저위교합환자의 보철적 접근법과 이론 : Pseudo Class III 교합환자 증례 (The prosthetic approach and principle for an collapsed VDO ; A clinical case of pseudo Class III patient)

  • 진단과 치료과정을 통해서 후천적으로 습득된 부정교합을 가진 부분무치악자 성인에게서, 교합고경을 회복해 줌으로써 3급 부정교합 안모에서 정상교합상태의 안모와 기능을 부여할 수 있었다. 대부분의 부분무치악자에서 치조골의 위축, 그리고 감소된 교합고경 인해 이루어진 부정교합자에게서 교합고경의 수복은 매우 어려운 작업이다. 적절한 안모의 심미는 대체로 interlabial gap이 약 0-2mm일 때 얻어진다. 이것과 함께, 제한된 입술 지지도(lip competence)가 교합고경 수복과 적절한 안모의 심미를 위한 지표로 사용될 수 있다. 소개된 증례의 경우는, 7년 이상의 계속 관찰 결과, 성공적인 치료였다고 판단된다. 교합고경을 증가시킨 경우이긴 하지만, 사실은 원래의 정상 교합고경으로 회복 시켜주었다고 하는 것이 더욱 옳은 표현일 것 같다. 교합고경을 증가시키는 작업은 안모의 수복, 절치부의 외상성 교합을 수정하고 하악을 후퇴시키는데 매우 어렵고도 중요한 일이다. 최종적으로, 3급 부정교합이 수정되고, 적절한 교합고경을 가지게 되었으며, 위축된 구치 무치악부가 rigid support 개념으로 제작된 가철성 의치로 수복되었다. 부분 무치악부를 가진 후천적 부정교합자의 치료의 결과로 획득한 치아 및 안모의 심미와 이것의 장기간 안정성은 다음의 관계를 잘 고려함으로써 지속될 수 있을 것으로 사료된다: 1) 이상적인 교합고경에서의 교합수복, 2) 상,하악골 관계에서 기능적으로 유도되는 최종 교합의 허용, 3) lip competence (입술 적응) 한계 내에서 기능적으로 유도되는 최종교합.

구개근 절제술을 이용한 상악 제일 대구치의 치료 (Palatal root resection of compromised maxillary first molars)

  • Purpose: Root resection can be a valuable procedure when the tooth in question has a high strategic value. The prognosis of root resection has been well documented in previous studies, but the results focused on the palatal root resection have not been discussed in depth. I represent here the short term effectiveness of palatal root resection of maxillary first molars. Methods: Palatal root resection was performed on maxillary first molars of three patients. All the palatal roots were floating state on the radiographic finding and showed full probing depth and purulent exudation at initial examination. Reduction of palatal cusp and occlusal table was performed concomitantly. Endodontic therapy was completed after root resection. Results: Compromised maxillary first molars were treated successfully by palatal root resection in 3 cases. The mobility of resected tooth was decreased a little bit. The probing pocket depth of remaining buccal roots was not increased compared to initial depth. All the patients satisfied with comfort and cost effective results and the fact they could save their natural teeth. Conclusions: Within the above results, palatal root resection is an effective procedure treating compromised maxillary first molar showing advanced palatal bone loss to root apex with or without pulp involvement when proper case selection is performed.

  • It is important to find a accurate root canal length or successful endodontic therapy. By X-ray method takes a long time and difficult in a curved canal. After developed electronic apex locator has allowed to measure the root canal length with easy, in a short time also in a curved canal. But most electronic apex locators have the disadvantage which is too short reading or sometimes the measurement itself becomes impossible if there are electrolytes in the canal. To overcome this drawback, impedance ratio method has been developed. In this study, we have developed frequency dependent electronic apex locator to minimize the interference of electrolytes. And based on that also some error in clinic use, we added the other method. Difference of two signals which are used in calculation of impedance ratio was can be represent the status of root canal fluid. As a result, using impedance ratio method and auto-calibration by voltage difference method can reduce the measurement error.

Dilemmas pertaining to three canals in the mesiobuccal root of a maxillary second molar: a case report

  • The mesiobuccal root of the maxillary molars is well known to pose a hindrance during endodontic therapy. Presented here is a case of a maxillary left second molar where three canals were located in its mesiobuccal root with the use of visual and diagnostic aids. Difficulties encountered during the process of unveiling the tooth's internal anatomy were discussed. The dilemmas encountered pertained to the root canal configuration, the nomenclature of the extra canals, and the justification for the presence of a third canal. The root canal configuration of 3-2-1 was confirmed for the mesiobuccal root using information gained from clinical, radiographic, and multidetector computed tomography (MDCT) scan findings. This case demonstrates the need for efforts to locate extra canals in the mesiobuccal root of the maxillary molars as their internal anatomy remains a mystery.

