Influences of Oral Health Behaviors, Depression and Stress on Periodontal Disease in Pregnant Women (임신여성의 구강건강행위, 우울, 스트레스가 치주질환에 미치는 영향)

  • Purpose: The purpose of this study was to identify the influences of oral health behaviors, depression, and stress on periodontal disease in pregnant women. Methods: The participants in this study were 129 pregnant women. Data were collected using questionnaires which included individual characteristics, oral health care behaviors, the Center for Epidemiological Studies-Depression scale (CES-D), a global measure of perceived stress, and pregnancy stress. A dentist measured periodontal probing depth and classified stages of periodontal disease according to the Community Periodontal Index (CPI). Data were analyzed using descriptive statistics, Pearson correlation, and multiple regression. Results: Periodontal disease had significant correlations with oral health care behaviors (r=-.56, p <.001), perceived stress (r=.44 p <.001), pregnancy stress (r=.37 p <.001), diet (r=-.33, p <.001) and depression (r=.18 p =.046). Factors influencing periodontal disease for these pregnant women were being in the 2nd (${\beta}=.27$, p <.001) or 3rd trimester (${\beta}=.45$, p <.001), having a pregnancy induced disease (${\beta}=.20$, p =.002), performing higher oral health behaviors (${\beta}=-.30$, p <.001), and having higher perceived stress (${\beta}=.17$, p =.028). The explanation power of this regression model was 61.6% (F=15.52, p <.001). Conclusion: The findings of this study indicated that periodic assessment of periodontal disease is essential for pregnant women who are in 2nd or 3rd trimester and have pregnancy induced diseases. Enhancing oral health care behaviors and reducing perceived stress are indicated as effective strategies to reduce periodontal disease in pregnant women.

  • Personal exposure relies on characteristics of time activity patterns of the population of concern as human activities impact the timing, location, and level of personal exposure. The information about time spent in microenvironments plays a critical role for personal exposure to air pollutants. It is useful to determine the precise times of the day that the subjects are in certain locations or engaging in specific activities because exposure to some air pollutants can depend on temporal trends. This study investigated time activity pattern for workers of Korean population over 19 years old with 8,778 workers in weekday. The residential indoor times were 12 hours. Time activity was different by gender and the mean times stayed at home in weekday were 12.9 hours in female and 11.42 hours in male, respectively. The major factors on residential indoor time and workplace time were age, monthly income, occupation and industry type, work position, education, and gender. Considering shorter than those in other countries, Korean workers spent less time at home after the working hours. Determinants of time activity pattern need to be taken into account in exposure assessment, epidemiological analyses, exposure simulations, as well as in the development of preventive strategies. Since there are substantial difference of Korean worker activity pattern, this information can be critical for exposure assessment in Korea.

  • Scabs caused by Venturia carpophila greatly reduce the quality of the fruits of the Japanese apricot (Prunus mume) when the disease is not properly managed. The disease produces a superficial blemish that is unlikely to affect the overall yield of processed fruit, but reduce the value of fruit intended for the fresh market. Incidence rates of scab at sprayed and unsprayed orchards range from 0% to 21.5% and from 30.2% to 100%, respectively, in the major cultivation regions of Jeonnam Province during the growing season of 2009. The trends in disease progress were quite similar, regardless of regions, and cultivar Namgo was relatively less damaged by scab compared to cultivar Cheonmae among the tested Japanese apricot cultivars. The fruits on branches 1.5 m above the infected Japanese apricot trees and the stem-end parts of the infected fruits were more severely damaged by scabs, possibly because of rain and run-off facilitate dissemination of conidia of V. carpophila and subsequent infection of the fruits or branches. The conidia of V. carpophila were dispersed from March 24 to April 26 in 2010, and more conidia were dispersed from 2-year-old branches than 1-year-old branches. Since the control efficacies were higher than 90% after more than two applications of Trifloxystrobin WG at 10-day-intervals from mid April, it is that effective fungicides be applied at least two times at 10-day-intervals from the middle of April to manage scabs of Japanese apricot in orchards.

