THE EFFECT OF ND : YAG LASER IRRADIATION ON EXPOSURE OF THE DENTINAL TUBULES ON ROOT SURFACE IN EXTRACTED HUMAN TEETH;A SCANNING ELECTRON MICROSCOPIC STUDY (Nd : YAG laser 조사가 치근면 상아세관의 노출에 미치는 효과에 관한 주사전자현미경적 연구)

  • The purpose of this study was to evaluate the effects of Nd:YAG laser treatment on removal of smear layer and exposure of dentinal tubules. The experimental specimens were obtained from root planed surface of 30 human teeth which were extracted due to severe periodontal disease. The specimens were assigned randomly of three groups: root planed group, Tetracycline HCl(100mg/ml, 5min) group, and Nd:YAG laser ($Laser-35^{TM}$, U.S.A. 5 seconds )group. Nd:YAG laser group was divided into 4 subgroups according to 3.0W, 3.5W, 4.0W, 4.5W of power. The surface change of specimens were evaluated by scanning electron microscopy. The number of exposed dentinal tubules and percentage of area occupied by dentinal tubule orifices per unit area between each group was statistically analyzed by paired Student t-test. The results were as follows: 1. By root planing only, dentinal tubule was not exposed, but scale-like smear layer and parallel instrument tracks were resulted. 2. Tetracycline HCl treated surfaces exhibited the small number of partially exposed dentinal tubules with long orifices. The number($3.80{\pm}0.79$) of exposed dentinal tubules on Tetracycline HCl group was significantly less than that in laser groups above 3.5W of power(P<0.001), and the percentage ($0.68{\pm}0.19$) of area occufied by dentinal tubule orifices per unit area($192\;{\mu}m^2$) was significantly lower than that of any laser group(P<0.001). 3. The laser group irradiated with 4.5W of power showed both the most number($10.60{\pm}0.97$) of exposed dentinal tubules among the experimental groups, and the highest percentage($3.75{\pm}0.55\;%$) of area occupied by dentinal tubule orifices. 4. Energy Surge during laser pulsing and overlapping passes of the fiber resulted in melted and resolidified surface textures with lava-like appearance.

Carbon Emission Model Development using Urban Planning Criteria - Focusing on the Case of Seoul (도시공간 계획요소를 이용한 이산화탄소 배출량 산정 모델 개발 - 서울시를 사례로)

  • Urban space is the main contributor of greenhouse gas emissions, a primary cause of global warming. In order to reduce greenhouse gas emissions, planning at a city-level is necessary. The aim of this research is to develop a carbon emission model which can be used to create and manage urban spaces. In order to achieve this aim, the following methodologies were utilized. First, urban planning criteria related to population, landuse, and activity level were selected through theoretical speculation. Second, carbon dioxide emission was calculated based on electricity, gas energy, heating, petroleum, and water usages. Third, Seoul was selected as a case study city, and a carbon emission model was developed through a relational analysis between Seoul's urban planning criteria and carbon emissions. Thus far, various efforts have been made to respond to climate changes in urban spaces, but these have been limited to analyzing contributing factors in terms of their total amounts of carbon emissions in the entire city. However, the carbon emission model of this study is derived from urban planing criteria at a detailed scale. This sets our study apart from other studies by demonstrating a specific model in a local setting which can be utilized for lowering carbon emissions at a city level.

The Effect of Partially Used High Energy Photon on Intensity-modulated Radiation Therapy Plan for Head and Neck Cancer (두경부암 세기변조방사선치료 계획 시 부분적 고에너지 광자선 사용에 따른 치료계획 평가)

  • Purpose: A selection of proper energy in treatment planning is very important because of having different dose distribution in body as photon energy. In generally, the low energy photon has been used in intensity-modulated radiation therapy (IMRT) for head and neck (H&N) cancer. The aim of this study was to evaluate the effect of partially used high energy photon at posterior oblique fields on IMRT plan for H&N cancer. Materials and Methods: The study was carried out on 10 patients (nasopharyngeal cancer 5, tonsilar cancer 5) treated with IMRT in Seoul National University Bundang Hospital. CT images were acquired 3 mm of thickness in the same condition and the treatment plan was performed by Eclipse (Ver.7.1, Varian, Palo Alto, USA). Two plans were generated under same planing objectives, dose volume constraints, and eight fields setting: (1) The low energy plan (LEP) created using 6 MV beam alone, (2) the partially used high energy plan (PHEP) created partially using 15 MV beam at two posterior oblique fields with deeper penetration depths, while 6 MV beam was used at the rest of fields. The plans for LEP and PHEP were compared in terms of coverage, conformity index (CI) and homogeneity index (HI) for planning target volume (PTV). For organs at risk (OARs), $D_{mean}$ and $D_{50%}$ were analyzed on both parotid glands and $D_{max}$, $D_{1%}$ for spinal cord were analyzed. Integral dose (ID) and total monitor unit (MU) were compared as addition parameters. For the comparing dose to normal tissue of posterior neck, the posterior-normal tissue volume (P-NTV) was set on the patients respectively. The $D_{mean}$, $V_{20Gy}$ and $V_{25Gy}$ for P-NTV were evaluated by using dose volume histogram (DVH). Results: The dose distributions were similar with regard to coverage, CI and HI for PTV between the LEP and PHEP. No evident difference was observed in the spinal cord. However, the $D_{mean}$, $D_{50%}$ for both parotid gland were slightly reduced by 0.6%, 0.7% in PHEP. The ID was reduced by 1.1% in PHEP, and total MU for PHEP was 1.8% lower than that for LEP. In the P-NTV, the $D_{mean}$, $V_{20Gy}$ and $V_{25Gy}$ of the PHEP were 1.6%, 1.8% and 2.9% lower than those of LEP. Conclusion: Dose to some OARs and a normal tissue, total monitor unit were reduced in IMRT plan with partially used high energy photon. Although these reduction are unclear how have a clinical benefit to patient, application of the partially used high energy photon could improve the overall plan quality of IMRT for head and neck cancer.

Strutural Change of Root Surface Following $CO_2$ Laser Treatment (($CO_2$)레이저 치료에 의한 치근표면의 구조적 변화)

  • The purpose of this study was to evaluate the structural change of root surface and the occlusion of dentinal tubule following $CO_2$ laser treatment. Seven extracted healthy human premolar werw curetted, sectioned, and four specimens were randomly assigned to each of 6 different treatment groups : 1) untreated EDTA etched control: 2) root plande only: 3) $CO_2$ laser treated with 2W mode 6(10msec/pulse, 20pps) for 1 minute: 4) $CO_2$ laser treated with 2W mode 6(lOmsec/pulse, 20pps) for 2 minutes: 5) $CO_2$ laser treated with 2W mode 7(20msec/pulse, 20pps) for 1 minute: 6) $CO_2$ laser treated with 2W mode 7(20msec/pulse, 20pps) for 2 minutes. Following the prescribed treatment, the specimens were prepared for SEM evaluation. Results showed that $CO_2$ laser may be effective to occlude dentinal tubules tor dentin sensitivity treatment. The effect of dentinal tubule occlusion was enhanced with increasing the total energy level lased to specimen regardless of lasing mode. The structural changes of root surfaces were restricted to superficies, and these changes included fissuring, charring, crater formation over the smooth lava like texture. The charring and crater formation implying root damage was observed in the case of the longer duration of a pulse. The results of the present study suggests that the pulsed $CO_2$ laser with shorter pulse duration and longer exposure time can be used effectively in order to obtain the optimal dentinal tubule occlusion with minimal root damage.
