynecologic, gastroenterologic, urologic, neurologic, and musculoskeletal origin. In this study, the etiology of chronic pelvic pain may remain obscure and the relationship between certain types of pathology, such as endometriosis or adhesions, and the pain response may be inconsistent and often inexplicable. Second, the causes of CPP through the literature of oriental medicine were reviewed as pains due to a wind-pathogen, a cold, disorder of Qi, disorder of blood stasis, a improper diet, disorder of fluid, and deficiency type etc... And the charateristic pains were concerned with a aching pain, a heavy pain, a distending pain. a pain due to mass in the abdoman, a pain likes pulling etc... The degree and classification of charateristic pains in current of time were dependent on subjective factors. Third. in oriental medicine, it wasn't to be suggested concretely recognition of etiological factor in pain. But they recognised that facters were influenced by pain. For example, diretic peripheral demages were concerned with a blood stasis, a phlegm, a damp phlegm, heart, and the pains that were occurrenced by sevn emotions were concerned with a stagnancy of Qi or a stagnancy of liver-Qi.

Mycobacterium genavense induced mycobacteriosis in an Indian peafowl (Pavo cristatus)

  • The report describes an avian mycobacteriosis in a captive wild bird. A 7-year-old female Indian peafowl (Pavo cristatus) maintained in a zoo of Korea presented a gradual cachexia and eventually was found dead. At necropsy, severely atrophied pectoral muscles exposing the keel bone were noticed. Yellowish thick nodules in varying sizes were scattered in all lobes of lungs, liver and spleen, suggesting mycobacteriosis. Histopathologically, multifocal to coalescing granulomas surrounded by multinucleated giant cells were observed. Numbers of acid-fast bacilli were revealed in granulomas. Then, a series of molecular diagnostic techniques were followed: a nested PCR, DNA sequencing and bioinformatics analysis. It resulted as Mycobacterium genavense. The identification of M. genavense as an etiological agent suggested that it might serve as a risk factor for other captive wild animals, and for a potential zoonotic risk since M. genavense have been a definite cause of disseminated mycobacterial infection in immunocompromised people. To the authors' knowledge, this is the first report of avian mycobacteriosis with M. genavense in a captive Indian peafowl.

  • Objectives : This research aimed to develop a new etiologic classification for traditional Korean Medicine in order to respond to the social and environmental change. Methods : We reviewed the existing theories on etiological classification for East Asian Medicine thoroughly and discussed the problems and limitations. Based on the experts' consensus, we abstracted disease factors and etiologic items. Results : The disease factors are classified into three parts: the human body, the environment, and the interaction between the human body and the environment. We defined them as the inner factor, the external factor, and the interaction between the inner and the external factors. The inner factor is free from the influence of the environment, and it causes diseases solely from the components of the human body. It is divided into genetic factors. The external factor is defined as a case when a disease occurs due to a factor outside the human body and includes external injuries, environmental pollution, and natural disasters. The interaction between the inner and the external factors is a disease factor that causes diseases by the interaction of the human body and the environment and includes emotions, habits, and social environment. As a result of the analysis, it was possible to see the meanings at a single glance as the scattered and fractional meanings were integrated with focus on medicinal herbs, but the increasing number of analyzed medicinal herbs tended to more and more complicate their relationships, thus, requiring additional work like filtering. Conclusions : The new etiologic classification of Korean Medicine fully reflects the perspectives on life in Korean Medicine while embracing the changes in modem society. Also, by avoiding the usage of ambivalent terms and wrong classification methods, the new classification system constructs intuitive and concise etiology and improves usability in clinical medicine.

  • To investigate the epidemiological trait of intestinal diseases of animals caused by thermophilic Campyllobacter spp., isolation of etiological agent was carried out and the profiles of plasmids and the transfer of resistance plasmid in the isolated Campylobacter spp. were examined. The results were as follows. 1. A total of 110 isolates of C jejuni and C coli were subjected to the test for the presence of plasmid DNA. Of the isolates examined, 60% of the isolates were noted to harbor plasmid DNA. Plasmid occurrencer ate from pigs, chickens and cattle were 76.2%, 61.7% and 37.7%, respectively. The plasmids of a large molecular weight, ranging from 36 Md to 86Md, were identified with the strains of tetracycline resistant. 2. Transfer frequency of tetracycline resistant plasmids was higher in the case of the filter mating method than in the broth mating method by the factor of 10~1,000. 3. Tetracycline resistant plasmids of C jejuni were transferrable to C jejuni and C coli by conjugation. In a low frequency, the transfer of tetracycline plasmid was also possible to Vibrio parahemolyticus. However, it was impossible to transfer to Streptococcus fecalis, E coli and Vibrio cholerae. 4. Tetracycline resistant plasmids of C jejuni were impossible to transfer to Campylobacter spp. and related bacteria by transformation.

Association between the Epidermal Growth Factor 61*A/G Polymorphism and Hepatocellular Carcinoma Risk: a Meta-Analysis

  • The epidermal growth factor (EGF) may play a pathological role in hepatocellular carcinoma (HCC). However, the conclusions of published reports on the relationship between the EGF $61^*A/G$ polymorphism and HCC risk remain controversial. To derive a more precise estimation we performed a meta-analysis based on 14 studies that together included 2,506 cases and 4,386 controls. PubMed, EMBASE, Web of Knowledge and the Chinese National Knowledge Infrastructure (CNKI) databases were used to retrieve articles up to August 1, 2014. The crude odds ratios (ORs) with 95% confidence intervals (95%CIs) were calculated to evaluate the association. Meta-analysis results showed a significant association between the EGF $61^*A/G$ polymorphism and HCC risk in all four genetic models (allele model: OR=1.25, 95%CI=1.12-1.40; dominant model: OR=1.32, 95%CI=1.14-1.54; recessive model: OR=1.33, 95%CI=1.12-1.58; ho-mozygous model: OR=1.59, 95%CI=1.33-1.90). Moreover, significant associations were observed when stratified by ethnicity, source of controls, etiology and genotype methods. Thus, this meta-analysis suggests that the G-allele of the EGF $61^*A/G$ polymorphism is associated with an increased risk of HCC, especially in Asians and Caucasians, without influence from the source of controls or etiological diversity. Further studies with larger population sizes are needed to confirm these results.

GENETIC STUDY IN AUTSTIC DISORDER - Chromosomal Analysis - (자폐장애 아동의 유전연구 - 염색체 분석 -)

  • The authors studied chromosomal abnormalities in 38 autistic children meeting the diagnostic criteria of DSM-III-R in order to investigate genere factor in autistic disorder There were 28 males and 10 females, with the mean age being $108.8{\pm}28.5months(70-156months).$ All samples were analyzed on short-term lymphocyre cultures in Medium 199 that contained FUdR. The fragile X chromosome was not found in any of the patients. There were other chromosomal abnormalities in 14(36.8%) of 38 patients, such as breakage, 11cases ; gap, 2case ; breakage and gap, 1 case. In grouping of chromosomal abnormalities, group A patients were 4 cases ; group C were 3 cases ; group A and B was 1 case ; group A and E was 1 case ; group C and E was 1 case ; group A, B and C was 1 case. There were no statistical significance in the 16 symptoms of autistic disorder of DSM-III-R between patients with chromosomal abnormalities and patients without chromosomal abnormalites. These results do not support the hypothesis that fragile X chromosome is an etiological factor in autistic disorder.
