Effects of Kochiae Fructus on Histamine-induced Itch, Erythema and Wheal Responses in Normal Healthy Adults (지부자 피부도포가 히스타민 유발 소양감, 홍반, 팽진에 미치는 영향)

  • Objectives : Kochiae Fructus (dried fruits of Kochia scoparia L. Schrad,) is used in the treatment of skin diseases as internal or external medicine in Oriental Medicine, but there has not been experimental study of Kochiae Fructus as an external medicine. The purpose of this study was to investigate if it has certain effects on the skin or not. Methods : By a skin prick test method, we evaluated the effects of wet dressing with Kochiae Fructus decoction in different concentrations on histamine-induced itch, erythema and wheal responses, and compared them with the effects of calamine lotion and distilled water in 8 healthy volunteers aged 26-32. Results : The mean intensity and duration of itch were most decreased after wet dressing with a Kochiae Fructus decoction of 100mg/150ml. The time required for erythema to reach the maximal size was around 5 minutes in all groups, and the size of erythema was smallest on the skin taking wet dressing with the Kochiae Fructus decoction of highest concentration. The size of wheal was also smallest after wet dressing with the highest concentration Kochiae Fructus decoction. The effects of wet dressing with Kochiae Fructus decoction on skin were dependent on their concentrations. Conclusions : Kochiae Fructus suppressed the histamine-induced skin responses, which supports the ancient herbal literatures which describe that Kochiae Fructus has a certain effectiveness on some skin diseases.

Effects of Gagam-Jawoonaek about Erythema by UV Exposure (가감자운액(加減紫雲液) 도포가 자외선으로 유발된 피부 홍반에 미치는 영향)

  • We studied to investigate the erythema reduction effects generated by Gagam-Jawoonaek(GJ) application(appl.) after UV exposure. Twenty women in their twenties to fifties with no skin diseases were recruited. We exposed UV as a 6 subsites on the left upper arm of subjects using multi-port solar simulator. After setting Gagam-Jawoonaek(GJ) application(appl.) subsites and non-appl. subsites, we measured erythema degrees($a^*$ values) of the subsites using spectrophotometer. We measured $a^*$ values four times(before UV exposure, before application of GJ, twenty-four and forty-eight hours after first application of GJ). We analyzed data using student's t-test. After UV exposure, $a^*$ values on the left upper arm increased. Twenty-four hours after first GJ treat., the changes of $a^*$ value on GJ treat. subsites($1.22{\pm}0.13AU$) were bigger than GJ non-treat. subsites($1.04{\pm}0.12AU$), but there was no statistically significance. Forty-eight hours after first GJ treat., the changes of $a^*$ value on GJ treat. subsites($1.95{\pm}0.11AU$) were bigger than GJ non-treat. subsites($1.58{\pm}0.13AU$), a statistically significance. Gagam-Jawoonaek could decrease erythema by UV exposure.

Polarization Spectral Imaging System for Quantitative Evaluation of Port Wine Stain Blanching Following Laser Treatment

  • Objective methods to assess quantitatively port wine stain (PWS) blanching in response to laser therapy are needed to improve laser therapeutic outcome. Previous studies have attempted to assess objectively PWS color based on point measurement devices. To date, these approaches have typically been limited by a number of factors such as small test area and need for contact. To address these issues, a polarization spectral imaging system and an image analysis method have been developed to evaluate quantitatively erythema and melanin content distribution in skin. The developed polarization spectral imaging system minimizes artifacts such as glaring, shadowing, and non-uniform illumination that interfere with image fidelity. Furthermore, the image analysis method has been employed to get images of skin melanin and erythema indices from the acquired color images for quantitative analysis. Finally, using PWS patient color image, the effectiveness in laser treatment of PWS was evaluated by calculating relative erythema index image that is the relative erythema index of PWS region to the normal region. The developed device and analysis method appears to be a simple and effective method for quantitative analysis of PWS blanching.

  • Objectives: The purpose of this study is to report the effectiveness of Sihochunggan-tang gagambang in two patients with erythema multiforme which ocurred repeatedly in stress condition.Methods: This case study was conducted in two erythema multiforme(EM) patients who have visited Korean Medicine Clinic. All two patients were treated with Sihochunggan-tang gagambang and acupuncture. VAS(Visual Analogue Scale) score was used to check the subjective change of skin symptoms and itching sense.Results & Conclusions: The VAS score were decreased compared to the initial visit in all two patients in our study. And most skin symptoms and itching sense of two patients were improved. As a result of examining VAS scores and photographs, Sihochunggan-tang gagambang can be effective on EM in two cases in our study.

A Clinical Case of Tinea Pedis Treated with Korean Medicine (한방치료로 호전된 족부백선 환자 1례)

  • Objectives : This study is to report a case of tinea pedis accompanied with erythema and pruritus, treated with Korean medicine treatment. Methods : A 26-year-old female patient has suffered from erythema and pruritus on her right 3rd toe. She applied desoximetasone ointment on it for a month, but the effect didn't last long and the symptoms got worse. After disappointing application of the corticosteroid, she received Korean medicine treatment including herbal medicine and herbal external medication for 1.5 months. Results : The skin symptoms such as erythema and pruritus were remarkably improved. Also, the Visual Analogue Scale(VAS) of pruritus changed from 4 to 0. Conclusions : This case report suggests that Korean medicine may have a role in treating tinea pedis with some skin symptoms such as erythema and pruritus.

