Causative Substance and Time of Mortality Presented to Emergency Department Following Acute Poisoning: 2014-2018 National Emergency Department Information System (NEDIS) (급성 중독으로 응급실에 내원하여 사망한 환자의 원인물질 및 시간 분포)

  • Purpose: The purpose of this study was to investigate the cause of acute fatal poisoning and the time of death by analyzing the National Emergency Department Information System (NEDIS) of South Korea. Methods: The NEDIS data from 2014 to 2018 excluding non-medical visits were used for this study. The patients with acute poisoning were extracted using diagnostic codes. The toxic substances were classified into pharmaceuticals, pesticides, gases, artificial poisonous substances, and natural toxic substances. Patients were classified according to the time of death, place of death, and region. In each case, the most causative substances of poisoning were identified. Results: There were 380,531 patients including poisoning-related diagnoses, of which 4,148 (1.1%) died, and the WHO age-standardized mortality rate was 4.8 per 100,000. Analysis of 2,702 death patients whose primary diagnosis was acute poisoning, the most common cause of poisoning death was pesticides (62%), followed by therapeutic drugs, gas, and artificial toxic substances. Herbicides were the most common pesticides at 64.5%. The proportion of mortality by time, hyperacute (<6 h) 27.9%, acute (6-24 h) 32.6%, subacute (1-7 d) 29.7%, and delayed period (>7 d) were 9.8%. Conclusion: This study suggests that the most common cause of poisoning death was pesticides, and 60% of deaths occurred within 24 hours. The 71% of mortality from pesticides occurred within 6-24 hours, but mortality from gas was mostly within 6 hours. According to the geographic region, the primary cause of poisoning death was varied to pesticides or pharmaceuticals.

  • This research proposed a design direction for the ambulance interior storage space based on the placement and access to the equipment to facilitate an emergency medical practice taking into account the safety and efficiency by the emergency medical technician ergonomic factors. Therefore, through a field survey for the current derivation of the storage space problem 'placement', 'enable', 'management' like these three items resulting clarity, accessibility, simplicity and functionality of the four functional elements by targeting ambulances in operation in this country. First, there is a need to improve standardized of cabinet sizes fit to loaded meet emergency equipment standards. Currently, the equipment designed to use a simple storage cabinet has had problems in the usability. Second, it must be located in the continuous exposure in progress to the user's traffic line. Third, the problem was the insufficient function of the cabinet to ensure the safety of patients by paramedics from the emergency rescue equipment and due to the leaning of the ambulance driving. Besides, it was pointed out that in addition to the proposed alternatives, including the agenda, states that safety belt replacement equipment, emergency personnel and patients with the handle of a guardian for the efficient use storage space within the 119 ambulance. However, in the present study suggest that there is a state proceeds mainly limited usability in the field, having been installed.

Clinical Characteristics of Patients with Acute Organophosphate Poisoning Requiring Prolonged Mechanical Ventilation (장기간 인공환기가 필요한 유기인계 중독환자의 연관인자 분석)

  • Purpose: The major complication of acute organophosphate (OP) poisoning is respiratory failure as a result of cholinergic toxicity. Many clinicians find it difficult to predict the optimal time to initiate mechanical ventilation (MV) weaning, and as a result have tended to provide a prolonged ventilator support period. The purpose of this study is to determine any clinical predictors based on patients characteristics and laboratory findings to assist in the optimal timing of mechanical ventilator weaning. Methods: We reviewed medical and intensive care records of 44 patients with acute OP poisoning who required mechanical ventilation admitted to medical intensive care unit between July 1998 and June 2007. Patient information regarding the poisoning, clinical data and demographic features, APACHE II score, laboratory data, and serial cholinesterase (chE) levels were collected. Base on the time period of MV, the patients were divided into two groups: early group (wean time < 7 days, n = 28) and delayed group (${\geq}$ 7 days, n = 16). Patients were assessed for any clinical characteristics and predictors associated with the MV weaning period. Results: During the study period, 44 patients were enrolled in this study. We obtained the sensitivity and specificity values of predictors in the late weaning group. APACHE II score and a reciprocal convert of hypoxic index but specificity (83.8%) is only APACHE II score. Also, the chE concentration (rho = -0.517, p = 0.026) and APACHE II score (rho = 0.827, p < 0.001) correlated with a longer mechanical ventilation duration. Conclusion: In patients with acute OP poisoning who required mechanical ventilation, the APACHE II scoring system on a point scale of less than 17 and decrements in cholinesterase levels on 1-3 days were good predictors of delayed MV weaning.

