Does blastomere biopsy in preimplantation genetic diagnosis affect early serum ${\beta}$-hCG levels?

  • Objective: To determine whether the serum ${\beta}$-human chorionic gonadotropin (hCG) profile following preimplantation genetic diagnosis (PGD) is lower than that of intracytoplasmic sperm injection (ICSI) cycles. Methods: A total of 129 PGD cycles and 1,161 age-matched ICSI cycles, which resulted in pregnancy (serum ${\beta}-hCG{\geq}5$ mIU/mL) on post-ovulation day (POD) 12 were included. We compared the mean serum ${\beta}$-hCG levels on POD 12, 14, 21, and 28, doubling time of serum hCG, and created a cut-off value for predicting a singleton pregnancy in each group. Results: The mean serum ${\beta}$-hCG concentration of the PGD group was significantly lower than that of the control group on POD 12, 14, and 21. The doubling time of serum ${\beta}$-hCG at each time interval showed no significant difference. The cut-off-value of serum ${\beta}$-hCG for predicting a single viable pregnancy was 32.5 mIU/mL on POD 12 and 113.5 mIU/mL on POD 14 for the PGD group, which was lower than that for the control group. Conclusion: Blastomere biopsy may decrease the ${\beta}$-hCG-producing activity of the trophoblasts, especially in early pregnancy. Setting a lower cut-off value of serum ${\beta}$-hCG for predicting pregnancy outcomes in PGD may be needed.

  • Partheno Embryo's research is known to play a very important role in identifying the development of embryonic cells or analyzing the genetic mechanisms of embryonic development, but the information on apoptosis formed during the early stage of development on Partheno Embryo is very little. Therefore, this study analyzed whether the embryonic cell death of unit embryos can be inhibited by adding Scriptaid, one of HDACi, which plays a role in demethylation of histone proteins as a method of regulating the cell cycle in the early embryo development of Partheno Embryo. As a result, the differentiation rate was higher in the group that added Scriptaid and FBS, but the cellular development was higher in the group that added pregnant serum to Scriptaid. As a result of analyzing the expression of the gene through IF and PCR, the group with the addition of gestational serum increased the expression of BCL2 and PCNA, which affects the anti-Casp3 action in cell survival. In addition, it is interpreted that treatment of Scriptaid for 16 hours, rather than 24 h treatment lowers the expression of Casp-3, a representative factor of apoptosis, and also increases embryonic development, thus affecting early embryo development. Therefore, it is concluded that the 16-hour treatment of Scriptaid and the use of gestational serum will inhibit cell death in the early embryonic development and increase the development rate of the embryo.

Evaluation of Pregnancy and Thyroid Function (임신과 갑상선 기능의 평가)

  • During early pregnancy, before the development of a functioning thyroid gland, thyroid stimulating hormone (TSH) is a very sensitive marker of thyroid dysfunction during pregnancy. Normal values have been modified during gestation with a downward shift. The fetus is influenced by the TSH supplied by the mother. TSH and free thyroxine (FT4) concentrations vary during pregnancy and conventional units can vary between laboratories. A downward shift of the TSH reference range occurs during pregnancy, with a decrease in both the lower and upper limits of maternal TSH, relative to the typical non-pregnant TSH reference range. Each laboratory produces its own reference TSH and FT4 concentrations because there are many different assays that yield different results in pregnancy. Therefore, automated immunoassays used for serum FT4 analysis are still used widely, but the important considerations discussed above must be noted. The use of population-based, trimester-specific reference ranges remains the best way to handle this issue The slight downward shift in the upper reference range of TSH occurring in the latter first trimester (7~12 weeks) of pregnancy, typically not observed prior to 7 weeks. Their use indicates high or low levels in a quantitative manner independent of the reference ranges. These data highlight the importance of calculating population-based pregnancy-specific thyroid parameter reference intervals. A precision medicine initiative in this area will require the collection and analysis of a large number of genetic, biological, psychosocial, and environmental variables in large cohorts of individuals. Large prospective randomized controlled trials will be needed to resolve these controversies.

In Vitro Effects of Female Sex Hormones on Collagenase Activity of Gingival Fibroblast and Periodontal Ligament Fibroblast (여성 호르몬의 변화가 치은 섬유아세포와 치주인대세포의 교원질 분해 효소의 활성에 미치는 영향)

