가토(家兎) 및 묘(猫)에서의 두개내압항진(頭蓋內壓亢進)에 의한 혈압반응(血壓反應)에 관하여 -Reserpine 및 6-Hydroxydopamine의 영향(影響)- (Blood Pressure Response to Raised Intracranial Pressure in Rabbits and Cats -Effect of Reserpine and 6-Hydroxydopamine-)

  • 1) 마취가토(痲醉家兎) 및 묘(猫)에서 경뇌막외강(硬腦膜外腔)을 통(通)한 가압방법(加壓方法)으로 두개내압상승(頭蓋內驅上昇)과 혈압(血壓) 및 심박(心搏)과의 관계(關係)를 조사(調査)하였다. 2) 양동물(兩動物)에서 두개내압(頭盞內壓)을 상승(上昇)시켜 두개내압(頭蓋內壓)과 혈압(血壓)의 차(差)가 아주 적어지면 현저(顯著)한 혈압상승(血壓上昇)이 나타났다. 3) 양동물(兩動物)에서 두개내압(頭盞內壓)이 혈압(血壓)보다 높아지면 현저(顯著)한 혈압하강(血壓下降)과 현저(顯著)한 일시적(一時的)인 심박감소(心搏減少)가 나타났다. 4) Reserpine 처리동물(處理動物)에서는 두개내압상승(頭蓋內驅上昇)은 혈압하강(血壓下降), 심박감소(心搏減少)를 일으켰다. 5) 6-Hydroxydopamine 처리(뇌내)동물(處理(腦內)動物)에서는 두개내압상승(頭蓋內驅上昇)에 의한 혈압상승(血壓上昇)은 비처리동물(非處理動物)에 비(比)하여 약(弱)하였다. 6) Reserpine 처리동물(處理動物)의 측뇌실내(側腦室內)에 norepinephrine을 투여(投與)한 후(後)에는 두개내압상승(頭蓋內驅上昇)은 현저(顯著)한 혈압상승(血壓上昇)을 일으켰다. 7) 두개내압상승(頭蓋內驅上昇)에 의한 혈압상승(血壓上昇)은 두개내압상승(頭蓋內驅上昇)으로써 뇌내(腦內) noradrenergic neuron이 자극(刺戟)되여 norepinephrine 유리(遊離)가 증가(增加)한 결과(結果) 일어나는 것으로 추리(推理)하였다.

자기공명분석기에 의한 반사성 교감신경성 위축증의 치험 (Experience with the Application of Magnetic Resonance Diagnostic $Analyser^{(R)}$ -A case of reflex sympathetic dystrophy-)

  • Reflex sympathetic dystrophy is a syndrome characterized by persistent, burning pain, hyperpathia, allodynia & hyperaesthesia in an extremity, with concurrent evidence of autonomic nervous system dysfunction. It generally develops after nerve injury, trauma, surgery, et al. The most successful therapies are directed towards blocking the sympathetic intervention to the affected extremity by regional sympathetic ganglion block or Bier block with sympathetic blocker; other traditional treatments include transcutaneous electrical stimulation, immobilization with cast & splint, physical therapy, psychotherapy, administration of sympathetic blocker, calcitonin, corticosteroid and analgesic agents. The purpose of this report is to evaluate and describe the effects of magnetic resonance following unsatisfactory results with traditional treatments of RSD. A 17 year old female patient, 1 year earlier, had received excision and drainage of pus at the right femoral triangle due to an injury caused by a stone. Afterwards, she experienced burning pain, knee joint stiffness, and muscle dystrophy of the right thigh, especially when standing and walking. Despite a year of number of traditional treatments such as: lumbar sympathetic block, continuous epidural analgesia, transcutaneous electrical stimulation, & administration of predisolone, her pain did not improve. Surprisingly, the patients was able to walk free from pain and difficulty after just one application of magnetic resonance. The patient has been successfully treated with further treatment of two to three times a week for approximately ten weeks. More recently, magnetic resonance has been demonstrated to produce effective results for the relief of pain in a variety of diseases. From our experiences we recognize magnetic resonance as a therapeutic modality which can provide excellent results for the treatment of RSD. It has been suggested that polysynaptic reflex which are disturbed in RSD may be modulated normally on the spinal cord level through the application of magnetic resonance.

백서에서 삼차신경 유발전위의 특성과 경로 분석 (Characteristics of Trigeminal Evoked Potential and It's Pathway in the Rat)