The Study on the Effectiveness and Satisfaction of the 'Disaster Safety and On-Site Emergency Management' weekend course in the High School-University affiliated career experience activities

  • This study investigates the satisfaction of students who participated in 'Disaster Safety and On-Site Emergency Management' weekend course, the high school-university affiliated program, to provide the basic data on university's major linked program developing and teaching methods. 98 high school students attended the courses at D General high school and B University in North Jeonlla Province. Among the participants, 52%(51 students) were sophomores, while 56.1%(55 students) were male and 43.9%(43 students) were female. The collected data was analyzed by using the SPSS statistics version 21.0 program. 80.6%(79 students) among the participants chose the weekend course program by themselves, 85.7%(84 students) were with clear motivation and goal, and 42.9%(42 students) answered "so interested studying Emergency at a college in the future" The most important reasons to choose this program are as follows: score 4.68 for 'the degree to which the useful program for youth', score 4.58 for 'the leader's expertise', and score 4.53 for 'reflecting the opinion of youth.' After the program's experience, the 'certificate for cardiopulmonary resuscitation' was the most important and the most satisfactory with score 4.78 and score 4.83 respectively. As the university career program using various job experience can be a meaningful experience that enhance the level of career status and career decisions of high school students, this program will strengthen the affiliation between high school and university curriculum and establish the sufficient national social system environment.

A Study on the Spatial Analysis Model to Decide Medical Institutions/Mental Health Centers for Disaster Victims

  • The National Emergency Management Agency of South Korea has established a Disaster Victims Psychology Support Center. The Disaster Victims Psychology Support Center can enable victims who got psychological damage from disasters to return to their daily lives through healing activity, field visits and advice of experts. The previous Psychology Support Center System managed the information of disaster victims through an independent database. However, this paper proposes a system that is developed to identify medical institutions and mental health centers within a distance of radius, based on the potential Hot-Spot areas of disaster victims using the GIS Systems. The proposed system can efficiently support selection of appropriate institutions for disaster victims using their location and age, classification of damage, and damaged parts of the body. Also, this spatial analysis can assist to decide on a policy based on the location of disaster victims and the extent of damage. Therefore, this paper can provide the required information to support decision making based on the concentrated areas for disaster victims.

Stress of the Pediatric Patient′s Parent in the Emergency Department (응급의료센터에 내원한 환아부모의 스트레스)

  • This study was performed to identify the level of stress recognized by the parents as a support system for their infant patients who were hospitalized in an emergency department, thus to provide the resulting data as the basic material of care intervention for the families of infant patients. This study subjected the parents with infant patients who were hospitalized in emergency department of C University Hospital in Daejeon, and the data was collected from questionnaires for them, dating from Mar. 20, to Jun. 24, 2001 This study used the tool(30 questions) which was originally developed by Ji, Dong-ok (1992) for measuring the stress of families of infant patients in emergency room, and then was modified and complemented by this researcher suitably for infant patients. Collected data was statistically analyzed with frequency, percentage, mean, standard deviation, t-test, ANOVA, by using SPSS WIN10.0 program, and the results were as follows: 1. The stress of parents with infant patients hospitalized in emergency department averaged 3.31 on the basis of 5-point measure, which means that they felt stress beyond means. 2. The factor with high average out of stress factors the parents of infant patients recognized included the followings: 'about the pain of infant patients due to examination(3.91±1.00)'; 'about rare opportunity to be in contact with physician(3.78±1.09)'; 'delay in emergent treatment required for infant patients(3.75±1.31)'; 'delay in the general treatment of infant patients(3.72±1.32)'. Factors of the stress level includes the followings: 'care and medical treatment'(3.46±.72); 'body and diseases'(3.41±.97)'; 'lack of information and supportable resources (3.25±.77)'; and so on. 3. For the stress level according to general characteristics, there were statistically significant differences in the result of the condition of infant patients, medical treatment(P<.01), religion, the procedure of treatment(P<.05). With little preceding studies for infant patients hospitalized in emergency department, this study looked through the level of stress recognized by the parents of infant patients. Based on the comprehension on the parents with infant patients and the knowledge on stress factors recognized by the parents, it is expected that we can seek the methods of care intervention such as explanation of care and treatment procedures, unit policies, continuous interest and emotional supports as well as the provision of information to understand the responses of parents and reduce accompanied stresses.

