Biomarkers for Evaluating the Inflammation Status in Patients with Cancer

  • Inflammation can be a causative factor for carcinogenesis or can result from a consequence of cancer progression. Moreover, cancer therapeutic interventions can also induce an inflammatory response. Various inflammatory parameters are used to assess the inflammatory status during cancer treatment. It is important to select the most optimal biomarker among these parameters. Additionally, suitable biomarkers must be examined if there are no known parameters. We briefly reviewed the published literature for the use of inflammatory parameters in the treatment of patients with cancer. Most studies on inflammation evaluated the correlation between host characteristics, effect of interventions, and clinical outcomes. Additionally, the levels of C-reactive protein, albumin, lymphocytes, and platelets were the most commonly used laboratory parameters, either independently or in combination with other laboratory parameters and clinical characteristics. Furthermore, the immune parameters are classically examined using flow cytometry, immunohistochemical staining, and enzyme-linked immunosorbent assay techniques. However, gene expression profiling can aid in assessing the overall peri-interventional immune status. The checklists of guidelines, such as STAndards for Reporting of Diagnostic accuracy and REporting recommendations for tumor MARKer prognostic studies should be considered when designing studies to investigate the inflammatory parameters. Finally, the data should be interpreted after adjusting for clinically important variables, such as age and cancer stage.

Effect of Job Stress, Emotional Labor, and Positive Psychological Capital on the Job Satisfaction of Physician Assistants (전담간호사의 직무 스트레스, 감정노동과 긍정심리자본이 직무만족도에 미치는 영향)

  • Purpose: The purpose of this study was to identify the effects of job stress, emotional labor, and positive psychological capital on the job satisfaction of physician assistants (PAs). Methods: The sample consisted of 166 PAs working in general hospitals in B city, Y city, and C city. Data were analyzed through the SPSS/WIN 18.0 program using t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson's correlation coefficients, and multiple regression. Results: The factors of job stress (${\beta}=-.488$, p<.001), positive psychological capital (${\beta}=.188$, p=.004), and total clinical career (${\beta}=-.147$, p=.014) had the greatest influence on the level of job satisfaction experienced by the PAs. The total explanatory power was 49.9%. Conclusion: It was found that job stress, positive psychological capital, and total clinical career were influencing factors in the job satisfaction of PAs. Therefore, to improve their job satisfaction, it is necessary to develop strategies to manage the job stress experienced by PAs, and to strengthen and develop positive psychological capital.

Retrospective evaluation of toceranib phosphate (Palladia) for treatment of different tumor types in 31 dogs

  • The purpose of this retrospective study was to provide additional data on the use of toceranib in a wide variety of tumor types in small breed dogs, especially < 8 kg (except 5 dogs). This was a retrospective study of 31 dogs with malignant tumors treated with a 2.5 mg/kg median dose of toceranib (Palladia; Zoetis, USA) on a Monday-Wednesday-Friday schedule. Clinical benefit was observed in 13 of 15 dogs (86.7%, 3 with complete response, 4 with partial response, 6 with stable disease) with gross disease. Distant metastasis, response to treatment, and treatment setting were significantly associated with survival time. Negative prognostic factors were multiple chemotherapy and distant metastasis (affecting progression-free survival [PFS]), surgery, regional enlarged lymph nodes, underlying disease, and toxicity (affecting median survival time [MST]). Positive prognostic factors were epithelial and round cell tumor (affecting PFS), epithelial tumor, microscopic disease, no evidence of disease response, and stable disease (MST). In conclusion, a clinical benefit from toceranib treatment was noted in most of the dogs with gross disease in our study. This study suggested that the toceranib is probably selective treatment to various tumor types in small breed dogs.

  • Coronavirus disease 2019 (COVID-19) presents as a mild-to-moderate respiratory illness in most children. However, a small proportion of children with COVID-19 develop severe or critical illnesses. Although pediatric clinical trials for the treatment of COVID-19 are sparse, some drugs are available for children and adolescents with severe COVID-19. This review summarizes clinical data focusing on antiviral agents and immunomodulators for use in treating COVID-19. In addition, current recommendations for therapeutics for children and adolescents with COVID-19 are discussed.