Nasal septal abscess with a dental origin: a case report and a review of the literature

  • Since the first report of a nasal septal abscess (NSA) from a dental origin (1920), six articles have been published in the English literature to date. The most common cause of NSA is an infection of the nasal septal hematoma after trauma. This is a report of an uncommon cause of NSA with a dental origin. A PubMed search performed regardless of year and country using the terms ("nasal septal abscess") OR ("nasal septum abscess") initially yielded 229 articles. After screening, seven articles (eight patients) were selected. Addition of two related articles produced a total of nine articles (10 patients) to be included. The age of the included patients ranged from 7 to 69 years (mean, 32.82 years; standard deviation, ±23.86 years). The sex composition was as followed: males (n=7; 63.6%), females (n=4; 36.4%). Dental histories were various: periapical lesions, caries, extraction, endodontic therapy, and cystic lesions. The maxillary incisor dominated as the tooth of origin. Early diagnosis and treatment of NSAs are important to avoid not only facial deformity, but also severe complications (e.g., intracranial infection). If NSA is suspected in patients without facial trauma, the possibility of a dental origin, especially from the maxillary incisor area, should be considered.

Procedural errors detected by cone beam tomography in cases with indication for retreatment: in vivo cross-sectional study

  • Objectives: This study aimed to investigate the frequency and type of endodontic procedural errors in cases indicated for retreatment through cone-beam computed tomography (CBCT) analysis. Materials and Methods: The sample consisted of 96 CBCT scans, encompassing 122 permanent teeth with fully formed roots. Errors included perforation, instrument fracture, canal transportation, missed canals, and inadequate apical limit of filling. Additionally, potential risk factors were analyzed and subjected to statistical modeling. Results: The most frequent procedural error observed was the inadequate apical limit of filling, followed by canal transportation, perforation, missed canal, and instrument fracture. Statistically significant associations were identified between various procedural errors and specific factors. These include canal transportation and root canal wall, with the buccal wall being the most commonly affected; missed canal and tooth type, particularly the palatine and second mesiobuccal canal canals; inadequate apical limit of filling and root curvature, showing a higher deviation to the mesial direction in severely curved canals; inadequate apical limit of filling and the presence of calcifications, with underfilling being the most frequent; canal transportation and periapical lesion, notably with deviation to the buccal direction; and the direction of perforation and periapical lesion, most frequently occurring to buccal direction. Conclusions: CBCT emerges as a valuable tool in identifying procedural errors and associated factors, crucial for their prevention and management.

두경부 악성 종양환자에서 조기 방사선치료를 위한 구강관리법에 대한 임상적 연구 (The clinical study of oral care for early radiation therapy in the head and neck cancer patients)

  • Introduction: Tooth requiring extraction before radiotherapy in head and neck cancer patients should be performed as long as possible before the initiation of radiation therapy. Conventionally, a minimum 2-week waiting primary healing period is recommended. Although the above 2-week period is ideal, it is not uncommon for the radiotherapist and cancer patient to feel an urgent need to proceed with radiotherapy despite the need for dental care. Therefore, alternative approaches for early radiotherapy, including conservative endodontic treatment and a 1-week waiting primary healing period after dental extraction at the time of radiotherapy were considered and applied based on a literature review Materials and Methods: The clinical study involved 120 head and neck cancer patients who were treated at Wonju Christian Hospital, Wonju College of Medicine, Yonsei University, from January 1995 to December 2004. Results: In the clinical study, there were no specific complications, such as, post-extraction wound infections, radiation osteitis and osteoradionecrosis over the recent 10 years despite the early radiotherapy. Conclusion: Based on the clinical study, a minimum 1-week waiting primary healing period for oral care before radiotherapy is suitable for early radiotherapy in head and neck cancer patients.

