Chinical and Nutritional Examination in Examination in Obese Children and Adolescents (일부 비만아동 및 청소년에 대한 임상영양학적 조사연구)

  • The purpose of this study was to investigate the relationship between body fat and risk factors of chronic diseases in obese children and eventually to contribute to the prevention and treatment of childhood obesity. Anthropometric parameters such as height, weight, skinfold thickness, circumferences, body fat content were measured in 55 obese subjects of age 5-20. Blood pressure, serum components, daily food intakes were also considered. The average age of the subjects was 12.5$\pm$3.3yrs and the mean obesity index was 64.7$\pm$21.3%. The average percentage of body fat was 35.6$\pm$5.4% and the mean weight of lean body mass was 45.3$\pm$13.9kg. Mean total muscle weight of the subjects was estimated to be 34.11$\pm$1.3kg. The mean systolic and diastolic blood pressures of the obese were 134.8$\pm$15.8mmHg and 69.6$\pm$11.6mgHg respectively. The concentrations fo serum triglyceride, total cholersterol, HDL-cholesterol, LDL-cholesterol were estimated to be 246.0$\pm$136.4mg/dl, 257.0$\pm$54.3mg/dl, 48.2$\pm$13.1mg/dl and 158.9$\pm$53.5mg/이 respectively. The concentrations of serum GOT and GPT were 34.6$\pm$17.1U/L and 24.7$\pm$15.3U/L. The mean content of uric acid is serum was 6.2$\pm$1.9mg/dl. Of these 55 children, 82% has hyperlipidemia, 26% was abnormal liver function and 24% was hyperuricemia. One patient was diabetic. Eighty seven% of them have developed more than one complications.

Comparison of the dietary intake and clinical characteristics of obese and normal weight adults

  • Obesity contributes to an increased risk for chronic diseases, including diabetes, cardiovascular diseases, and certain types of cancer. The prevalence of obesity has increased in Korea. We compared the clinical and dietary characteristics of obese adults (n = 30, 17 men and 13 women, mean age 29.9) to those with a normal weight (n = 15, 8 men and 7 women, mean age 26.5). We determined lipid profiles, fasting blood sugar (FBS), blood pressure, and serum free fatty acid (FFA). Dietary intake was estimated using a food frequency questionnaire (FFQ) and a 3-day dietary record. Exercise patterns and average alcohol intake were determined. The average body mass index was 28.3 kg/$m^2$ in the obese and 21.2 kg/$m^2$ in the normal weight groups. The obese group had significantly higher levels of total cholesterol, LDL cholesterol, and triglycerides, lower levels of HDL cholesterol, and higher blood pressures compared to the normal weight group. FBS was not significantly different between the two groups. The obese group had significantly more subjects with metabolic syndrome (26.7%) compared to the normal weight group (0%). Serum FFA levels tended to be higher in the obese (P = 0.087). No significant difference in caloric intake was observed between the two groups. No differences in carbohydrate, protein, or fat intake between two groups were observed from the FFQ. However, results from the 3-day dietary record showed that the percentage of energy from fat was significantly higher in the obese group. The frequency and amount of exercise did not differ between the two groups. Alcohol consumed per drinking episode was significantly higher in the obese group. These results confirm that excessive weight is associated with disturbances in lipid metabolism in these fairly young and otherwise healthy adults. Dietary factors, including higher fat intake and alcohol consumption, seem to be contributing to the obesity of these subjects.

A Study on Blood Lipids and Blood Pressure of Adult Men and Women According to Vegetable Intake (일부 성인남녀의 채소 섭취량에 따른 혈중 지질 및 혈압에 관한 연구)