  • Objective : There are some advantages of trigeminal evoked potential(TEP) recording compared to other somatosensory evoked potential(SSEP) recordings. The trigeminal sensory pathway has a pure sensory nerve branch, a broader receptive field in cerebral cortex, and a shorter pathway. Despite these advantages, there is little agreement as to what constitutes a normal response and what wave forms truly characterize the intraoperative TEP. This study presents the normative data of TEP recorded on the epidural surface of the rat with a platinum ball electrode. Materials & Methods : Under general anesthesia with urethane, the adult Sprague-Dawley male rats(300-350g) were given electrical stimulation with two stainless steel electrodes which were inserted into the subcutaneous layer of the area around whiskers. A reference electrode was positioned in the temporalis muscle ipsilateral to the recording site. Results : TEPs were recorded in the Par I area of somatosensory cortex and recorded most apparently on the point of 2mm posterior from the bregma and 6mm lateral from the midline. The typical wave form consisted of 5 peaks (N1-P1-N2-P2-N3 according to emerging order, upward negativity). Each latency to corresponding peaks was not influenced by the different intensities of stimulation, especially from 1 to 5mA. Average latencies of 5 peaks were in the following order ; 7.7, 11.1, 15, 22.3, 29.4ms. There was also no significant difference between latencies before and after administration of muscle relaxant(pancuronium). For the electrophysiological localization of recorded waves, the action potential of a single unit was recorded with glass microelectrode(filled with 2M NaCl, $3-5M{\Omega}$) in the thalamus of rat. A sharp wave was recorded in the VPM nucleus, in which the latency was shorter than that of N1. This suggests that all 5 peaks were generated by neural activities in the suprathalamic pathway. Conclusion : In terms of recording near-field potentials, our data also suggests that TEP in the rat may be superior to other SSEPs. In overall, these results may afford normative data for the studies of supratentorial lesions such as hydrocephalus or cerebral ischemia which can have an influence on near-field potentials.

Yohimbine이 가토두개내압상승(家兎頭蓋內壓上昇)에 따른 혈압상승(血壓上昇)에 미치는 영향(影響) (Effects of Yohimbine on the Pressor Response to Raised Intracranial Pressure in Rabbits)

  • 1) Urethane 마취가토(痲醉家兎)에서 경뇌막외강내(硬腦膜外腔內)에 삽입(揷入)한 balloon을 통(通)한 가압(加壓) 및 두개내압기종방법(頭蓋內壓記種方法)에 의하여 내압(內壓)을 상승(上昇)시키고, 이 내압상승(內壓上昇)에 따른 혈압상승(血壓上昇)에 미치는 ${\alpha}_2-adrenoceptor$ antagonist인 yohimbine의 영향(影響)을 관찰(觀察)하였다. 2) 내압(內壓)은 가압(加壓)balloon 내(內) 식염수주입(食鹽水注入)으로 주입초기(注入初期) 에는 완만(緩慢)하게 그 후 점차(漸次) 급격(急激)하게 상승(上昇)하였다. 이에 따라 혈압(血壓)은 처음에 경미(輕微)한 하강경향(下降傾向)을 보인 후 급격(急激)하게 상승(上昇)하였고, 더욱 내압(內壓)을 상승(上昇)시키면 혈압(血壓)은 심(甚)한 하강(下降)을 보였다. 최고혈압상승정도(最高血壓上昇程度)는 원혈압(元血壓)의 $49{\pm}2.4%$(32예(例) 평균(平均)${\pm}SE)$의 증가(增加)였으며, 이때에 가압(加壓)balloon 내(內)로 주입(注入)된 식염수양(食鹽水量)은 $1.22{\pm}0.15\;ml$, 내압(內壓)은 $165{\pm}6.4\;mmHg$였다. 3) 측뇌실내(側腦室內) yohimbine$(50{\mu}g)$은 혈압자체(血壓自體) 영향(影響)을 미치지 못하였으나, 본양(本量)의 yohimbine 투여후(投與後)에는 가압(加壓)balloon내(內)에 대조동물(對照動物)에서보다 훨씬 적은 양(量)의 식염수주입(食鹽水注入)으로 내압(內壓) 및 혈압(血壓)이 상승(上昇)하였다. 즉(卽) 최고혈압상승(最高血壓上昇) (6예평균(例平均, $57{\pm}4.5%)$이 나타날 때의 가압(加壓)balloon 내(內)에 주입된 식염수양(食鹽水量)은 $0.83{\pm}0.02\;ml$, 내압(內壓)은 $164{\pm}9.6\;mmHg$였다. 4) $Clonidine(30\;{\mu}g)$의 측뇌실내(側腦室內) 주입후(注入後) 혈압자체(血壓自體)는 하강(下降)되었고 이때 가압(加壓)balloon 내 식염수주입(食鹽水注入)에 의한 내압상승(內壓上昇)은 대조군(對照郡)보다 순화(純化)되었으며 혈압상승(血壓上昇)은 거의 볼 수 없었다. 5) $Clonidine(30\;{\mu}g)$투여(投與)로 하강(下降)된 혈압(血壓)은 $yohimbine(500\;{\mu}g)$투여(投與)로 거의 원혈압(原血壓)으로 회복(回復)되었고, 이때 내압(內壓)을 상승(上昇)시키면 대조군(對照郡)에서와 같은 내압상승(內壓上昇)에 따른 혈압상승(血壓上昇)이 나타났다. 6) 본실험(本實驗) 성적(成績)은 가토(家兎)에서 ${\alpha}_2-adrenoceptor$ antagonist가 뇌(腦)에 존재(存在)할 때는 두개뇌압상승(頭蓋腦壓上昇)에 따른 혈압상승(血壓上昇)이 촉진(促進)되고 또한 ${\alpha}_2-adrenoceptor\;agonist$에 의한 두개뇌압상승(頭蓋腦壓上昇)에 따른 혈압상승(血壓上昇)의 억제(抑制)가 나타나지 않음을 가리키고 있다.
