  • The purpose of this paper is to simulate the high ozone concentration in Shiwha Banwol indusrial complex. High pollution episodes (ozone alert) of this area are the results of geographical location and its air pollutants emission. This research has used meteorological model (RAMS) and photochemical air pollution Model (CIT model). As first step of the evaluate of this combined model system simulations are done in terms of meteorological characteristics like wind fields, PBL-height, etc.. Numerical simulations are carried out with real meteorological synoptic data on June. 24-25, 2010. In comparison with real measurement and another research the model reflects well local meteorological phenomena and shows the possibility to be utilized to analyse the pollutant dispersion over irregular terrain region. The high ozone concentration is deeply correlated to the ambient air temperature, wind speed and solar radiation. Local meteorological phenomena like sea-land breeze impact on horizontal dispersion of ozone. This analysis of meteorological characteristics can, in turn, help to predict their influences on air quality and to manage the high ozone episodes.

SPM AND CONIDIA OF MOLDS DURING THE ASIAN DUST EPISODES

  • High SPM concentrations(199.8~249.4${\mu}{\textrm}{m}$/㎥) were detected in the west Korea during the Yellow Sand Periods, 2000. Majority of the total SPM were composed of about 5${\mu}{\textrm}{m}$ sized coarse particles over the periods. However, fine particles sized about 1 ${\mu}{\textrm}{m}$ and coarse particles sized about 5-6${\mu}{\textrm}{m}$ showed peaks at the graph of SPM size distribution in the Non Yellow Sand Period. Airborne fungal spores at the SPM samples were cultured and identified. Full-grown colonies during the Yellow Sand Periods, Fusarium, Aspergillus, Penicillium and Basipetospora are hyphomycetes in the division Fungi imperfecti(Deuteromycota). And morphologically more diversified mycelia of hyphomycetes were grown on the sample captured from 1.1~2.1${\mu}{\textrm}{m}$ sized SPM than on other sized samples during the Yellow Sand Period. But no mold was observed on the sample of 1.1~2.1${\mu}{\textrm}{m}$ sized SPM in the Non Yellow Sand Period. It was thought that several sorts of fine sized fungal spores were suspended in the atmospheric environment of the west Korea during the Asian dust episodes.

아동간호사의 상위의사소통 행위 (Qualitative Analysis of Nurses′ Metacommunicative Behaviors in a Pediatric Unit)

  • The purpose of this study was to identify metacommunicative behaviors between nurses and patients in a pediatric unit. The research method included observation using videotaping. Data were collected from December, 2001 to February, 2002. Total six nurses, and eight patients and their mothers in a pediatric unit participated in this study. The interactions were videotaped under the participants' consent. The participants were observed for total 8 hours over 2-day period. Special episodes which were identified as metacommunicative behaviors in the taped interactions were transcribed. Transcription included verbal and nonverbal interactions. Selected episodes were classified using Mitchell's definition. Each classified definitions were named, and categorized by its purpose. The results were as follows: Nineteen metacommunicative behaviors which used frequently by nurses-approaching, mediating eye level, eye contact, touching, encouraging, turnabout, mimic voice, giving choices, friendly demand, expansion, tagging, repeating and confirming, identification, reflection, baby talk, symbolization, description of acts, relaxed posture, turning away- were identified and organized into four categories. They were call for attention, facilitating response, empathy, and tension release. In conclusion, nurses in this study used metacommunicative behaviors frequently and these behaviors were effective in interacting with children. It is suggested that any educational programs to teach communication skills to nurses need to include techniques on metacommunicative behaviors. This will help nurses to be more sensitive to different characteristics of their patients.

