Particulate matter air pollution is a serious problem affecting human health and visibility. The variations in $PM_{10}$ concentrations are influenced by not only local emission sources, but also atmospheric circulation conditions. In this study, we investigate the temporal features of $PM_{10}$ concentrations in South Korea and the atmospheric circulation patterns associated with high concentration episodes of $PM_{10}$ during winter (December-January-February) 2001-2016. Based on those analyses, a Korea Particulate matter Index (KPI) is developed to represent the large-scale atmospheric pattern associated with high concentration episodes of $PM_{10}$. The atmospheric patterns are characterized by persistent high-pressure anomalies, weakened lower-level north-westerly anomalies, and northward shift of the upper-level meridional wind anomalies near the Korean Peninsula. To evaluate the change in occurrence of high concentration episodes of $PM_{10}$ under a possible future warmer climate, we apply KPI analysis to CMIP5 climate simulations. Here, historical and two representative concentration pathway (RCP) scenarios (RCP 4.5 and RCP 8.5) are used. It is found that the occurrence of atmospheric conditions favorable for high $PM_{10}$ concentration episodes tends to increase over South Korea in response to climate change. This suggests that large-scale atmospheric circulation changes under future warmer climate can contribute to increasing high $PM_{10}$ concentration episodes in South Korea.
Breakthrough cancer pain is a transient exacerbation of pain that occurs despite relatively well controlled background pain with around-the-clock analgesia. It is highly prevalent in patients with cancer pain, with an overall prevalence of 70~90%. Breakthrough cancer pain has several negative effects on quality of life, including a decrease in functional status and social relationship, and higher incidence of anxiety/depression. It also places a detrimental burden on their families, society, and the healthcare system. According to the pathogenic mechanism, breakthrough cancer pain is classified into two categories: idiopathic (or spontaneous) pain and incident pain. Episodes of breakthrough cancer pain have typical characteristics, including rapid onset (5~10 min), severe intensity, and short duration (30~60 min). However, there are some variations in timing and severity of pain among patients and episodes. Therefore, a thorough assessment of pain episodes is needed and management plan must be individualized to provide optimal treatment. Several immediate-release formulations such as oxycodone, morphine, and hydromorphone are widely used despite relatively slow onset of action. Recent studies have shown that transmucosal fentanyl preparations were effective for faster control of breakthrough pain. We hope to improve management of breakthrough cancer pain with more efficient analgesics in line with currently available evidence.

Cyclosporine (CsA) has become well established as a potent immunosuppressive agent in the renal transplantation. However, therapy is complicated by large intraindividual and interindividual variability in pharmacokinetics of CsA and frequent undesirable clinical outcomes such as graft rejection and nephrotoxicity. The objective of this study was to determine the CsA trough blood concentrations that were associated with acute graft rejection and renal toxicity in renal transplant patients. Also, the ability of the current recommendation of therapeutic range for CsA to prevent graft rejections and CsA-associated renal toxicity was assessed. The clinical courses of the patients on CsA as an immusuppressive agent for preventing the graft rejection with renal ransplantation performed at Seoul National University Hospital from January 1995 to September 1998 were retrospectively reviewed. Total of 78 patients were included and three of them were retransplantation cases. Twenty-two acute episodes of rejection were identified, but only 16 episodes were clinically significant. Of these all the episodes occurred during the first month after transplantation except one. Mean daily doses of CsA were $427.2\pm72.1,\;352.6\pm56.8,\;308.62\pm48.3\;and\;268.47.1\;mg$ at posttransplant 1, 3, 6, and 12 months, respectively. Mean CsA whole blood though levels were $259.8\pm36.2,\;238.5\pm39,\;200.8\pm45.8\;and\;161.9\pm25.8\;ng/ml$ at posttransplant 1, 3, 6 and 12 months, respectively. Mean daily doses/weight were $7.9\pm1,\;6.4\pm1,\;5.3\pm0.7\;and\;4.6\pm0.7\;mg/kg$ at posttransplant 1, 3, 6 and 12 months, respectively. CsA doses decreased significantly as months progressed (p<0.001). During the first month after transplantation, only $12.5\%$ of the patients in rejection group had CsA concentration in therapeutic range, and 87.5, 93.8, and $100\%$ were within the therapeutic range at posttransplant 3, 6, and 12 months, respectively. These results suggested that CsA concentrations of $250\sim300\;ng/ml$ might be appropriate for preventing the acute rejection during the first posttransplant month.
