Surgical Treatment of Cor Triatriatum (삼방심의 외과적 치료 -수술치험 24례-)

  • Cor triatriatum is a relatively rare cardiac anomaly, whose major feature is a fenestrated membrane separating an upper common pulmonary venous chamber from a lower true left atrial cavity. Interatrial communications may be present between the right atrium and the common pulmonary venous chamber or the true left atrium. From April 1981 to April 1992, 24 patients with cor triatriatum were treated at Seoul National University Hospital. Ages ranged from 1 month to 24 years with mean of 7.4 years. Twenty patients had interatrial communications through a patent foramen ovale, primum or secundum defect of the atrial septum. Four had no interatrial communications. Fourteen patients had associated anomalies; partial anomalous pulmonary venous connection in 3, total anomalous pulmonary venous connection in 2, persistent left superior vena cava in 3, and other anomalies in 6 patients. Surgical corrections were performed through right atriotomy in 18 patients, left atriotomy in 4, and both atriotomy in 2. Three patients [12.5%] died early after operation; two of them were associated with single ventricle. Six out of 21 survivors [28.6%] experienced complications; recurrent pneumonia, pulmonary embolism, ischemic encephalopathy, diaphragmatic palsy and tachyarrhythmias. At the time of follow up, all survivors, except one, were in functional class I. Surgical correction of cor triatriatum restored normal hemodynamic status with relatively low operative mortality, especially in patients not complicated with severe anomalies. This report summarizes the clinical diagnosis, associated anomalies, interatrial communications, surgical approach and late result of 24 patients underwent surgical corrections in our hospital.

Clinical Course of Bipolar Disorder in Children and Adolescents (소아청소년 양극성 장애의 임상 경과)

  • Objectives : The early onset of mood symptoms in bipolar disorder has been associated with poor outcomes in many studies. However, aspects of the clinical course of bipolar disorder in children and adolescents are controversial. The goal of this article is to review the clinical characteristics and longitudinal course of children and adolescents with bipolar disorders. Methods : Searches were conducted in MedLine, PsycINFO, KISS, and RISS using the terms phenomenology, clinical course, outcome, BPD, pediatric, children and adolescents. Twenty-one reports were selected : either original articles reporting symptoms and clinical characteristics of subjects (ages 5-18 years), or published articles in reviewed journals about bipolar disorder in children and adolescents. Results : Approximately 70% of subjects with bipolar disorder recovered from their index episode, and 50% had at least 1 syndromal recurrence, particularly depressive episodes. For 60% of the follow-up time, subjects had syndromal or subsyndromal symptoms with numerous changes in symptoms and shifts of polarity. Approximately 20% of BP-II subjects converted BP-I. Conclusion : Bipolar disorders in children and adolescents are characterized by episodic illness with subsyndromal and syndromal episodes with mainly depressive and mixed symptoms and rapid mood changes. Extensive follow-up time is needed to evaluate the continuity of bipolar disorder symptoms from childhood to adulthood.

Factors Influencing the Level of Diabetic Retinopathy in Patients with Type 2 Diabetes Mellitus (제2형 당뇨병 환자의 당뇨병성망막증 정도에 영향을 미치는 요인)

  • Purpose: This study was a descriptive survey research to identify whether characteristics of patients with type 2 diabetes mellitus, their knowledge about diabetes, and self-care behavior impacted on the level of diabetic retinopathy. Methods: Participants were 133 patients who had type 2 diabetes mellitus and were being seen at a hospital in Korea. The scale for knowledge about diabetes had 24 items, the scale for self-care behavior, 20 items, and the level of diabetic retinopathy was classified according to the international clinical diabetic retinopathy severity measurement standards. Results: The influence of the independent variables on the level of diabetic retinopathy showed that age, job, time since onset of Diabetes Mellitus, regular ophthalmologic examinations, and systolic blood pressure were identified as factors affecting the level of diabetic retinopathy. The explanation power of this regression model was 23.0% and it was statistically significant (F=5.42, p<.001). Conclusion: Early education about occurrence of diabetes related diseases, specifically diabetic retinopathy should be provided for patients from younger ages. Moreover, for disease management, social support is needed from co-workers and friends. Efforts to encouraged prevention and delay of diabetic retinopathy should include control of blood sugar and blood pressure.

