Kinetic analysis of the elbow joint in human motion (인체운동에 있어서 주관절의 운동학적 분석)

  • Objectives : We find that the reaction force on the elbow joint during elbow flexion, extension with and without an object in the hand can be calculated the equations of motion that the sum of the torque and the sum of the force acting on the elbow joint must be zero and (moment of inertia x angular acceleration) and (mass x acceleration). Methods : we have calculated the equations of motion (${\Sigma}F=0$, ${\Sigma}{\tau}=0$, ${\Sigma}F=ma$, ${\Sigma}{\tau}=Ia$) to investigate the reaction force on the elbow joint during elbow flexion, extension by means of the simplified free-body technique for coplanar forces. Results : we found that the reaction force on the elbow joint during elbow flexion, extention as constant acceleration motion is more than constant velocity, static motion. Also, we found that the relation between during flexion and during extension like this ; $J_{flexion}$ < $J_{extension}$.

Assessment of discomfort in elbow motion from driver posture (운전자 자세에 따른 팔꿈치 동작의 불편도 평가)

  • The human arm is modeled by three rigid bodies(the upper arm, the forearm and the hand)with seven degree of freedom(three in the shoulder, two in the elbow and two in the wrist). The objective of this work is to present a method to determine the three-dimensional kinematics of the human elbow joint using a magnetic tracking device. Euler angle were used to determine the elbow flexion-extension, and the pronation-supination. The elbow motion for the various driving conditions is measured through the driving test using a simulator. Discomfort levels of elbow joint motions were obtained as discomfort functions, which were based on subjects' perceived discomfort level estimated by magnitude estimation. The results showed that the discomfort posture of elbow joint motions occurred in the driving motion.

A Study on the Reliability and Validity of Measuring the Range of Motion of the Elbow Joint Using a Smartphone Application

  • The purpose of this study was to assess the intra-tester and inter-tester reliability and validity of elbow flexion range of motion using a smart phone used by the general public. Thirty-one general people participated in this study. The range of motion in active elbow flexion was measured with a goniometer and smart phone over two times by two observers. The intra-taster and inter-tester reliability were evaluated using the intraclass correlation coefficient(ICC). The validity was measured by Pearson's correlation coefficient. The intra-observer reliability was good in all measured items ICC>0.900. The inter-observer reliability was high with ICC>0.806. All correlation coefficients of android app and goniometer were greater than 0.868 and showed a significant positive correlation (p<0.01). The range of motion measurement with a smart phone showed acceptable reliability. Therefore, using a smartphone to measure the range of motion of the elbow joint could provide convenience and economic benefits.

Total elbow arthroplasty for posttraumatic destroyed or unreduced elbow joint (외상 후 파괴된 주관절 또는 진구성 탈구에서의 주관절 전치환술)

  • Purpose: To evaluate the results in patients who received total elbow replacement for posttraumatic destroyed or unreduced elbow joint. Materials and Methods: Six patients with posttraumatic destroyed, or unreduced elbow joint, who were nearly impossible to move actively and had pain and grossly unstable joint, were followed up average 42 months. 3 cases were soft tissue injuries and bone defects which were caused by severe comminuted fracture, 1 was a nonunion with comminuted fracture, and 2 were unreduced elbow joint. Total elbow replacement was performed average 10 months after the injury. All the cases were used by semiconstrained prosthesis, and the results were estimated by Mayo elbow perfomance score. Results: Pain was decreased in all the cases postoperatively. Average ranges of motion were improved with active extension 20° and flexion 120°. Mayo elbow performance scores were pain 42.5 points, range of motion 17.5 points, stability 8.3 points, function 19.2 points and totally 87.5 points, and final results were 3 excellent and 3 good. Loosening of prosthesis was not found in all the cases by final follow-up radiograph. Conclusicon: Semiconstrained TER can be used as a effective treatment improving pain and active ranges of motion caused by posttraumatic destroyed or unreduced elbow joint, however, long term follow-up is needed because early loosening of TER can be occurred due to severe bone defects.

  • Background: Arm swing plays a role in gait by accommodating forward movement through trunk balance. This study evaluates the biomechanical characteristics of arm swing during gait. Methods: The study performed computational musculoskeletal modeling based on motion tracking in 15 participants without musculoskeletal or gait disorder. A three-dimensional (3D) motion tracking system using three Azure Kinect (Microsoft) modules was used to obtain information in the 3D location of shoulder and elbow joints. Computational modeling using AnyBody Modeling System was performed to calculate the joint moment and range of motion (ROM) during arm swing. Results: Mean ROM of the dominant elbow was 29.7°±10.2° and 14.2°±3.2° in flexion-extension and pronation-supination, respectively. Mean joint moment of the dominant elbow was 56.4±12.7 Nm, 25.6±5.2 Nm, and 19.8±4.6 Nm in flexion-extension, rotation, and abduction-adduction, respectively. Conclusions: The elbow bears the load created by gravity and muscle contracture in dynamic arm swing movement.

New Fixation Method Using Two Crossing Screws and Locking Plate for Cubitus Varus Deformity in Young Adult Elbow: Case Report

  • Many types of osteotomy have been proposed for the treatment of cubitus varus deformity of the elbow, and various methods for fixation of the osteotomy site have also been described. However, no method has been perfect. We treated two cases of cubitus varus elbow deformity with step-cut osteotomy using a new fixation method with two crossing screws and an anatomically designed locking plate. Active assisted elbow range of motion (ROM) exercise was permitted at postoperative 3 days, after removal of the drainage. Preoperative and postoperative humerus-elbow-wrist angles and ranges of motion of the two patients were compared. At 3 months followup, each patient had recovered the preoperative elbow ROM, and achieved the complete bony union of the osteotomy site and proper correction of the cubitus varus deformity. In addition, the appropriate remodeling of the lateral bony protrusion was observed. Therefore, we introduce a new fixation method for achievement of stable fixation allowing immediate postoperative elbow motion after corrective osteotomy for cubitus varus deformity in young adults.

