A Proposal of the Comprehensive 'Aging in Place' Support Institution through the Analysis of the Legal System and Recognition Survey of the Elderly Housing Modification (국내의 노인주거 개조 지원 관련 법제도 현황 및 인식조사를 통한 Aging in Place 종합지원기관 도입 필요성 제안)

  • This study suggests the general support center with comprehensive services for the elderly. In general, the modifications of the elderly housing have a direct influence on the "Quality of life" and "Aging in place" for the elderly. But until now, the elderly housing modification has been carried out separately on the elderly welfare category. It is very inefficient and inconvenient for the elderly. Therefore, it is necessary to have a comprehensive organization that includes the implementation of modifications. To provide reasons for this insistence, this study analyzed the previous research, legal system guidelines, and others related things. And it researched the perception of the comprehensive institution and the need for housing modification through the recognition survey of the elderly and the early elderly. The study found that the current domestic legal system does not have an integrated system to carry out the entire process of applying and implementing the elderly housing modification. On the other hand, the results of the survey showed that the elderly hoped to introduce a comprehensive and user-friendly institution for housing modification.

Analysis of the Characteristics of Hospital Visits and Medical Utilization of Elderly Patients at an Emergency Medical Center in the Gyeongbuk Region (경북지역 일개 응급의료센터 노인환자의 내원관련 특성과 의료이용 분석)

  • Objectives : The purpose of this study was to examine the health service utilization of elderly patients who visited an emergency medical center in the Gyeongbuk region and to provide basic information for the effective management of emergency medical centers. Methods : This study analyzed the characteristics of the hospital visits and the actual situation for the use of emergency medical care of 10,264 elderly patients that visited an emergency medical center in the Gyeongbuk region from January, 2014 to December, 2014. Frequency analysis and chi-square test were done in this study. Results : This study showed that there is a difference in the characteristics of health service utilization which included hospital visits, duration of hospital visits and mode of arrival to the emergency medical center according to age, gender and other characteristics. Conclusions : Providing efficient emergency services is necessary as well as establishing an emergency medical center management plan that takes into consideration the difference in health service utilization of elderly patients.

Outcome Assessment of Endodontic Treatment of Mandibular Second Molars with C-shaped Canals in Elderly Patients

  • Purpose: The aim of this study was to investigate the outcomes of endodontic treatment of mandibular second molar with C-shaped canal in elderly patients and related factors affecting the survival of the tooth. Materials and Methods: From 2010 to 2015, the survival rate following endodontic treatment was evaluated in elderly patients over 60 years who visited the Veterans Health Service Medical Center for endodontic treatment. The presence of C-shaped canals was confirmed using clinical records and radiographic features. Patient's age, sex, systemic diseases, tooth location, vitality, signs and symptoms, fractures, caries, apical radiolucency, canal shaping methods, sealer leakage, filling voids, and restoration of prosthesis were included in the analyses as confounding variables. The survival rate of teeth was analyzed using Kaplan-Meire analysis and the relationship between the survival rate and variables was analyzed using Simple and Multiple Cox regression analysis. Result: In total, 107 teeth in elderly patients had C-shaped canal. The survival rate of teeth that received endodontic treatment was 63.70%. None of the factors investigated significantly influenced the survival rate (P>0.05). Conclusion: In elderly patients with C-shaped canal, the survival rate after root canal treatment was not significantly different from that of other mandibular molars.

Pneumonectomy for Clinical Stage I Non-Small Cell Lung Cancer in Elderly Patients over 70 Years of Age

  • Background: Lobectomy is the generally accepted standard treatment for early-stage non-small cell lung cancer (NSCLC). However, especially in elderly patients, it is often necessary to perform pneumonectomy in order to maximize the likelihood of curative treatment, although pneumonectomy is a challenging procedure. Methods: We analysed patients who were clinically diagnosed with stage I NSCLC and underwent pneumonectomy with curative intent from 2004 to 2011. The patients were divided into an elderly group (${\geq}70$ years) and a younger group (<70 years). We retrospectively analysed the outcomes of these groups of patients in order to characterize the role of pneumonectomy as a treatment for elderly patients with clinical stage I NSCLC. Results: Thirty patients younger than 70 years of age (younger group) and fourteen patients 70 years of age or older (elderly group) who underwent pneumonectomy were enrolled in the present study. The median follow-up period was 35 months (range, 0 to 125 months). The perioperative mortality rate (within 90 days after the operation) was 7.1% in the elderly group and 6.7% in the younger group (p=0.73). No significant differences between the two groups were observed regarding the occurrence of pneumonia, acute respiratory distress syndrome, cardiac arrhythmia, bronchopleural fistula, and vocal cord paralysis. The overall five-year survival rate was 79.4% in the younger group and 35.7% in the elderly group, which was a significant difference (p=0.018). The five-year disease-free survival rate was 66.7% in the younger group and 35.7% in the elderly group, but this difference was not statistically significant (p=0.23). Conclusion: Although elderly patients with early stage lung cancer showed a worse long-term survival rate after pneumonectomy than younger patients, the outcomes of elderly patients were similar to those of younger patients in terms of perioperative mortality and postoperative complications. Patients should not be denied pneumonectomy solely due to old age.

