폐교를 리모델링한 노인시설에 관한 연구 - 일본의 시나가와구 리모델링 사례를 중심으로 - (A Study on the Recycling of the Closed Schools in Japan - A Case Study on the Remodeling of Shinagawa in Japan -)

  • Recently (2019), more than 3,800 schools have been closed in Korea due to the decrease in the number of students. Among them, 1,000 closed schools were used, 400 unused closed schools were sold, and the remaining 2,400 were sold. However, considering the absolute lack of elderly facilities due to the aging population, it seems necessary to recycle these public assets into elderly facilities. The purpose of this study is to explore the cases of remodeling closed schools in Japan and recycling them as elderly facilities, and to find ways to resolve the very serious shortage of elderly facilities while seeking measures to closed schools in Korea. The results of this study are summarized as follows: 1) In the case of Shinagawa in Tokyo, it was easy to change its use because the building was owned by the old district. 2) The existing classroom space was used as it was to create an elderly living space consisting of two or three rooms and one bathroom unit. 3) In case 2, even if the earthquake-resistant structure was reinforced, the overall construction cost was reduced by 30% compared to the new construction.

일부 지역사회 노인들의 의료서비스 접근성에 관한 조사 연구 (Accessibility of Medical Care in an Elderly Population)

  • A quantitative descriptive survey was conducted to assess the needs of elderly people in relation to accessibility of medical care. Identifying their functional status was done and accessibility of medical care was ascertained. Using a convenient sampling method, 856 elderly aged 65 and over in Taejon-city participated. A multi-dimensional questionnaire containing sections on health status, Activities of Daily Living(ADLs) and accessibility of medical care in terms of use, time, distance, transport, cost and feeling at ease was developed by the researchers of this study to collect data. Data were collected between October and December, 1999. In general, the result of the self-reports from this study found that approximately 40% of participants had difficulties with their health status and ADLs while about 45% of the population did not access medical care. The majority of respondents stated that they had no difficulties with time distance and transport to access medical care. About two-thirds of the respondents felt that they had many or some difficulties with expenses and strategies for emergency to access medical care. Even though these findings need to be generalized, several recommendations for appropriate medical care delivery for the elderly still can be outlined from the study findings. Recommendations suggested are: To identify impediments to access medical care in emergency in elderly people and to remove those factors preventively are required. Responsible governmental involvement for solving problems of the medical care cost and additional costs in relation to separation of dispensary from medical practice for the elderly is required. More constructive and practical uses of public health community centers are recommended.

질적 연구를 통한 삶의 맥락적 관점에서의 노인의 도시락 및 밑반찬 배달 서비스에 대한 수용 태도 및 적응에 대한 이해 (A Qualitative Study on Attitude, Acceptability, and Adaptation for Home-delivered Meal Services in the Korean Elderly from the Perspective of Life Context)

  • Objectives: The purpose of this study was to suggest the strategies for improvement of home-delivered meal services for the elderly, to identify reasons for recipients to get started with the services and to evaluate the attitude, acceptability and adaptation of recipients to the services from the perspective of life context. Methods: The data was collected through face-to-face in-depth interviews with eighteen low-income elderly recipients of home-delivered meals and analyzed using a qualitative research method. Results: The results were deduced as four themes which comprised of long-term vulnerable socioeconomic contexts resulted in entry to the services, conflicting acceptability to the services, passive adaptation to taking the services, and positive practices to cope with supplement free meals or other services. The service participation was initiated because of a combination of prolonged, vulnerable socioeconomic contexts, including poverty and unexpected life events such as diseases, disability, living alone, aging and unemployment. With regard to taking the services, conflicting acceptability was observed: positive aspects including saving living cost and good quality of meals, and negative aspects including lack of a tailored service and feeling of stigma. Although the recipients needed an individualized service, they did not express their needs and demands for the services and they accepted the unavailability as an accustomed, prolonged vulnerable socioeconomic context. With regard to lack of tailored services, either self-solution such as modification of eating patterns or community-based network and services were used. Conclusions: We suggest that a system to concretely identify recipients' attitude, acceptability and adaptation for home-delivered meal services should be developed in the establishment of a tailored nutrition support system for the low-income elderly.

