Effect of Dietary Supplementation of DHA and AA on the Incorporation of Long Chain Fatty Acid and DNA Content in the Developing Brain of Rats (쥐에서 뇌발달 과정중에 식이에 첨가한 DHA와 AA가 뇌조직의 지방산조성과 DNA 함량에 미치는 영향)

  • The aim of this study was to observe whether the dietary supplementation of docosahexaenoic acid(DHA). In growing rats requires extra supplementation of arachidonic acid(AA) for brain development. Sprague-Dawley rats were divided into three groups, each fed a different diet. In the FO group, dams were fed a DHA-rich FO diet during pregnancy and lactation and pups were fed the same diet until 10 weeks old. In the AO group dams and pups were similarly fed a FO diet after weaning. DHA and AA were most effetively deposited in the developing brain during pregnancy and lactation in rats. However, FO-W pups showed significantly lower level of DHA at 0-3 weeks compared with the FO and AO groups and than slowly increased DHA levels to about 87% of other groups at 10 weeks with the introduction of the FO diet after weaning. The total amount of DNA in whole brain rapidly reached a maximum level at 3 weeks and then was sustained at a constant level after 5 weeks of age. The DNA content was positively correlated with DHA level but not with AA level in the developing brain. DNA content was significantly lower in the FO-W group compared to the FO and AO group at 3 weeks of age. However, the DNA content of brain in FO-W pups increased to 80% of the FO group level at 10 weeks after feeding the FO diet after weaning. The relative percentage of AA in brain lipids was significantly reduced in the early stage of brain development when only DHA was supplemented. However, DHA supplementation had no significant effect on the incorporation of AA when the approximately 35% of LA in the FO diet was substituted by preformed AA. These results suggest that large quantities of DHA could interfere with the normal conversion of LA to AA if LA is not supplemented enough together with DHA. Therefore, high DHA supplementation may require preformed AA in the diet even though AA has no significant correlation with the DNA content in brain. DHA supplementation after weaning also improved the incorporation of DHA into brain and content of DNA even though brain development was almost completed, suggesting that a low level of DHA supplementation without AA addition might be necessary to improve brain development during infancy as well as during pregnancy and lactation.

  • Objective: To evaluate the effectiveness of GnRH antagonist multiple dose protocol applied during early and late follicular phase (MDP-EL) in comparison with standard GnRH agonist luteal long protocol (LP) in each non-obese and obese polycystic ovary syndrome (PCOS) women undergoing IVF. Methods: Two hundred eleven infertile women with PCOS were recruited and randomized to undergo either GnRH antagonist MDP-EL (antagonist group) or standard GnRH agonist luteal LP (agonist group). IVF cycle outcomes were compared between the two groups. Results: Total dose and days of recombinant human follicle stimulating hormone (rhFSH) administered were significantly fewer in the antagonist group than in the agonist group. Incidence of severe ovarian hyperstimulation syndrome was significantly lower in the antagonist group. However, IVF and pregnancy outcomes were similar in the two groups. When all subjects were divided into non-obese and obese subgroups, in non-obese PCOS subgroup, IVF and pregnancy outcomes were comparable in the antagonist and agonist groups but total dose and days of rhFSH were also significantly fewer in the antagonist group. Similar findings were also observed in obese PCOS subgroup. Conclusion: GnRH antagonist MDP-EL is at least as effective as GnRH agonist LP and may be a more patient-friendly alternative in controlled ovarian stimulation for PCOS patients undergoing IVF, independent of body mass index.