Arthroscopic Treatment of Stiff Elbow (주관절 관절경을 이용한 구축의 치료)

  • Purpose: Limitation of motion of the elbow joint due to stiffness affect on life quality of the patients. So contracture of the elbow should be treated as soon as possible. Among the many treatment modalities, we described the result of arthroscopic treatment. Materials and Methods: From Mar. 2000 to Mar. 2003, 40 patients, who received the arthroscopic treatment by author for contracted elbow, were the subjects. We estimated the range of motion (ROM) of elbow joint before and after surgery by goniometer. The clinical result was evaluated by Severance elbow scoring system. The final ROM was evaluated at the point of no further increasement of joint motion. Male ware 30 cases, female ware 7 cases, average 42.6 years old and mean follow up period were 31 months. During arthroscopic treatment we had done release of the joint capsule or resection, synovectomy, removal of loose bodies. We used traditional portals. Results: The avarage preoperative ROM of elbow joint was 72.5 degree(range, 5 - 132 degree) and the increasement of ROM was totally 49.3 degree in flexion 26.5 degree and extension 22.8 degree. There was no other complication. Conclusion: Arthroscopic treatment for contracted elbow permit early joint ROM and it decrease the secondary injury to the elbow joint. Also there are few complications. It is thought to be a good treatment modality in contracted elbow joint.

A study of measurement on range of arm joint motion of Korean male in twenties (한국인 20대 청년의 팔 관절 동작범위 측정 연구)

  • This paper measured the range of arm joint motion for Korean 54 males in twenties. The range of the arm joint motion of the subjects was measured directly using Goniometer and protractor. The number of the static and the dynamic anthropometric variables are seven and thirteen, respectively. The anthropometric data are analyzed by basic statistical analysis (four group), correlation analysis and regression analysis using commercial SAS program. The results of analysis are compared with American students anthropometric data by Laubach(1978). Thin subjects have larger movement angle as wrist flexion, wrist abduction, elbow flexion, and elbow wupination and have smaller as wrist adduction and shoulder flexion. Fat subjects have larger movement angle as shoulder flexion and are smaller wrist abduction, elbow flexion, pronation, shoulder extension, shoulder adduction, shoulder abduction, and shoulder medial rotation Korean are more flexible than American in wrist and ranges of elbow flexion and elbow rotation. The shoulder movement is similar to that of American, but shoulder flexion is less flexible.

Debridement Arthroplasty for Post-Traumatic Stiff Elbow (후외상성 주관절 강직에서의 변연 관절 성형술)

  • Stiffness of the elbow joint is relatively common after trauma, ectopic ossification, bum, postoperative scar, and etc. Mild flexion deformity can be reduced by use of active or passive motion exercise, dynamic sling, hinged distractor device, or turnbuckle orthosis. But these methods have disadvantages of difficulty in gaining acceptable range of motion only with stretching exercise, re-contracture after conservative managements and poor results that flexion contracture remained. The common described operative exposures for treatment of the stiff elbow are anterior, lateral, posterior, and medial approach. Through Anterior, lateral and medial approach each has not access to all compartments of the elbow. But, posterior approach has benefits that access to posterior, medial and lateral aspects of the elbow and as needed, fenestration to the olecranon fossa that produces a communication between the anterior and posterior compartments of the elbow are possible. From June 1991 through April 1997, 11 patients who had posttraumatic stiff elbow, were treated with debridement arthroplasty through the posterior approach. The purpose of this study are to introduce technique of the debridement arthroplasty and to evaluate final outcomes. With regarding to preoperative pain degree, mild degree matches to 3 cases, moderate to 3 cases, and severe to 2 cases. In preoperative motion, flexion was average 85° and extension was 30°. Postoperatively nine patients had got the complete relief of pain and two patients continued to have mild pain intermittentely. Postoperative flexion improved to 127° and extension to 2°, so that elbow flexion had improved by an average of 42° and elbow extension by 28°. On the objective scale all patients had good or excellent results and they all felt that they were improved by operation. Debridement arthroplasty is one of excellent procedures for the intractable stiff elbow if it is not unstable or it has not incongrous. But it need a meticulous operative technique and a well-programmed rehabilitation.

Characteristics of the Muscular Activities on the Feedback Control of Elbow Orthosis Using Pneumatic Rubber Artificial Muscle (공압 고무 인공근육을 장착한 주관절 보조기 피드백 제어 시 근력 특성)

  • An elbow orthosis with a pneumatic rubber actuator has been developed to assist and enhance upper limbs movements and has been examined for the effectiveness. The effectiveness of the elbow orthosis was examined by comparing muscular activities during alternate dumbbell curl motion wearing and not wearing the orthosis. The subjects participated in the experiment were younger adults in their twenties. The subjects were instructed to perform dumbbell curl motion in a sitting position wearing and not wearing orthosis in turn and a dynamometer was used to measure elbow joint torque outputs in an isokinetic mode. Orthosis was controlled using contractile muscle force that is measured from force sensor through cDAQ-9172 board. The air pressure of the pneumatic actuator was 0.3MPa the most suitable air pressure. For the analysis of muscular activities, Electromyography of the subjects was measured during alternate dumbbell curl motion. The experiment results showed that the muscular activities wearing the elbow orthosis were reduced. With this, we confirmed the effectiveness of the developed elbow orthosis.