Recent Advances in Cell Therapeutics for Systemic Autoimmune Diseases

  • Systemic autoimmune diseases arise from loss of self-tolerance and immune homeostasis between effector and regulator functions. There are many therapeutic modalities for autoimmune diseases ranging from conventional disease-modifying anti-rheumatic drugs and immunosuppressants exerting nonspecific immune suppression to targeted agents including biologic agents and small molecule inhibitors aiming at specific cytokines and intracellular signal pathways. However, such current therapeutic strategies can rarely induce recovery of immune tolerance in autoimmune disease patients. To overcome limitations of conventional treatment modalities, novel approaches using specific cell populations with immune-regulatory properties have been attempted to attenuate autoimmunity. Recently progressed biotechnologies enable sufficient in vitro expansion and proper manipulation of such 'tolerogenic' cell populations to be considered for clinical application. We introduce 3 representative cell types with immunosuppressive features, including mesenchymal stromal cells, Tregs, and myeloid-derived suppressor cells. Their cellular definitions, characteristics, mechanisms of immune regulation, and recent data about preclinical and clinical studies in systemic autoimmune diseases are reviewed here. Challenges and limitations of each cell therapy are also addressed.

The Frequency and Distribution of Unexpected Antibodies at a Tertiary Hospital in Daejeon (대전지역 대학병원에서 동정된 비예기항체의 분포와 빈도)

  • Antibody screening and identification tests before blood transfusion are important because unexpected red antibodies can cause acute or delayed hemolytic transfusion reactions. Although a tube method was used for detecting unexpected antibodies, a column agglutination method has recently been used because of its simple procedure and a high detection of warm antibodies. This study investigated the frequency and distribution of unexpected antibodies in transfusion candidates during the recent 5 years, and transfusion characteristics in the identified cases. From January 2005 to December 2009, 46,923 sera of the cases from E hospital were screened and 98 sera were identified by the DiaMed-ID System. 272 cases (0.58%) showed positive results out of all 46,923 cases that underwent unexpected antibodies screening. Among them, unexpected antibodies were identified in 98 cases. The anti-Rh antibodies included in warm antibodies were the most frequently detected in 47 cases (47.96%). Anti-Lewis and anti-MNSs antibodies were detected in 11 cases (11.22%) and 6 cases (6.12%), respectively. Unidentified antibodies were detected in 6 cases (6.12%). Among the patients with unexpected antibodies, 43 cases (43.88%) had a history of previous transfusion. Anti-E was the most frequently detected antibody (4/14 cases, 30.77%) in the cases who had a previous history of transfusion and showed different screening results from negative to positive, This study may provide the basic data for the frequency and characteristics of red cell antibodies.

The Effect of Angelica gigas Nakai Extract and Bacillus Polyfermenticus KJS-2 on Atopic Dermatitis induced by DNCB in mice (DNCB로 유도된 아토피 피부염에 대한 당귀 추출물과 Bacillus polyfermenticus KJS-2의 효과)

  • Objectives: The purpose of this study was to investigate the effects of Angelica gigas Nakai extract(AGNE) and Bacillus polyfermenticus KJS-2 (BP2) on atopic dermatitis (AD) induced by 2, 4-dinitrochlorobenzene (DNCB) in mice. Methods: In the experiment, we divided mice into four groups: a control group, a DNCB group, an AGNE group, and an AGNE+BP2 group. Then we examined the changes in scratching frequency, clinical aspects on dorsum skin, immunoglobulin (IgE), cytokines ($TNF-{\alpha}$, IL-6) and expression of COX-2. Resutls: From the experiment, the scratching frequency was significantly dropped in AGNE group and AGNE+BP2 group. Clinical observations of dorsum skin, there were a severe keratotic lesion and drop of dead skin cell in DNCB group, but symptoms of AD were decreased 39.6% in AGNE group and 49.6% in AGNE+BP2 group during 3 weeks. IgE, $TNF-{\alpha}$ and IL-6 were decreased significantly in both AGNE and AGNE+BP2 group. Expression of COX-2 was also decreased significantly in both groups. Conclusions: In conclusion, these data suggest that AGNE can decrease symptoms of AD and BP2 makes AGNE more effective. So AGNE can be useful herbal therapy for AD.

