다문화가족 여성 배우자의 건강에 대한 탐색연구: 출신국가에 따른 차이를 중심으로 (Exploring the health of female spouses from multicultural families: Examining the differences by their country of origin)

  • 본 연구의 목적은 국내에 거주하는 다문화가족 여성 외국인 배우자의 건강수준과 이에 영향을 미치는 요인을 파악하며, 특히 출신국가에 따라 이들의 건강수준과 영향요인이 다르게 나타나는지 살펴보는데 있다. 이를 위해 본 연구에서는 2012년 전국다문화가족실태조사 자료를 활용하여 가중치를 적용한 총 165,451명의 한국계 중국인, 중국, 베트남, 필리핀 출신 여성 배우자를 대상으로 분석을 실시하였으며 그 결과, 베트남과 필리핀출신 여성이 인식하는 건강수준이 한국계 중국인과 중국인 여성배우자의 건강수준보다 유의하게 높게 나타났다. 더 나아가 이들의 건강수준을 설명하는 요인의 상대적 영향력 역시 출신국가별로 다르게 나타났으며, 특히 한국계 중국인 여성과 베트남 여성이 차별경험에 더 취약함을 알 수 있었다. 또한 공통적으로 모든 여성들에게 있어 배우자와의 관계 만족도는 건강수준을 설명하는 가장 큰 요인으로 파악되어 이들의 건강에 대한 이해와 실천적 예방, 개입에 있어서는 이에 대한 고려가 필수적임을 알 수 있었다. 종합적으로 본 연구결과는 이민자 건강 연구에 있어서는 출신국가에 따른 특성 및 이들이 이민자로서 가지는 한국에서의 지위 및 경험에 대한 이해가 선행되어야 하며, 이에 기반한 보건정책 및 실천전략의 마련이 요구됨을 시사한다.

  • Modern society is a multi-cultural consumer society, and there are multiple trends to cater to the tastes of diverse consumers with different sociocultural background. To grasp fashion trends in fast-changing society, how consumer life is changing and what sort of trend is prevailing should be understood above all. A major fashion trend keeps on changing in every season, and that is an extensive and compound measure of what affects the lives and values of cultural receivers who take the lead in it. The purpose of this study was to delve into what sorts of trends were presented in the 21st century's different cultures, how those cultures were reflected in fashion trends, and how design elements predicted by fashion trends could serve as the sources of design that could create a new fashion. The findings of the study were as follows: First of all, the theories of popular culture and trends were reviewed to describe how general receivers found meaning and delight in the products of cultural industry in their own way and how the products were converted into diverse cultural media. Secondly, consumer styles were discussed by classifying consumers into six groups, twixter, duppie, Ubi-Nomad, NONOS, LOHAS and chav, who were generated by changing cultural codes. Thirdly, sociocultural trends and consumer changes brought a lot of diverse changes to fashion trends. The visual materials about the 2005 S/S, F/W Collection were examined to track how changing trends affected fashion style.

Evaluation of emotion-based messages designed to motivate Hispanic and Asian parents of early adolescents to engage in calcium-rich food and beverage parenting practices

  • BACKGROUND/OBJECTIVES: Setting healthful beverage expectations, making calcium-rich foods and beverages (CRF/B) available, and role modeling are parenting practices promoting calcium intake among early adolescents. This study aimed to evaluate emotion-based messages designed to motivate parents of early adolescents to perform these practices. SUBJECTS/METHODS: Emotion-based messages were developed for each parenting practice and tested in 35 parents from 5 states. Findings were used to modify messages and develop a survey administered via Amazon MechanicalTurk to a convenience sample of Asian (n = 166) and Hispanic (n = 184) parents of children 10-13 years. Main outcome measures were message comprehension, motivation, relevance, acceptability, and novelty. Engagement in the parenting practices was also assessed. RESULTS: Message comprehension was acceptable for the majority of parents. Most also agreed that messages were motivational (setting healthful beverage expectations (69.0%), making CRF/B available (67.4%), and role modeling (80.0%)), relevant and acceptable. About 30-50% indicated they had not seen the information before. Many parents indicated they were already engaging in the practices (> 70%). No racial/ethnic differences were observed for responses to messages or engaging in parenting practices. CONCLUSIONS: Results indicate that emotion-based messages designed to motivate parents to engage in parenting practices that promote calcium intake among early adolescents were motivating, relevant, and acceptable.