재가노인을 위한 식사배달서비스 제공기관의 실태분석 (Analyzing the Current Practice of the Home-Delivered Meal (HDM) Service Program for Homebound Elderly)

  • The purpose of this study was to research the current home delivered meal (HDM) service programs for seniors living in the community. Fifty seven centers which operated a HDM service program were surveyed with respect to their administrative structure, menu management, food purchasing and production management, hygiene and equipment and facility. -Statistical data analyses were completed using the SAS 8.1 program for descriptive analysis and t-test. The results showed that 55 percent of the study group were from 70 to 79 years old. All of the participants received free HDM. As a result of the meal cost analysis, the meal cost at 56.1% of the HDM service centers was from ₩2,000 to ₩2,499 per meal. A total of 68.4% of the HDM service centers were operated without the services of a dietitian. According to the menu analysis, all nutrients except Vitamin B2 were at levels of more than 33% of the Recommended Dietary Allowances for Koreans. Although 96.6% of the HDM service centers required a therapeutic diet menu for the health of the elderly recipients, 68% of the directors responded that they could not afford to serve therapeutic meal. Food purchasing, menu planning and other foodservice management processes were handled by non-professionals, such as volunteers, cooks or social workers. Forty two percent of the HDM service centers never used standard recipes. For determining portion sizes, 75.4% of the HDM service centers depended on personal experience. Finally, the current HDM service programs for the homebound elderly were not operated systematically. It is suggested that professionally trained personnel should be included among the staff members to provide a more effective HDM service. The HDM service programs should be supported financially and systematically by the government.

  • Mid of 1980s witnessed the beginning of a new trend regarding the elderly as the Third Age Group, which is capable of positive selection for their later life. These changes are caused not only by the rising cost of caring them as the dependent to the society, but also by paradigm shift from the clinical and biomedical model to political economy and critical gerontology. The paradigm shift is based on the social constructionism as a perspective on the later life. In this communication the development of social constructionism is reviewed as an approach on the living environment for later life through literature survey. Emphases were made on the anti-ageism instead of ageism in the analysis of the social constructionism perspective for living environment in later life. Individual practice, influencing the practice of others, influencing agency policy, and developing theory were addressed and emphasized for the practice of anti-ageism. This study found the importance of the development of the social constructionism perspective on the study and practice regarding living environment for ‘the Third Age’ in Korean situation.

장기 보호 여성노인의 수발에 관한 연구 - 수발특성과 우울을 중심으로 - (Caregiving for the Long-Term Care Elderly Women - Focusing on Caregiving Characteristics and Depression -)

  • The present study was initially designed to figure out the general condition of care giving system for the elderly women who need long term care and the level of their depression according to the conditions of care. And This research is intented to present appropriate policy that could help the establishment of supporting system for the fragile elderly women.1 used the data from <2001 National Study on the Needs for the Long-Term Care Elderly> by Korea Health and Population Institute. The results are as follows: First, Two third of all the respondents had serious problems (2-9 activities limits) in Instrumental Daily Living Ability(DAL). Most respondents reported “low” in satisfaction level related to receiving care, meaning the elderly had negative perception for the care from the family. The elderly expected their children to be as the primary care giver and mostly wanted to live with them in the future. Second, The majority of the long term care elderly women haven't used community service facilities very often and said they are not likely going to use the facilities in the future. Third, The respondents reported high in depression level as to lower satisfaction with their children's support, poorer health condition, more reluctant to use service facilities due to the cost, and fewer friends and neighbors resources around them. Therefore I could say that negative factors for the elderly women's psychological health were having unsatisfactory relationship with intimate people, developing physical illness, being in economic difficulties. That is, receiving less help from close family members, shrinking social network, and experiencing economic hardship would have negative effects on elderly women's psychological health. In the basis of these results, I suggest that in the mean time we shouldn't overlook the importance of the private support when we develop the public elderly support system.

Nutritional Assessment of the Older Population: Practical Application and Limitation

  • Evaluation of nutritional status is an essential element in providing appropriate intervention strategies to achieve the highest level of health, Nutritional assessment of the older population is complicated by many factors which do not significantly affect the nutritional status in young adults, therefore, it should be considered in two ways; community-dwelling elders group and hospitalized or institutionalized elderly group. To sort out the individuals with nutritional problems in a community efficiently, nutrition screening tools must be simple, relatively inexpensive, and applicable to a large number of subjects. Combination of tools and indicators such as 24-hour food recall, body weight and height, and questionnaires on eating practices, and the presence of chronic diseases is practically applicable as basic tools of nutritional screening of older age group. However, the lack of validated screening techniques remains a barrier in improving nutrition. Validation is only limited to energy, BMI, protein intake of the older populations living in western countries. Further refinement of nutritional assessment tools is demanded to figure out whether those are practically applicable to community-living older adults in Asian Society. A careful and systematic evaluation of nutritional assessment tools should be carried out prior to implementation of stepwise nutrition service to the heterogeneous older population. For an in-depth nutritional assessment at the individual level, we need to extend research efforts to clarify the requirements of nutrients due to aging and diseases. More cost-effective method that will allow rapid analysis of survey results are needed so that information can be readily available to policymakers.