  • It has been suggested that vegetables protect the cardiovascular system in part by attenuating blood pressure. The purpose of the present research was to examine blood lipids according to vegetable intakes. Anthropometric measurements, blood pressures, nutrient intakes using the 24-hour recall method, and serum lipids of <50th percentile vegetable intake group (<50th percentile VIG; men=66, women=111) and ${\ge}50th$ percentile vegetable intake group (${\ge}50th$ percentile VIG; men=83, women=94) were estimated. The average age, height, and BMI were 54.7 years, 158.2 cm, 62.2 kg, and $24.9kg/m^2$ for <50th percentile VIG and 53.7 years, 159.6 cm, 63.0 kg, and $24.7kg/m^2$ for ${\ge}50th$ percentile VIG, respectively. The daily food intake of the ${\ge}50th$ percentile VIG was significantly higher than that of the <50th percentile VIG (p<0.001). Also, daily intakes of cereals (p<0.001), legumes (p<0.05), nuts (p<0.05), vegetables (p<0.001), and fruits (p<0.05) of the ${\ge}50th$ percentile VIG were significantly higher than those of the <50th percentile VIG. The daily energy intakes of ${\ge}50th$ percentile VIG and <50th percentile VIG were 1342.7 kcal and 1782.0 kcal (p<0.001), and most nutrient intakes of the ${\ge}50th$ percentile VIG was significantly higher than that of the <50th percentile VIG. Serum cholesterol of the ${\ge}50th$ percentile VIG were significantly lower than that of the <50th percentile VIG (p<0.01). Also, vegetable intake showed significantly negative correlations with total cholesterol (p<0.05) and LDL-cholesterol (p<0.05). Based on these results, it should be emphasized that increase of vegetable intake improves the blood lipid profile.

Dietary iodine intake and the association with subclinical thyroid dysfunction in male workers (직장인 남성에서 식이 요오드섭취량과 불현성 갑상선기능이상과의 연관성)

  • The prevalence rate of thyroid dysfunction (hypothyroidism and hyperthyroidism) has increased within the Korean population and seems to be affected by iodine dietary habits. Some studies reported that the prevalence of thyroid dysfunction increase both in the area of iodine deficiency and excess. In this study, we tried to discover the difference in iodine intake, anthropometric measurements, and blood parameters between male subjects with or without subclinical thyroid dysfunction. A total of 5,249 subjects (Euthyroid: 4706, SubHypo: 454, SubHyper: 89) were used in this study. There were no significant differences in BMI, body fat, visceral fat, waist circumference, SBP, DBP, total cholesterol, HDL-cholesterol, LDL-cholesterol, TG, fasting serum glucose, HbA1c, alcohol intake, however significant differences were noticed in both age and smoking status. Through a food frequency questionnaire (FFQ), iodine intake per day was estimated. The average iodine intake was similar (SubHypo $392.9{\pm}279.0{\mu}g$, Euthyroid $376.5{\pm}281.7{\mu}g$, SubHyper $357.3{\pm}253.8 {\mu}g$) among groups. The main source of iodine intake was eggs (52.8%, 54.2%, 52.4%) followed by milk (16.3%, 15.8%, 17.8%), then sea mustard & sea tangle (12.4%, 11.9%, 11.6%). The prevalence of subclinical hypothyroidism and subclinical hyperthyroidism was higher in subjects whose intake was higher than the recommended nutrient intake (RNI). These results suggest that the excess consumption of iodine intake may act as one of the risk factors regarding thyroid dysfunction in Korea. Therefore, an adequate amount of iodine intake is necessary in order to prevent subclinical thyroid dysfunction and clinical thyroid dysfunction.

Protective effect of silymarin in streptozotocin-induced diabetic dyslipidaemia in rats

  • The present study investigated the effect of silymarin, a flavonoid, on streptozotocin (STZ) - induced diabetic dyslipidaemia in rats. Experimental diabetes was induced by a single intraperitoneal injection of STZ (60 mg/kg). Silymarin (25 mg/kg and 50 mg/kg) was orally administered to diabetic rats for a period of 15 days. Blood glucose levels, serum lipid profile and liver glycogen levels were estimated following the established procedures. Biochemical observations were supplemented with histological examination of liver sections. Oral administration of silymarin to diabetic rats significantly (P < 0.001) decreased the blood glucose levels ($259.99{\pm}23.64$ vs. $99.90{\pm}2.62$ [25 mg] & $89.17{\pm}3.32$ [50 mg]). The most interesting finding was the significant (p < 0.001) increase in HDL-cholesterol levels ($26.99{\pm}0.61$ vs. $40.55{\pm}0.52$ [25 mg] & $41.12{\pm}0.37$ [50 mg]) whereas, there was a significant decrease in serum total cholesterol (TCh), triglycerides (TG), low density lipoprotein (LDL) and very low density lipoprotein (VLDL) cholesterol levels observed in silymarin treated diabetic rats. STZ treatment caused significant degeneration of liver parenchyma, which was normalized to near normal morphology by administration of silymarin. The findings indicate that silymarin effectively improved the overall lipid profile and restored the glycogen stores in the liver of STZ-induced diabetic rats, in a dose dependent manner. The results indicate existence of abnormalities in lipid metabolism in STZ-induced diabetic rats and suggest a protective effect of silymarin in this animal model.

