상부 식도에 발생한 식도 평활근종 -2례 보고- (Esophageal Leiomyoma in the Upper Thoracic Esophagus)

  • Though leiomyoma is the most common benign esophageal tumor, it is rare in the upper third of esophagus. Recently, we experienced two cases of esophageal leiomyoma in the upper third of esophagus. The patient were a 53 year old female and a 40 year old male. On esophagoscopy, masses were found at 20\ulcorner23 cm, 22\ulcorner26 cm from upper incisor with normal overlying mucosa. Enucleation was done through a right posterolateral thoracotomy without complications.

  • In recent era, the incidence of combined injury of cervical trachea and esophagus by penetrating wound has been increased considerably. If initial operative repair is unsatisfied, devastating complications and even death may be considered. A 5 years old boy and 67 years old female were admitted to our department for deep cervical stab wounds. On exploration, cervical trachea and esophagus was nearly total transected anteroposteriorly. Emergent reconstructive surgery for trachea and esophagus had done. Postoperative course was uneventful without any complications.

  • Among varieties of the mediastinal tumors, benign developmental cysts [Entergenous cysts] occur about 10% of them. From the primitive foregut, tracheobronchial tree and esophagus develop at 3 weeks of its embryonal age, and bronchogenic cyst arises from accessory or supernumerary lung bud. Usually it remains isolated with surrounding structures, and causes no specific symptoms. But few cases of bronchogenic cysts have fistulous communication with esophagus causing compressive symptoms. We report a case of unusual complicated case of bronchogenic cyst with review of literatures.

Boerhaave syndrome의 외과적 치험 (Surgical Experiences of Boerhaave`s Syndrome -10 Cases analysis-)

  • All 10 cases of spontaneous rupture of esophagus had violent vomiting as precursor. 9 patients were male, 1 case was female. Chief complaints were chest pain and dyspnea. Chest P \ulcornerA and esophagogram were mainly used as confirm diagnostic tool. Perforation sites of all cases were at distal esophagus near the G-E junction. 6 cases were received primary repair within 24 hrs, other cases were managed with surgical drainage after exclusion and diversion of esophagus. Empyema was the most frequent complication. Other complications were sepsis, pneumonia, leaking etc. Overall mortality rate was about 70.0%.

  • Despite the improved diagnostic and treatment modalities recently introduced for a variety of esophageal disorders, a perforation or leak from the esophagus remains a sources of morbidity and mortality regardless of the cause of leak. After the perforation of esophagus, the contamination of mediastinum and pleural cavity with food, bacteria and corrosive gastric juice leads to sepsis and cardiopulmonary dysfunction. The early diagnosis and early treatment are very important, and the delayed treatment leads to high risk of morbidity and mortality. We experienced one case of esophageal perforation, after forced vomiting in 48 years old male patient. We used omentum on the treatment of ruptured esophagus, and it was successfully managed.

기관지 식도루를 동반한 내엽형 폐격리증 -1예 보고- (Intralobar pulmonary sequestration communicating with the esophagus -1 cases report-)

  • We have experienced a case of intralobar pulmonary sequestration communicating with the esophagus. A 24 year old female patient was admitted of chronic cough related to food, purrlent sputum and left lower chest pain. Esophagography and bronchography revealed bronchoesophageal fistula and bronchiectasis with cystic lesion of the left lower lobe. There was multiple anomalous feeding vessels arinig from the intercostal arteries and no draining systemic vein on aortography. division of the bronchoesophageal fistula and left lower lobectomy was performed. Communication with the esophagus in rarely associated with intralobar pulmonary sequestration and esophagogram is useful method of diagnosis for this communication.

  • The coincidental occurrence of squamous cell carcinoma and leiomyoma in the esophagus is rare. A 56 year-old male referred to chest surgery department to evaluate the mediastinal mass which had found on a health examination. The diagnosis was confirmed esophagoscopy and surgery. We report a case of synchronous squamous cell carcinoma and leiomyoma in esophagus and reviewed references to the literature.

식도에서 발생한 상피육종암 2례보고 (Carcinosarcoma of the Esophagus : 2 Case Report)

  • Carcinosarcoma of the esophagus have an interesting pathologic feature of admixture of carcinomatous and sarcomatous lesion and accounts for 0.5∼1.5% of all esophageal neoplasm. Generally, it has been reported that these have better prognosis than the squamous cell carcinoma. We have experienced two cases of carcinosarcoma occurring at the mid-esophagus. In both two cases, Ivor Lewis operation was performed and lymph node metastasis was absent. The pathologic diagnosis was confirmed as carcinosarcoma. Postoperative course was uneventful and the patients have been followed up for 3 months and 3 years, respectively, without any problems. We report these cases with a brief review of the literatures.

식도 자연 천공 치험 1례 (Spontaneous Rupture of the Esophagus (Boerhaave's Syndrome) -A case report-)

  • Spontaneous esophageal perforation occurred rarely but often lead to a high mortality and morbidity. We had experienced one case of spontaneous rupture of esophagus. A 52 - year old male patient was admitted to our hospital because of the chest pain and massive hematemesis after emetic strain. Esophagogram that taken at private clinic revealed leakage of dye into the right pleural space. Under the diagnosis of the thoracic esophageal perforation, two stage operation was planned because the size of perforation was large and pyothorax was developed on the right side. At first, exclusion and diversion of the esophagus were carried out. After six months, the esophageal reconstruction with left colon was performed.

후천성 양성 식도기관지루 -수술치험 1례- (Surgical Repair of Acquired Benign Esophagobronchial Fistula - A case Report -)

  • Acquired esophagobronchial fistula is relatively rare disease. Its causes are malignancy of esophagus or bronchus, infection, trauma, and diverticulum of esophagus. Malignant esophagobronchial fistula is more frequent than benign origin. The patient was 21-year-old female and had typical Onos sign. On esophagogram, fistulous tract was identified between esophagus and left lower lobe bronchus. The cause was nonspecific inflammation of mediastinum. The fistulous tract was resected and reinforced by mediastinal pleura. Postoperative course was uneventful.