  • Many factors may affect periodontal changes during the physiologic conditions of woman(e.g. puberty, menstrual cycle, pregnancy, menopause). Recently many research has focused on the immunological changes of host, but the exact mechanism is not clear. Collagen is a major constituent of periodontium, and collagenase specifically digests the collagen and plays a role in destruction of periodontal tissue. So, I suppose that it participates with the cytokines in the inflammation of gingiva and vascular response during the changes of female sex hormones. Because there are some evidences of the existence of the receptors of estrogen and progesterone in the gingiva, it may be a target tissue of female sex hormones. In this experiment, gingival fibroblast and periodontal ligament cell were cultured in the presence of various concentrations of estrogen or progesterone corresponding to the menstrual cycle and pregnancy. Collagenase activity of the supernatant of culture media was determined by Spectrophotometric collagenase assay. The enzyme activity was calculated by the % decrease of the coated collagen. 1. The estrogen at both concentrations had no effect on the activity of collagenase of the gingival fibroblast. 2. The progesterone had some effect on the collagenase activity of the gingival fibroblast at low and high concentration of menstrual cycle, and elevated the enzyme activity at all range of pregnancy concentrations. 3. In periodontal ligament cells, estrogen elevated the enzyme activity at the early pregnancy concentration and progesterone elevated at the concentration just before menstruation. In this experiment, pregesterone elevated the collagenase activity of gingival fibroblast and periodontal ligament cells. But the mechanism of the up-regulation of the enzyme activity was not confirmed. The more experiments of direct effect of progesterone on gingival at the molecular level(e.g. northern blot analysis) can reveal the exact mechanism.

Immune Cells in the Female Reproductive Tract

  • The female reproductive tract has two main functions: protection against microbial challenge and maintenance of pregnancy to term. The upper reproductive tract comprises the fallopian tubes and the uterus, including the endocervix, and the lower tract consists of the ectocervix and the vagina. Immune cells residing in the reproductive tract play contradictory roles: they maintain immunity against vaginal pathogens in the lower tract and establish immune tolerance for sperm and an embryo/fetus in the upper tract. The immune system is significantly influenced by sex steroid hormones, although leukocytes in the reproductive tract lack receptors for estrogen and progesterone. The leukocytes in the reproductive tract are distributed in either an aggregated or a dispersed form in the epithelial layer, lamina propria, and stroma. Even though immune cells are differentially distributed in each organ of the reproductive tract, the predominant immune cells are T cells, macrophages/dendritic cells, natural killer (NK) cells, neutrophils, and mast cells. B cells are rare in the female reproductive tract. NK cells in the endometrium significantly expand in the late secretory phase and further increase their number during early pregnancy. It is evident that NK cells and regulatory T (Treg) cells are extremely important in decidual angiogenesis, trophoblast migration, and immune tolerance during pregnancy. Dysregulation of endometrial/decidual immune cells is strongly related to infertility, miscarriage, and other obstetric complications. Understanding the immune system of the female reproductive tract will significantly contribute to women's health and to success in pregnancy.

  • The present investigation has been undertaken to understand the mechanism of implantation process, by demonstrating the role of ovarian steroids in connection with phosphatase activity in the differentiation of uterine endometrium for implantation. The results obtained are as followings: The differentiation of the uterine endometrial tissue was closely influenced by the ovarian steroid hormones; at first, 17${\beta}$-estradiol initiated the differentiation of the uterine luminal and glandular epithelial cells, and then progesterone induced differentiation of stromal cells, and thereby two steroids maintain decidualization of the uterine tissues. We observed that the phosphatase activities seem to be dependent upon the ovarian steroids; that is the activity showed higher level in progesterone treated group than in estradiol treated one, and the highest activity was found in the group treated with both estradiol and progesterone. Acid phosphatase showed the highest activity whereas alkaline phosphatase showed the lowest in the rat uterine endometrium during early pregnancy.

Survey of Maternity wear Purchasing Status in Pregnant Women (임산부의 임부복 구매 실태)

  • The purpose of this study is to most appropriately develop maternity wear according to the preferences of specific age groups of pregnant women. The specific age groups were women in their twenties, early thirties, and late thirties. This survey was conducted in the metropolitan area around Seoul, South Korea through a questionnaire given to 150 pregnant women. The study found that pregnant women purchased significantly cheaper clothes after their pregnancy than before. The participants responded that they mostly bought clothes using the Internet, but they also reported many instances of returning the clothes due to fit and sizing. It was shown that women in their twenties tended to consider the price, design, and activity more than women in their thirties when purchasing maternity wear. The study showed that pregnant women tended to wear maternity wear from early in the pregnancy-preferring leggings, one-piece dresses, pants, shirts, briefs, and bras with the girdle on the outside. More women in their late thirties purchased maternity support belts than younger age groups. In any case, in purchasing maternity wear, clothing features that the respondents focused on were comfort, functionality, fit, the covering of their body changes, and fashion. Still, many respondents 35 years and older reported more interest in safety rather than fashion in comparison to the responses of respondents under the age of 35.

  • A study was conducted to improve the reproductive performance of Korean native cows. Plasma progesterone concentrations were determined by radioimmunoassay during the estrous cycle and early pregnancy. Plasma progesterone concentrations during the estrous cycle in the 15 cows were the lowest (0.30ng/ml) at estrus, gradually increased after 6 days, remained high from 8 to 17 days (3.87~5.59ng/ml) and drastically decreased thereafter to reach minimal concentrations at the next estrus. Plasma progesterone concentrations of the 10 pregnant cows at 20 days after insemination were $5.82{\pm}1.33ng/ml$ (mean${\pm}$SD) and those of 10 non-pregnant cows were $0.30{\pm}0.21ng/ml$, respectively. Therefore, plasma progesterone concentrations in the pregnant cows at 20 days after insemination were significantly higher than in non-pregnant cows (p<0.001).