  • Currently, hospitals and medical practices have a large amount of unstructured information, gathered in time at each ward or practice by physicians in a wide range of medical branches. The data requires processing in order to be able to extract relevant information, which can be used to improve the medical system. It is useful for a physician to have access to a patient's entire medical history when he or she is in an emergency situation, as relevant information can be found about the patient's problems such as: allergies to various medications, personal history, or hereditary collateral conditions etc. If the information exists in a structured form, the detection of diseases based on specific symptoms is much easier, faster and with a higher degree of accuracy. Thus, physicians may investigate certain pathological profiles and conduct cohort clinical trials, including comparing the profile of a particular patient with other similar profiles that already have a confirmed diagnosis. Involving information technology in this field will change so the time which the physicians should spend in front of the computer into a much more beneficial one, providing them with the possibility for more interaction with the patient while listening to the patient's needs. The expert system, described in the paper, is an application for medical diagnostic of the most frequently met conditions, based on logical programming and on the theory of probabilities. The system rationale is a search item in the field basic knowledge on the condition. The web application described in the paper is implemented for the ward of pathological anatomy of a hospital in Romania. It aims to ease the healthcare staff's work, to create a connection of communication at one click between the necessary wards and to reduce the time lost with bureaucratic proceedings. The software (made in PHP programming language, by writing directly in the source code) is developed in order to ease the healthcare staff's activity, being created in a simpler and as elegant way as possible.

  • This paper describes the implementation of wireless realtime monitoring system using modified CSMA/CA protocol. This system consists of wireless modem, central monitor, mobile station, DB server, and offer advantage of mobility, reduced installation time, long-term cost savings, and so on. And this system offers patient position pursuit service. Patient position pursuit service must be offered to deal with emergency which can be occured during patient movement. This paper proposes modified CSMA/CA protocol and patient position pursuit algorithm, implements wireless realtime monitoring system using it.

  • The book, "Euibangyoochui醫方類聚"(A.D. 1445) is called the treasure-house of the knowledge of medical science, due to the 50,000 prescriptions and an enormous amount of medical information that it contains. But in spite of the importance of this book, a convenient and efficient method to use the information in this book isn't developed yet and as a result the use of this book in a low state. Therefore before than anything else, there is necessity to establish the database on "Euibangyoochui". Before the establishment of database on "Euibangyoochui", there are basic works that should be preceded as follows; like the correction through investigating and comparing between the texts that are related with "Euibangyoochui", interpretation, proofread and translation to Korean of the original. And also bibliographical research on "Euibangyoochui". The compilatory course and transmissional system of the medical books in first term of Chosun(朝鮮) was able to apprehend through the bibliographical research about "Euibangyoochui". Also, many emergency medical books and books about the prevention of the infectious diseases mostly based on "Euibangyoochui" were wrote and put to practical use. And this fact, tells us that the compilation of "Euibangyoochui" was supported by the government of Chosun as means to accumulate the medical knowledge.

  • The disaster preparedness system in Korea has been developed in spite of many obstacles, but there are still many problems for response to various kinds of disasters in 21th century. Disaster response system in Korea was focused on policy, administration, hardwares in the past. But in the future it is necessary to change the system to adapt the global needs about the human based disaster response system and capacity to assist the international disaster by official assistance and research for that field. Because nearly all the disasters are associated with the safety, welfare, injury, disability and survival of human, health disaster preparedness and response system should be a important part in the whole disaster system considering the common value of human right to disaster preparedness for human.