Profiles of Epstein-Barr Virus Associated Gastric Carcinomas in Brunei Darussalam

  • Background: Gastric cancer is the second most common gastrointestinal cancer and is largely attributed to Helicobacter pylori (H. pylori) infection. In addition, studies have also shown association with Epstein-Barr virus (EBV) in 10% of gastric cancers. This study assessed the characteristics of EBV associated gastric cancers (EBVaGC) in Brunei Darussalam. Materials and Methods: This study included gastric cancers diagnosed between 2008 and 2012, registered with the Department of Pathology RIPAS Hospital, Brunei Darussalam. Clinical case notes were systematically reviewed. Histology specimens were all stained for EBV and also assessed for intestinal metaplasia and H. pylori. Results: There were a total of 81 patients (54 male and 27 females) with a mean age of $65.8{\pm}14.8years$ included in the study. Intestinal metaplasia and active H. pylori infection were detected in 40.7% and 30.9% respectively. A majority of the tumors were proximally located (55.6%), most poorly differentiated (well differentiated 16%, moderately differentiated 30.9% and poorly differentiated 53.1%) and the stages at diagnosis were; stage I (44.4%), stage II (23.5%), stage III (8.6%) and stage IV (23.5%). EBV positivity (EBVaGC) was seen in 30.9%. Between EBVaGC and EBV negative gastric cancers, there were no significant differences (age, gender, ethnic group, presence of Intestinal metaplasia, tumor locations, stages of disease and degree of tumor differentiation). Conclusions: This study showed that a third of gastric cancers in Brunei Darussalam were positive for EBV, higher than what have been reported in the literature. However, there were no significant differences between EBVaGC and EBV negative gastric cancers. This suggests that the role of EBV in gastric cancer may be mostly incidental rather than any causal relation. However, further studies are required.

3R Variant of Thymidylate Synthase 5'-untranslated Enhanced Region Contributes to Colorectal Cancer Risk: A Meta-analysis

  • Background: Studies investigating the association of 2R/3R polymorphism in the thymidylate synthase 5'-untranslated enhanced region (TSER) and colorectal cancer (CRC) risk have reported conflicting results. Thus, a meta-analysis was performed to summarize the data on the potential association. Methods: Pubmed, Embase and CBM databases were searched for all available studies. Links between the TSER 2R/3R polymorphism and CRC risk were estimated by odds ratios (ORs) with 95% confidence intervals (CIs). Results: Seven case-control studies with a total of 2723 cases and 4030 controls were included in this meta-analysis. The results showed that the 3R variant of TSER 2R/3R polymorphism contributes to CRC risk in two comparison models (OR 3R vs. 2R =1.10, 95%CI 1.02-1.18, P = 0.015; OR Homozygote comparison model = 1.22 1.04-1.43, 95%CI 1.04-1.43, P = 0.012). Subgroup analyses by ethnicity further demonstrated a contribution in Caucasians with three comparison models (OR 3R vs. 2R = 1.10, 95%CI 1.02-1.19, P = 0.015; OR Homozygote comparison model = 1.21, 95%CI 1.03-1.41, P = 0.019; OR Recessive comparison model = 1.18, 95%CI 1.05-1.33, P = 0.008). However, the association in the Asian population was still uncertain due to the limited data (all P values were more than 0.05). Conclusions: Our meta-analysis suggests that the 3R variant of Thymidylate synthase 5'-untranslated enhanced region 2R/3R polymorphism contributes to gastric cancer risk in the Caucasian population, while any association in Asian populations needs further study.

Clinical Profile, Treatment and Survival Outcome of Testicular Tumors: A Pakistani Perspective

  • Background: Testicular cancer management is considered a marvel of modern science with excellent treatment results. Pakistan has a distinct ethnic variation and geographic distribution but data regarding clinical presentation of testicular tumors and their management is under reported. The objective of this study was to determine clinical profile, treatment modalities and survival outcome of testicular tumors in the Pakistani population. Materials and Methods: A retrospective review of patients who received treatment for testicular cancer at Shaukat Khanum Cancer Hospital from January 2009 to December 2012 was performed. Patient demographics, clinical features at presentation and treatment modalities were assessed. For categorical variables chi square test was used. Survival was calculated using Kaplan Meier survival curves and Log rank test was employed to determine significance. Results: The most common tumor was mixed germ cell tumor in 49% patients. For all tumor variants except seminoma, stage III was the most common clinical stage at presentation. Majority of patients with non seminomatous germ cell tumors presented in the15-30 year age group as compared to seminoma which was most prevalent in the 30-40 year age group. Orchiectomy followed by chemotherapy was the most common treatment modality in 80% patients. Expected 5 year survival for seminomas and non-seminomatous germ cell tumors was 96% and 90% respectively which was not significantly different (p=0.2). Conclusions: Despite a distinct clinical profile of testicular tumors in Pakistani population, survival is comparable with published reports.

