포괄적 중재 프로그램이 취약계층 허약노인의 기능 상태와 골밀도에 미치는 효과 (The Effect of a Comprehensive Intervention Program on the Functional Status and Bone Density of the Socially-Vulnerable and Frail Elderly)

  • Purpose: The purpose of this study is to provide a disease management, nutrition education, and group exercise program for three months to the moderately frail elderly whose physical functions have deteriorated, and to investigate its effects in order to develop an intervention program. Methods: As a quasi-experiment, this study was conducted based on non-equivalence studies designed as a similar experiment. The milk intake group and calcium intake group participated in the disease education, individual nutrition education, and group exercise program for three months, and the control group was visited once in the three months by a nurse who provided disease education, nutrition education, and oral instruction of exercise, and asked them to exercise on their own every day. For the data analysis, ${\chi}^2$-test, ANOVA and $Scheff{\acute{e}}$ test were used. Results: After three months of intervention, there was a significant difference in the frailty level (p=.029) and bone density (p=.001) between the groups. Conclusion: The comprehensive intervention program had an effect on the bone density and the frailty level of the socially-vulnerable and moderately frail elderly, suggesting that the program can be used as a nursing intervention to prevent functional deterioration and damage of the moderately frail elderly.

노인의 영양섭취상태에 영향을 미치는 인구사회학적 요인 분석 (Sociodemographic Factors Associated with Nutrients Intake of Elderly in Korea)

  • In recent years, the number and proportion of Korean elderly have grown rapidly, and elderly individuals show a disproportionate risk for poor nutritional status. The purpose of this study was to examine the relationship of sociodemographic background to nutrient intake of persons 65 years of age or older, living in 15 cities in Korea. Data on 1973 subjects (603 males, 1370 females), who participated in the Korean Elderly Nutrition Survey (2000), were analyzed. Their mean age was 72.3 years and their mean body mass index (BMI) was 24.2 kg/$m^2$. Basic sociodemographic data were obtained through personal interviews. The 98-item semi-food frequency questionnaire, developed and previously validated for Korean middle-aged and elderly subjects, was administered. “Percentage of subjects who consumed under 75% Korean RDA,” “number of nutrients consumed below 75% Korean RDA,” “mean nutrient adequacy ratio,” and “nutrient density” were used to determine nutritional status. Male elderly had better nutritional quality than female elderly. Nutritional quality decreased with age, especially in older elderly (over 75). Elderly who were underweight (BMI 〈 20 kg/$m^2$) showed poorer nutritional quality than those who were normal weight (BMI 20∼25 kg/$m^2$) and overweight (BMI $\geq$ 25 kg/$m^2$). Elderly who lived alone had significantly poorer nutritional quality than those who lived with a spouse, and/or with children. Lower education level and economic dependence also showed lower nutritional quality. A stepwise multiple regression analysis was performed to examine the effects of specific sociodemographic factors on nutritional quality. For number of nutrients under 75% RDA as a dependent variable, education level explained 4.8% of the variance, followed by living status, age, body mass index, gender, and living expense support (Model $R^2$ = 0.091). For mean nutrient adequacy ratio as a dependent variable, model $R^2$ was 0.098. Therefore, sociodemographic variables such as gender, age, body mass index, living status, educational level, and economic status influenced elderly nutrition status. These results indicate that an elderly nutrition intervention should focus on subjects who are poorly educated, living alone, age 75 or older, and/or underweight.

노인대상 영양정보 웹사이트 컨텐츠 개발 : 내용분석과 카드소팅과정(Card-sorting process) (The Development of Web-based Nutrition Information Contents for Older Adults : Content Analysis and Card-sorting process)

  • This study was undertaken to develop web-based nutrition information contents for the older adults. Twenty six domestic web-sites were analyzed and then 12 foreign web-sites and 4 education materials for the elderly of foreign university were benchmarked. Also a lot of literatures on elderly education program were reviewed. A card-sorting task was performed with 8 older adults to ascertain how the target audience organized information about nutrition. The results were as fellows. Among 26 domestic web-sites, 2 sites(7.7%) were only for the elderly. Main topics of information contents for the elderly included 'Importance of Healthy Eating', 'DRI', 'Dietary Guidelines'. Four of twelve foreign web-sites were for the elderly nutrition education. Topics of 'Dietary Guideline', 'Meal Program' were found in 4 sites and 'Importance of Healthy Eating', 'Diet & Disease', 'DRI, 'Food Guide Pyramid', 'Nutrition Fact Labels' were found in 3 sites. Education materials of foreign university dealt with basic information on 'nutrient needs changes related with aging', 'Heart & Bone Healthy Eating Plan', 'Food Guide Pyramid'. Also topics on 'Eating on a budget', 'Eating Out Guideline' were included for practical use for the elderly. Based on card-sorting process, contents framework for web-site was developed and 4 main menus for framework were respectively named as 'Nutrition', 'Meals', 'Foods'. 'Check up Nutritional Health' by panel discussion. Finally we developed nutrition information contents for 4 main menus. We focused on helping older adults recognize the importance of healthy eating and apply the nutrition information to practical use. We expect that the developed framework of contents can be a guideline for indentifying the information needs of older adults in developing effective nutrition intervention program. And we suggest that the survey for target people should be peformed for the web-site to be user-friendly designed and the developed contents be evaluated and revised in the near future.

