Epidemiological Characterization of Influenza Viruses detected from Acute Respiratory Patients in Seoul during 2010-2012 (서울지역 급성호흡기 환자들로부터 분리한 인플루엔자 바이러스의 유행양상 (2010-2012))

  • Objectives: For our survey of the incidence of influenza viruses among respiratory viral infections in Seoul, we evaluated their prevalence among infectious acute respiratory viral patients in Seoul from 2010 to 2012 through regular surveillance. Methods: For influenza virus detection, we conducted real-time PCR analyses on 2,544 throat specimens collected from patients with respiratory viral infections in Seoul between 2010 and 2012. They were collected and then tested for the presence of influenza viruses through reverse transcription (RT) - polymerase chain reaction (PCR). Results: 19.1% (486/2,544) of the throat specimens were determined to be positive for influenza viruses. The incidences of influenza viral infection in the case of respiratory viral infections through regular surveillance in Seoul were 23.0% (212/923) in 2010, 6.4% (47/738) in 2011, and 25.7% (227/883) in 2012, and 10.8% (275/2,544) of type A, and 8.3% (211/2,544) type B influenza viruses. In addition, the greatest prevalence was in the 20-49 age group (51.6% ), which shows that influenza viruses constituted a major causative agent of acute respiratory viral infections. Conclusions: The distributions of influenza viruses and the epidemiologic patterns of the viral pathogen in acute respiratory viral infectious patients may provide potentially effective data for epidemiological studies in Seoul, Korea.

Radiation Exposure and Cancer Mortality Among Nuclear Power Plant Workers: a Meta-analysis (원전종사자의 방사선 노출과 암사망 위험도와의 관련성에 대한 메타분석)

  • Objectives: We conducted a meta-analysis to investigate the relationship between low external doses of ionizing radiation exposure and the risk of cancer mortality among nuclear power plant workers. Methods: We searched MEDLINE using key words related to low dose and cancer risk. The selected articles were restricted to those written in English from 1990 to January 2009. We excluded those studies with no fit to the selection criteria and we included the cited references in published articles to minimize publication bias. Through this process, a total of 11 epidemiologic studies were finally included. A publication bias was tested for using Egger's test. The homogeneity test was performed before the integration of each of the standardized mortality ratios (SMRs) and the result proved that the studies were heterogeneous. Results: We found significant decreased deaths from all cancers (SMR = 0.75, 95% CI = 0.62 - 0.90), all cancers excluding leukemia, solid cancer, mouth and pharynx, esophagus, stomach, rectum, liver and gallbladder, pancreas, lung, prostate, lymphopoietic and hematopoitic cancer. The findings of this meta-analysis were similar with those of the 15 Country Collaborative Study conducted by the International Agency for Research on Cancer. A publication bias was found only for liver and gallbladder cancer (p = 0.015). Heterogeneity was observed for all cancers, all cancers excluding leukemia, solid cancer, esophagus, colon and lung cancer. Conclusions: Our findings of low mortality for stomach, rectum, liver and gallbladder cancers may explained by the health worker effect. Yet further studies are needed to clarify the low SMR of cancers, for which there is no useful screening tool, in nuclear power plant workers.

Changes in Characteristics of Patients with Acute Intoxication in a Regional Emergency Medical Center (일개 응급센터에 내원한 급성 중독 환자군의 특성 비교)

  • Purpose: The purpose of this study is to compare the toxicologic characteristics of two groups of patients with acute intoxication for two different time periods and to make recommendations based on the results of this study. Methods: We reviewed retrospectively the medical records of patients with acute intoxication in our emergency center from June 1997 to May 1998 (group A) and from June 2000 to May 2003 (group B), and we evaluated differences in the epidemiologic and the toxicologic characteristics between the two groups. Results: The ratios of the number of patients with acute intoxication to the total numbers of patients who visited our emergency department were $0.49\%$ and $0.52\%$ for groups A and B, respectively. In both groups many poisoned patients visited our emergency center from 4:00 pm to midnight. The interval between the time of intoxication and arrival at the hospital was significantly shorter in group B. The number of patients transferred to our emergency center was larger in group B. Attempted suicide was the major cause of acute intoxication in both groups. Major toxic substances in both groups were centrally active drugs and insecticides. The number of comatose and mechanically ventilated patients was larger in group B. However, there was no statistically significant difference in the mortality rates. Conclusion: Emergency physicians who manage intoxicated patients should recognize regional characteristics and differences in the toxicologic characteristics of poisoning. In addition, the establishment of a poisoning control center in the regional emergency center is necessary to integrate data control and to enhance specialized management of intoxicated patients.

