How to Integrate the Fourth Industrial Revolution in the Healthcare Industry? (의사인력정책, 근거 중심으로 접근하자)

  • The issue of increasing the number of physicians is emerging. Because the physician workforce is a critical component of the health care system, and substantial costs are involved in training personnel, a cautious approach is required. The demand to increase the number of physicians is based on the contention that there are difficulties in accessing essential health services and the need to prepare for future demands such as fostering physician-scientists. However, simply increasing the number of physicians is not an appropriate approach to address these demands, especially considering that the effects of such an increase will appear 10 years later. Moreover, it is concerned that the current argument for increasing the physician workforce is intertwined with political interests. When considering the impact on the health care system, decisions regarding the expansion of the physician workforce should be based on evidence. Additionally, rather than temporarily responding to social issues, it is expected that a governance system will be established to continuously discuss and decide on fostering medical personnel.

Job Analysis of Hospital Coordinator Based on the DACUM Method (DACUM기법에 의한 병원코디네이터의 직무분석)

  • The purpose of this study was to analyze of Hospital Coordinator based on the DACUM(Developing A Curriculum) method. The contents of this study were to extract the duties, tasks consisting of job of Hospital Coordinator and to investigate levels of importance, difficulty, frequency and entry level on each task, and to make out a job model of Hospital coordinator. A DACUM committee was composed to analyze job of Hospital Coordinator and the committee members were total 17, a facilitator, 15 hospital administrator and a recorder. The major findings of this study were as the followings ; first, duties in job of Hospital Coordinator were total 10, which were organization of Hospital Coordinator affairs, customerfacing services, consultation support, customer counsel, customer management, financial management, medical service planning, medical service marketing, organizational management, image making management, and self-development. And total tasks in job of Hospital coordinator were 76. Second, the tasks which were important, difficult, frequent and essential in entry step of occupation were counseling before consultation, explaining treatment plan after consultation, checking progress of consultation, answering teleconsultation, and finding out customer's consultation information. Third, a job model of Hospital Coordinator was constructed based on the results of DACUM job analysis.

A Study on the Health Research of Sexual Minorities in Korea (한국 성 소수자 건강연구에 대한 고찰)

  • This paper reviewed the past social discussion and health research results on sexual minorities to suggest a future direction. Studies have been done to increase the social inclusion of sexual minorities and promote the health of sexual minorities. However, there is still a lot of exclusive awareness of sexual minorities in Korean society, and the researchers have not identified their health status. Studies have shown that factors that hinder lesbian, gay, bisexual, transgender, and intersex (LGBTI) people from accessing medical services are the lack of knowledge and awareness of LGBTI people and attitudes not inclusive of LGBTI people's culture. Thus, it is necessary to identify the health status of each sexual minority type and prepare health promotion measures accordingly in national health research. In addition, it is essential to provide medical personnel with sexual minority education and introduce a sexual minority education curriculum that includes theory and practice into the health care curriculum. The result of this paper may be able to act as a guide for future operation plans in health research on sexual minorities.

A Prediction Model for Psychiatric Counseling for Depression among Subjects with Depressive Symptoms (우울증 대상자의 정신 상담 경험 여부 예측 모형)

  • Purpose: The number of patients suffering from depression is rapidly increasing worldwide, and by 2030, it is expected to pose a severe social and economic burden. Reports suggest that approximately 30% of subjects with symptoms of depression do not attempt treatment. Therefore, predicting the characteristics of subjects with depressive symptoms who have not even attempted counseling treatment is essential to increase the participation rate for such treatment. This study intends to predict the participation rates for psychological counseling treatment for depression among subjects with depressive symptoms. Methods: This study used data from the 2021 Korea Community Health Survey (KCHS). Data analysis was carried out using a decision tree to design a model that predicted participation in psychological counseling for depression. Results: The results showed that subjects aged 65 to 74 had difficulty understanding the explanations of medical staff even though they did not have cognitive impairment. Only 11.1% of this group received psychological counseling, which was the lowest rate among the various age groups. Among the subjects, 62.4% of those aged 19-44 or 45-64, who had suicidal thoughts and attempted suicide, received psychological counseling and this was the highest rate among the age groups surveyed. Conclusion: The identification of people showing depressive symptoms is crucial for encouraging them to undertake treatment. Also, proper depression-oriented medical services should be developed and implemented for people with depressive symptoms who exhibit a blind spot towards attempting treatment.