Skeletal Manifestations of Mucolipidosis II/III

  • Mucolipidosis (ML) is a kind of skeletal dysplasia. Characteristic X-ray findings of the bone may contribute to the early diagnosis and treatment of ML II/III. Skeletal radiographs show distinctive patterns at different ages: neonatal hyperparathyroidism, osteodystrophy (similar to chronic osteitis fibrosa cystica), and dysostosis multiplex. Patients with ML II/III show a mixture of osteodystrophic bone changes and atypical changes of dysostosis multiplex: proximal pointing of the metacarpals in the wrist, dysplastic changes in the lower third of the ilia, marked broadening of the ribs becoming oar-shaped, and beaking of the lower thoracic and lumbar vertebrae. In ML II, the osteodystrophy has clinical and radiographic features of neonatal hyperparathyroidism. In some neonatal subjects, chemical hyperparathyroidism is also demonstrated. After transient hyperparathyroidism in newborns, the progressive osteitis fibrosa cystica develops from 3-6 months of age. Patients with ML III show prominent skeletal involvement, particularly the destruction of vertebral bodies and the femoral heads. Intravenous pamidronate treatment is well tolerated, and it can produce clinical effects, with a reduction in bone pain and improvements in mobility in patients with ML III. In this review, the skeletal manifestations of ML II and III are investigated.

The effect of Art Therapy apply to ADHD child's problem behavior and child relationship (미술치료가 ADHD 아동의 문제행동과 또래관계에 미치는 효과)

  • This study shows that Art therapy has positive effects on ADHD child's problem behavior and child relationship. Also we want to provide more data to education field. We work with 3 children. They are recommended by their class teacher and they are attending J elementary school which is located in K city in Chunbuk. The experiment period was 7 weeks - 2 times a week, total is 14th- and each time required was 40-60 minutes when they finish their school. Since we want to see each ADHD child's results, we use ADHD teacher evaluating measure and Child relationship measure to compare the results after the fact. The results are Art therapy has positive effects on the ADHD child's problem behavior specially concentration, excess behavior, social skill, hostile behavior and aggregation with same ages, sociality, lack of sociality, exclusive, unstable, and hypersensitiveness.

The Prevalence of Obesity in Children and Adolescents (서울지역의 학동기 소아 및 청소년의 비만증 이환율 조사)

  • The prevalence of childhood and adolescents obesity at ages 6 to 17 in Seoul were investigated on 1992. The number of children and adolescents comprised of 3310 boys and 3270 girls. Obesity was defined as weight that exceeded the standard weighter for heigher and sex by more than 20%(relative weight>120%) The results were as follows : 1) The overall prevalence rate of childhood and adolescent obesity was 14.45% The prevalence rate of boys was 15.83% and that of girls was 13.06% on 1992. The overall difference of prevalence rate between boys and girls in significant(p<0.001) 2) The highest peak age is 11 years old. There were two peaks in the prevalence rate of obesity one is from 9 to 11 years old. the other is from 15 to 17 years old. 3) The prevalence rate of overweight that exceeded the standard weight by more than 20% to 29% was 8.62% by more than 30% to 49% was 5.58% and by more than 50% was 0.25% Our study suggests than the prevalence rate of obesity among the children and adolescent in Seoul, Korea is gradually increasing which will affect the future adult population. We feel it is needed further investigation to determine the causes of increasing rate of obesity and early establishment of preventive programs.

