만성(慢性) Acohol 섭취(攝取) mouse에서 alcohol 대사(代謝) 효소활성(酵素活性)에 미치는 인삼(人蔘)의 영향(影響) (Effect of Ginseng on the Hepatic Alcohol Metabolizing Enzyme System Activity in Chronic Alcohol-Treated Mouse)

  • The present study was undertaken to investigate an effect of ginseng butanol fraction(total saponin) on the hepatic ethanol metabolism, we used experimental animals for the subject of study. When, in case of ADH and MEOS, ginseng butanol fraction was added, enzyme activity was increased in a small dose, and on the contrary, in a large dose, showed inhibitory effect. in catalase, the activity showed no significant effect by adding ginseng butanol fraction. In the light of kinetic aspect, when, in reaction mixture, ethanol and ginseng butanol fraction were concurrently added and reacted, Km value of ADH and MEOS was decreased. After pretreated with ginseng butanol fraction and inducement of acute toxic state by ethanol, the activities of ADH and MEOS were increased to an extent of about 25% compared to controls. But catalase activity was not significantly affected. In case that ginseng butanol fraction was given to mice fed with 5% ethanol instead of water for 60 days, the activities of ADH and MEOS were increased about 20% to 50% compared to ethanol-treated group. On the contrary, catalase activity was not affected. But blood concentrations of ethanol were decreased due to ginseng butanol fraction treatment. All these observations suggested that reduction of ethanol blood concentration should be dependent upon increased activities of ADH and MEOS. Thereby it suggests the recovery from alcohol intoxication can be prompted by treatment with ginseng.

The Significance of Clinical Examination for Brain Lesion Differentiation of Patients with Head Trauma after Alcohol Intoxication

  • Purpose: There are many patients visited to ED in an alcohol intoxicated state. For these patients, it is difficult to predict by only clinical examination whether he/she would have brain lesion. The purpose of this study is to research whether it is possible to predict brain lesion by only clinical examination findings, with comparing patients with/without actual brain lesions. Methods: A retrospective study was performed at a university hospital for the period 11 months with the medical records. As for the inclusion group, head trauma patients with objectively proved drunk, judging by their blood ethanol concentration, and performed the brain CT were selected. In terms of medical record, Glasgow coma scale (GCS), the presence of neurologic abnormalities, the presence of lesion on brain CT of the patients, were examined. From laboratory results, blood ethanol concentration, platelet count, prothrombin time (PT), activated partial thromboplastin time (aPTT) and glucose concentration were identified. Results: For this study, there were total 80 patients of inclusion group. There was no statistically significant difference in terms of GCS score and neurological examination abnormalities, between the group with brain lesion and the group without brain lesion on brain CT. Conclusion: Alcohol intoxicated patient with head trauma visits the ED, it is not possible to distinguish or determine whether brain lesion exists or not by only clinical findings. In order to check the lesion existence, the image examination, therefore, should be considered and performed.

마우스의 아세트알데히드 대사에 미치는 인삼 부탄올 분획의 영향 (The Effect of Ginseng Butanol Fraction on the Acetaldehyde Metabolism in Mice)

  • The present study was undertaken to investigate the possible effect of ginseng butanol fraction on the hepatic acetaldehyde metabolism. Experimental animals were used for the subject of the study. When, in case of mitochondrial aldehyde dehydrogenase (Ald DH), ginseng butanol fraction was added, enzyme activity was increased in a small dose, while, in a large dose, it showed inhibitory effect. In terms of kinetic aspect, ginseng butanol fraction has the effect to decrease the Km values of Ald DH. In vivo studies, the activity of Aid DH increased by induction of acute intoxication of ethanol was further increased through pretreatment with ginseng butanol fraction. When ginseng butanol fraction was given to mice fed with 5% ethanol instead of water for 60 days, the activity of Ald DH in mitochondrial fraction decreased to about 35% in chronic alcoholism, but after pretreatment of ginseng butanol fraction the activity was restored to the control level. By the pretreatment with disulfiram, the Ald DH activity was inhibited in normal and alcohol-treated groups, but after the treatment with ginseng butanol fraction the activity was restored to the control level. The results suggest that ginseng butanol fraction enhance the Ald DH activity inhibited by the treatment of disulfiram with no relation to NAD. It was observed that ginseng butanol fraction markedly decrease the acetaldehyde levels in plasma and liver. All these observations suggested that reduction of acetaldehyde in blood and liver should be dependent upon increased activity of mitochonclrial Ald DH. It is concluded that the recovery from alcohol intoxication should be prompted by treatment with ginseng.