Relationship between Dietary, Blood and Urinary Levels of Lead, Blood Pressure and Serum Lipids in Korean Rural People on Self-Selected Diet (일부 농촌 성인 남녀의 식이, 혈액 및 뇨중의 납수준과 혈압, 혈청지질과의 관계)

  • This study was performed to evaluate the relationship between dietary, blood and urinary levels of lead and blood pressure and serum lipids in 30 healthy adult living in rural area of Korea. Analysis for the nutritional status of subjects were performed by 3-day dietary intake record, duplicated diet collection, 24-hour urine collection, and venous blood sampling before anthropometry. The mean daily intakes of lead estimated for 3 days was $120.1{\pm}22.0\mu\textrm{g}$. The blood levels and 24-hour urinary excretion of lead were $10.8{\pm}3.6\mu\textrm{g}/dl\;and\;36.5{\pm}9.5\mu\textrm{g}$, respectively. The systolic and diastolic blood pressure were $113.0{\pm}16.9mmHg\;and\;76.7{\pm}12.1\;mmHg$. The serum levels of total cholesterol, triglyceride, HDL-cholesterol, LDL-cholesterol and ${\beta}-lipoprotein$ were $158.8{\pm}32.9mg/dl,\;104.6{\pm}48.8mg/dl,\;45.7{\pm}9.9mg/dl,\;92.2{\pm}28.5mg/dl\;and\;426.4{\pm}141.5mg/dl$, respectively. There was no significance in the relation between lead and blood pressure. In the relation between lead and serum lipids, it showed negative correlation with lead intake and HDL-choleterol at the level of significance of p<0.01. But there was no significance in the relation between lead and serum levels of otehr lipids.

Comparison of Adverse Events between High-intensity and Moderate- to Low-intensity Statin Group (의약품부작용보고시스템 데이터베이스를 이용한 고강도 statin과 중·저강도 statin 관련 이상사례 비교 분석)

  • Background: 3-Hydroxy-3-methylglutaryl coenzyme A reductase inhibitors (statins) effectively reduce serum levels of low-density lipoprotein (LDL) and total cholesterol. High-intensity statins are recommended for all patients aged ${\leq}75$ with clinical atherosclerotic cardiovascular disease (ASCVD), diabetes mellitus aged 40-75 with ${\geq}7.5%$ estimated 10-year ASCVD risk and LDL-C ${\geq}190mg/dL$. High-intensity statins associated with more frequent adverse events (AEs) compared to moderate- to low-intensity statins. The aim of this study was to compare AEs between high-intensity and moderate- to low-intensity statin group using the Korea Adverse Event Reporting System (KAERS) database. Methods: Adults (${\geq}18years$) with statin-associated AEs from July 2009-June 2014 were included. Only AEs classified as "certain", "probable" and "possible" based on the WHO-Uppsala Monitoring Center criteria were analyzed. Results: In total, 247 AEs from 196 patients [high-intensity statin group (HG), n = 25 (13%); moderate- to low-intensity statin group (MLG), n = 171 (87%)] were included. Mean age was higher in HG compared with MLG ($67{\pm}14$ vs $62{\pm}12$). The HG showed a significant higher frequency of liver/biliary system disorders (37% vs 14%, p = 0.001). Hepatic function abnormal was reported more frequently in HG compared to MLG (26% vs 9%, p = 0.006). Conclusion: According to KAERS data, liver/biliary system disorders were more frequently reported in HG compared to MLG.

The Effects of Exercise on the Vitamin C and E Intakes and Their Plasma Levels of Vitamin C, $\alpha$-tocopherol and $\gamma$-tocopherol in Young Male Adults (운동량이 젊은 성인 남자의 비타민 C와 E 섭취량 및 혈장 비타민 C, $\alpha$-tocopherol 및 $\gamma$-tocopherol 수준에 미치는 영향)

  • We investigated the effect of physical exercise on the intakes and plasma levels of vitamin C and E in young male adults. The study population were divided into two groups: small-amount exercisers($\leq$ 30min/day, n=29) and moderate-amount exercisers(> 30min/day, n=32) according to their physical exercise habits measured by a questionnaire. Dietary intakes of vitamin C and E of the subjects were estimated by the 24 hour recall method. Plasma lipid profiles, vitamin C, $\alpha$-tocopherol and ${\gamma}$-tocopherol levels were analyzed. No significant differences were observed in total cholesterol, TG, HDL-C and LDL-C of plasma between two groups. Plasma vitamin C and $\alpha$-tocopherol levels in moderate-amount exercisers were significantly higher than those in small-amount exercisers, although dietary intakes of vitamin C and vitamin E were not significantly different between two groups. However, plasma ${\gamma}$-tocopherol level of moderate-amount exercisers did not significantly differ from small-amount exercisers. There was a positive correlation between the amount of exercise and plasma $\alpha$-tocopherol level. These results suggest that moderate amount of exercise over 30 min per day would lead to a healthy life with an activated antioxidant defence system against oxidative stress.(Korean J Nutrition 34(3) : 306~312, 2001)