Clinical Analysis of Frostbite (동상의 임상적 분석)

  • Purpose: Frostbite can affect still soldiers. Initial clinical manifestations are similar for superficial and deep frostbite, so early treatment is identical. It is under-estimated by physicians. We try to identify the challenges of managing these complex tissue injuries. Methods: A retrospective analysis of 84 patients hospitalized at AFCH from 2009 to 2015 was conducted. We investigated differences of epidemiological characteristics, identification of soft tissue injury, treatment and complications between superficial (SF: 43; 51.2%) and deep (DF: 41; 48.8%) frostbite. Results: The major (94.0%) developed frostbite in dry circumstances (89.3%). Wet circumstances (66.7%) were more susceptible to DF rather than dry (46.7%). The 38 (45.2%) arrived to specialist within 7days. Most prone sites were feet, followed by hands. Toes had more deep injuries. DF presented more increased levels of ALT, CPK, CKMB, CRP. The bone scan of W+S+ was 48.3%, 87.1% and W+S- was 20.7%, 12.9%, respectively. The treatment resulted in improved or normalized perfusion scan with matching clinical improvement. It was a good tool to assess treatment response. Eighteen normal and 8 stenotic type of PCR resulted in normal with matching clinical improvement. One continuous obstructive waveform led to minor amputation. Twelve underwent both PCR and MRA. Among 6 normal PCR, 5 showed normal and one stenosis in MRA. All 5 stenosis and one obstruction showed the same findings in MRA. It was a good tool to evaluate vascular compromise. They were treated with rapid rewarming (11.6%, 22.0%), hydrotherapy (16.3%, 29.3%), respectively. Six (14.6%) underwent STSG, 2 (4.9%) had digital amputation in DF. Berasil, Ibuprofen, Trental were commonly administered. PGE1 was administered selectively for 6.8, 10.8 days, respectively. Raynaud's syndrome (16.3%), CRPS (4.7%), LOM (14.6%) and toe deformity (4.9%) were specific sequelae. Conclusion: We should recommend intensive foot care education, early rewarming and evacuation to specialized units. The bone scanning and PCR should allow for a more aggressive and active approach to the management of tissue viability.

Characteristics of Salmonella species isolated from domestic poultry and environmental samples in Korea (우리나라의 가금과 환경에서 분리한 Salmonella species의 특성)

  • This study was conducted to investigate the isolation prequency, serotypes, and related epidemiological properties of 341 Salmonella spp from domestic poultry and environmental samples during the period 1993-1995. A total of 1,918 samples was collected during the three years period in nationwide. Most of Salmonella spp were isolated from the intestinal contents of poultry, especially cecal(46.0%) and rectal(35.8%) contents. Among the tested samples, rat(28.5 %) was the most predominant Salmonella reservoirs and followed by duck(24.8%), broiler(18.8%), layer(14.8%) and feed(7.1%), in order. More than twelve Salmonella serovars were identified among the 341 Salmonella isolates. The most prevalent serotypes isolated from non-human sources were S enteritidis (22.3%) and S pullorum (21.9%), S muenchen (13.9%), S typhimurium (12.6%), S gallinarum, S meleagridis, S heidelberg, and S senftenberg were followed, in order. In layer chickens, S pullorum (26.0%) was the most predominant serotype but S muenchen (33.0%) was in broiler chickens, S enteritidis (28.4%) was in ducks, and S typhimurium (60.0%) was in rats, respectively. As a results, S enteritidis was identified as the most prevalent serotype in non-human Salmonella isolates in Korea during the period 1993-1995. A preliminary study on the phage typing of 19 S enteritidis selected from the nationwide scale was shown that S enteritidis phage type(PT) 4 was the most predominant PT, and SEPT 1, SEPT 6a, SEPT 7 and SEPT 7a variant were also found in the same period.