갈화 약침이 알콜 중독 흰쥐의 치상회에서 신경세포 생성에 미치는 영향 (Effects of Puerariae flos herb-acupuncture on cell proliferation and neurogenesis in the dentate gyrus of ethanol-induced Sprague-Dawley rats)

  • The purpose of this study was to determine the effects of Puerariae flos herb-acupuncture on hippocampal neural cell proliferation. Sprague-Dawley rats were randomly assigned into 4 groups; control group, control with herb-acupuncture group, alcohol group, alcohol with herb-acupuncture group group. Control groups were received with NaCl, while alcohol intoxication groups were injected intraperitoneally with alcohol (2 g/㎏) twice per day for 3 days. Herb-acupuncture groups were injected on Zhongwan (CV 12) for 5 consecutive days. Bromo-deoxyuridine (BrdU) was injected into all animal per day for 5 days. For the detection of BrdU-positive cells in dentate gyrus of hippocampus, immunohistochemistry was performed. In alcohol group, a significant decrease in BrdU-positive cells was observed compared to control group. In alcohol with herb-acupuncture group, BruU-positive cells increased significantly compared to alcohol group. In conclusion, the present results revealed that new cell proliferation is enhanced in the dentate gyros of young Sprague-Dawley rats through Puerariae flos herb-acpuncuture in an acute alcoholic intoxication condition.

  • Objective: The purpose of this study was to determine the effects of Puerariae radix(PR) herb-acupuncture on nitric oxide synthase(NOS) expression in hippocampus of alcohol-intoxicated Sprague-Dawley(SD) rats. Methods: SD rats were randomly assigned into 6 groups; normal group, control group, alcohol with herb-acupuncture group(0.3, 3, 30 and 300mg/kg PR). Normal groups were received with NaCl, while alcohol intoxication groups were injected intraperitoneally with alcohol(2g/kg) twice per day for 3 days. Herb-acupuncture groups were injected on Zhongwan(CV12) for 5 consecutive days. For the detection of NADPH-d-positive cells in hippocampus, immunohistochemistry was performed. Results: n control group, a significant decrease in NADPH-d-positive cells was observed compared to normal group. In alcohol with herb-acupuncture group, NADPH-d-positive cells increased significantly compared to control group. Conclusions: The present results revealed that NOS expression is enhanced in the hippocampus of SD rats through PR herb-acupuncture in an acute alcoholic intoxication condition.

Hepatoprotective Evaluation of Ganoderma lucidum Pharmacopuncture: In vivo Studies of Ethanol-induced Acute Liver Injury

  • Objectives: Alcohol abuse is a public issue and one of the major causes of liver disease worldwide. This study was aimed at investigating the protective effect of Ganoderma lucidum pharmacopuncture (GLP) against hepatotoxicity induced by acute ethanol (EtOH) intoxication in rats. Methods: Sprague-Dawley (SD) rats were divided into 4 groups of 8 animals each: normal, control, normal saline pharmacopuncture (NP) and GLP groups. The control, NP and GLP groups received ethanol orally. The NP and the GLP groups were treated daily with injections of normal saline and Ganoderma lucidum extract, respectively. The control group received no treatment. The rats in all groups, except the normal group, were intoxicated for 6 hours by oral administration of EtOH (6 g/kg BW). The same volume of distilled water was administered to the rats in the normal group. Two local acupoints were used: Qimen (LR14) and Taechung (LR3). A histopathological analysis was performed, and the liver function and the activities of antioxidant enzymes were assessed. Results: GLP treatment reduced the histological changes due to acute liver injury induced by EtOH and significantly reduced the increase in the alanine aminotransferase (ALT) enzyme; however, it had an insignificant effect in reducing the increase in aspartate aminotransferase (AST) enzyme. It also significantly ameliorated the superoxide dismutase (SOD) and the catalase (CAT) activities. Conclusion: The present study suggests that GLP treatment is effective in protecting against ethanol-induced acute hepatic injury in SD rats by modulating the activities of ethanol-metabolizing enzymes and by attenuating oxidative stress.

급성 중독환자에서 삼투압 계산식으로 추정된 에탄올 농도의 유효성 검증 (Discrepancies and Validation of Ethanol Level Determination with Osmolar Gap Formula in Patients with Suspected Acute Poisoning)

  • Purpose: Osmolar gap (OG) has been used for decades to screen for toxic alcohol levels. However, its reliability may vary due to several reasons. We validated the estimated ethanol concentration formula for patients with suspected poisoning and who visited the emergency department. We examined discrepancies in the ethanol level and patient characteristics by applying this formula when it was used to screen for intoxication due to toxic levels of alcohol. Methods: We retrospectively reviewed 153 emergency department cases to determine the measured levels of toxic ethanol ingestion and we calculated alcohol ingestion using a formula based on serum osmolality. Those patients who were subjected to simultaneous measurements of osmolality, sodium, urea, glucose, and ethanol were included in this study. Patients with exposure to other toxic alcohols (methanol, ethylene glycol, or isopropanol) or poisons that affect osmolality were excluded. OG (the measured-calculated serum osmolality) was used to determine the calculated ethanol concentration. Results: Among the 153 included cases, 114 had normal OGs (OG≤14 mOsm/kg), and 39 cases had elevated OGs (OG>14). The mean difference between the measured and estimated (calculated ethanol using OG) ethanol concentration was -9.8 mg/dL. The 95% limits of agreement were -121.1 and 101.5 mg/dL, and the correlation coefficient R was 0.7037. For the four subgroups stratified by comorbidities and poisoning, the correlation coefficients R were 0.692, 0.588, 0.835, and 0.412, respectively, and the mean differences in measurement between the measured and calculated ethanol levels were -2.4 mg/dL, -48.8 mg/dL, 9.4 mg/dL, and -4.7 mg/dL, respectively. The equation plots had wide limits of agreement. Conclusion: We found that there were some discrepancies between OGs and the calculated ethanol concentrations. Addition of a correction factor for unmeasured osmoles to the equation of the calculated serum osmolality would help mitigate these discrepancies.

