  • The intelligent human-machine interface (HMI) has been developed to enhance the safety and availability of a nuclear power plant by improving operational reliability The key elements of the HMI are the large display panels which present synopsis of the plant status and the compact, digital work stations for the primary operator control and monitoring functions. The work station consists of four consoles such as a dynamic alarm console (DAC), a system information console (SIC), a computerized operating-procedure console (COC), and a safety related information console (SRIC). The DAC provides clean alarm pictures, in which information overlapping is excluded and alarm impacts are discriminated, for quick situation awareness. The SIC covers a normal operation by offering all necessary plant information and control functions. In addition, it is closely linked with the DAC and the COC to automatically display related system information under the request of these consoles. The COC aids the operator with proper emergency operation guidelines so as to shutdown the plant safely, and it also reduces his physical/mental burden by automating the operating procedures. The SRIC continuously displays safety related information to allow the operator to assess the plant status focusing on plant safety. The proposed HMI has been validated and demonstrated with on-line data obtained from the full-scope simulator for Yonggwang Units 1,2.

  • Purpose: Fingertip amputations are the most common type of upper limb amputations. Composite grafting is a simple and cost-effective technique. Although many factors have investigated the success of composite grafting, the success rate is not high. Therefore, this study was conducted to investigate whether the microscopic procedure process during composite grafts improves the success rate. Materials and Methods: Thirteen cases of unreplantable fingertip amputation underwent a microscopic resection procedure for composite graft in the operating room. The principle of the procedure was to remove the least devitalized tissue, maximize the clean tissue preservation and exact trimming of the acral vessel and to remove as many foreign bodies as possible. Results: All fingertips in the thirteen patients survived completely without additional procedures. Conclusion: Composite grafting allows for the preservation of length while avoiding the donor site morbidity of locoregional flaps. Most composite grafts are performed as quickly as possible in a gross environment. However, we take noticed the microscopic resection. This process is thought to increase the survival rate for the following reasons. First, the minimal resection will maximize the junction surface area and increase serum imbibition. Second, sophisticated trimming of injured distal vessels will increase the likelihood of inosculation. Third, accurate foreign body removal will reduce the probability of infection and make it possible to increase the concentration and efficiency in a microscopic environment. Although there is a need for more research into the mechanisms, we recommend using a composite graft under the microscopic environment.

  • We experienced the seven cases of penetrating and non-penetrating cardiac injuries combined with cardiac tamponade from June 1986 to June 1989 at Seoul and Chun-An Hospital of SOONCHUNHYANG medical college. The results were as follows. l. In sex distribution, 7 cases were male. In age distribution, The fourth decades occupied about 58 % of all cases. 2. In mode of injury, 4 cases were stab wounds, 1 case penetration by metallic fragment, 2 cases blunt chest trauma. 3. We routinely checked the CVP with subclavian vein catheterization in case of suspicious cardiac tamponade. Significant increments were showed in 4 cases. 4. Becks triad [low blood pressure, raised central venous pressure, distant heart sound] were recorded in 43 % of the cases with proven tamponades. 5. The sites of injury included RV in 4 cases, LV in 1 case, RA in 1 case and branch of RCA in 1 case. The RV injuries were the most common. 6. Coronary artery damage occurred in 2 cases. LADA was severed in 1 case combined with RV rupture and branch of RCA was torn 1 case. 7. Pericardiocentesis was performed 1 case at another hospital before referring to our hospital. We have never used the procedure because we think that it is potentially dangerous with no clear benefit. 8. Subxyphoid pericardial window was performed in 2 cases of severe cardiac tamponade. We have employed this method to stabilize the patients who had systolic hypotension. 9. Surgical approaches were performed with median sternotomy in 3 cases, thoracotomy in 4 cases. 10. We undertook the simple closure in 6 penetrating cardiac wounds. The removal of impacted metallic fragment was performed under the cardiopulmonary bypass. Simple ligation was performed in 2 cases of coronary artery severance 11. One patient with no sign of life was urgently intubated and undertaken an emergency room thoracotomy on the stretch car without antiseptic preparation. The cardiorrhaphy in 6 cases were performed in the operating theater 12. One patient undertaken emergency room thoracotomy did not survive due to refractory hypovolemic shock. But the remaining 6 patients recovered.

  • The Unmanned Air Vehicle (UAV) systems are indispensable tools of air surveillance and reconnaissance nowadays. Via this systems, hazardous end risky intelligence gathering activities are handled easily. Although they are named as "Unmanned" the UAV systems are commanded by pilots/operators. So, because of weather conditions, enemy attacks etc. as well as pilot error it is possible to face with sudden Round per Minute (RPM) drops and subsequently engine cut/stop during a mission flight at high altitudes. In this case, there are some very urgent decisions to make and rapid "emergency procedure" steps to take in a very short time before Line of Sight (LOS) is lost. The time before crash and the distance to landing air base need to be calculated, the Return Home route need to be checked and the landing/crash side need to be determined. Therefore it is a vital necessity that UAV pilots have some extra qualifications like being determined, well instructed and trained, experienced apart from operating ability. Within this scope, for an education process of a UAV pilot experience sharing and lessons learned are as important as simulators even more. By means of lessons learned it is possible to find out the reasons, mistakes and prevent the likely UAV accidents. In this study it is told about a real UAV crash, experienced of the pilot, the dos and don'ts and the difficulties. Thus it is aimed to help the people who can experience the same or similar situations in future.