Prevalence and Treatment Pattern of Korean Patients with Facial Palsy (안면신경마비 환자의 최근 5년간 연도별 진료경향 분석)

  • Objectives : While there are many studies about treatments of facial palsy, no study has been performed on general population of Korea, especially concerning about comparison between western medicine and oriental medicine. This study aimed to investigate magnitude of health visits and treatment patterns for Korean patients with facial palsy through the computerized database of Health Insurance Review and Assessment Service(HIRAS). Methods : According to the HIRAS database over 5 years' period from 2004 to 2008, the medical records of patients with facial palsy as a main diagnosis were extracted. Inclusion criteria of facial palsy are Bell's palsy(G510), Geniculate ganglionitis(G511), Melkersson's syndrome(G512), Other disorders of facial nerve(G518), Disorder of facial nerve, unspecified(G519) in western medicine. And Paralytic face(G016), Deviated eye and mouth(J01), The other facial palsy(J013) were included in oriental medicine. We compared the claim number of western medical care with that of oriental medicine treatment by year and month. Results : The total claim number of facial palsy was increasing on both western medicine and oriental medicine from 2004 to 2008. In western medicine, the claim number of Bell's palsy(G510) is the most. In oriental medicine the inpatients claim number of Deviated eye and mouth(J01) is the most, while outpatients claim number of the other facial palsy(J013) is the most. Conclusions : Medical database of HIRAS provided comprehensive and vast information on epidemiologic characteristics and treatment, which can be more reliable data to expect medical demand for facial palsy in condition that accurate diagnosis and standardized treatment is delivered in clinical settings.

Factors Influencing Depression among Middle-School Girls (한국 여자 중학생의 우울에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to investigate the prevalence of depression among school girls in Korea and identify factors influencing the tendency to depression. Methods: A self-report survey was conducted with South Korean middle schoolgirls who were in the 8th and 9th grades. Four hundred and one schoolgirls were included in the study. The instruments utilized in this study were the Center for Epidemiologic Studies - Depression Scale and Social Problem Solving Inventory-Revised. Data were analyzed using descriptive statistics, Pearson correlation and multiple logistic regression with SPSS WIN 14.0 program. Results: The average depression score of the participants was 20.68, which indicates moderate levels of depression. About 35% of the schoolgirls in this study reported a tendency to depression. Significant predictors for depression were 'decreased problem-solving abilities', 'no family members with whom they can discuss their concern', 'decreased satisfaction in relationship with friends', and 'increased negative self-perception of body-image'. Conclusion: The study findings suggest that schoolgirls require special concern regarding the risk of developing depression. Regular depression screening could be beneficial for early detection of depression in schoolgirls and enhancing problem-solving ability could be considered as an effective strategy to reduce the risk of depression among schoolgirls.

Epidemiologic characteristics of type 1 diabetes in children aged 14 years or under in Korea, 1985-2000 (1985-2000년에 15세 미만 한국인에서 발생한 1형 당뇨병의 역학적 특징)

  • Type 1 diabetes mellitus (T1 DM) develops in genetically susceptible individuals as a result of progressive autoimmune destruction of beta cells. There is larger global variation in incidence among children aged 0-14 years. The incidence of T1 DM in Korea is very low. The latest survey in Korea was conducted in 2001 by Korean Society of Pediatrics to analyse the childhood (0-14 years) national-wide incidence during 1 995-2000. The average childhood incidence (/100,000 and year) was 1.36 (95% CI: 1.23-1.48), higher than 1.06 of Korea in 1 994. This survey reported that higher mean annual incidence was observed in female (girls, 1.67 vs boy 1.07), older age group (10-14 years, 1.98; 5-9 years, 1.43; 0-4 years, 0.73), Kangwondo (2.09) and Seoul (1.99), and more new patients developed in spring (spring, 27.1%; winter, 26.6%; summer, 24.8%; autumn, 21.5%). The increase of childhood T1 DM incidence has occurred in every region (4.0% in Asia, 3.2% in Europe during 1990-1999). Also, steady increase in incidence was observed during 1985-2000 in Korea. This trends may be maintained or accelerated by environmental factors, known as triggers, modifiers and promoters of the T1 DM occurrence. The rising incidence of T1 DM suggests the need for continuous monitoring of incidence by using standardized methods in order to plan or assess prevention strategies.

Molecular typing of uropathogenic Escherichia coli isolated from Korean children with urinary tract infection

