Resuscitative endovascular balloon occlusion of the aorta (REBOA) has been increasingly used in recent years as a resuscitative adjunct for trauma patients with life-threatening non-compressible torso hemorrhage. By blocking the aorta temporarily with an inflated balloon, REBOA preserves cerebral and coronary perfusion while diminishing exsanguination below the balloon, thereby providing time for resuscitation and definitive bleeding control. When determining the occlusion zone during the REBOA procedure, factors such tortuosity of the aorta, co-occurring minor chest injuries, and the severity of shock must be considered, as well as the main injury site. This paper describes a case of high Zone I REBOA in an elderly patient with a tortuous aorta who had concomitant injuries of the chest and pelvis.
Objectives: This report aims to present the case of Korean medicine therapies including Samilsingi-hwan on recurrent hyponatremia in the elderly. Method: A laboratory test was conducted on a patient with general weakness, anorexia, and excess salivation. The patient was diagnosed with hyponatremia and so received intravenous fluid treatment. Results: We continued fluid treatment until electrolyte levels of sodium returned to normal, but the levels gradually dropped repeatedly when the fluid treatment was discontinued. After changing the herbal medicine to Samilsingi-hwan, recurrence of hyponatremia was not seen, even after stopping the fluid treatment. Clinical symptoms also improved during 35 days of hospitalization. Conclusion: This case demonstrates that Samilsingi-hwan may be effective in recurrent hyponatremia in the elderly. Further research is needed.
Methods: This study presents a comprehensive case study of an elderly male diagnosed with acute kidney injury (AKI) resulting from severe dehydration, supported by an extended follow-up with laboratory findings. Results: An 83-year-old male patient experienced severe diarrhea overnight, leading to hospitalization due to symptoms of dehydration and hypotension. His laboratory results displayed a typical AKI pattern, including a significant increase in creatinine levels (5.19 mg/dL) and the presence of hyperkalemia and hyponatremia. Following general treatments, including the administration of an herbal drug (Bulhwangeumjeonggi-san), the estimated glomerular filtration rate (eGFR) improved from 10 ml/min (Stage 5) to 34 ml/min (Stage 3) within five days when he was discharged. Although subsequent eGFR tests, conducted one and two months later as an outpatient, revealed an improvement of 42 ml/min, the patient still experienced mild chronic dysfunction as a consequence. Conclusion: This study presents a noteworthy case of acute kidney injury attributed to severe dehydration, emphasizing the importance of medical awareness regarding diarrhea-induced kidney function impairment, especially in the elderly population.
In Japan, a new nursing insurance system was enforced in April 2000, where premiums were paid according to the level of necessary care. Our project, Nutrition Care and Management(NCM) for the elderly was started in 1995, funded by the Ministry of Health and Welfare of Japan. The NCM project was to provide appropriate nutrition care for the elderly and to see that it effectively functions as part of the health care services. There were 4 stages to the project : the first stage was to find out the PEM status among the elderly patients in hospital and home-care settings in Japan. The 2nd stage was to develop and evaluate nutritional assessment methods, anthropometry, resting energy expenditure measuring methods using of portable indirect calorimeter, and the convenient protein energy intake assessment methods, etc. for the elderly patients with PEM risk. The 3rd stage was to examine the effectiveness of the nutrition care plan induced of protein energy supplement and team care in improving nutrition among the elderly patients. The last stage was to develop the NCM set for the elderly patient based on the past three years of scientific evidence. it is expected that the NCM system for the elderly will provide adequate nutritional care management, improve the elderly care environment and create effective resource management.

In this paper, it is realized by developing the dialog system that is controlled by three kinds of modules such as speech recognition engine, graphical agent, or database classified by a nursing schedule. The system was evaluated in an actual environment of a nursing facility by introducing it to an older male patient with dementia. The comparison study between dialog system and professional caregivers was then carried out at nursing home for 5 days in each case. The evaluation results showed that the dialog system was more responsive in catering to needs of dementia patient than professional caregivers. Moreover, the proposed system led the patient to talk more than caregivers did.

Purpose: This study was to explore and describe the reported experiences of elderly spouses who care their bedridden spouse in the home. Methods: The participants of this study were 14 male and female elderly spouses who live in B metropolitan city and have provided care for more than six months. Data were collected from July 3 to November 6, 2014. Data analysis was done simultaneously with data collection, using the analytical methods of Strauss and Corbin for Grounded theory. Results: The core category was identified as 'going together bearing a heavy burden of care in old age.' In this study, the caring process of elderly spouses can be explained in terms of three stages such as 'a period of trial and error,' 'a period of mastering a role,' and 'a period of role transcendence'. Conclusion: The results of this study can provide an intervention framework to reduce the heavy burden of caring for an elderly spouse.
Pneumonia is an inflammation that forms at the mesopulmonum and alveolus that is also an acute infection with fever and cough. It is difficult for doctors to diagnose pneumonia in elderly patients, due to the variety of symptoms. For this reason, it, with its complications, is a major cause of death for the elderly. In this case, improvement was seen in an elderly patient diagnosed with pneumonia by X-ray, and by using Si-Kyung-Ban-Ha-Tang-Ka-Mi-Bang as a way of Kong-Bo-Kyum-Si, because in this case pneumonia was presumed Dam-Yel-Ong-Pye and Ki-Eum-Yang-Huh due to the weakness associated with old age. This thesis proposes that the elderly are better treated differently from other pneumonia patients due to the weakness that comes with old age.
Background: Small cell lung cancer (SCLC) tends to grow more rapidly and spread much faster than non-small cell lung cancer (NSCLC). A concurrent combination of chemotherapy and thoracic radiotherapy is suggested as the standard conventional treatment, but it is more challenging for elderly patients having pulmonary and cardiovascular comorbidities. Case presentation: Here we present a case of an 80-year-old male, current smoker diagnosed with SCLC in limited stage T3N0M0 (36mm right upper lobe, satellite nodule) in Dec, 2015. The standard concurrent chemoradiotherapy was not available for his comorbidities, which included chronic obstructive pulmonary disease (COPD) and angina pectoris. Furthermore, he and his family refused the recommended chemotherapy or radiotherapy exclusively. Alternatively, he received various non-conventional treatments including local radiofrequency hyperthermia, mistletoe, and Traditional Korean medicine including acupuncture, moxibustion and herbs since Jan. 2016. Despite the progression in primary tumor size, there have been no other distant relapse so far, and the patient has been in stable condition ever since. Conclusion: We suggest that a combination of various alternative treatments could be a candidate for elderly patients intolerable to conventional cytotoxic treatments.
Purpose: This study was conducted to examine the effect of a fall prevention education on the fall related knowledge and fall prevention behavior of the elderly patients in comprehensive nursing care service wards. Method: A quasi experimental study with control group and experimental group was used. A total 62 elderly patients in comprehensive nursing care service wards were randomly divided into the experimental group(n=30) and the control group(n=32). The data collection period was from August to December, 2017. Data were analyzed with $X^2-test$, paired t-test, independent t-test and Pearson's correlation coefficient by using SPSS 25.0. Results: Experimental group reported higher score in fall related knowledge than the control group(t=-10.28, p<.001). Participants who viewed DVD and received the leaflet education reported higher score than those with routine admission education(t=-6.51, p<.001). The experimental group showed significant improvement in fall related knowledge and fall prevention behavior(r=.21, p=.015). Conclusion: The fall prevention program was effective in improving fall-related knowledge and fall prevention behavior among elderly patients. Since the DVD plus leaflet education was effective in improving prevention behaviors, it might be included in routine orientation on admission for the elderly patient in comprehensive nursing care service wards.
