이 연구는 19세 이상 성인의 최근 한 달 이내 전자담배 흡연율과 인구사회학적 요인과 건강수준 및 건강행태 요인 측면의 관련요인을 파악하여, 전자담배 흡연율 감소를 위한 금연교육과 보건정책 수립의 근거를 생성하기 위해 시도되었다. 2018년 서울시 지역사회 건강조사에 참여한 19세 이상 22,908명의 설문자료를 이용한 이차자료 분석 연구이며, 분석은 SAS 프로그램을 이용하였다. 최근 한 달 이내 전자담배 흡연율은 4.5%이었으며, 다변량 로지스틱 회귀분석에서 전자담배 흡연 관련요인은 성, 연령, 교육수준, 직업, 일반담배 흡연상태이었다. 전자담배 흡연을 예방하고 흡연율을 낮추기 위하여 우선적으로 대졸이상의 학력을 가진 전문직을 대상으로 전자담배의 위험성을 알리는 교육을 실시하여야 한다. 또한 일반 국민을 대상으로 전자담배 흡연도 일반담배 흡연과 차이가 없음으로 알리는 홍보와 캠페인을 시행하고 나아가 전자담배를 일반담배와 동일하게 취급하는 동일한 금연정책의 전개가 요구된다.
Traditionally, most cases of nicotine poisoning have been due to ingestion of nicotine pesticides. However, the increasing use of electronic cigarettes (e-cigarettes) has resulted in both intentional and unintentional exposure to concentrated liquid nicotine or "e-liquid" leading to an increase in nicotine poisoning cases. However, fatalities following the ingestion of the e-liquid are extremely rare. We report a rare case of cardiac arrest and severe encephalopathy following the intentional ingestion of e-liquid. We present the case of a 20-year-old woman who intentionally ingested liquid nicotine intended for e-cigarette use. She was found in asystole and experienced a return of spontaneous circulation (ROSC) after undergoing approximately 46 mins of cardiopulmonary resuscitation. Her plasma nicotine levels were >500 ng/ml. Despite aggressive supportive care, she was found to have encephalopathy consistent with severe anoxic brain injury on magnetic resonance imaging. In recent times, there have been some reports of deaths following liquid nicotine ingestion. Our case illustrates the potential for fatal nicotine toxicity from ingestion of e-cigarettes.
Purpose: Acute nicotine poisoning by liquid nicotine in electronic cigarettes is becoming an increasing problem worldwide. The current systematic review aimed to determine the harm of acute nicotine poisoning by reviewing published case reports. Methods: An online literature search with PubMed, Embase, Cochrane Library, and KoreaMed database was performed to identify relevant studies addressing acute nicotine poisoning with electronic cigarettes. Two investigators searched the case reports written in English or Korean. Results: Twenty-six cases were included in this study. The routes of intoxication included ingestion in 18 cases, intravenous injection in three cases, subcutaneous injection in two cases, and ocular exposure in two cases. Ten cases had a cardiac arrest, and seven of them died. Seven out of 12 cases with intentional poisoning had a cardiac arrest. Nine children under 18 years were reported, and three of them had a cardiac arrest. Sixteen cases without a cardiac arrest recovered well, except for one case with sudden sensorineural hearing loss. Conclusion: The authors reviewed the risks of electronic cigarette liquid in terms of acute poisoning through a systematic review. The nicotine solution of an e-cigarette can be life-threatening in cases of acute poisoning. Therefore, active emergency treatment with early recognition is necessary. In addition, various management methods and regulations for preventing acute nicotine poisoning, such as restriction of distribution and nicotine concentration, should be considered.

Electronic cigarettes are novel tobacco products that are frequently used these days. The cartridge contains liquid nicotine and accidental poisoning, even with a small oral dose, endangers children. We present here a mortality case of a 15-month-old child who ingested liquid nicotine mistaking it for cold medicine. When the emergency medical technicians arrived, she was found to have pulseless electrical activity. Spontaneous circulation was restored after approximately 40 minutes of cardiopulmonary resuscitation. The cotinine level in her urine was 1,716 ng/mL. Despite intensive supportive care, severe anoxic brain injury was found on computed tomography and the child ultimately died. This fatality highlights the need for public health efforts to minimize such accidents.
This study was conducted to evaluate the effect of additives in low sidestream cigarette papers, such as Mg$(OH)__2,\;TiO_2\;and\;KH_{2}PO_4$ on the delivery of mainstream and sidestream smoke. From the analysis of tar and nicotine in sidestream and mainstream smoke, the delivered ratios of tar and nicotine by sidestream to mainstream smoke in common cigarette paper were 5.32 and 8.60, respectively. However, the delivered ratios of those of the paper containing $Mg(OH)_2\;were\;2.25\~3.23,\;4.86\~7.14,\;Mg(OH)_2\;and\;KH_{2}PO_4\;were\;2.12\~2.92,\;4.67\~6.89,\;TiO_2\;was\;3.21,\;7.51 $ respectively. The deliver patterns of semi-volatile components in the cigarettes were similar each there, but a slight different pattern in the amount was observed depending on the kinds of compounds added in cigarette papers. In the cigarettes made of $Mg(OH)_2$ added paper, the aromatic components such as benzene, toluene and phenol were generated more while the aliphatic components like neophytadiene, ethyl decanoate were delivered less than the cigarettes made of common cigarette papers. However, the cigarettes manufactured with $Mg(OH)_2\;and\;KH_2PO_4$ added paper showed an opposite trend. The cigarettes made of $TiO_2$ added paper showed relatively low delivery in the most compounds measured. In sensory evaluation, cigarette papers tested were noticed a distinguishable sensory character between the low sidestream smokes with additive cigarette papers except $TiO_2$ added one. Moreover, aroma patterns detected by a electronic nose system in TPM were a similar tendency.