Real Exchange Rate Misalignment in Pakistan: An Application of Regime Switching Model

  • This study investigates the key determinants of exchange rate (RER) misalignment for the period 1991 to 2020. The BEER technique has been used to estimate the degree of the equilibrium exchange rate. To explore the actual exchange rate misalignment and to assess the behavior of variables that are different in different regimes of undervaluation and overvaluation, the nonlinear technique of Markov regime-switching (MSM) was applied. The mean and variance of each regime are highly significant and show that undervaluation episodes have a low mean (116.139) and more volatility (1.229) while overvaluation episodes have a high mean (126.732) with less volatility (0.871). The findings show that MSM accurately identifies exchange rate misalignment in both regimes as separate incidents of overvaluation and undervaluation. Results further depict that misalignment of the RER is affected by terms of trade, net foreign assets, interest differential, government investment, and consumption decision. Results recommend that if policymakers want to use the exchange rate as a policy tool, they must first consider the drivers of the equilibrium exchange rate. As a result, any deliberate actions to address exchange rate misalignment must focus on the underlying fundamentals that drive the exchange rate.

Benign Recurrent Intrahepatic Cholestasis Type 2 in Siblings with Novel ABCB11 Mutations

  • Benign recurrent intrahepatic cholestasis (BRIC), a rare cause of cholestasis, is characterized by recurrent episodes of cholestasis without permanent liver damage. BRIC type 2 (BRIC2) is an autosomal recessive disorder caused by ABCB11 mutations. A 6-year-old girl had recurrent episodes of jaundice. At two months of age, jaundice and hepatosplenomegaly developed. Liver function tests showed cholestatic hepatitis. A liver biopsy revealed diffuse giant cell transformation, bile duct paucity, intracytoplasmic cholestasis, and periportal fibrosis. An ABCB11 gene study revealed novel compound heterozygous mutations, including c.2075+3A>G in IVS17 and p.R1221K. Liver function test results were normal at 12 months of age. At six years of age, steatorrhea, jaundice, and pruritus developed. Liver function tests improved following administration of phenylbutyrate and rifampicin. Her younger brother developed jaundice at two months of age and his genetic tests revealed the same mutations as his sister. This is the first report of BRIC2 confirmed by ABCB11 mutations in Korean siblings.

Effectiveness of Intra-anal Biofeedback and Electrical Stimulation in the Treatment of Children With Refractory Monosymptomatic Nocturnal Enuresis: A Comparative Randomized Controlled Trial

  • Purpose: To compare the effects of intra-anal biofeedback (BF) and intra-anal electrical stimulation (ES) on pelvic floor muscles (PFMs) activity, nocturnal bladder capacity, and frequency of wet night episodes in children with refractory primary monosymptomatic nocturnal enuresis (PMNE). Methods: Ninety children of both sexes aged 8-12 years with refractory PMNE participated in this study. They were randomly assigned to 3 groups of equal number: control group (CON) that underwent behavioral therapy and PFM training, and 2 study groups (BF and ES) that underwent the same program in addition to intra-anal BF training and intra-anal ES, respectively. PFMs activity was assessed using electromyography, nocturnal bladder capacity was evaluated by measuring the first morning voided volume, and a nocturnal enuresis diary was used for documenting wet night episodes before treatment and after 3 months of treatment. Results: After training, all groups showed statistically significant improvements in all measured outcomes compared to their pretreatment findings. The ES group showed significantly greater improvements in all measured outcomes than the CON and BF groups. Conclusions: Both intra-anal BF training and ES combined with behavioral therapy and PFMs training were effective in the treatment of PMNE, with intra-anal ES being superior to BF training.

Nutrition Evaluation Screening Tool: An Easy to Use Screening Tool for Hospitalised Children

  • Purpose: Nutrition screening is vital to ensure patients are appropriately managed in hospital. In paediatrics there is currently no universally accepted nutrition screening tool. The Nutrition Evaluation Screening Tool (NEST) was developed as an easy to use and practical screening tool for hospitalised children. We aim to evaluate compliance of the NEST and assess agreement of the NEST with the already validated nutrition screening tools, Screening Tool for Risk on Nutritional Status and Growth (STRONGkids), Screening Tool for the Assessment of Malnutrition in Paediatrics (STAMP) and the Subjective Global Nutritional Assessment (SGNA) tool. Methods: Retrospective review of 102 patient episodes at the Evelina London Children's Hospital. Electronic records were used to assess NEST compliance and to complete the nutrition tools for each patient episode. Cohen's kappa was used to determine the level of agreement between each nutrition tool. Results: There was moderate agreement between the NEST and the two screening tools, STRONGkids (κ=0.472) and STAMP (κ=0.416) for patients on initial screening at admission. 87.2% of patient episodes were NEST compliant within 24 hours of admission to hospital. Conclusion: The moderate agreement between these two already validated screening tools enhances the NEST's validity as a paediatric screening tool. The NEST had the strongest correlation with the SGNA tool compared to other screening tools. The NEST is user friendly screening tool for hospitalised children.