  • 충북지역 65세 이상 노인들의 조리실습을 병행한 영양교육의 효과를 연구한 결과에서 위생, 식습관, 영양지식, 만족도 모든 항목에서 1, 2차연구에 지속적으로 참여한 지역 (괴산, 보은, 진천)이 위생, 식습관, 영양지식, 만족도 점수 모두 유의적 (p < 0.001)으로 높은 것으로 나타났다. 고령사회에서 65세 이상 인구가 20%이상인 초고령 사회로 빠르게 진입하고 있는 우리나라 농촌지역에서는 고령층의 식생활 향상을 통한 건강증진 및 질병예방을 위한 효과적인 영양교육이 필수적이라 사료된다.

노년층의 영양교육 프로그램 개발을 위한 기초조사 : 영양지식 및 태도 (Nutrition knowledge and attitude analysis of elderly people for the development of nutrition education program)

  • The purpose of this study was to examine the nutrition knowledge and attitude of the elderly for developing nutrition education program. The nutrition knowledge test was consisted of nutrition, nutrients, weight control, food safety, and shopping tips by developing the table of specifications. The nutrition attitude test was comprised of seventeen questions for invention, self-efficacy, control, and outcome expectation. A total of 140 elderly living at home responded to both nutrition knowledge and attitude test. According to the results of nutrition knowledge test, the mean was 13.3 at the maximum 18 points and 51.4% respondents got more than 14 points. Significant differences were found with age (p<0.05, $x^2$=21.46), charge of income (p<0.05, $x^2$=8.86) and monthly expenses(p<0.05, $x^2$=22.95). The results of nutrition attitude test showed that the mean value of 40.89 points at the maximum 51 points and 58.6% of respondents rated more than 40.89 points. In exploring the relationship between the demographic variables(sex, age, educational level, and monthly expenses) and for attitude concepts, a significant correlation was found. Correlation among the knowledge for nutrition, nutrients, and the attitudes for nutrition was significant(p<0.05).

노인세대의 편의식 이용현황과 요구도 조사 (Survey on Present Use and Future Demand for the Convenience Food in the Elderly Group)

  • This study is to research the use and the demand of the elderly generation that pursue economic ability and active lifestyle. The composition of the meals were "homemade meals" 65.3%, "homemade & convenience meals" 26.7% overall. For the "never miss a meal" category, 70-74 years old was 48.3%, over 80 years old was 60.0%. The elderly under 70 years old showed a high frequency in the use of alternative meals (p < 0.05) and the elderly with higher education used the alternative meals more frequently (p < 0.01). The rice-cakes, bread, tuna cans, Korean side-dishes, beverages, meat, cup-noodles and seafood as alternative meals are in the order of most frequency. Also, 96.0%, 59.0% and 46.5% of the elderly had experience in using "Ready to eat (RTE)", "Ready to heat (RTH)" and "Ready to cook (RTC)". The oldest group surveyed, the elderly at the age of 80 years old and above showed lower preference for "completely cooked state" but showed highest results in the "half-cooked state". This shows the difference between people's perception and the behavior that showed high frequency of 96.0% in the experience with "RTE". This can be inferred that the elderly needs the development of convenience food that is moderately packaged according to the newer lifestyle where the elderly live independent from their children. Moreover, there is a higher demand in the healthy convenience food, which means that there is a need for development in the quality of the convenience food that will allow more faith in convenience food by the elderly.

노년기 자살생각의 요인과 변화추이 분석: 국민건강영양조사 3개년도(2001, 2005, 2010)자료를 활용하여 (Influencing Factors and Trend of Suicidal Ideation in the Elderly: Using the Korea National Health and Nutrition Examination Survey(2001, 2005, 2010))

  • Objective: The purpose of this study was to analysis the determinants and trend of suicidal ideation the elderly in Korea. Methods: This study participants were selected the elderly over the age of 55 from the Korea National Health and Nutrition Examination Survey in 2001(n=1,122), 2005(n=2,098), and 2010(n=2,402). Statistical analysis methods used in this study were $x^2$-test, logistic regression analysis and other basic statistics such frequency, percentage using SPSS version 21.0. Results: In 2001, the influencing factors of suicidal ideation was spouses, subjective health status and stress recognition. In 2005, the influencing factors of suicidal ideation were spouses, subjective health status, chronic disease amount, activity limitation, depression experience and stress recognition. In 2010, the influencing factors of suicidal ideation were elderly, education level, subjective health status, activity limitation, depression experience and stress recognition. Conclusions: The health education considering the characteristics of each elderly group should be developed and applied to prevent adults' suicidal ideation because the factors influencing suicidal ideation were revealed differently between the elderly group.