  • Purpose: We investigated the molecular types of uropathogenic Escherichia coli (UPEC) by using conventional phylogrouping, multilocus sequence typing (MLST), and fimH genotyping. Methods: Samples of patients younger than 18 years of age were collected from the Chung-Ang University Hospital over 2 years. Conventional phylogenetic grouping for UPEC strains was performed by polymerase chain reaction (PCR). Bacterial strain sequence types (STs) were classified on the basis of the results of partial sequencing of seven housekeeping genes. In addition, we analyzed nucleotide variations in a 424-base pair fragment of fimH, a major virulence factor in UPEC. Results: Sixty-four UPEC isolates were analyzed in this study. Phylogenetic grouping revealed that group B2 was the most common type (n=54, 84%). We identified 16 distinctive STs using MLST. The most common STs were ST95 (35.9%), ST73 (15.6%), ST131 (12.5%), ST69 (7.8%), and ST14 (6.3%). Fourteen fimH allele types were identified, of which 11 had been previously reported, and the remaining three were identified in this study. f1 (n=28, 45.2%) was found to be the most common allele type, followed by f6 and f9 (n=7, 11.3% each). Comparative analysis of the results from the three different molecular typing techniques revealed that both MLST and fimH typing generated more discriminatory UPEC types than did PCR-based phylogrouping. Conclusion: We characterized UPEC molecular types isolated from Korean children by MLST and fimH genotyping. fimH genotyping might serve as a useful molecular test for large epidemiologic studies of UPEC isolates.

Caries Preventive Effects on Permanent Teeth by Fluoride Mouthrinsing Program in Elementary School (초등학교 불소용액 양치사업의 영구치 우식예방 효과)

  • Objectives: This study was conducted to find out caries preventive effect on permanent teeth among children who brush teeth with fluoride-containing toothpaste by supervised weekly fluoride mouthrinsing program in elementary schools. Methods: The epidemiologic dental survey was performed on the elementary schoolchildren of supervised weekly fluoride mouthrinsing program (FMR) with 0.2 percent neutral sodium fluoride solution and control group in 2007. Caries preventive effect on permanent teeth by fluoride mouthrinsing program were calculated by DMFT index and DMFS index between FMR group and control group. Results: By DMFT index between FMR group and control group, caries rates on permanent teeth of the fourth-, fifth- and sixth-grade children participating in FMR program were 34.1%, 40.8% and 31.5%, respectively. By the DMFS index between FMR group and control group, caries preventive rates on permanent teeth of the fourth-, fifth- and sixth-grade children participating in FMR program were calculated 25.4%, 37.7% and 33.5%, respectively. Conclusions: We suggest that fluoride mouthrinsing program should be developed to all elementary schools to prevent dental caries.

Analysis of reports on orphan lung diseases in Korean children

  • Purpose: Orphan lung diseases are defined as lung diseases with a prevalence of 1 or less in 2,000 individuals. Despite an increase in the numbers of patients with such diseases, few studies on Korean children have appeared. To obtain epidemiologic and demographic data on these diseases, we systematically reviewed reports on pediatric orphan lung diseases in Korea over the last 50 years. Methods: We reviewed 223 articles that have appeared since 1958 on orphan lung diseases in Korean children. These articles described a total of 519 patients aged between 0 and 18 years. We classified patients by year of publication, diagnosis, geographic region, and journal. Results: Of 519 patients, 401 had congenital cystic lung diseases and 66 had bronchiolitis obliterans. About 80% of patients were described in reports published in three journals, Pediatric Allergy and Respiratory Disease (Korea), the Korean Journal of Pediatrics, and the Korean Journal of Thoracic and Cardiovascular Surgery, in which papers on 157 (30.2%), 138 (26.6%), and 111 (21.4%) patients appeared, respectively. The frequency of publication of case reports has increased since 1990. Of the 519 patients, 401 (77.3%) were from Seoul/Gyeonggi-do and 72 (13.9%) from Busan/Gyeongsangnam-do. Conclusion: The prevalence of pediatric orphan lung disease has increased since 1990, and some provinces of Korea have a higher incidence of these diseases than do others. Studies exploring the incidence of pediatric orphan lung diseases in Korea are needed for effective disease management.

The Characteristics of Symptoms of Depression and Anxiety Associated with Suicidal Ideation in Korean Employees (한국 직장인의 자살생각과 관련된 우울 및 불안증상의 특성)

  • Objective : We have explored the relationship between suicidal ideation and some very specific characteristics of the symptoms of depression and anxiety in Korean employees. Methods : During the period 2013 through 2014, 100,793 employees who visited the Health Screening Center were asked to complete questionnaires, including the Center for Epidemiologic Study-Depression scale (CSE-D), the Beck Anxiety Inventory (BAI) and the presence of suicidal ideation. We attempted to determine which specific subscale, of each the CES-D and BAI would be more closely related to suicidal ideation by different gender and age by logistic regression. Results : Depression has been found to be a more prevalent and predominant factor in suicidal ideation, when compared to the presence of the condition of anxiety. Of four subscales of CES-D, issues arising from interpersonal relationships were most often associated with suicidal ideation, regardless of the chronologic age or gender of the subject. In BAI, cognitive anxiety showed a more significant association with suicidal ideation than did somatic anxiety in our study participants, showing same pattern in different gender and age groups. Conclusions : Among subscales of CES-D and BAI, the interpersonal relationship in CES-D and cognitive anxiety in BAI showed a higher and more significant association with suicidal ideation in Korean employees.