Association between dietary flavanones intake and lipid profiles according to the presence of metabolic syndrome in Korean women with type 2 diabetes mellitus

  • BACKGROUND/OBJECTIVES: This study was aimed at examining the association between dietary flavanones intake and lipid profiles according to the presence of metabolic syndrome (MetS) in Korean women with type 2 diabetes mellitus (T2DM). SUBJECTS/METHODS: A cross-sectional analysis was performed among 502 female T2DM patients (non-MetS group; n = 129, MetS group; n = 373) who were recruited from the Huh's Diabetes Clinic in Seoul, Korea between 2005 and 2011. The dietary intake was assessed by a validated semi-quantitative food frequency questionnaire (FFQ) and the data was analyzed using the Computer Aided Nutritional Analysis program (CAN-Pro) version 4.0 software. The intake of flavanones was estimated on the basis of the flavonoid database. RESULTS: In the multiple linear regression analysis after adjustment for confounding factors, daily flavanones intake was negatively associated with CVD risk factors such as total cholesterol, LDL-cholesterol, and apoB and apoB/apoA1 ratio only in the MetS group but not in the non-MetS group. Multiple logistic regression analysis revealed that the odds ratio for a higher apoB/apoA1 ratio above the median (${\geq}0.74$) was significantly low in the $4^{th}$ quartile compared to that in the $1^{st}$ quartile of dietary flavanones intake [OR: 0.477, 95% CI: 0.255-0.894, P for trend = 0.0377] in the MetS group. CONCLUSIONS: Dietary flavanones intake was inversely associated with the apoB/apoA1 ratio, suggesting a potential protective effect of flavanones against CVD in T2DM women with MetS.

Effect of Mixed Extract of Ephedrae Sinica and Fibrosum Gypsum on Differentiation of Preadipocytes and Obesity of Rats (마황(麻黃)과 석고(石膏) 혼합물이 지방전구세포 분화와 흰쥐의 비만에 미치는 영향)

  • Objectives This study was designed to evaluate the effects of Ephedra Sinica and Fibrosum Gypsum extract on obesity by using 3T3-L1 cells and high fat diet rats. Methods In vitro, Ephedra Sinica and Fibrosum Gypsum extract (50, 100, 200, $500{\mu}g/ml$) were added in 3T3-L1 cells. Cytotoxicity was measured by MTT assasy. Adipocyte differentiation was measured by Oil Red O staining, GPDH activity and $C/EBP{\alpha}$ protein expression. In vivo, Sprague-Dawley rats were divided into 5 groups : Normal diet group (Normal group), taken high fat diet and no treatment group (Control group), taken high fat diet and orally administered Ephedra Sinica and Fibrosum Gypsum extract daily (Group I: 50 mg/kg, Group II: 100 mg/kg, Group III : 200 mg/kg, oral). For 6 weeks of administration, body weight and the amount of food intake were measured once a week. After administration, blood analysis (AST, ALT, T-Bilirubin, BUN, RBC, Hb, HCT), serum lipid level (triglyceride, Total cholesterol, HDL, LDL), serum leptin level, epididymal adipose tissue weight and histological finding of liver were estimated. Results In vitro, The cytotoxicity was not significant. 3T3-L1 cell's differentiation was significantly decreased in Oil Red O staining, GPDH activity and $C/EBP{\alpha}$ protein expression. In vivo, Body weight and the amount of food intake, AST, ALT, Total cholesterol, TG, LDL, serum leptin, epididymal adipose tissue weight showed significant decrease in group I, group II and group III. There were no significant difference in T-bilirubin, BUN, RBC, Hb and HCT between all groups. HDL showed significant increase in group I, group II and group III. In histological finding of liver tissue, there were decreased adiposity and cytopathic effect in group I, group II and group III. Conclusions It is suggested that Ephedra Sinica and Fibrosum Gypsum extract can be used in the treatment of obesity.