성견에 있어서 Cobalt-60 조사가 치수치유과정에 미치는 영향에 관한 연구 (AN EXPERIMENTAL STUDY ON THE EFFECTS OF DOG'S PULPAL HEALING AFTER COBALT-60 IRRADIATION)

  • Irradiation is frequently employed as the sole therapy for oral cancer. These irradiated patients presents peculiar and progressive dental problems. But there is only scanty informations concerning specific approaches to endodontic treatment for head and neck cancer patients who have been subjected to tumorcidal doses of radiation therapy. The purpose of the present study was to determine the effects of cobalt-60 radiation on the pulpal healing of dogs after the direct pulp capping. As the experimental animals, 10 dogs (above 7-8 months after birth) were divided into 3 groups (Control, Group I, Group II). The cobalt-60 was irradiated to the Group I and Group II each 1,009 and 1,562.5 rads as single dose. As the capping material Dycal$^{(R)}$(L.D. Caulk company) was selected. After the direct pulp capping the dogs were sacrified 1, 2, 3, 4, week interval and made the original slides cut with a thickness of 8 microns and stained with hematoxylin and eosin. After examination and comparision of all specimen, the results of this study were drawn as follows; 1. The formation of reparative dentin was observed from the 1st week in the Control group, the 2nd week in the Group I & II. The few and irregular tuble structure was appeared in the 4th week in the Control group only, but failed in the Group I & II. 2. The continuity of dentin bridge was appeared in the 3rd week in all group and the degeneration of odontoblast in the 1st week of the Group II. 3. The congestion and hemorrhage in the pulp tissue were observed in all groups until 3rd week. The inflammation was appeared within the 2nd week in the Group I and especially marked in the Group II, but absent in the Control group. 4. In cases Dycal into the pulp tissue deeply, the local necrosis of pulp and decrease of dentin formation was observed.

화학방사선 요법이 치아발육에 미치는 영향에 관한 증례보고 (EFFECT OF CHEMORADIATION THERAPY ON THE DEVELOPING DENTITION : A CASE REPORT)

  • 하악 좌측부위의 횡문근육종으로 2년동안의 화학요법과 6주간의 방사선요법으로 치료를 받았고, 이에 따른 후유증으로 치아의 발육장애를 동반한 8세 환아에 대해, 임상적, 방사선학적으로 관찰을 시행하고, 치료한 결과 다음을 보고하는 바이다. 1. 구강내 소견으로서 맹출한 영구치의 경미한 법랑질 형성부전과 치아우식이 관찰되었으며, 하악 좌측 부위의 미맹출된 제 1 대구치로 인해 수직고경이 상실되어 과개교합이 형성되었다. 2. 방사선학적으로 맹출한 영구치의 치근은 짧은 치근에서부터 무형성증에 이르기까지 다양한 치근으 발육장애를 보였으며, 미맹출 상태인 제 2 소구치와 제 2 대구치는 왜소치나 형성부전의 소견을 보였다. 3. 치료는 보존치료 및 저석회화 소견을 보이는 영구치를 위해 불소도포를 시행하였고, 향후 적절한 수직고경의 회복과 심미적 개선을 위한 포괄적인 보철치료가 수행되어야 할 것으로 사료된다.

근관협착된 치근파절 치아에서 의도적 재식술 치험례 (Intentional Replantation of a Root-Fractured Tooth with Pulp Canal Obliteration)

  • 상아질, 백악질, 치수를 포함하는 파절로 정의되는 치근파절은 영구치에서 발생하는 외상 중 0.5-7%를 차지하며, 대부분 11-20세군의 상악 전치부에 호발한다. 영구치 치근파절의 처치는 기본적으로 변위된 치관부 파절편을 정복시키고 고정하는 것이다. 60-80%는 치수생활력이 유지되나 치수괴사나 염증성 치근흡수의 징후가 나타난다면 근관치료를 시행하게 되며 대부분에서 치근부 파절편의 치수생활력은 유지되기 때문에 근관치료는 치관부 파절편에 한하여 수행한다. 그러나 치관부 파절편에서 적절한 apical stop을 얻는 것은 어렵다. 의도적 재식술이란 통제된 환경에서 의도적으로 치아를 발거한 후 구강 외에서 치근단 치료를 시행하고 재식립하는 방법으로 완벽한 근관치료와 수복을 목표로 한다. 통상적인 근관치료가 실패한 경우, 기존의 수복물이 존재하거나 석회화된 근관으로 인해 재근관치료가 어려운 경우, 공간적으로 접근이 불가능하여 치근단 수술을 시행하지 못하는 증례에서 의도적 재식술이 계획될 수 있다. 본 증례에서는 이전의 외상으로 인해 석회화된 근관을 보이는 상악 중절치에서 발생한 수평 치근파절을 치료하기 위해 의도적 재식술을 이용하였고 임상적, 기능적으로 만족할만한 결과를 얻었기에 이를 보고하는 바이다.