Pathological observations on diseased cockerels in rural areas of Bangladesh

  • The poultry farmers of rural area in Bangladesh usually prefer raising cockerel to broiler due to availability, low price of chicks, requirement of less space and feed, and high price of meat and the farmers believe that the cockerels are less susceptible to diseases in comparison to broilers. This study was carried out to observe the pathology of diseased cockerel in 3 farms of rural area in Bangladesh. A total of 974 birds were examined and the diagnoses of different disease/conditions were based on the history, clinical signs, characteristic gross, tissue alterations, clinical pathology and isolation and identification of the pathogenic organisms. The diseases in this study included infectious bursal disease, yolk sac infection, vitamin E deficiency, coccidiosis, and other diseases. The proportionate mortality rate were $7.29\%,\;0.62\%,\;0.72\%,\;0.21\%\;and\;0.10\%$, respectively, which indicated that most of the fatal causes of death were due to infectious bursal disease. Age group of 2-8 week old were the most susceptible to this disease and E coli was suggested as a cause of yolk sac infection. The data also suggested that the coccidiosis in rural areas of Bangladesh has decreased due to awareness of the farmers and routine use of coccidiostates.

Acoustic outputs from clinical extracorporeal shock wave lithotripsy devices (임상에서 사용중인 체외충격파쇄석기의 음향 출력 분포)

  • Survey was carried out on the acoustic outputs from 12 shock wave fields produced by the 10 extracorporeal shock wave lithotriptors whose technical documents are available, among the 33 devices approved by the Ministry of Food & Drug Safety (MFDS).The results show that the acoustic outputs (P+, P-, efd, and E), critical to the therapeutic efficacy and the patient safety, are largely different between the devices. The maximum values of P+, P-, efd, and E vary up to 2.08, 3.72, 3.89, and 15.98 times, respectively. The acoustic output parameters are not thoroughly provided in the technical documents, and some of data (eg. efd) are suspected to be abnormal outside usual ranges. The large device to device differences in the shock wave outputs are likely to undermine equivalence between the ESWL devices approved for the same indication. To verify the reliability of the data in the technical documents of the approved devices and to confirm if the acoustic outputs from the devices in clinical use are the same as those in their technical documents, an authorized test laboratory should be available. A postapproval monitoring led by the regulatory agency is suggested to maintain the acoustic outputs from the ESWL devices that suffer from degrading in performance due to aging.

Clinical and Phylogenetic Characteristics of Escherichia coli Urinary Tract Infections (대장균에 의한 요로감염의 연령별, 계통군별 특징)

  • Purpose : We aimed to investigate the clinical and phylogenetic characteristics of Escherichia coli Urinary Tract Infections (E. coli UTI). Methods : We enrolled patients with culture-proven E. coli UTI, who were admitted at the study hospital from September 2008 to August 2009. We investigated clinical data of patients with E. coli UTI and characteristics of isolated E. coli strains. The phylogenetic groups were classified using triplex polymerase chain reaction (PCR), and the distribution of nine virulent genes was determined by multiplex PCR. Results : A total of 47 patients have participated in this study. Thirty (63.8%) were under 6 months; eight (17.0%) were between 6-12 months; and nine (19.1%) were over 12 months. We compared two age groups between under 6-month and over 6-month. In the age group under 6-month, higher proportion of male (P =0.002) and group B2 strains (P =0.020) were observed. In contrast, higher proportion of female and group non-B2 strains were observed in age group over 6-month. Frequencies of papC, papGII, papGIII, sfa/foc, hlyC, cnf1, fyuA, iroN and iucC were estimated as 68.1%, 57.4%, 42.6%, 46.8%, 46.8%, 31.9%, 87.2%, 48.9% and 63.8%, respectively. In the comparison of phylogenetic groups, group B2 showed higher distribution of virulent genes, while group D included more strains resistant to trimethoprim/sulfamethoxazole (TMP/SMZ) than other groups. Conclusion : We showed the age group-specific difference in the distribution of sex ratios and phylogenetic groups; more male and group B2 strains in age group under 6-month, while more female and group non-B2 in age group over 6-month. However, further evaluation including larger number of patients will be necessary to confirm above thesis in future molecular epidemiological studies.