The effects of elastic band exercises and nutritional education on frailty, strength, and nutritional intake in elderly women

  • [Purpose] The purpose of this study was to examine the effects of elastic band exercises and nutritional education, as well as to identify the factors influencing frailty, strength, and nutritional intake of elderly women. [Methods] The subjects in this study were 30 elderly women who were divided into four groups. All groups agreed to participate in four programs: health education only (HE), elastic band exercises only (EX), nutritional education only (NU), and elastic band exercises plus nutritional education (EX+NU). Frailty was evaluated by measuring the frailty factors according to Fried et al. Leg strength was measured using a leg-extension machine. Nutritional intake was assessed by the 24-hour recall method and food records. Nutritional intake was analyzed by CAN Pro 5.0 program. [Results] After three months, the prevalence of frailty significantly decreased in the EX+NU group (P=0.013) compared with that of the HE group (P=0.088). There was significant improvement in leg strength in both the EX (P=0.012) and EX+NU groups (P=0.003) compared with that of the HE group (EX, P=0.005; EX+NU, P=0.002). The nutritional intake significantly decreased in the EX group compared with that of the HE group (P<0.05, P<0.05). [Conclusion] The combination of elastic exercises and nutrition education had positive effects on frailty and leg strength, while having negative effects on total calories, carbohydrate, sodium, and iron intake in elderly women. Elastic exercises only had positive effects on leg strength while having negative effects on nutritional intake in elderly women.

경북 농촌지역 여성 독거노인과 가족동거노인의 계절별 영양소섭취 비교 (Seasonal Nutrient Intakes of Elderly Women Living Alone as Compared to Those Living with Family in the Gyeongpuk Rural Area)

  • For the rapidly growing elderly population, the achievement and maintenance of good nutritional status is critical to health, functioning and quality of life. Elderly women living alone have been identified as a group associated with poor nutrition. The purpose of this study was to assess dietary intakes of elderly women living alone as compared to those of elderly women living with family in a rural area and to examine seasonal variation. The subjects are 49 elderly women living alone and 41 elderly women living with family who reside in Goryeong-gun, Gyeongbuk, and their food intakes were assessed once each time in summer 2005, winter 2005-2006, and spring 2006. The average ages were 74.7 years for living alone and 72.8 years for living with family. Education level was not different between the two groups. Height, weight, body mass index, systolic and diastolic blood pressures, and fasting blood glucose were not significantly different between the two groups. Average intakes of major nutrients, nutrient adequacy ratio, mean adequacy ratio and index of nutritional quality were lower in the elderly women living alone compared with the elderly women living with family in summer, but the differences in intakes of most nutrients became insignificant both in winter and in spring. High carbohydrate and low fat diet was prevalent and intakes of carbohydrate and fat in summer deviated from macronutrient acceptable distribution ranges. Percentages of the subjects who consumed energy less than 75% of the estimated energy requirement and nutrients less than the estimated average requirement were higher than those reported by the Third National Health and Nutrition Examination Survey. In summer, the percentage of the subjects who consumed energy less than both 75% of the estimated energy requirement and 4 nutrients less than estimated average requirements was 58.5% of the elderly women living alone, which was higher than 26.5% of the elderly women living with family and that of National Nutrition Survey. Therefore, nutrition policies including nutrition education and support are necessary to improve nutritional status of elderly, especially elderly women living alone and should reflect regional and seasonal characteristics.

울산지역 보건소 이용 노인들의 영양상태와 인지상태 (Nutritional Status and Cognitive Status of the Elderly Using Public Health Center in Ulsan)

  • This study was designed to obtain the information concerning food intake, dietary habits, functional status, health condition and cognitive status of the elderly using public health center in Ulsan area. The subjects of this study consisted 154 elderly persons aged 60~82 years. Interviews were conducted using the health habits and food frequency questionnaires to provide basic information for nutrition education program. We evaluated the current food intake, dietary cholesterol intake (cholesterol index), functional status, cognitive function and blood analysis of the subjects. The results of this study were as follows : Mean age of the subjects was 68.7 $\pm$ 6.7years. The average cognitive status score of the subjects was 7.9 $\pm$ 2.0 (full score was 10.0). Male had a higher cognitive status score than female. There was significant difference between cognitive status score and age, education level, pocket money, physical activity and living condition. The subjects who had a higher cognitive status score ate more fish and meats group and milk and milk products than the subjects had a lower cognitive status score. And hemoglobin level, serum triglyceride, HDL-cholesterol and atherogenic index affected to cognitive status while fasting blood glucose and LDL-cholesterol did not any effect on cognitive status. These results have demonstrated that various socioeconomic variables and food intake pattern and nutritional status affect on cognitive status with aging and suggest that proper nutrition education and adequate nutrient intake in quality and quantity are essential in maintaining cognitive status in later life.
