  • There can be several states of emergency when the train is in operation. In this paper we consider passengers how to escape from emergency states and what facilities they can use by each situation. In general, passengers can escape from a train by manual door openning, emergency ramp and breaking window. Now, we consider which facility is proper to each scenario.

Emergency Care Conditions where the Nurse is posted in the 119 Relief Squad (119구급대의 간호사 배치여부에 따른 응급처치 실태)

  • The purpose of this study is to identify the difference of emergency care conditions where the nurse is posted in the 119 relief squard or not. The data composed a total of 777 cases of emergency activity of one police stand in Seoul during March, June. September and December in 1998. The results of this study were as follows: 1. The age of the subjects was 20-60 age 54.3%, over 60 age 35.4% with an average age of 50.2. Among them men were 55.0%, the unemployed were 60.8%. The place of the occurrence of an accident was the house, 49.7% and the cause as illness was 59.6%. 2. The main symtom was pain the 36.2% the main consciousness state was alert 76.9%: The state of the patients as chronic was 59.6%. 3. The consulting hospital of the 119 relief squad as a third medical center was 79.9%. The distance to the medical center as less than 5 km was 77.2%, and an average transfer distance was 5.38 km. The cases of doctor guided emergency care was 0.9%, the cases that had posted nurses in 119 relief squads was 48.6%. 4. The case of the best emergency care operation was difficulty in breathing, 62.1% and the second was unconsciousness, 46.1 %. The more serious the consciousness state, the higher the rate of emergency care operation. There wasn't any difference in emergency care operation concerning transfer distance. 5. The cases that had the posted nurses was 19.1%, the cases of no nurse was 11.7% among the cases of emergency care operation during transfering; the cases that had posted nurses had the higher emergency care operation. (p<0.05). Airway maintenance was 14.8% in cases that had the posted nurses, while in the cases of no nurse, 10.9%; and oxygen inspiration was 16.0% in cases that had the posted nurses while in the cases of no nurse 7.6% (p<0.01); spinal fixation was 6.6% in cases that had the posted nurses while in the cases of no nurse 4.6%(p<0.05). With these results, we can conclude that the cases that had the posted nurse showed higher emergency care operation.

  • Zoletil is a non-opioid, non-barbiturate animal anesthetic and proprietary combination of two drugs, a dissociative anesthetic drug, tiletamine, with the benzodiazepine anxiolytic drug, zolazepam. Zoletil has greater potency than ketamine. Zoletil is abused for recreational purposes, especially by people with easy access to medicine. However, in Korea, it is available over-the-counter. Here we report on a case of an 83-year-old woman who received injection of seven vials of "Zoletil 50" by her daughter and presented with an altered mental change. Her mental state was stupor and vital sign was hypotension, bradycardia. Her blood tests indicated metabolic and respiratory acidosis and hyperkalemia. She was treated with intravenous naloxone and flumazenil but was not responsive. She was admitted to the ICU and treated with supportive therapy. Her mental state showed transient recovery, however, her clinical manifestation worsened and she expired.

Prehospital care status and improvement plan of 119 emergency medical technician to anaphylaxis patients (119구급대원의 아나필락시스 환자에 대한 병원 전 응급처치 현황 및 개선 방안)

  • Purpose: This study aims to analyze the current state of emergency care for patients with anaphylaxis and to identify problems and areas for improvement in prehospital care. Methods: This study was conducted using 119 emergency running sheets and medical records of 109 patients diagnosed with anaphylaxis. Questionnaires were also given to emergency medical technicians (EMTs) and emergency physicians. The data were analyzed using SPSS 21.0. Results: Prehospital emergency care included oxygen administration in 64.2%, an intravenous line in 15.6%, and medication injection in 11.0%. The most commonly administered medications were antihistamines in 66.7% and epinephrine in 8.3%. Of EMTs surveyed, 47.0% suggested an epinephrine injection and using direct medical control, while 53.8% of the emergency physicians suggested an epinephrine injection and using indirect medical control. Most emergency physicians 88.8% responded that epinephrine could be administered by EMTs. Conclusion: The data support epinephrine injection of patients with anaphylaxis by EMTs, but a larger sample size of EMTs is required. Education about the treatment of anaphylaxis should be improved for EMTs. The scope of paramedic responsibilities should also be redefined.

The Severity of the Pediatric Patients Visiting Emergency Center (응급실 방문 환아의 중증도)

  • This study was attempted to help in explore new direction about classification of the severity of the pediatric patients visiting emergency center. Data were collected from 276 patients who visited emergency center of E University Hospital during 3 months period from March 1, to May 31,1999. The results were as follows: 1. The degree of severity of the pediatric patients visiting emergency center shown ranged 0-18 and averaged .87. 2. With the respect to the severity of the pediatric patients visiting emergency center, there were statiscally significant difference in patients' visiting time(F=2.607, p=.025), disease classification(F=9.606, p=.000), consciousness level(F=71.499, p=.000), period of symptom manifestation (F=2.262, p=.030), pediatric patients protector's thinking about pediatric patients state (F=16.833, p=.000), treatment outcome (t=5.362, p=.000), duration of stay at emergency center(F=23.944, p=.000).