  • We evaluate the technical and organizational aspects of the severe accident management guideline (SAMG), focusing on the decision-making process in the technical support center (TSC). From the technical aspects, we conclude that the present SAMG is a good tool that can assist the TSC in efficiently managing probable severe accidents. However, we suggest that the clear separation of the emergency operating procedure (EOP) and SAMG, which shifts plant control from the main control room (MCR) to the TSC, might not be an effective framework from an organizational perspective. Studies on organizational behavior demonstrate that a group decision made under a risky situation might be polarized in either a risky or cautious way. We recognize that we cannot be free from the polarization effect since the current SAMG recommends that the TSC evaluate the advantages and disadvantages of strategies to be implemented and choose the best one based on a group decision process. Illustrative examples of accident management under risky conditions are recapitulated from previous studies of the authors and we propose that the SAMG should be more proceduralized to remove this polarization from the decision-making process.

  • The present study is to assess the applicability of the best-estimate thermal-hydraulic codes, RELAP5/MOD3.2 and CATHARE2V1.3U, to the analysis of thermal-hydraulic behavior in PWRs during midloop operation following the loss of RHRS. The codes simulate an integral test, BETHSY 6.94, which was conducted in the large scale test facility of BETHSY in France. The test represents the accident where the loss of RHRS occurs during midloop operation with the pressurizer and upper head vents open and the sight level indicator broken. Besides, the hot legs are half filled with water and the upper parts of the primary cooling system are filled with nitrogen, with a letdown line open and only one SG available. The purposes of this study are to understand the physical phenomena associated with reflux cooling in the 5G U-tubes when noncondensable gas is present under low pressure and to assess the applicability of the codes to simulate the loss of RHRS event by comparing the predictions with the test results. The results of the study may contribute to actual applications for plant safety evaluation and description of the emergency operating procedure.

  • When a loss of coolant accident (LOCA) occurs in a nuclear power plant, accident scenarios which can prevent core damage are defined based on break size. Current probabilistic safety assessment evaluates that core damage can be prevented under small-break LOCA (SBLOCA) and steam generator tube rupture (SGTR) with rapid cool down (RCD) strategy when all safety injection systems are unavailable. However, previous research has pointed out a limitation of RCD in terms of initiation time. Therefore, RCD success criteria estimation based on allowable coping time under a SBLOCA or SGTR when all safety injection systems are unavailable was performed based on time-line and thermal-hydraulic analyses. The time line analysis assumed a single emergency operating procedure flow, and the thermal hydraulic analysis utilized MARS-KS code with variables of break size, cooling rate, and operator allowable time. Results show while RCD is possible under SGTR, it is impossible under SBLOCA at the APR1400's current cooling rate limitation of 55 K/hr. A success criteria map for RCD under SBLOCA is suggested without cooling rate limitation.

  • Purpose: The present study aimed to evaluate the influence of how the trauma care system applied on the management of trauma patient within the region. Methods: We divided the patients in a pre-trauma system group and a post-trauma system group according to the time when we began to apply the trauma care system in the Halla Hospital after designation of a trauma center. We compared annual general characteristics, injury severity score, the average numbers of the major trauma patients, clinical outcomes of the emergency department, and mortality rates between the two groups. Results: No significant differences were found in the annual patients' average age ($54.1{\pm}20.0$ vs. $52.8{\pm}18.2$, p=0.201), transportation pathways (p=0.462), injury mechanism (p=0.486), injury severity score (22.93 vs. 23.96, p=0.877), emergency room (ER) stay in minutes (199.17 vs. 194.29, p=0.935), time to operation or procedure in minutes (154.07 vs. 142.1, p=0.767), time interval to intensive care unit (ICU) in minutes (219.54 vs. 237.13, p=0.662). The W score and Z score indicated better outcomes in post-trauma system group than in pre-trauma system group (W scores, 2.186 vs. 2.027; Z scores, 2.189 vs. 1.928). However, when analyzing survival rates for each department, in the neurosurgery department, in comparison with W score and Z score, both W score were positive and Z core was higher than +1.96. (pre-trauma group: 3.426, 2.335 vs. post-trauma group: 4.17, 1.967). In other than the neurosurgery department, W score was positive after selection, but Z score was less than +1.96, which is not a meaningful outcome of treatment (pre-trauma group: -0.358, -0.271 vs. post-trauma group: 1.071, 0.958). Conclusions: There were significant increases in patient numbers and improvement in survival rate after the introduction of the trauma system. However, there were no remarkable change in ER stay, time to ICU admission, time interval to emergent procedure or operation, and survival rates except neurosurgery. To achieve meaningful survival rates and the result of the rise of the trauma index, we will need to secure sufficient manpower, including specialists in various surgical area as well as rapid establishment of the trauma center.