국내 반복입원의 현황과 환자 특성: 외래진료 민감질환을 중심으로 (Current Status of Repeated Hospitalization in South Korea: Focused on Ambulatory Care Sensitive Conditions)

  • Purpose: Repeated hospitalization could be a proxy of unnecessary or preventive admission in South Korea where barriers to hospitalization are relatively low. This study aimed to estimate the current status of repeated hospitalization due to ambulatory care sensitive conditions (ACSC) in South Korea. Methods: Using the National Health Information Database, repeated hospitalization databases were constructed in units of episodes for patients who had been admitted more than twice between January 2017 and December 2018. The number of hospitalizations, total in-hospital days, and total medical expenditure were calculated and compared by patient characteristics in both of the entire patient group and the ACSC patient group. Results: Of total hospitalization episodes, 26.6% reported repeated admission, and 6.7% of repeated hospitalization was due to ACSC. A total of 183,110 patients with ACSC had been admitted an average of 2.9 times and spent an average of KRW5,630,118. In other words, KRW1,309 billion had been spent for repeated hospitalization due to ACSC. The scale of medical expenditure was relatively large in the highest and lowest socioeconomic status. Conclusion: Repeated hospitalization for ACSC can be considered a simple and intuitive indicator when assessing unnecessary hospitalizations or evaluating healthcare policy.

Impact of CO2 Emissions, Exchange Rate Regimes, and Political Stability on Currency Crises: Evidence from South Asian Countries

  • This study uses the panel probit model to investigate and evaluate the relationship between exchange rate regimes, political stability, and carbon dioxide during currency crises. To understand currency crisis times, we study a panel dataset of seven South Asian nations that contain annual observations from 1996 to 2020. Furthermore, we created the EMPI exchange market pressure indicator to detect crises. Our results strongly suggested that fixed exchange rate is negatively associated with currency crises, with good regulatory quality and better effective governments. Simultaneously, the floating exchange rate is positively related to the currency crises in those countries where the rule of law has less adequately flowed. However, CO2, exports, and interest rates are buoyantly associated with crises. The floating exchange rate, the rule of law, exports, and interest rate are associated positively and contribute more prone to the crisis episodes. Negatively associated variables contributed less amid crises episodes: fixed exchange rate regime, government effectiveness, and regulatory quality. Meanwhile, CO2 has a positive relationship with a currency crisis and contributes more likelihood to the probability of a currency crisis. Countries that adopted the fixed exchange rates with effective governments and regulatory quality faced more minor currency crises.

양극성 장애에서의 수면장애: 일주기 리듬의 교란과 수면 무호흡증을 중심으로 (Sleep Disorders in Bipolar Disorders: A Narrative Review on Circadian Rhythm Disturbances and Sleep Apnoea)

  • Bipolar disorders are a group of mood disorders characterised by relapsing mood episodes throughout the course of illness. Patients with bipolar disorders commonly present with various sleep problems. Patients in a manic episode generally show decreased need of sleep and those in a depressed episode frequently complain about hypersomnia. Current literature even points to evidence that patients with bipolar disorder in euthymic state may still show signs of sleep disturbances when compared to the general population. Clinicians may also note intricate interactions between changes of circadian rhythm and evolution of mood episodes in patients with bipolar disorder. Also, commonly prescribed medications which plays a crucial role in treatment of bipolar disorders including mood stabilisers and antipsychotic medications often cause significant weight gain over time. Being a risk factor of sleep apnoea, weight gain can predispose the patient to develop sleep apnoea. In this narrative review, we summarised current evidence and literature regarding characteristics of circadian rhythm and comorbid sleep apnoea in patients with bipolar disorder. We also present literature regarding implications of circadian disturbance and comorbid sleep apnoea in managing patients with bipolar disorder.