Psychosocial Risk Factors of Postpartum Depression (산후우울증의 심리사회적 위험요인)

  • Objective : Postpartum depression(PPD) was known to be caused by many factors including various psychosocial risk factors. This study was performed to identify the psychosocial risk factors for ppd, preliminarily in Korea. Methods : A group of 119 postpartum women, each of whom was at 6 to 8 weeks after delivery was identified at the time when they visited to the child health clinic or postnatal check-up clinic. The risk factors were surveyed by the self-reported questionnaire. The items of questionnaire were consisted of known risk factors in other studies and other possible stress-related factors. PPD was assessed by the Edinburgh Postnatal Depression Scale(EPDS) and the degree of postpartum depression was determined by its score. Results : 16 women(13.45%) in the high risk group were diagnosed as PPD among the 119 women. Risk factors including past experience of depressive symptoms and low level of marital satisfaction were founded more frequently in women in the high risk group than in the low risk group. The score of EPDS was significantly high in the group who experienced depressive symptoms in the past, anxiety or depression during pregnancy, stressful life event during the period of recent pregnancy and postpartum, and who had low level of marital satisfaction. There was a positive correlation between age and the score of EPDS. However, the postpartum depressive symptoms were not influenced by the level of education, job, retirement due to pregnancy and delivery, wanted or unwanted pregnancy, delivery method, feeding method, the hospitalization of infant, expected and real gender of infant. Conclusion : These results suggest that PPD is quite frequent at postpartum period. Various risk factors contribute to the development of PPD. If clinicians pay attention to the risk factors of PPD and give appropriate psychiatric intervention to the mothers during pregnancy and postpartum, it will be easy for the clinicians to recognize and treat PPD in the early stage.

  • This study was performed to determine whether the length of the feeding of the controlled experimental diet to the dam resulted in changes to the dam milk or pup brain fatty acid composition. As a first method, females have been obtained at 3 weeks of age and fed the experimental diet throughout their growth to adulthood including mating, pregnancy, and lactational periods. As a second method, in order to shorten this long and expensive process, time-pregnant dams were obtained as early as possible from a commercial supplier, on day 3 of gestation, and immediately switched to the experimental diet. At birth, the milk of dams prepared by these two different methods was compared by collecting the stomach contents of the pups. This showed a slight increase in docosahexaenoic acid (DHA) and arachidonic acid (AA) in the pup stomach contents from the time-pregnant dams. There were no significant changes in the brain fatty acid composition of pups between the two different lengths of the experimental diet intake. By the 10 days of age, there were only minor differences in the milk fatty acid composition of pup stomach contents from the two sets of dams. However, the pup brains of the time-pregnant groups at 10 days showed increased AA and DHA due to intake of the chow diet including AA, DHA and eicosapentaenoic acid (EPA). Thus, the history of the maternal feeding could affect the results under these particular circumstances, but the differences were minimal.

  • With advances in the methods of cancer treatment used in modern medicine, the number of breast cancer survivors has been consistently rising. As the number of women who wish to become pregnant after being diagnosed with breast cancer increases, it is necessary to consider fertility preservation in these patients. However, medical doctors may be unaware of the importance of fertility preservation among cancer patients because most patients do not share their concerns about fertility with their doctors. Considering the time spent choosing and undergoing treatment, an early referral to a reproductive specialist is the best way to prevent a delay in cancer treatment. Since it is not easy to make decisions on matters related to cancer diagnosis and fertility, patients should be provided with enough time for decision-making, and to allow for this, an early referral will provide patients with sufficient time to choose an appropriate method of fertility preservation. The currently available options of fertility preservation for patients with breast cancer include cryopreservation of embryos, oocytes, and ovarian tissue and gonadotropin-releasing hormone agonist treatment before and during chemotherapy. An appropriate method of fertility preservation must be selected through consultations between individual patients and health professionals and analyses of the pros and cons of different options.