A Case-Control, Restrospective Study on Tsutsugamushi Disease Occurred in Gyeongju and Pohang Provinces, Korea (경주 및 포항 지역에서 발생한 쯔쯔가무시병에 관한 연구 - 항체 유무를 중심으로 -)

  • Objectives: Tsutsugamushi disease is one of the most significant acute febrile illnesses, increasing in frequency of occurrence during the late autumn in rural areas of Korea. Methods: I have conducted a case-control study on 30 cases who had the fever as a chief complaint, then had been ruled out as having the Tsutsugamushi disease. Data was collected retrospectively by review of chart regarding their general characteristics, clinical and laboratory findings, then was analysed by chi-test. Results: Of 30 cases, 16 cases were seropositive and 14 cases were seronegative against O. tsutsugamushi. Of seropositive for tsutsugamushi disease, 56.3% were female; 37.5% were in their seventies; 50.0% were farmers; 62,6% had chances of exposure to fields or mountains. And 75.0% occurred in November. The main symptoms and signs were fever and chill(100.0%), headache(75.0%). weakness and fatigue(93.8%), and eschar(68.8%). The characteristic laboratory findings were elevated AST(50.0%), ALT(62.5%), and abnormal urinalysis(56.3%). On the other hand, of seronegative cases, 57.1% were male; 50.0% were in their fifties; 42.9% were farmers; 57.2% had chances of exposure to fields or mountains. And 71.4% occurred in November. The symptoms and signs were fever and chill(100.0%), headache(85.7%), eschar(64.3%). weakness, fatigue and skin(57.1%). The laboratory findings were elevated AST(71.4%) and ALT(64.3%), and abnormal urinalysis(42.9%). However, there were no significant differences between the seropositive and seronegative cases(P>.05). Conclusions: Acute febrile community inhabitants who have the epidemiological, clinical as well as laboratory features should be focused upon for the early diagnosis and treatment for tsutsugamushi disease whether or not possessing the serological antibody against O. tsutsugamushi.

  • This research, sponsored by the Korean Ministry of Environment in 2014, was the first epidemiological study in Korea that investigated the health impact assessment of radon exposure. Its purpose was to construct a model that calculated the annual mean cumulative radon exposure concentrations, so that reliable conclusions could be drawn from environment-control group research. Radon causes chronic lung cancer. Therefore, the long-term measurement of radon exposure concentration, over one year, is needed in order to develop a health impact assessment for radon. Hence, based on the seasonal correction model suggested by Pinel et al.(1995), a predictive model of annual mean radon concentration was developed using the year-long seasonal measurement data from the National Institute of Environmental Research, the Korea Institute of Nuclear Safety, the Hanyang University Outdoor Radon Concentration Observatory, and the results from a 3-month (one season) survey, which is the official test method for radon measurement designated by the Korean Ministry of Environment. In addition, a model for evaluating the effective annual dose for radon was developed, using dosimetric methods. The model took into account the predictive model for annual mean radon concentrations and the activity characteristics of the residents.

  • The novel coronavirus disease 2019 (COVID-19) is spreading globally. Although its etiologic agent is discovered as severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2), there are many unsolved issues in COVID-19 and other infectious diseases. The causes of different clinical phenotypes and incubation periods among individuals, species specificity, and cytokine storm with lymphopenia as well as the mechanism of damage to organ cells are unknown. It has been suggested that in viral pneumonia, virus itself is not a direct cause of acute lung injury; rather, aberrant immune reactions of the host to the insults from viral infection are responsible. According to its epidemiological and clinical characteristics, SARS-CoV-2 may be a virus with low virulence in nature that has adapted to the human species. Current immunological concepts have limited ability to explain such unsolved issues, and a presumed immunopathogenesis of COVID-19 is presented under the protein-homeostasis-system hypothesis. Every disease, including COVID-19, has etiological substances controlled by the host immune system according to size and biochemical properties. Patients with severe pneumonia caused by SARS-CoV-2 show more severe hypercytokinemia with corresponding lymphocytopenia than patients with mild pneumonia; thus, early immunomodulator treatment, including corticosteroids, has been considered. However, current guidelines recommend their use only for patients with advanced pneumonia or acute respiratory distress syndrome. Since the immunopathogenesis of pneumonia may be the same for all patients regardless of age or severity and the critical immune-mediated lung injury may begin in the early stage of the disease, early immunomodulator treatment, including corticosteroids and intravenous immunoglobulin, can help reduce morbidity and possibly mortality rates of older patients with underlying conditions.