본 연구의 목적은 '제11차(2015년) 청소년건강행태온라인조사'를 이용하여 흡연을 경험한 청소년의 흡연특성과 흡연유형에 따른 특성을 파악하여 복합흡연에 관련된 요인을 알아보고자 하였다. '제11차(2015년) 청소년건강행태온라인조사'에 응답한 68,043명 중 일반담배 및 전자담배 흡연 경험이 있는 중 고등학생 5,347명을 대상으로 분석하였다. 사용된 데이터는 가중치가 적용된 복합표본분석을 이용하여 빈도분석, 카이제곱 검정, 로지스틱 회귀분석으로 분석하였다. 전체 흡연 청소년의 20%가 복합흡연자였으며 전자담배 이용자 85.6%가 니코틴을 사용하였다. 다중 로지스틱 회귀분석 결과 남학생인 경우(OR 1.63; 95% CI 1.15-2.31), 대도시에 거주하는 경우(OR 1.76; 95% CI 1.32-2.33), 하루에 패스트푸드를 1-2회 먹는 경우(OR 2.48; 95% CI 1.33-4.64), 최초흡연 경험 시기가 고등학교인 경우(OR 0.76; 95% CI 0.58-0.98), 간접흡연 경험이 있는 경우(OR 1.24; 95% CI 1.07-1.44), 음주경험이 있는 경우(OR 2.03; 95% CI 1.65-2.48), 일주일간 용돈수준이 높을수록 복합흡연의 위험이 높았다. 본 연구는 청소년의 전자담배와 복합흡연에 대한 기초자료를 제공하고 금연 프로그램 및 정책 개발에 도움이 되리라 판단된다.
Purpose: This study examined the characteristics differences associated with 1-year continuous smoking abstinence in Korean young adult and middle-aged male workers. Methods: 'Korea National Health and Nutrition Examination Survey (KNHANES)' is an ongoing surveillance system that assesses the health and nutritional status of Koreans. For a secondary analysis of cross-sectional data from KNHANES VI-1, this study selected 683 Korean male workers as a subpopulation, who were young adult or middle-aged. The subpopulation was composed of success or failure group of 1-year continuous smoking abstinence. Binomial logistic regression analysis was performed. Results: Success group was more likely to be older, live in urban areas, rate their health status as excellent or good, and have ever been diagnosed with hypertension than failure group. Additionally, Success group was less likely to have used electronic cigarettes (e-cigarettes), drink more than once a month, and have been exposed to secondhand smoking in workplace within a week. Conclusion: Developing smoking cessation programs requires consideration of resources in rural areas, promoting self-efficacy, restricting e-cigarettes and drinking, and promoting a smoke-free workplace.
This study was carried out to determine the relationship between smoke components and sensory evaluation by changes tobacco leaf blending. Seven different cigarettes were made by adding different types of oriental, reconstituted and expanded tobacco leaves. 62 kinds of smoke components which were 6 of general components, 34 of semi-volatile and volatile components, 9 of acid components, and 13 of phenolic components were analyzed. Eight kinds of sensory item were evaluated and also electronic nose system data was collected. All smoke components and sensory characteristics of mainstream smoke were changed by the different blending. To determine the relationship between smoke components and sensory test, the correlation and regression analysis were carried out by using SPSS statistical program. Tar, pH, and CO showed a high correlation with sensory evaluation item. As tar related to hotness, CO have a high correlation with offensive aroma. Semi-volatile and volatile components of smoke related to sensory characteristics such as aroma, taste, irritation, hotness and smoothness. When propylene, l,3-butadiene, butane, isoprene, and 2-methylfuran showed a high correlation with aroma; methyl chloride, methanol, toluene, ethyl benzene showed a high correlation with irritation. Some acidic components and phenolic components of smoke also had a high relation to smoke volume. Especially the acidic components such as 2-furoic acid, 2-hydroxybutyric acid, phenylacetic acid and palmitic acid; the phenolic components such as 4-vinyl phenol, pyrocatechol, 3-methyl catechol, hydroquinone showed a high correlation with smoke volume. As using regression analysis, it was possible to estimate the results of sensory evaluation from the smoke analysis data. From the results of electronic nose system analysis, we can find the different pattern by adding expanded tobacco leaf.

The aim of this study was to present a framework for clinicians to use when discussing electronic cigarette (e-cigarette) use and its association with pre-diabetes. A communication tool was designed using evidence-based strategies from the academic literature. A four-step framework is presented, which includes: step (1) helping patients to understand the association between e-cigarette use and pre-diabetes; step (2) the synergistic health impacts of e-cigarette use and pre-diabetes; step (3) management of diabetes-related lifestyle factors; and step (4) stages of change assessment related to e-cigarette reduction. This communication tool provides support for clinicians to discuss the risk of pre-diabetes associated with e-cigarette use. Moving forward, implementation and evaluation of this model are needed.