AN EXPERIMENTAL STUDY OF THE EFFECT OF THE METHYLCHLOROTETRACYCLINE ON GROWTH OF CORONAL SUTURE IN RATS (Demethylchlorotetracycline이 백서의 관상봉합 발육에 미치는 영향에 관한 실험적 연구)

  • To study some informations on the morphogenesis and developmental process of the coronal suture in rats, the author performed daily oral administrations of demethylchlorotetracycline, a kind of tetracycline group, in the amount of 30mg/kg of body weight to the female rats from the 7th day of pregnancy to the time of delivery. Microscopic evaluation was undertaken on the fetal rats in the experimental group. The subject of this experiment were defined to the fetal rats of each group at 1st, 3rd, 7th and 14th day after birth. All these fetal rats were sacrificed and the heads were removed. All the tissue sections were fixed with $10\%$ formalin, Bouin' and Carnoy' solution and then stained by Hematoxylin-Van Gieson stain, or Feulgen and Rossenbeck, Periodic acid Schiff, and prepared for alcian blue reaction. The results were as follows; 1. The directions of osteogenic fibers were arranged irregulary during first 3 days, but after the 7th day they tended to change radial directions like control group. 2. The density of deep stained cells by Feulgen-Rossenbeck reaction were shown leu in the experimental group than that in the control group in first 3 days, but there was shown no significant difference between both groups after the 7th day. 3. PAS reaction in early stage was generally negative in the experimental group unlike as in the control group, but diffuse reaction was observed in the loose middle zone like as in the control group after 14th day. 4. Alcian blue reaction was negative in cambial zone, and slightly positive in uniting zone compared with control group in early stage. After 14th day, however, there was observed a tendency of moderately positive reaction.

Physical Changes in and Coping with Marriage by Immigrant Women at an Early Stage of Immigration (이주초기에 나타나는 결혼 이주여성의 신체변화와 대처)

  • Purpose: To provide an in-depth analysis of the physical changes in and marital experiences of immigrant women in Korea, considering the differences in their cultural backgrounds. Methods: A qualitative research methodology with a phenomenology perspective was used. Data were collected through interviews from four focus groups and through in-depth interviews from five individuals. Data analysis was carried out using Colaizzi's phenomenological analysis method. Results: Twenty-four participants from nine different nations were interviewed. Three phenomenological theme clusters were identified and six sub-themes were derived. These comprise: "emergence of physical changes", "experienced symptom with negative result", and "coping with my body". The derived themes comprise: "struggling for my body to survive", "changed body after pregnancy and delivery", "diagnosed as normal but", "neglected my health", "using familiar care", and "unfamiliar health service system". Conclusion: Immigrant women by marriage in Korea are new subjects of nursing care. Their physical changes and experiences in coping with marriage at an early stage of immigration as described by themselves provide valuable information for nursing professionals. Cultural differences, problems specific to women, and our social conditions regarding minorities and our patriarchal tradition that discriminates against women affect their health problems. We strongly recommend that nurses should actively determine and engage in the health problems of immigrant women.

The Effects of Irradiation and Calcium-deficient Diet on the Expression of Bone Morphogenetic Protein-2/4 during Early Tooth Development (치아발육시 방사선조사와 칼슘결핍이 골형성단백질-2/4의 분포에 미치는 영향에 관한 여구)

  • Purpose: To investigate the expression of bone morphogenetic protein (BMP)-2/4 during eary tooth development after irradiation and calcium-deficient diet. Materials and Methods: The pregnant three-week-old Sprague-Dawley rats were used for the study. The control group was non-irradiation/normal diet group (Group 1), and the experimental groups were irradiation/normal diet group (Group 2) and irradiation/calcium-deficient diet group (Group 3). The abdomen of the rats at the 9th day of pregnancy were irradiated with single dose of 350 cGy. The rat pups were sacrificed at embryonic 18 days, 3 days and 14 days after delivery and the maxillae tooth germs were taken. The tissue sections of specimen were stained immunohisto-chemically with anti-BMP-2/4 antibody. Results: At embryo-18 days, immunoreacivity for BMP-2/4 of the Group 1 was modetate in stratum intermedium of dental organ and weak in dental papilla and dental follicle, but that of Group 2 was weak in cell layer of dental organ, and no immunoreacivity was shown in dental papilla and dental follice of Group 2 and in all tissue components of the Group 3. At postnatal-3 days, immunoreacivity for BMP-2/4 of the Group 1 was strong in cell layer of dental organ, odontoblasts and developing alveolar bone, but that of Group of 2 and Group 3 was weak in odontoblasts and developing alveolar bone. At postnatal-14 days, immunoreacivity for BMP-2/4 of the Group 1 was strong in newly formed cementum, alveolar bone and odontoblasts, but that of Group 2 was weaker than that of Group 1. In the Group 3, tooth forming cell layer showed weak immunoreactivity, but other cell layers showed no immunoreactivity. Couclusion : The expression of bone morphogenetic protein (BMP)-2/4 during early tooth development was disturbed after irradiation and calcium-deficient diet.