Injuries in Female Elite Korean Field Hockey Athletes: an Epidemiological Study (국가대표 여자 필드하키 선수들의 스포츠 손상 역학조사)

  • PURPOSE: This study examined the incidence, location, and characteristics of sports injuries according to the position in Korean female national field hockey athletes. METHODS: Eighteen female national field hockey athletes (two goal keepers, five forwards, six midfielders, and five defenders) who trained at the national training center between January 2018 and December 2018 were enrolled. The event, position, and location of injury were recorded per IOC's daily injury reports form. The incidence of sports injuries was examined with the 95% confidence interval (CI), and the difference in the incidence of injury according to the position was presented as the rate ratio (RR). Moreover, differences in the location of sports injury according to the position were examined using χ2 tests. RESULTS: A total of 92 sports injuries occurred among field hockey players, with an incidence of 6.47 cases per 1,000 hours of training (95% CI 4.98-7.96). In addition, 29.12 cases of injuries occurred per 1,000 exposures (95% CI 23.17-35.07), and the incidence of injury was highest among the defenders, followed by forwards, midfielders, and goal keepers. The most common location of sports injury was the lower extremity, followed in order by the trunk, upper extremity, head, and neck, and there were significant differences in the location of the sports injury according to the position (p=.046). In addition, lumbar spine/lower back, thigh, ankle, lower leg, knee, and foot/toe were common sites of sports injury among female field hockey players. CONCLUSION: These results provide a baseline for predicting sports injuries in athletes during games, and would provide useful information for developing performance enhancement and injury prevention programs.

Characteristics of the Molecular Epidemiology of CTX-M-Producing Escherichia coli Isolated from a Tertiary Hospital in Daejeon, Korea

  • The aims of this study were to characterize the molecular epidemiological profiles of CTX-M-producing uropathogenic Escherichia coli isolates from a tertiary hospital in Daejeon, Korea, and to investigate the genetic diversity and compare the prevalence of sequence types (STs) in different areas. Extended spectrum β-lactamase-producing E. coli strains isolated from urine were analyzed for CTX-M, integrons, and insertion sequence common regions (ISCRs) by PCR and sequencing. Multilocus sequence typing (MLST), pulsed-field gel electrophoresis (PFGE), phylogenetic analysis, and rep-PCR were also used for molecular typing of the isolates. Of 80 CTX-M producers, 31 and 46 expressed CTX-M-15 and CTX-M-14, respectively. MLST analysis indicated that the most prevalent ST was ST131 (n = 34, 42.5%), followed by ST38 (n = 22, 27.5%), ST405 (n = 8, 10.0%), and ST69 (n = 6, 7.5%). Most CTX-M producers harbored class 1 integrons. ST131 strains belonged to phylogenetic group B2 and showed identical rep-PCR patterns, whereas ST69, ST38, and ST405 strains belonged to phylogenetic group D; the ST38 and ST405 strains displayed the same rep-PCR pattern, respectively. ST131 and ST38 isolates showed 21 and 19 distinct types, respectively, by PFGE. In Daejeon, D-ST38 CTX-M-14 producers were relatively more prevalent than in other countries and Korean cities. Our results indicate that CTX-M-producing E. coli isolates belonged mostly to ST131 or ST38 and were more related to hospital-onset than to community-onset infections and that the blaCTX-M gene may vary according to the ST.