Birth and Infant Death Reporting System via Computer Network (출생 및 영아사망 신고체계 및 전산정보체계 개발)

  • Accurate vital statistics are essential for a national health planning and evaluation. Among various vital statistics, birth and death rates, and infant and matemal mortality rates together with the causes of death are the very basic ones for above purposes as well as for the maternal and child health management. These statistics are based on the birth and death reports. It is required by law to report every birth and death within one month after its occurrence. However, in case of a neonatal death occurring prior to the birth report, most of the birth and death are not reported. Thus accurate infant and maternal mortality rates are not available yet in Korea. The main objective of this study is to develop a birth and infant death reporting system via computer network. We designed a new birth report form based on the current form and data from the analysis of medical record forms of 14 hospitals. A new form is basically addition of essential medical information to the current birth report form. Since a revision of the rules and regulations related wtih the birth report is necessary to use a new form, we kept the current from intact to make it acceptable to the government office for a field trial. We also developed computer programs for data input for birth and death reports at a medical faciltiy, data processing for production of maternal and child health indices at a health center, and management of maternal and child health services including immunization and postantal care at health center. The birth certificate and birth report can be printed out at a medical facility. The computer packages were programmed by Borland Delphi 3.0 and can be run under Windows 95 system. We proposed a new birth and death reporting system via computer network after a field trial for data input, transmission, and processing. The medical and demographic data o birth and death at medical facilities will be sent to health centers directly via computer network. The health center will retain the medical data for analysis and forward only the data for birth and death reports required by current regulations to the Dong, Up, or Myun Office. Once the birth or death is reported via computer network to the Dong Office, then the Dong Office will notify the baby's mother of the birth report and request to submit the baby's name by mail. When the baby's name its submitted. the Dong Office will forward the birth reports to the Common Court and Statistics Agency in the same way as the current system, Upon the completion of birth registration of the Common Court, the court will issue the birth certificate to mother which will be used in lieu of the family record. The advantages of proposed birth and death reporting system via computer network ar as follows ; I) The accuracy, timing, and completeness of reporting will be improved and more accurate maternal and child health indices can be obtained, ii) The maternal and child health services of health center will be obtained, iii) Epidemiologic data for pregnancy and birth can be obtained, iv) Manpower for birth and death reporting will be saved.

Modified Orbitozygomatic Approach without Orbital Roof Removal for Middle Fossa Lesions

  • Objective : The purpose of the present study was to describe an OrBitoZygomatic (OBZ) surgical variant that implies the drilling of the orbital roof and lateral wall of the orbit without orbitotomy. Methods : Design : cross-sectional study. Between January 2010 and December 2014, 18 patients with middle fossa lesions underwent the previously mentioned OBZ surgical variant. Gender, age, histopathological diagnosis, complications, and percentage of resection were registered. The detailed surgical technique is described. Results : Of the 18 cases listed in the study, nine were males and nine females. Seventeen cases (94.5%) were diagnosed as primary tumoral lesions, one case (5.5%) presented with metastasis of a carcinoma, and an additional one had a fibrous dysplasia. Age ranged between 27 and 73 years. Early complications were developed in four cases, but all of these were completely resolved. None developed enophthalmos. Conclusion : The present study illustrates a novel surgical OBZ approach that allows for the performance of a simpler and faster procedure with fewer complications, and without increasing surgical time or cerebral manipulation, for reaching lesions of the middle fossa. Thorough knowledge of the anatomy and surgical technique is essential for successful completion of the procedure.

A Disital Siver Care for the Health and Rehabilitation of the Elderly (노인 건강과 재활을 위한 디지털 실버케어)

  • This study introduces trends on silver care implemented with the fourth industrial revolution technology, and discusses the use of digital technology for elderly health and rehabilitation by reviewing relevant literature to examine and present directions for future digital-based silver care commercialization. First, health smart home, which is a smart residential service available through digital technology or IT technology that supports independent living in your home, is available. Second, there are technical services using artificial intelligence(AI) and robots. Robots based on advanced intelligence can serve as an assistant for the health and rehabilitation of senior citizens by supporting services that enable the daily lives of senior, checking their health conditions, and high-quality medical care. For the commercialization of these silver care systems, information and services appropriate to the current situation, such as the physical ability and health status of the elderly, should be provided, and it would be desirable to gradually expand the use of essential technology to reflect the needs of the elderly in use so that the digital alienated.