Usefulness of presepsin as a prognostic indicator for patients with trauma in the emergency department in Korea: a retrospective study

  • Purpose: Trauma is an important public health concern, and it is important to increase the survival rate of patients with trauma and enable them to return to society in a better condition. Initial treatment in the emergency department (ED) is closely associated with the prognosis of patients with trauma. However, studies regarding laboratory biomarker tests that can help predict the prognosis of trauma patients are limited. Presepsin is a novel biomarker of inflammation that can predict a poor prognosis in patients with sepsis. This study aimed to determine whether presepsin could be used as a prognostic indicator in patients with polytrauma. Methods: The study included patients with trauma who had visited a single regional ED from November 2021 to January 2023. Patients who had laboratory tests in the ED were included and analyzed retrospectively through chart review. Age, sex, injury mechanism, vital signs, surgery, the outcome of ED treatment (admission, discharge, transfer, or death), and trauma scores were analyzed. Results: Overall, 550 trauma patients were enrolled; 59.1% were men, and the median age was 64 years (interquartile range, 48.8-79.0 years). Patients in a hypotensive state (systolic blood pressure, <90 mmHg; n=39) had higher presepsin levels (1,061.5±2,522.7 pg/mL) than those in a nonhypotensive state (n=511, 545.7±688.4 pg/mL, P<0.001). Patients hospitalized after ED treatment had the highest presepsin levels (660.9 pg/mL), followed by those who died (652.0 pg/ mL), were transferred to other hospitals (514.9 pg/mL), and returned home (448.0 pg/mL, P=0.041). Conclusions: Serum presepsin levels were significantly higher in trauma patients in a hypotensive state than in those in a nonhypotensive state. Additionally, serum presepsin levels were the highest in hospitalized patients with trauma, followed by those who died, were transferred to other hospitals, and returned home.

Implementation of Intelligent Speech Recognition System according to CCTV Emergency Information (CCTV 응급상황에 따른 지능형 음성인식 시스템 구현)

  • For the emergency detecting in general CCTV environment of our daily life, the monitoring by only images through CCTV information occurs some problems especially in cost as well as man power. Therefore, in this paper, for detecting emergency state dynamically through CCTV as well as resolving some problems, we propose our advanced speech recognition system. For the purpose of it, we adopt HMM(Hidden Markov Model) in our system to do a feature extraction. Also, we adopt Wiener filter technique for noise elimination in many information coming from on CCTV environment. In this paper, our system send only the emergency speech information to a manager to deal with emergency state effectively.

Risk Communication Networks in South Korea: The Case of the 2017 Gangneung Wildfire

  • Wildfires have become increasingly common and intense in South Korea because of climate change, but few have recognized the catastrophic level of the problem. Given the significant impact of wildfires, emergency management stakeholders must have effective risk communication structures for rapidly responding to such phenomena and overcoming geographical difficulties. Despite the country spending billions of dollars to build a big databased early warning system, risk communication flow during the 2017 Gangneung wildfire was ineffective, thereby causing substantial economic, social, and environmental losses. To examine the patterns of information exchange in South Korea's risk communication networks and their structural characteristics during the wildfire, we conducted semantic and network analyses of real-time data collected from social media. The results showed that the inefficient flow of risk information prevented emergency responders from adequately assessing the emergency and protecting the population. This study provides new insights into effective risk communication responses to catastrophic events and methods of research on webometric approaches to emergency management.

The Study of Design and Implementation of RFID Emergency Medical Information System(REMIS) (RFID와 HL7을 이용한 응급 의료 정보 시스템 설계 및 구현에 관한 연구)

  • In this paper, we designed the RFID(Radio Frequency Identification) Emergency Medical Information System(REMIS). This REMIS offers the emergency patient's medical information using RFID and HL7(Health Level 7) to an emergency medical technician. In emergency situation as like coma, if the communication, from the patient's current location to the hospital, is possible, REMIS offer the medical information of the patient through REMIS server to an emergency medical technician. In the state of communication blocked, REMIS can offer the patient identification and the emergency information through RFID tag, which the patient wear, to an emergency medical technician. When this system was designed, the protection of the patient's medical information and their privacy was considered, and the HL7 was used to be compatible with another medical systems. Therefore, in this paper, REMIS was designed that it is always possible to offer the emergency patient's information to an emergency medical technician regardless of any communication status and to improve the emergency rescue process, effectively.

Detection and Recognition Method for Emergency and Non-emergency Speech by Gaussian Mixture Model (GMM을 이용한 응급 단어와 비응급 단어의 검출 및 인식 기법)

Therefore, in this paper, for detecting emergency state dynamically through CCTV as well as resolving some problems, we propose a detection and recognition method for emergency and non-emergency speech by GMM. The proposed method determine whether input speech is emergency or non-emergency speech by global GMM. If emergeny speech, local GMM is performed to classify the type of emergency speech. The proposed method is tested and verified by emergency and non-emergency speeches in various environmental conditions.