  • Purpose: The present study purposed to examine induced characteristic or erythema and safety by medium wave ultra violet(UVB) lamp. Methods: We compared sunshine and UVB lamp using spectroradiometer and UV radiometer. For measuring sunshine irradiation, we used spectoradiometer and detected from 8 to 18 o'clock every each hour on the beach, playground and rooftop of a 5 story building. The subjects for erythema examination were 5 healthy subjects who have no pathologic history of photosensitivity reaction, psoriasis and vitiligo. They were exposed to UVB radiation at the abdominal area for 2 hours and after irradiation, we observed the change of skin color every 12 hours over a period of 1 week. Results: Between sunshine and UVB lamp, sunshine had higher data on the chromaticity coordinates, dominant and peak wavelength, bandwidth and purity than the UVB lamp but on the color temperature, brightness the UVB lamp had higher data than the sunshine. In comparison of sunshine and UVB lamp, UVB lamp irradiated constantly such as $3.9-4.4{\mu}W/cm^2$ at a distance of 100cm between bed and lamp which was same as early morning irradiation on the sunshine. The erythema didn't appear to any subject. Conclusion: This results suggest that the UVB lamp has lower irradiance as much as early sunshine. Therefore the UVB lamp had no influence of inducing erythema at a distance of 100cm between bed and lamp.

  • Poncet's disease is an aseptic polyarthritis developing in the presence of active Tuberculosis occurring elsewhere, and is not due to direct involvement of joints but to an immunological reaction to tuberculoprotein. We experienced a case of Poncet's disease accompanying erythema nodosum in a 55-year-old female patient with pulmonary tuberculosis. She had multiple tender erythematous nodules on both lower limbs for 3 months and a cough and sputum from one month ago. She felt severe pain in both knees and ankles with swelling one week before admission. Her chest X-ray, computed tomography (CT) scan and positive sputum AFB stain results revealed that she had active pulmonary tuberculosis accompanying erythema nodosum and aseptic polyarthritis. Her arthritis and erythema nodosum were dramatically improved within four weeks after anti-tuberculosis therapy. We report a case of Poncet's disease in pulmonary tuberculosis accompanying erythema nodosum.

  • UV irradiation causes a variety of biologic effects on the skin. These effects can be devided to acute reactuons and chronic reacxtions by duration of UV irradiation. Acute reactions are erythema reaction, pigment reactions and changes in epidermal thickness. Among them erythema reaction is most common and conspicuous acute effects of the skin. Upon exposure to sun or artificial UV soures, a faint redness response of skin may begin. Larger exposure causes sunburn reaction which is exaggerated erythema reactionassociated with pain, swelling, vesicle and dulla. Extent and time course of erythema reaction depend upon several factors including wavelength and dose of UVR, skin conditions likeas skin type, site, color, temperature, humidity and environmental factors. Evaluation of erythema erythema induced by UV irradiation is difficult to quantify. Degree of redness of skin are usually estimated by subjective visual evaluation. The lowest exposure dose required to protuce erythema is called minimal erythema dose (mod). Repeated exposures of UVR result in photaging skin. In this condition we can see wrinkling, skin atrophy, dilated blood vessels and keratoses. In sensitive persons photocarcinogenesis is can Be developed on exposed area of skin. Recently skin canser is increasing now in our country. An effective public education and photopreventive method must be developed.

Effects of Scutellaria baicalensis Extract on Skin Lesion of Contact Dermatitis induced by DNFB in Mice (황금 추출물이 생쥐에 유발된 접촉피부염의 피부 증상에 미치는 영향)

  • Scutellaria baicalensis Georgi is one of major herbs used to treat patients with skin diseases diagnosed by wind-heat syndrome. This study was designed to investigate the effects of Scutellaria baicalensis Extract (SBE) on skin lesion of Contact dermatitis (CD) in mice. We investigated the effects of SBE on skin thickness, skin lesions, erythema index and melanin index in vivo. The effects on body weights and spleen/body weight ratio were also investigated in mice with CD induced by topical application of 1-Fluoro-2,4-dinitrofluorobenzene (DNFB). In our results, topical application of SBE lowered skin thickness significantly. In addition, SBE ameliorated skin lesions such as erythema, exudate and petechia in mice with CD. SBE also inhibited erythema index elevated by DNFB significantly. finally, SBE did not affect body weight and spleen/body weight ratio. In conclusion, these data imply that SBE can ameliorate skin lesion of CD such as erythema, exudate and petechia, and be used to treat CD patients with relative safety.

Detecting Boundary of Erythema Using Deep Learning (딥러닝을 활용한 피부 발적의 경계 판별)

  • Skin prick test is widely used in diagnosing allergic sensitization to common inhalant or food allergens, in which positivities are manually determined by calculating the areas or mean diameters of wheals and erythemas provoked by allergens pricked into patients' skin. In this work, we propose a segmentation algorithm over U-Net, one of the FCN models of deep learning, to help us more objectively grasp the erythema boundaries. The performance of the model is analyzed by comparing the results of automatic segmentation of the test data to U-Net with the results of manual segmentation. As a result, the average Dice coefficient value was 94.93%, the average precision and sensitivity value was 95.19% and 95.24% respectively. We find that the proposed algorithm effectively discriminates the skin's erythema boundaries. We expect this algorithm to play an auxiliary role in skin prick test in real clinical trials in the future.