Analysis of Hospital-based Home Care Service Utilization Using National Health Insurance Claim Data from 2008 to 2017 (건강보험 자료를 이용한 전국 의료기관 가정간호 실시 및 이용 현황 분석: 2008년-2017년)

  • Purpose: The purpose of this study is to analyze the status quo of hospital-based home care utilization in Korea from 2008 to 2017. Methods: Data from a total of 1,396 medical institutions, 350,390 patients, and 3,563 home care nurses were analyzed using claim data from the Health Insurance Review & Assessment Service. Results: The number of hospital-based home care agencies decreased from 177 in 2008 to 115 in 2014. This number started to increase in 2015 and reached 179 in 2017. The number of hospital-based home care patients declined from 35,056 in 2008 to a low of 26,848 in 2013. This number started to increase in 2014 and reached 67,863 in 2017. Essential hypertension was the most common disease among hospital-based home care patients from 2008 to 2015. The number of hospital-based home care visits declined from about 500,000 in 2008 to a low of 362,000 in 2013. This number started to increase in 2014 and reached 658,000 in 2017. Conclusion: It is necessary to vigilantly monitor hospital-based home care agencies, patients, and the utilization of services. This may help establish platforms for providing community and home-based nursing services for the super-aged society in Korea.

Current Status of the Pediatric Palliative Care Pilot Project in South Korea Based on the Experience of a Single Center

  • Purpose: We evaluated the status of patients enrolled in South Korea's pediatric palliative care pilot project based on the experience of a single center. This study examined factors related to end-of-life services and differences in medical costs. Methods: The medical records of 120 patients referred by a pediatric palliative care team were analyzed retrospectively. Data from July 1 to February 28, 2022 were collected and analyzed using the chi-square test and the Mann-Whitney U test. Results: Volunteer programs and psychological support (100%), family support and education (99.2%), and financial support through institutional linkage (62.5%) were provided to the participants. In the deceased group, there were no significant differences in general characteristics, which included age, gender, primary disease, religion, duration of hospitalization in an intensive care unit (ICU) and non-intensive care unit (non-ICU). However, the ICU group had fewer opportunities to access individual pain and physical symptom management than the non-ICU group and there were limitations in linking with external resources. Medical expenses were significantly different for the ICU group, with a 3-times higher average cost than the non-ICU group. Conclusion: Although an individualized approach is needed for each patient in pediatric palliative care, psychosocial care is essential. In addition, if early intervention for end-of-life pediatric patients is available from a palliative care team, the cost burden of medical care for patients and their families should be minimal.

Problems with Transferring Major Trauma Patients to Emergency Medical Center of a University Hospital from Another Medical Center (대학병원 응급의료센터로 전원되는 중증 외상환자의 현황 및 문제점)

  • Purpose: The incidence of multiple trauma is increasing nowadays and is the leading cause of death among young adults. Initial treatment is well known to be crucial in multiple trauma victims. However, many indiscriminate transfers occur due to the lack of a well-organized trauma system in Korea. The objective of this study is to demonstrate the current serious state in which major trauma patients are transferred to the Emergency Medical Center of a university hospital from another medical center. Methods: From November 2009 to October 2010, we performed a retrospective study to analyze the characteristics of patients who visited the Ajou University Medical Center located in Gyeonggi-do. We evaluated the ISS (injury severity score), and a score over 15 point was identified as major trauma. The major trauma patients were separated into two groups according to the visit route, and the characteristics of each group were analyzed. Results: Among the 88,862 patients who visited to the Emergency Medical Center, trauma patients accounted for 19,950, and 343 of them were evaluated as major trauma patients. Among the 343 patients, 170 patients had been transferred from other medical centers. The proportion of males to females was 3.3:1, and the mean ISS was 22.7. The leading cause of trauma was motor vehicle accidents. Of the total 170 patients, 77.6% were admitted to the Intensive care unit and 36.3% underwent surgery. The 170 patients that had been transferred to our medical center, 78.8% were transferred from Gyeonggi-do, 15.3% were transferred from other regions, and 5.9% were miscellaneous. Conclusion: Almost half of the major trauma victims treat at our medical center had been transferred from other medical centers. Establishing a traumatic system, supported by well-organized trauma centers and emergency medical services, that can reduce inappropriate transfers among medical facilities is essential.