Difference in the Incidences of the Most Prevalent Urologic Cancers from 2003 to 2009 in Iran

  • Background: Urological cancers represent a major public problem associated with high mortality and morbidity. The pattern of these cancers varies markedly according to era, region and ethnic groups, but increasing incidence trends overall makes focused epidemiological studies important. The aim of the present study was to assess the incidence of most prevalent urological cancers in Iran from 2003 to 2009. Materials and Methods: The data for this study were obtained from the population-based Cancer Registry Center of the Iran Ministry of Health and Medical Education. Differences of mean age and age distributions of each cancer were compared between 2003 and 2009 in men and women. Results: Bladder cancer was the most common urologic cancer in both genders. The rate difference of age standardized ratio (ASR) of bladder and renal cell carcinoma in women were 1.54 and 2.01 percent per 100,000 population from 2003 to the 2009, respectively. In men, the rate difference of age standardized ratio of prostate, testis, kidney and bladder cancer was also 2.23, 1.2, 1.8 and 1.5 percent per 100,000 population from 2003 to 2009, respectively. The mean ages of patients in all cancers in both genders did not differ significantly through time (p value>0.05) but the distribution of ages of patients with bladder and prostate cancer changed significantly from 2003 to 2009 (p value<0.001). Conclusions: The results of present study suggest the general pattern and incidence of urological cancers in Iran are changing, the observed increase pointing to a need for urological cancer screening programs.

Health Disparities between Black Hispanic and Black Non-Hispanic Cervical Cancer Cases in the USA

  • Background: Globally, cervical cancer is a major public health concern. Cervical cancer is the second most common cancer among women, resulting in approximately 500,000 cases per year. The purpose of this study is to compare disease characteristics between Black Hispanic (BH) and Black non-Hispanic (BNH) women in the US. Materials and Methods: We used stratified random sampling to select cervical cancer patient records from the SEER database (1973-2009). We used Chi-square and independent samples t-test to examine differences in proportions and means. Results: The sample included 2,000 cervical cancer cases of Black non-Hispanic and 91 Black Hispanic women. There were statistically significant differences between black Hispanic and black non- Hispanics in mean age at diagnosis (p<0.001), mean survival time (p<0.001), marital status (p<0.001), primary site of cancer (p<0.001); lymph node involvement (p<0.001); grading and differentiation (p<0.0001); and tumor behavior (p<0.001). Black women were more likely to develop cervical cancer and to have the highest mortality rates from the disease. Conclusions: Findings from this study show clear racial and ethnic disparities in cervical cancer incidence and prognosis that should be addressed.

  • A series of immoral conducts are repeatedly occurring by some commanders and high-ranking officers in the ROK navy. They are seriously compromising not only harmony and solidarity of the organization, but also pride and morale of members of the service. They seem to be results of the fragile ethical background of the service as well as of failures in military education and training on ethics. This paper sees backgrounds of weak ethical foundations of the navy as follows: the enduring legacies of cultural factors, such as the toxic military culture of the Japanese colonization, dark side of mariners' traditions, the sea-aversive Confucian culture, and unique characteristics in any small organization or society, etc... This paper also suggests the current ethnic systems of the navy are primarily based on responsive measures against misdeeds, rather than fundamental prescriptions for immoral behaviors, in the absence of ethical instrument for warriors. The paper also emphasizes the importance of developing character-based leadership, founded on, above all, respects for human integrity and strong sense of honor. Moreover, the importance of accountability in charging the command, as advocated by the US navy, is also stressed in order to establish a more robust ethic ground within the navy. Based on these analyses, the author proposes that in order to become a stronger and healthier fighting force, the navy needs to make continuous command efforts and organizational innovations to improve ethic principle and moral standard of the service, to strengthen ethics of naval leaders by a robust system of ethical education and training programs, and ultimately to solidify a navy culture in which moral passions and energies are spreading over.

Cognitive Distance Mapping: a Survey-Based Experiment Using GPS and GIS

  • 본 연구는 인간이 인지거리를 지도화 함에 있어, 성별, 연령, 학년, 수학기간 등의 개인적인 요인과 거리 추정에 사용된 기준점의 상대적인 위치가 거리 추정에 어떤 영향을 주는 지를 위치정보 시스템을 이용한 현장 실험을 통해 파악하고자 하였다. 연구 방법으로는, 미국 캔자스대학교에 재학 중인 학부생과 대학원생을 대상으로 캠퍼스 방문을 통한 설문조사를 실시하였다. 자료분석 결과, 여학생 그룹의 거리추정 정확도가 인종이나 학년에 관계없이 남학생 그룹에 비해 높았으며, 전체적으로 학년과 연령이 높아지고 수학기간이 길어짐에 따라 거리 추정 정확도의 분산정도가 감소하는 것으로 나타났다. 인지거리 지도화를 위해 제시된 2개 기준점에 가까이 위치한 건물 일수록 거리 추정의 정확도가 높았으며, 설문조사 지점이 기준점에 가까울수록 거리 추정의 오차가 감소하였다. 상관관계 분석 결과, 남학생 그룹의 거리추정 오차값이 여학생 그룹에 비해 건물-기준점 간 거리, 그리고 피실험자-기준점 간 거리와 더 강한 상관관계를 가진 것으로 보아, 남학생이 여학생에 비해 기준점에 근거한 공간적 축척을 더 면밀하게 이용하는 것으로 판단된다.