Analysis on the Middle School Teachers' Level of Interest of Achievement Standards-based Assessment (성취평가제에 대한 중학교 교사의 관심수준 분석)

  • The purpose of this study is to analyze the level of interest in the achievement evaluation system, under the assumption that the teacher is an important element of curriculum. For this study, surveys targeting the 150 middle school teachers from 6 schools in Pusan was conducted. A questionnaire based CBAM model was used as research tools. For data analysis, mean of relative strength by teacher's level of interest was calculated. And MANOVA was used to investigate the difference among level of interest according to teachers' background such as sex, age, career, job position, and the grade they taught. The results of the analysis are summarized as follows. First, the middle school teachers' overall interest for achievement evaluation system is low. Teachers showed a typical pattern appeared in the early stages of change. They have little interest in the achievement evaluation system. And they should try to fix and change the achievement evaluation system rather than cooperate it. Second, as a result of this analysis of the difference according to teachers' background, no significant differences were found in most of the variables. Significant differences were found only in age, career, and job position from step 0. In running the post hoc, teachers who have less careers, lower positions, and younger ages had less interest in achievement evaluation system.

A Study on the Development of Patterns for Pants for Men in their Early 20s and Evaluation of Subjective Wear Sensation (20대 초반의 남성을 위한 다양한 팬츠 패턴 제안 및 착용평가)

  • This investigates the type of pants preferred by Korean men in their 20s through a subjective wear test. Six types of men's pants were developed as experimental garments. The male subjects were university students between the ages of 20 and 24. Subjective wear sensations of the experimental pants were rated using seven Likert scales. A 7-point Likert-type scale was used for the evaluation, with 7 indicating the best fit. As a result, a basic men's sloper for lower body clothing was developed based on SizeKorea 2004 anthropometric measurements surveyed between 2003 and 2004. A basic pattern (A) was manipulated to tight-fit pants (E) and loose-fit pants (D, L, M, and N) by considering the ease at hip level and the style. Among the six pants, pants D and L (4cm ease at hip level) were found to be superior to others in terms of comfort, fitting, and crotch depth. With regard to ease and comfort of the crotch part, pants E and A (0cm ease at hip level) were found to be the least preferred.

A Study on Preschoolers' Intelligent Ability, Reward Choice in Assumed Situation and Delay of Gratification Ability (유아의 지적능력, 가상상황에서의 보상선택유형 및 만족지연능력에 관한 연구)

  • This study has been performed to identify intelligent ability, reward choice in assumed situation of delay of gratification, and delay of gratification ability. The subjects for this study were 100 preschoolers between the ages of 4 and 5, their mothers, and 15 teachers of three day-care centers in Seoul. T-test, F-test, Correlation analysis and multiple regression analysis were used for data analysis. The main results of this study were as follows: First, preschoolers' delay of gratification ability by mothers' educational background was significant and delay of gratification ability by sex was significant. This means that mothers who had a higher educational background were positively related to preschoolers' delay of gratification ability. Second, in an assumed situation of delay of gratification, preschoolers' delay of gratification ability by reward choice was not significant. Third, delay of gratification by intelligent ability was significant. Fourth, the correlation among intelligent ability, reward choice in assumed situation of delay of gratification and delay of gratification ability were significant. Finally, preschoolers' delay of gratification ability was significantly influenced by two factors: reward choice in assumed situation of delay of gratification and preschoolers' intelligent ability.

Nano-Silica effect on the physicomechanical properties of geopolymer composites

  • Addition of nano-$SiO_2$ (NS) to geopolymer composites has been studied through measurement of compressive strengths, FTIR and XRD analysis. Alumino-silicate materials are coarse aggregate included waste concrete and demolished walls with its cementing binder, cement kiln dust (CKD) used and can possess a pronouncing activation for the geopolymer reaction resulting from the high alkali contents within. Materials prepared at water/binder ratios in a range of 0.30: 0.40 under curing of $40^{\circ}C$ and 100% Relative Humidity (R.H.), while the used activator is sodium hydroxide in the ratio of 2 wt. %. First, CKD is added in the ratio from 10 up to 50 wt., %, and the demolished walls was varied depending on the used CKD content, while using constant ratio of waste concrete (40 wt., %). Second step, depending on the optimum CKD ratio resulted from the first one (40 wt. %), so the control geopolymer mix composed of cement kiln dust, demolished walls and waste concrete in the ratio (40:20:40, wt %). Nano-silica partially replaced waste concrete by 1 up to 8%. Results indicated that, compressive strengths of geopolymer mixes incorporating nano-silica were obviously higher than those control one, especially at early ages and specially with 3%NS.