Are Women in Kuwait Aware of Breast Cancer and Its Diagnostic Procedures?

  • The aim of this study was to examine the knowledge and awareness of women in Kuwait with regard to risk factors, symptoms and diagnostic procedures of breast cancer. A total of 521 questionnaires were distributed among women in Kuwait. Results showed that 72% of respondents linked breast cancer factors to family history, while 69.7% scored abnormal breast enlargement as the most detectable symptom of the disease. Some 84% of participants had heard about self-examination, but knowledge about mammograms was limited to 48.6% and only 22.2% were familiar with diagnostic procedures. Some 22.9% of respondents identified the age over 40 years as the reasonable age to start mammogram screening. Risk factor awareness was independent on age groups (p>0.05), but both high education and family history increased the likelihood of postivie answers; the majority knew about a few factors such as aging, pregnancy after age 30, breast feeding for short time, menopause after age of 50, early puberty, and poor personal hygiene. In conclusion, 43.1% of participants had an overall good knowledge of breast cancer with regards to symptoms, risk factors and breast examination. Very highly significant associations (p<0.005) were evident for all groups except for respondents distributed by nationality (p=0.444). Early campaigns for screening the breast should be recommended to eliminate the confusion of wrong perceptions about malignant mammary disease.

Neutrophil Functions and Cytokines Expression Profile in Buffaloes with Impending Postpartum Reproductive Disorders

  • The study was conducted to correlate the periparturient immune status in terms of neutrophil functions and cytokine expression in peripheral blood mononuclear cell culture with impending postpartum reproductive disorders in buffaloes. Forty pregnant buffaloes were observed for occurrence of postpartum reproductive disorders (PRD), i.e., metritis, endometritis and delayed uterine involution etc., during one week prepartum to four weeks postpartum period. A representative number (n = 6) of buffaloes that did not develop any PRD were included in group I (healthy, control), while the animals which experienced PRD were assigned into group II (PRD, n = 8). The blood samples were collected at weekly interval from one week prepartum to four weeks postpartum period considering the day of calving as 'd 0'. Differential leucocytes counts, superoxide and hydrogen peroxide production activity in isolated neutrophils and the mRNA expression profile of cytokines i.e., IL-2, IL-4 and IFN-${\gamma}$ in PBMC culture were studied in all the samples. A higher total leucocytes, neutrophil and band cells count along with impaired neutrophil functions i.e., lowered level of production of superoxide and hydrogen peroxide before parturition and during early postpartum period were observed in buffaloes developing PRD. Further, a lower expression of IL-2, IFN-${\gamma}$ and IL-4 mRNA in PBMC culture was observed at calving in buffaloes that subsequently developed PRD at later postpartum. Thus, suppression in neutrophil function and cytokine expression at prepartum to early postpartum period predisposes the buffaloes to develop postpartum reproductive disorders. Hence, monitoring of neutrophils function and cytokine expression profile would be effective to predict certain reproductive disorders at late pregnancy or immediately after parturition in buffaloes. In future, this may be a novel approach for determining suitable management and therapeutic decisions for prevention of commonly occurring reproductive disorders in farm animals.