도시형 실버타운의 입지특성 분석 연구 (Analysis of the Location Characteristics of Urban Silver Towns)

  • 최근 우리나라에서 급속도로 증가하고 있는 노년층은, 과거와 달리 '액티브 시니어'로 불린다. 액티브 시니어는 높은 거주 비용에도 불구하고 도심의 실버타운에 거주하는 것을 선호하며 그들만의 새로운 도시콘텐츠를 만들어 간다. 액티브 시니어의 생활특성을 반영한 새로운 도시 콘텐츠는 그들의 거주지 선택에도 중요한 영향을 미친다. 본 연구에서는 액티브 시니어의 도시생활 콘텐츠를 바탕으로 액티브 시니어의 주요 거주지로 선호되는 도시형 실버타운의 입지 특성을 파악하는 데 목적을 두고 있다. 액티브 시니어의 도시생활 콘텐츠를 고려한 도시형 실버타운 입지 특성으로는 1) 대중교통, 2) 주변간선도로, 3) 문화 쇼핑시설, 4) 의료시설, 5) 아파트 단지, 6) 자연적 환경으로 구분할 수 있다. 특히 자연적 환경보다는 주변의 의료시설 및 아파트 단지 및 문화 쇼핑 시설이 도시형 실버타운의 주요한 입지 특성으로 나타났다. 본 연구의 결과는 액티브 시니어의 새로운 도시 콘텐츠인 도심 실버타운의 입지선정뿐만 아니라 고령자 주택 관련 정책에도 도움이 될 것으로 기대된다.

전국 법정복지대상 노인의 일상생활 수행능력과 치매와의 상관관계 (A Study on ADL and Dementia of Aged Person with Medicaid in Korea)

  • The purpose of this study was to analyze characteristics related to the activity of Daily Living (ADL) and dementia among the elderly people who have Medicaid. The cross-sectional descriptive survey study was a nationwide randomization sampling among the population of elderly families who have Medicaid. The data were collected during the month of October, 1999 and total sample was 1,027 elderly people. There were major findings according to the studies. In the results of the ADL assessment most of elderly people were within the 24 to 45 point range. Also, 63.3% of elderly people who made 45 points do not need help when performing daily activities according to the 15 areas of activity components, and 4.9% of these people couldn't do their daily activities. The results of the Dementia assessment were 70.6% of elderly people were in the normal range, 21.7% have a mild case, and 2.8% have severe case of dementia. These were found by using instruments for mental states, which simplified to items of detection of early dementia. In the result of these tests, there was a significantly positive correlation between ADL and degree of dementia with the pearson correlation coefficients. As a result of these studies, the author recommend to strengthen function and organization of public health like a visiting nurse center for elderly people who are over 65 years old. In addition, the government should apply early detection and management system for dementia in the community continuously and cost-effectively, especially for elderly people who live alone and are vulnerable elderly as our priority.

농촌 노인의 사회관계망이 생활서비스 필요도에 미치는 영향 (The Effects of Social Networks on the Need for Life Services for the Elderly in Rural Areas)

  • The purpose of this study was to identify the social networks used by the rural elderly and to analyze the effects of these social networks on the need for life services. The research subjects were 1,000 people aged 65 years and over living in the rural areas of Korea. Among the total 1,000 respondents, 979 elderly who have children were included in this study. The data were analyzed using frequency analysis, descriptive statistics, and multiple regression analysis. The results of this study were as follows. First, the frequency of contact with friends/neighbors was higher than that of children, whereas the social support of children was higher than that of friends/neighbors(excluding physical support). Second, the rate of use of home services and the elderly welfare center was very low. Promotion, a minimum cost burden, convenient transportation (circulation bus operation, etc.), and the provision of visiting services are examples of ways to be using them much more. Third, the elderly who do not live with their children have a higher need for life services. As well unlike the elderly who live with their children, in the case of the elderly who do not live with their children, the necessity for life services was influenced not only by their children but also by the relationship with their friends/neighbors and by home services. This implies that comprehensive social networks are important for the elderly who do not live with their children. Therefore, it is necessary to activate the social support of the community as well as of children and friends/neighbors.