  • The purpose of this study is to investigate on the effect of the elderly reading education activity for the children in local child care center. For this study, the data was collected, based on questionnaires by children who participated in the elderly reading assistant education activity program in 14 local child centers. And The study employs a survey observation of reading guidance lesson and individual interviews with the workers at the local child care center. As a result, this shows that the elderly reading guidance activity effects on draws children' interest in reading effectively, and that the workers' opinions about the elderly reading education activity are positive.

Analysis of Nursing Records for Elderly Patients with Abdominal Pain in the Emergency Medical Center (응급의료센터에 내원한 복부통증 노인 환자에 대한 간호기록 분석)

  • Purpose: This study was done to analyze nursing assessment and nursing care for pain in the electronic nursing records for the elderly patients with abdominal pain visiting the Emergency Medical Center. Methods: This study is a descriptive study based on nursing records from January to December 2015. A total of 1155 records for elderly patients with abdominal pain were gathered. Results: The mean age of elderly patients whose records were analyzed was 75.2 years. Analysis of nursing records regarding pain management showed that semi-urgent severity (93.7%), direct emergency room visits (58%), and 6.01 hours of emergency room stay (6.01 hours)were the most frequently documented characteristics of the elderly patients with pain complaints. Recording time of nursing assessment for abdominal patients was 1.01 hour; the average pain intensity was 3.97. The mostly used nursing intervention for abdominal pain was medication (65.1%). There was no record of non-pharmacological pain nursing interventions. Conclusion: The results of this study showed that improving knowledge and nursing practice for pain management is much of necessity. In particular, development of the non-pharmacological nursing interventions for pain is needed. Further research is also imperative to develop and evaluate record systems for pain management that can be used in the emergency room.

  • The research on the health promotion effect of the elderly through the health care service has been going on for a long time, but there is insufficient research to grasp the needs of elderly people in order to effectively provide health care services. In order to solve these problems, this study suggested the direction of health care service for elderly people by analyzing regional characteristics and demand among rural areas. To this end, the direction of improvement of customized healthcare service model was suggested through the analysis of the health - related program utilization status, health management method, health care service type, and contents demand of the elderly by urban area and rural area.

  • The purpose of this study is to investigate the effect of information education satisfaction of the elderly welfare center on information utilization satisfaction and life satisfaction of the elderly. For this purpose, we surveyed the elderly who were enrolled in the elderly welfare center from September 02 to September 24, 2015. First, in the information cognition improvement of the elderly, it was found that the content of education of information, education instructor, and educational environment are important. And in the improvement of the information experience of the elderly, it was found that the education contents, the education method, the instructor and the educational environment factors of the satisfaction of information education were important factors. Second, in the improvement of personal factors satisfaction of the elderly, it was found that the content of education of information and education and the factors of education instructor were important factors. The purpose of this study was to investigate the relationship between information literacy and satisfaction of the elderly.

A Study on the Nutrient Intake, Health Risk Factors, Blood Health Status in Elderly Korean Women Living Alone (일부지역 저소득층 독거노인의 영양소 섭취, 영양위험도 및 생화학 지표에 관한 조사연구)

  • This purpose of this study was to investigate the nutrient intake, the health status as determined self-assessment checklist, biochemical indicators of elderly Korean women. We interviewed and 55 female subjects living alone in the Urban aged over 65 years. Information on their dietary intake was collected by 24-hour Recall method. Their health status was determined by a NSI checklist. Biochemical indicators were performed in whole blood and plasma of subjects. Except for protein, Fe, all of the elderly subjects belonged to over moderate nutritional risk. The average daily nutrient intake of the elderly was below the level of the recommended dietary allowances (RDA) for Koreans. A relationship between their health risk score and nutrient intake was observed negatively (not significantly). They had a risk of anemia as hemoglobin and hematocrit of subjects were under the normal value. Therefore, the reason that health risk score and health status badly was thought for lower nutrient intake.

Model Development of an Elderly Health Promotion Center: The Effect of a Social Support Program at a Community Health Center (노인 건강증진센터 모형개발을 위한 연구: 일 지역보건소에 적용한 사회적지지 프로그램의 효과)

  • Purpose: The purpose of this study was to develop a model of an elderly health promotion center after witnessing the effect of a social support program on dependent variables in older adults at a local community health center. Method: The subjects were 60 female adults over 65 years dwelling in a rural area, and they were divided into experimental and control groups each with 30 people. A social support program was implemented 6 hours a day, 3 times a week, for 4 months in the experimental group. Included was health assessment, health education, counseling, consultation, exercise, physical & occupational therapy, primary care, recreation, lunch & transfer service. Data was collected from May 1stto September 14th, 2002 by questionnaires, and analyzed by $x^2$-test, t-test, Pearson's correlation coefficient and stepwise multiple regression using SAS. Result: The social support program in the elderly was very effective on all dependent variables of physical health (t=4.68, p=.001), health knowledge (t=3.60, p=.001), life satisfaction (t=8.65, p=.001), and health promoting behaviors (t=5.23, p=.001). Conclusion: The Social Support Program at a Community Health Center was effective on health promoting behaviors in the elderly.