This study presents the K-means clustering analysis-based classification of the meteorological patterns affecting the occurrence of high PM10 concentration in the southeastern region of the Korean peninsula for the last five years (2014-2018). Regional differences in Busan, Ulsan, and Gyeongnam related to high PM10 episodes, were clarified through the statistical comparison study using synoptic scale meteorological elements using NCEP (National Centers for Environmental Prediction/FNL (Final Operational Global Analysis) re-analysis meteorological data. Meteorological patterns were classified into a total of five categories (C1-C5). The incidence of each cluster was 24.8% (C1), 21.3% (C2), 20.4% (C3), 17.3% (C4), and 16.2% (C5), respectively. The high PM10 concentration in the southeastern region resulted from long and short range transports (C1, C3, C5) from outside of the region, and the emissions (C2, C4) inside the region. In the high PM10 episodes in Busan, Ulsan, and Gyeongnam regions, meteorological characteristics such as different geopotential height and wind speed at 500 hPa in each cluster and the change in the location of high pressure over Korean Peninsula is strongly associated with the dispersion of PM10 around inventories in the region and the tendency of long-range transportation of PM10 emitted from outside of region.
Purpose: Besides interferencence of esophageal motor function by the nasogastric tube, a decline of the positive gastro-esophageal pressure gradient caused by intermittent positive pressure ventilation seems to have a major role in the pathogenesis of gastroesophageal reflux (GER) in mechanically ventilated preterm infants. The aim of this study was to determine the incidence of GER and associated risk factors in mechanically ventilated preterm infants. Methods: Twenty four hour esophageal pH monitorings were performed using a antimony electrode on 11 mechanically ventilated preterm infants in Neonatal Intensive Care Unit in Pusan National University Hospital. We evaluated the following reflux parameters; reflux index, reflux episodes/hour, reflux episodes ${\geq}5min/hour$, duration of longest episode, and percent episodes ${\geq}5min$. Patients were considered to have significant GER if more than 2 among 5 parameters were satisfied. Results: The mean gestational age of the patients was 30.9 weeks, mean birth weight was 1,568 g, and mean age at the time of pH monitoring was 2.8 days. Significant GER was detected in 4 patients (36.4%). There was no relationship between the incidence of GER and gestational age, birth weight, postnatal age, or the ventilator settings. Conclusion: The incidence of GER in mechanically ventilated preterm infants was similar, compared with other previous studies. Associated risk factors of GER in these patients were not detected. Therefore, mechanical ventilation in preterm infants does not seem to be the high risk factor of GER.
Purpose: This research was a descriptive study of nursing activities observed in nursing simulation during a senior nursing student practicum. Content and frequencies of nursing activities during the simulation practice were identified. Methods: Thirty-six episodes of pediatric nursing simulation were videotaped. Both verbalizations and descriptions of nonverbal behaviors were recorded from the videotapes. The data were coded and analyzed. The coded nursing activities were evaluated for frequency and purpose of interaction. Results: Average time per simulation episodes was 27 minutes and ranged from 3.30 to 32.54 minutes. Nursing activities in these simulation episodes included nursing assessments such as vital sign measurement, associated symptom assessment, and check of patient condition, nursing interventions such as medication, tepid water massage, fluid therapy, provision of oxygen, suctioning, hyperglycemia and hypoglycemia management, communication such as parent education, procedure guidance, and communication among providers. Activities in assessment were most frequent, and among them, vital sign measurement and check of patient condition were more frequent than others. Conclusion: Students showed enhanced nursing activities such as more frequent nursing assessment, communication and interventions in their simulation experience. Therefore simulation experience can be considered as one strategies to provide nursing students with better and more intense practicum experience.
Purpose: Natural history and consequences of the novel 2009 influenza A H1N1(2009 H1N1) infection in immunocompromised pediatric patients are not yet fully understood. In this study, we investigated the clinical features and outcomes of the 2009 H1N1 infection in pediatric patients with hematological and oncological diseases. Methods: We retrospectively reviewed the medical records of 528 patients who had hematological and oncological diseases and who were treated at 7 referral centers located in the Yeungnam region. Among the 528 patients, 27 with definite diagnosis of 2009 H1N1 infection were the subjects of this study. All patients were divided into the following 3 groups: patients who were receiving chemotherapy (group 1), patients who were immunosuppressed due to a nonmalignant hematological disease (group 2), and patients who were off chemotherapy and had undergone their last chemotherapy course within 2 years from the influenza A pandemic (group 3). Results: All 28 episodes of 2009 H1N1 infection were treated with the antiviral agent oseltamivir ($Tamiflu^{(R)}$), and 20 episodes were treated after hospitalization. Group 1 patients had higher frequencies of lower respiratory tract infection and longer durations of fever and hospitalization as compared to those in group 2. Ultimately, all episodes resolved completely with no complications. Conclusion: These results suggest that early antiviral therapy did not influence the morbidity or mortality of pediatric patients with hematological and oncological diseases in the Yeungnam region of Korea after the 2009 H1N1 infection. However, no definite conclusions can be drawn because of the small sample size.