한국당귀(Angelicae gigantis Radix) 엑스가 흰쥐의 약물성 간장해에 대한 보호 효과 (Protective Effects of Angelicae gigantis Radix Extract on Hepatic Injury Induced by Toxic Drugs in Rats)

  • This study was attempted to investigate the effect of 'Angelicae gigantis Radix extract (AG.EX.)' and 'Angelicae acutilobae Radix extract (AA.EX.)' on the activities of GOT, GPT and alkaline phosphatase (A1.P), the contents of total cholesterol in serum of $CCl_4$ and D-galactosamine intoxicated rats, and the weight change ratio of body, liver and spleen in $CCl_4-intoxicated-rats$ by administering the extract of 300 and 500 mg/kg P.O.. Significant test was performed by comparision with the biochemical values between intoxicated-control group and extract-administered group. The activities of s-GOT, s-GPT and the contents of total cholesterol elevated by $CCl_4-intoxication$ were significantly decreased in all dose (300, 500 mg/kg) of Angelicae gigantis Radix-water extract (AG.WEX.) and alcohol extract (AG.AEX), and Angelica acutilobae Radix-water extract (AA.WEX.) and alcohol extract (AA.AEX.), respectively, as compared with the control group. And administered group of 300 mg/kg showed more significant decreasing effect than 500 mg/kg, and more significantly decreased in water extract of AG.EX. and ethanol extract of AA.EX. But in the activities of s-A1.P. inhibition effect were significantly decreased only in a dose of 300 mg/kg of AA.WEX. and AA.AEX. The activities of s-GOT and s-GPT elevated by D-galactosamine were not decreased in all samples, as compared to intoxicated-control group. But the activities of s-Al.p was significantly decreased as compared with control groups, in all samples and administration of 300 mg/kg was more significantly decreased than 500 mg/kg. The contents of total cholesterol remarkably decreased than the normal groups by D-galactosamine intoxication was not recovered in all samples. The increasing rate of the body weight increased by $CCl_4-intoxication$ were not decreased than the $CCl_4-control$ group in all sample groups. The increasing rate of liver weight increased by $CCl_4-intoxication$ were significantly decreased in 300 mg/kg of AA.AEX.AG.WEX. and AA.WEX., respectively, as compared with $CCl_4-control$ group.

  • The ameliorating effect of the MeOH extract of the root of Platycodon grandiflorum on the ethanol-induced cognitive dysfunction in mice was investigated. The total MeOH extract was devided into two fractions, saponin rich fraction and non-saponin fraction. The mice with repeated administration of the total MeOH extract and those with saponin rich fraction both showed a markedly prolongεd latency time upon the step-through test (passive avoidance performance) performed after acute ethanol intoxication, while those with non-saponin rich fraction did not. The results suggest that the extracts from the root of Platycodon grandiflorum exert a beneficial effect on ethanol-induced memory impairment in mice and also suggest that these effects of the extracts might be ascribed to the saponin constituents.

Effects of Dried Persimmon Snacks on Alcohol Metabolism in Men

  • Persimmon has been known to help alcohol intoxication in Korea for a long time and is prepared as a processed food. The effect of dried persimmon snacks on alcohol metabolism was determined in vivo. Eight Korean men (25~27 years old) were administered 3.5 mL/kg of alcohol (22.5%, w/v) with or without a dried persimmon snack (2g/kg). The levels of alcohol in the blood and of acetaldehyde in the blood and the urine were determined by alcoholmeter and assay using alcohol dehydrogenase and HPLC, respectively. All subjects showed decreased levels of alcohol in blood, and six subjects showed a decrease of alcohol in urine after consumption of ethanol with dried persimmon snacks. Concentration of acetaldehyde in the blood an urine decreased significantly in three and five of the eight subjects, respectively. Reduction was more significant for alcohol than for alcohol than for acetaldehyde with administration of ethanol and dried persimmon snacks. This in vivo result suggests that dried persimmon snacks are effective in decreasing th concentration of alcohol after alcohol intake.