The ambient TSP data measured at Seoul, Incheon. Taean, Daegu, Busan in Korea were used to explain the chemical composition and general features of Asian Dust (AD) observed in Korea. 9 episodes out of 19 were sampled from 2000 through May 2002, and measurements were conducted covering ionic and metal components with mass concentration. The results showed that daily averaged mass concentration (TSP) during the AD episodes was 458 $\mu\textrm{g}$/㎥, and ionic and metal concentrations were 27.93 $\mu\textrm{g}$/㎥ and 71.7 $\mu\textrm{g}$/㎥, respectively, accounting for 6.1 % and 15.5% of the total aerosol mass. TSP concentrations during episodes were varied from 120 to 1742 $\mu\textrm{g}$/㎥ according to the impact of Asian Dusts and had a tendency of showing higher values at sites in the west side of Korea, which can be explained by the effect of diffusion and deposition. In this study, ionic components like Ca (NO$_3$)$_2$, CaSO$_4$, NaNO$_3$, Na$_2$SO$_4$ were prominent types in secondary aerosol during AD periods and also indicated that V, Co as well as soil elements such as Ca, Fe, Mg, Mn, K correlated well with Al, while Cu, Cd, Pb, Zn didn't agree well with it. In addition, enrichment factors (EFs) for each metal component were obtained to provide simple information about source contribution of Asian Dust, and the results were compared with those from other AD studies. In this study, the results showed that aerosol properties in Korea during the Asian Dust were considerably different from those of general atmospheric condition and specially varied from case to case rather than site to site, which implies that there are certain variations in the soil of source region, pathways of air mass, and meteorological condition. For the enhanced study, those factors should be combined with the features of Asian Dust resolved from this study.

Characteristics of carbonaceous components and organic compounds in PM2.5 over the atmosphere of the Yellow Sea were investigated. PM2.5 samples were collected onboard the meteorological research vessel, GISANG 1, over the Yellow Sea during the YES-AQ campaign in 2018 and 2019, respectively. The average concentrations of carbonaceous components in this region were 2.59 ± 1.59 ㎍ m-3 for the OC, 0.24 ± 0.10 ㎍ m-3 for the EC, 2.14 ± 1.30 ㎍ m-3 for the WSOC and 1.17 ± 0.94 ㎍ m-3 for the HULIS-C, respectively. The total concentration of 56 organic compounds (ΣOCs) accounts for 10% of OC. The main group among organic compounds were dicarboxylic acids which account for 57% of ΣOCs, followed by n-alkanoic acids accounting for 34% of ΣOCs. In n-alkanoic acid distribution, hexanoic (C6:0) and octanoic (C8:0) acids which are low molecular weight n-alkanoic acids and known as emitted from marine biogenic activities were dominant in this region. Furthermore, non-HULIS-C fraction increased when the air mass originated from the marine region rather than the continental region. When the Asian dust episode was observed, the WISOC concentrations along with the levoglucosan were increased, while the haze episodes caused the increase of WSOC, HULIC-S and DCAs. In this study, we found that the components of carbonaceous and organic aerosols in PM2.5 over the Yellow Sea were changed with the specific air pollution episodes. It indicates that the physicochemical properties of PM2.5 can be changed by the air pollution episodes in this region.

This study aims to understand human rights sensibility in university varsity teams and to compare and analyze differences in human rights sensibility by variables related to university varsity teams. In order to achieve the purpose of this study, 188 student-athletes from 10 universities were selected. The collected data were analyzed in descriptive analysis, reliability analysis, one-way ANOVA, Scheffe using IBM SPSS 24.0. First, based on the episodes, the episodes with the highest human rights sensitivity are the right to labor of migrant workers and happiness rights, and the episodes with the lowest human rights sensitivity are the right to freedom of detention and privacy rights. In addition, among the sub-factors of human rights sensitivity, perception of responsibility and perception of behavior are higher than perception of outcome. Second, there are differences in the human rights sensitivity of the university varsity team depending on the size and the level of performance of the university varsity team. Third, there are differences in the human rights sensibility of the university varsity team depending on the educational characteristics (volunteer activity and human rights education) of the university varsity team.
